Point of Care Testing is een innovative stroming binnen de geneeskunde. Aan de hand van een voorbeeld, waarbij het doel is flebitis in een vroeg stadium te kunnen onderkennen, wordt in dit artikel het belang van deze methode toegelicht.

An influenza epidemic can greatly increase the workload in primary care and the emergency department (ED) and can even disrupt the healthcare system. It is difficult to diagnose influenza by history taking and physical examination. A fast diagnosis usinginfluenza point-of-care tests (POCTs) could reduce unnecessary antibiotic prescriptions, diagnostic tests, consultations and hospital admissions. Moreover, length of stay on EDs and length of admission could be shortened. The analytical accuracy of antigen detection tests for influenza is relatively low compared to the well performing RT-PCR assays (sensitivity and specificity approximately 95%). Only 1 randomized controlled trial has shown the effect of a (combined) RT-PCR assay for influenza detection on clinically relevant outcome measures. Observational research suggests that introduction of RT-PCR assays for influenza detection reduces length of stay on the ED and decreased time from sample reception to result. For practical reasons, we should embrace the introduction of RT-PCR assays for influenza detection on EDs. Before POCTs can be implemented in primary care (family medicine) the analytical accuracy and time to receive results should be improved and effects of its clinical impact should be proven.

We propose that diagnostic tests should only be used when their results could affect treatment and prognosis of the diagnosed condition, taking costs into account. The cost-effectiveness of MRI has been evaluated in a randomised clinical trial in which 356 patients were divided over one group in which MRI was done within two weeks after experiencing trauma and one group that received standard care. No difference in QALYs was found between the groups. The number of referrals did not decrease in the MRI group, whereas costs for treatment increased. The point-of-care CRP-test for dealing with patients with acute cough presenting in daily general practice is one example of the opposite. CRP led to a decrease of antibiotic prescriptions and was found to be cost-effective. We conclude that more experts on diagnostics should be consulted in general practice and that diagnostic tests and procedures should receive attention in the medical curriculum and vocational training for general practice.

Ziekenhuispersoneel gebruikt point-of-care (POC-)testen om buiten het laboratorium sneller een diagnose te kunnen stellen en vervolgens een behandeling te starten of aan te passen. Naar aanleiding van enkele incidenten heeft de Inspectie voor de Gezondheidszorg (IGZ) in 2008 alle Nederlandse ziekenhuizen aanbevelingen gestuurd om het veilig gebruik van bloedglucosemeters te bevorderen. Uit onderzoek van het RIVM blijkt dat het merendeel van de Nederlandse ziekenhuizen deze aanbevelingen heeft opgevolgd. Een aanzienlijk deel daarvan had zelfs al eerder maatregelen getroffen. Het onderzoek is uitgevoerd in opdracht van de IGZ. De aanbevelingen uit de IGZ-circulaire betreffen onder andere de wijze waarop ziekenhuispersoneel bloedmonsters afneemt, de hygiëne tijdens de monsterafname, het werken via een protocol, de aansluiting van POC-meters op het netwerk van het ziekenhuis en (bij)scholing van ziekenhuispersoneel. Uit het onderzoek blijkt dat de meeste ziekenhuizen beschikken over POCbloedglucosemeters om de bloedsuikerspiegel te meten. Andere POC-testen die op grote schaal worden gebruikt op diverse afdelingen van veel ziekenhuizen zijn bijvoorbeeld testen om de mate van bloedstolling te meten of het hemoglobinegehalte (het eiwit dat zuurstof door het lichaam vervoert). De aanbevelingen uit de IGZ-circulaire zijn ook voor andere POC-testen opgevolgd. Wel varieert per POC-test hoeveel ziekenhuizen actie hebben ondernomen. Om erop toe te kunnen zien of POC-testen naar behoren worden uitgevoerd, vallen ze in de meeste ziekenhuizen onder het kwaliteitssysteem van het eigen klinisch chemische laboratorium. Deze laboratoria voeren routinematig veel analyses uit op patiëntmonsters, en kunnen door deze ervaring het gebruik van de POC-testen goed beoordelen.

Point-of-care testing (POCT) is being used increasingly in general practice and other healthcare contexts outside the hospital. Recommendations for correct and safe use of POCT in Dutch general practice have been laid down in the guideline 'Point-of-care testing in general practice'. The recommendations in this guideline are based on existing regulations and guidelines, both national and international, and respect the different roles and responsibilities within the healthcare chain.
