Inleiding. Dit onderzoek had als doel een beknopt instrument te ontwikkelen voor het tijdig herkennen van kinderen met een taalontwikkelingsstoornis (TOS), dat een acceptabele voorspellende validiteit heeft. Methode. In een nested case-controlonderzoek werden leerlingen, in de leeftijd van 4 tot 11 jaar, van een school voor speciaal onderwijs cluster 2 (voor kinderen met ernstige spraaktaalmoeilijkheden) met bewezen TOS gematcht op geboortedatum en geslacht met kinderen die het reguliere basisonderwijs volgden. Vervolgens analyseerden wij de voorspellende validiteit voor het hebben van TOS op grond van combinaties van de scores op de taalkenmerken van het Van Wiechenonderzoek op de leeftijd van 24, 36 en 45 maanden. We konden gebruikmaken van de gegevens van 253 paren van kinderen met en zonder TOS. Resultaten. Bij een combinatie van uitval op twee taalkenmerken op één leeftijdsmoment varieerde de specificiteit van 85 tot 100 % en de sensitiviteit van 32 tot 78 %. Wanneer vijf taalkenmerken op de drie verschillende leeftijden werden gecombineerd, werd een specificiteit van 96 % (95 %-betrouwbaarheidsinterval (BI) 94–99 %) en een sensitiviteit van 71 % (95 %-BI 66–77 %) gevonden. Conclusie. De gevonden combinatie van vijf taalkenmerken kan beschouwd worden als een beknopt instrument met een acceptabele voorspellende validiteit om kinderen met TOS tijdig te kunnen herkennen.

EUSUHM-congres: kennisdelen voor en samenwerken aan een gezonde jeugd! Changing contexts—influencing youth and their surroundings. Het thema van het congres was ‘Changing contexts – influencing youth and their surroundings’. Met dit thema komt tot uiting dat gezondheid meer is dan lichaam en geest en dat lichaam en geest worden beïnvloed door de context waarin zij zich bevinden. Dit is waar publieke gezondheid over gaat.

In dit hoofdstuk worden enkele veelvoorkomende klinische problemen besproken vanuit kinderneurologisch perspectief, zoals afwijkende ontwikkeling, hoofdpijn en afwijkende vorm en grootte van het hoofd. Daarnaast komt een aantal aandoeningen van het zenuwstelsel aan de orde waar de kinderarts regelmatig mee te maken krijgt, zoals koortsconvulsies, epilepsie, neurocutane aandoeningen en schedel-hersenletsel. Daarbij wordt geprobeerd de kinderarts handvatten te bieden om met behulp van anamnese en onderzoek tot een juiste differentiaaldiagnose te komen en daaruit voortvloeiend verantwoorde beslissingen te nemen ten aanzien van zinvol en noodzakelijk aanvullend onderzoek.

© 2015 American Medical Association. All rights reserved. Importance: Patients with 22q11.2 deletion syndrome (22q11DS) have an elevated (25%) risk of developing schizophrenia. Recent reports have suggested that a subgroup of children with 22q11DS display a substantial decline in cognitive abilities starting at a young age.Objective: To determine whether early cognitive decline is associated with risk of psychotic disorder in 22q11DS.Design, Setting, And Participants: Prospective longitudinal cohort study. As part of an international research consortium initiative, we used the largest data set of intelligence (IQ) measurements in patients with 22q11DS reported to date to investigate longitudinal IQ trajectories and the risk of subsequent psychotic illness. A total of 829 patients with a confirmed hemizygous 22q11.2 deletion, recruited through 12 international clinical research sites, were included. Both psychiatric assessments and longitudinal IQ measurements were available for a subset of 411 patients (388 with≥1 assessment at age 8-24 years).Main Outcomes And Measures: Diagnosis of a psychotic disorder, initial IQ, longitudinal IQ trajectory, and timing of the last psychiatric assessment with respect to the last IQ test.Results: Among 411 patients with 22q11DS, 55 (13.4%) were diagnosed as having a psychotic disorder. The mean (SD) age at the most recent psychiatric assessment was 16.1 (6.2) years. The mean (SD) full-scale IQ at first cognitive assessment was lower in patients who developed a psychotic disorder (65.5 [12.0]) compared with those without a psychotic disorder (74.0 [14.0]). On average, children with 22q11DS showed a mild decline in IQ (full-scale IQ, 7.04 points) with increasing age, particularly in the domain of verbal IQ (9.02 points). In those who developed psychotic illness, this decline was significantly steeper (P < .001). Those with a negative deviation from the average cognitive trajectory observed in 22q11DS were at significantly increased risk for the development of a psychotic disorder (odds ratio = 2.49; 95%CI, 1.24-5.00; P = .01). The divergence of verbal IQ trajectories between those who subsequently developed a psychotic disorder and those who did not was distinguishable from age 11 years onward.Conclusions And Relevance: In 22q11DS, early cognitive decline is a robust indicator of the risk of developing a psychotic illness. These findings mirror those observed in idiopathic schizophrenia. The results provide further support for investigations of 22q11DS as a genetic model for elucidating neurobiological mechanisms underlying the development of psychosis.

To investigate susceptible groups and risk factors for childhood drowning in the Netherlands with the aim of improving prevention campaigns.Prospective and partly retrospective.Information about drowning accidents in 2010 and 2011 was collected using two methods. Firstly, drowning - with or without a fatal outcome - was added to the list of conditions to be reported to the Netherlands Paediatric Surveillance Unit (NSCK). Paediatricians received a comprehensive questionnaire when they had reported on drowning. Additional information on drowning was collected from online media (news websites and online journals).A total of 82 reports of drowning were analysed (63 from the paediatricians' survey and 19 additional reports from online media). Twenty-three children died as a result of drowning in the Netherlands in 2010 and 2011. Fifty-four percent of all cases of drowning were in children younger than 4 years. Boys were victims in 71% of the cases. Half (51%) of the accidents happened because the parents or caregivers lost sight of the children. In 27% of cases the accident took place in or around the home. Twenty-one children (26%) were victims of drowning in a public swimming pool and 5 of these children died.Better and continuous supervision of young children could help prevent more cases of drowning. Open water in proximity to the home should be fenced off. Continuous good supervision is also essential in public swimming pools.

To determine the level of independence and social functioning in young people with Down syndrome.Cross-sectional study.Via the Dutch Down Syndrome Foundation (Stichting Downsyndroom), we asked parents of children with Down syndrome born in 1992, 1993 or 1994 to complete a written questionnaire about their child. This questionnaire contained the following standardised lists: the 'Dutch social competence rating scale for people with a learning disability', the 'Child behaviour checklist' and the 'Children's social behaviour questionnaire', and additional questions on background characteristics. The results of this Dutch cohort were compared with available data on peers without Down syndrome.Data from 322 young people with Down syndrome, mean age 18.4 years (range 16.8-19.9 years), were collected (response 63%). Almost 60% of participants mastered basic skills of independent functioning, such as maintaining adequate standards of personal hygiene, preparing breakfast and being able to spend at least 30 minutes at home alone. About 10% of the participants had basic skills such as cooking and paying in a shop. Nine out of ten participants had more problems with social functioning than peers without Down syndrome, mainly with social interaction, processing information and regulating their emotions. Half of the participants had clinically relevant behavioural problems.The results of this study show that young people with Down syndrome have limited practical and social skills, and more behavioural problems than their peers without Down syndrome. In daily life they are to a greater or lesser extent dependent on others, and need lifelong support.

Doel Bepalen welke mate van zelfredzaamheid en sociaal functioneren jongeren met Down-syndroom bereiken. Opzet Dwarsdoorsnedeonderzoek. Methode We vroegen ouders van een kind met Down-syndroom dat was geboren in 1992, 1993 of 1994, via de Stichting Downsyndroom een schriftelijke vragenlijst in te vullen over hun kind. De vragenlijst bevatte de gestandaardiseerde lijsten ‘Sociale redzaamheid voor zwakzinnigen’, ‘Child behavior checklist’ en ‘Vragenlijst voor inventarisatie van sociaal gedrag van kinderen’, en aanvullende vragen over achtergrondkenmerken. We vergeleken de uitkomsten van dit Nederlandse cohort met beschikbare gegevens van leeftijdsgenoten zonder Down-syndroom. Resultaten We verkregen de gegevens van 322 jongeren met Down-syndroom met een gemiddelde leeftijd van 18,4 jaar (uitersten: 16,8-19,9) (respons: 63%). Bijna 60% van de deelnemers beheerste basisvaardigheden van zelfstandig functioneren, zoals zichzelf verzorgen en wassen, ontbijten en ten minste 30 min alleen thuis kunnen zijn. Ongeveer 10% van de deelnemers beheerste vaardigheden als koken of betalen in een winkel. 9 van de 10 deelnemers hadden meer problemen met sociaal functioneren dan hun leeftijdsgenoten zonder Down-syndroom, met name in sociale interactie, het verwerken van informatie en het reguleren van emoties. De helft van de deelnemers had klinisch relevante gedragsproblemen. Conclusie De resultaten van dit onderzoek wijzen erop dat jongeren met Down-syndroom beperkte praktische en sociale vaardigheden hebben en meer gedragsproblemen dan hun leeftijdsgenoten zonder Down-syndroom. Ze zijn in het dagelijks leven in meer of mindere mate afhankelijk van anderen en hebben hun leven lang begeleiding nodig.

Na de geboorte van een kind is één van de eerste keuzen waarvoor de ouders worden gesteld: hoe voeden wij ons kind? Borstvoeding is de meest natuurlijke en gezonde voeding, en de meeste moeders (91%) starten dan ook vol enthousiasme, maar na de eerste week is de borstvoeding slechts bij 80% goed op gang gekomen. Vaak haakt een aanzienlijk deel van de moeders in de periode daarna snel weer af. Te vaak nog is dat terug te voeren op een te kort schietende begeleiding van de ouders. Er is weliswaar veel informatie over borstvoeding beschikbaar, maar deze wordt gefragmenteerd aangeboden en is (lang) niet altijd evidence-based en up to date. Een professionele kijk op borstvoeding voorziet in die leemte. Anatomie, fysiologie, toxicologie en psychosociale aspecten van borstvoeding komen aan de orde, maar ook is er volop aandacht voor de klinische, de praktische en de beleidsmatige aspecten. De wetenschappelijke achtergronden zoals de immunologische werking van moedermelk – de belangrijkste reden waarom borstvoeding zo gezond is – komen aan de orde. De uitgave is bestemd voor alle professionals die zijn betrokken bij de zorg voor borstvoeding: verloskundigen, gynaecologen, kinderartsen, verpleegkundigen van de afdeling neonatologie en gynaecologie, huisartsen, nurse practitioners, artsen en verpleegkundigen werkzaam in de jeugdgezondheidszorg, lactatiekundigen, diëtisten, kraamverzorgenden en doktersassistenten en zij die daarvoor in opleiding zijn. De uitgave biedt overigens ook volop nuttige informatie voor studenten en docenten van opleidingen in de gezondheidszorg, medische studenten en co-assistenten. Met dit eerste wetenschappelijke (leer)boek over borstvoeding verwacht de redactie professionals – meer dan voorheen – op één lijn te krijgen ten aanzien van het geven van uniforme adviezen en een wetenschappelijke onderbouwing te leveren voor de overtuiging dat ‘breast is best’.

A 12-year-old boy with Down's syndrome presented with fever, arthritis and petechiae. Investigations revealed the presence of disseminated gonorrhoea. The patient made a good recovery after antibiotic therapy. A contact investigation was carried out, as well as talks with the family and school, and the case was reported to the Child Abuse Advice and Report Centre. The gonorrhoea was found to be caused by incest. Sexual abuse should be suspected in every child (certainly over the age of 1 year) with a sexually transmitted disease.

Four children of Turkish origin, three boys aged 12, 8 and 7 years, and a girl aged 5 years, presented with clinical symptoms of familial Mediterranean fever. They had the characteristic episodes of fever combined with abdominal pain, thoracic pain, general malaise or arthralgia. Familial Mediterranean fever is an autosomal recessive genetic disorder restricted to people originating from the Middle East. The causative gene (MEFV) and many missense mutations have been identified. The clinical syndrome is characterised by self-limiting febrile episodes accompanied by inflammation of the serous membranes, resulting in peritonitis, pleuritis or synovitis. In untreated patients systemic amyloidosis may develop, which manifests as renal insufficiency. The diagnosis is based on the characteristic medical history and is confirmed by DNA analysis. Meanwhile, treatment with colchicine can be started. This is effective in 90% of affected patients. Being aware of the prevalence of familial Mediterranean fever in immigrant populations can improve the quality of life and prevent long-term complications.

Approximately 1 to 2 per thousand live-born infants suffer from a serious perceptive hearing loss. Normal hearing from birth is essential for optimal human development (language and speech, social and emotional development, communicative skills and learning). The earlier the hearing loss is diagnosed the better the prognosis for the infant with a hearing impairment. Suitable methods are now available for neonatal hearing screening: automated measurement of auditory brain stem response and measurement of oto-acoustic emissions. Screening must be viewed as only the first step in a program of diagnosis, treatment and habilitation of these children. The ultimate goal of the implementation of neonatal hearing screening is: identification of bilateral hearing losses before the age of 3 months and start of therapy and counselling before the age of 6 months.

To determine whether incubator home care is desirable and feasible.Inventory.Four neonatal units representative of the type of care in general hospitals in the Netherlands.The relevant data on all infants with a birth weightor = 2000 g admitted in the last 3 months of 1996 to one of four hospitals were analysed. Conditions for incubator home care were determined (e.g. absence of need for special care, vital function monitoring or nasogastric tube feeding).Forty-nine infants were enrolled. Mean hospital stay was 28.7 days in an incubator plus 19.7 days in a cot. When infants were placed in a cot they usually still needed tube feeding and monitoring of vital functions and sometimes parenteral nutrition, medication or extra oxygen which made home discharge impossible. Therefore a pilot study of actual home care could not be carried out.Although early home discharge is very desirable for newborn infants, the number of infants eligible for incubator home care is so small that further attempts to organise it are not useful.
