Objective: To assess the current trends in the field of artificial intelligence in medicine by analysing 100 most cited original articles relevant to the field. Methods: The systematic review was conducted in September 2022, and comprised literature search on Scopus database for original articles only. Google and Medical Subject Headings databases were used as resources to extract key words. In order to cover a broad range of articles, original studies comprising human as well as non-human subjects, studies without abstract and studies in languages other than English were part of the inclusion criteria. There was no specific time period applied to the search and no specific selection was done regarding the journals in the database. The screening was done using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines to extract the top 100 most cited articles in the field of artificial intelligence usage in medicine. Data was analysed using SPSS 23. Results: Of the 11,571 studies identified, 100(0.86%) were analysed in detail. The studies were published between 1986 and 2021, with a median of 43 citations (IQR 53) per article. The journal ‘Artificial Intelligence in Medicine’ accounted for the highest number 9(9%)) of articles, and the United States was the country of origin for most of the articles 36(36%). Conclusion: The trends, development and shortcomings in field of artificial intelligence usage in medicine need to be understood to conduct an effective research in areas that still need attention, and to guide the authorities to direct their funding accordingly.

Abstract Artificial intelligence (AI) is rapidly moving from an experimental phase to an implementation phase in many fields, including medicine. The combination of improved availability of large datasets, increasing computing power, and advances in learning algorithms has created major performance breakthroughs in the development of AI applications. In the last 5 years, AI techniques known as deep learning have delivered rapidly improving performance in image recognition, caption generation, and speech recognition. Radiology, in particular, is a prime candidate for early adoption of these techniques. It is anticipated that the implementation of AI in radiology over the next decade will significantly improve the quality, value, and depth of radiology's contribution to patient care and population health, and will revolutionize radiologists' workflows. The Canadian Association of Radiologists (CAR) is the national voice of radiology committed to promoting the highest standards in patient-centered imaging, lifelong learning, and research. The CAR has created an AI working group with the mandate to discuss and deliberate on practice, policy, and patient care issues related to the introduction and implementation of AI in imaging. This white paper provides recommendations for the CAR derived from deliberations between members of the AI working group. This white paper on AI in radiology will inform CAR members and policymakers on key terminology, educational needs of members, research and development, partnerships, potential clinical applications, implementation, structure and governance, role of radiologists, and potential impact of AI on radiology in Canada. Artificial intelligence (AI) is becoming increasingly important, especially in the medical field. While AI has been used in medicine for some time, its growth in the last decade is remarkable. Specifically, machine learning (ML) and deep learning (DL) techniques in medicine have been increasingly adopted due to the growing abundance of health-related data, the improved suitability of such techniques for managing large datasets, and more computational power. ML and DL methodologies are fostering the development of new "intelligent" tools and expert systems to process data, to automatize human–machine interactions, and to deliver advanced predictive systems that are changing every aspect of the scientific research, industry, and society. The Italian scientific community was instrumental in advancing this research area. This article aims to conduct a comprehensive investigation of the ML and DL methodologies and applications used in medicine by the Italian research community in the last five years. To this end, we selected all the papers published in the last five years with at least one of the authors affiliated to an Italian institution that in the title, in the abstract, or in the keywords present the terms "machine learning" or "deep learning" and reference a medical area. We focused our research on journal papers under the hypothesis that Italian researchers prefer to present novel but well-established research in scientific journals. We then analyzed the selected papers considering different dimensions, including the medical topic, the type of data, the pre-processing methods, the learning methods, and the evaluation methods. As a final outcome, a comprehensive overview of the Italian research landscape is given, highlighting how the community has increasingly worked on a very heterogeneous range of medical problems. Spurred by advances in processing power, memory, storage, and an unprecedented wealth of data, computers are being asked to tackle increasingly complex learning tasks, often with astonishing success. Computers have now mastered a popular variant of poker, learned the laws of physics from experimental data, and become experts in video games - tasks that would have been deemed impossible not too long ago. In parallel, the number of companies centered on applying complex data analysis to varying industries has exploded, and it is thus unsurprising that some analytic companies are turning attention to problems in health care. The purpose of this review is to explore what problems in medicine might benefit from such learning approaches and use examples from the literature to introduce basic concepts in machine learning. It is important to note that seemingly large enough medical data sets and adequate learning algorithms have been available for many decades, and yet, although there are thousands of papers applying machine learning algorithms to medical data, very few have contributed meaningfully to clinical care. This lack of impact stands in stark contrast to the enormous relevance of machine learning to many other industries. Breast cancer is an illness that affects many women and can cause even death; this is a case of not being detected on time, which could be due to a human error during the analysis of radiographic images or not going on time in a health center. For this, using machine learning (ML) to analyze radiographic images is proposed as a support tool for radiologists aiming to reduce false diagnostic rates. While researching information, it was detected that this technology has many benefits in the health area; however, it also has limitations or disadvantages. The importance of this paper is to demonstrate that there are not enough clinical tests nor details about the methodologies that were used; there should be more to assert that ML is defined at the moment of making a diagnosis, which generates no conclusive results regarding effectiveness and therefore creates mistrust in doctors, and some people might rather use deep learning (DL) for its application in the detection of breast cancer because DL has more practical tests and fewer limitations than machine learning. With the swift pace of the development of artificial intelligence (AI) in diverse spheres, the medical and healthcare fields are utilizing machine learning (ML) methodologies in numerous inventive ways. ML techniques have outstripped formerly state-of-the-art techniques in medical and healthcare practices, yielding faster and more precise outcomes. Healthcare practitioners are increasingly drawn to this technology in their initiatives relating to the Internet of Behavior (IoB). This area of research scrutinizes the rationales, approaches, and timing of human technology adoption, encompassing the domains of the Internet of Things (IoT), behavioral science, and edge analytics. The significance of ML in medical and healthcare applications based on the IoB stems from its ability to analyze and interpret copious amounts of complex data instantly, providing innovative perspectives that can enhance healthcare outcomes and boost the efficiency of IoB-based medical and healthcare procedures and thus aid in diagnoses, treatment protocols, and clinical decision making. As a result of the inadequacy of thorough inquiry into the employment of ML-based approaches in the context of using IoB for healthcare applications, we conducted a study on this subject matter, introducing a novel taxonomy that underscores the need to employ each ML method distinctively. With this objective in mind, we have classified the cutting-edge ML solutions for IoB-based healthcare challenges into five categories, which are convolutional neural networks (CNNs), recurrent neural networks (RNNs), deep neural networks (DNNs), multilayer perceptions (MLPs), and hybrid methods. In order to delve deeper, we conducted a systematic literature review (SLR) that examined critical factors, such as the primary concept, benefits, drawbacks, simulation environment, and datasets. Subsequently, we highlighted pioneering studies on ML methodologies for IoB-based medical issues. Moreover, several challenges related to the implementation of ML in healthcare and medicine have been tackled, thereby gradually fostering further research endeavors that can enhance IoB-based health and medical studies. Our findings indicated that Tensorflow was the most commonly utilized simulation setting, accounting for 24% of the proposed methodologies by researchers. Visual attention estimation is an active field of research at the crossroads of different disciplines: computer vision, deep learning, and medicine. One of the most common approaches to estimate a saliency map representing attention is based on the observed images. In this paper, we show that visual attention can be retrieved from EEG acquisition. The results are comparable to traditional predictions from observed images, which is of great interest. Image-based saliency estimation being participant independent, the estimation from EEG could take into account the subject specificity. For this purpose, a set of signals has been recorded, and different models have been developed to study the relationship between visual attention and brain activity. The results are encouraging and comparable with other approaches estimating attention with other modalities. Being able to predict a visual saliency map from EEG could help in research studying the relationship between brain activity and visual attention. It could also help in various applications: vigilance assessment during driving, neuromarketing, and also in the help for the diagnosis and treatment of visual attention-related diseases. Machine learning (ML), artificial neural networks (ANNs), and deep learning (DL) are all topics that fall under the heading of artificial intelligence (AI) and have gained popularity in recent years. ML involves the application of algorithms to automate decision-making processes using models that have not been manually programmed but have been trained on data. ANNs that are a part of ML aim to simulate the structure and function of the human brain. DL, on the other hand, uses multiple layers of interconnected neurons. This enables the processing and analysis of large and complex databases. In medicine, these techniques are being introduced to improve the speed and efficiency of disease diagnosis and treatment. Each of the AI techniques presented in the paper is supported with an example of a possible medical application. Given the rapid development of technology, the use of AI in medicine shows promising results in the context of patient care. It is particularly important to keep a close eye on this issue and conduct further research in order to fully explore the potential of ML, ANNs, and DL, and bring further applications into clinical use in the future. We present in this paper some of the existing machine learning and sound processing techniques involved in the medical process and show how they can be applied in the context of a vocal disease detection task. The machine learning techniques are the usual, ranging from decision trees to neural networks, and when it comes to the sound processing approaches some scientists probably are familiar with them from the speech processing or music analysis areas. However, the techniques are adapted to the particularities of the medical field - data collection, verification, etc. Through the concrete example of the vocal disease detection task we worked on, we made some interesting observations, both on the classification ability of the model and on the influence of the data. Abstract Acute appendicitis is a typical surgical emergency worldwide and one of the common causes of surgical acute abdomen in the elderly. Accurately diagnosing and differentiating acute appendicitis can assist clinicians in formulating a scientific and reasonable treatment plan and providing high-quality medical services for the elderly. In this study, we validated and analyzed the different performances of various machine learning models based on the analysis of clinical data, so as to construct a simple, fast, and accurate estimation method for the diagnosis of early acute appendicitis. The dataset of this paper was obtained from the medical data of elderly patients with acute appendicitis attending the First Affiliated Hospital of Anhui University of Chinese Medicine from January 2012 to January 2022, including 196 males (60.87%) and 126 females (39.13%), including 103 (31.99%) patients with complicated appendicitis and 219 (68.01%) patients with uncomplicated appendicitis. By comparing and analyzing the prediction results of the models implemented by nine different machine learning techniques (LR, CART, RF, SVM, Bayes, KNN, NN, FDA, and GBM), we found that the GBM algorithm gave the optimal results and that sensitivity, specificity, PPV, NPV, precision, recall, F1 and brier are 0.9167, 0.9739, 0.9429, 0.9613, 0.9429, 0.9167, 0.9296, and 0.05649, respectively. The GBM model prediction results are interpreted using the SHAP technology framework. Calibration and Decision curve analysis also show that the machine learning model proposed in this paper has some clinical and economic benefits. Finally, we developed the Shiny application for complicated appendicitis diagnosis to assist clinicians in quickly and effectively recognizing patients with complicated appendicitis (CA) and uncomplicated appendicitis (UA), and to formulate a more reasonable and scientific clinical plan for acute appendicitis patient population promptly.

Abstract This paper presents a comparison of machine learning (ML) methods used for three-dimensional localization of partial discharges (PD) in a power transformer tank. The study examines ML and deep learning (DL) methods, ranging from support vector machines (SVM) to more complex approaches like convolutional neural networks (CNN). Multiple case studies are considered, each with different attributes, including sensor position, frequency content of the PD signal, and size of the transformer tank. The paper focuses on predicting the PD location in three-dimensional space using single-sensor electric field measurements. Various aspects of each method are analyzed, such as the input signal, core methodology, correlation coefficient between the predicted location and the actual location, and root mean square error (RMSE). These features are discussed and compared across the different methods. The results indicate that the CNN model exhibits superior performance in terms of location accuracy among the methods considered.

One problem in the quantitative assessment of biomechanical impairments in Parkinson's disease patients is the need for scalable and adaptable computing systems. This work presents a computational method that can be used for motor evaluations of pronation-supination hand movements, as described in item 3.6 of the Unified Parkinson's Disease Rating Scale (MDS-UPDRS). The presented method can quickly adapt to new expert knowledge and includes new features that use a self-supervised training approach. The work uses wearable sensors for biomechanical measurements. We tested a machine-learning model on a dataset of 228 records with 20 indicators from 57 PD patients and eight healthy control subjects. The test dataset's experimental results show that the method's precision rates for the pronation and supination classification task achieved up to 89% accuracy, and the F1-scores were higher than 88% in most categories. The scores present a root mean squared error of 0.28 when compared to expert clinician scores. The paper provides detailed results for pronation-supination hand movement evaluations using a new analysis method when compared to the other methods mentioned in the literature. Furthermore, the proposal consists of a scalable and adaptable model that includes expert knowledge and affectations not covered in the MDS-UPDRS for a more in-depth evaluation. Abstract Active learning has become an increasingly popular method for screening large amounts of data in systematic reviews and meta-analyses. The active learning process continually improves its predictions on the remaining unlabeled records, with the goal of identifying all relevant records as early as possible. However, determining the optimal point at which to stop the active learning process is a challenge. The cost of additional labeling of records by the reviewer must be balanced against the cost of erroneous exclusions. This paper introduces the SAFE procedure, a practical and conservative set of stopping heuristics that offers a clear guideline for determining when to end the active learning process in screening software like ASReview. The eclectic mix of stopping heuristics helps to minimize the risk of missing relevant papers in the screening process. The proposed stopping heuristic balances the costs of continued screening with the risk of missing relevant records, providing a practical solution for reviewers to make informed decisions on when to stop screening. Although active learning can significantly enhance the quality and efficiency of screening, this method may be more applicable to certain types of datasets and problems. Ultimately, the decision to stop the active learning process depends on careful consideration of the trade-off between the costs of additional record labeling against the potential errors of the current model for the specific dataset and context.

We present our work to develop digital objects to represent and convey a specific category of scientific knowledge: computable biomedical knowledge (CBK). Properly developed, validated, implemented and stewarded, CBK has the potential to accelerate the translation of actionable knowledge from scientific discovery to clinical application. Our research takes an infrastructural approach to CBK, initially by focusing on the creation of a conceptual model for packaging computable biomedical knowledge - the Knowledge Object (KO) - and on corresponding efforts to create an architecture for KO management and implementation. Additionally, our work is grounded in the FAIR principles, such that KO artefacts should be findable, accessible, interoperable and reusable and we are exploring aligning KOs with emerging best practices for FAIR Digital Objects (FDO). The outcomes of this work resonate in clinical contexts, health professions education, healthcare quality improvement, biomedical and translational research and population care. Our KO model is also of interest to researchers and practitioners interested in knowledge science, including those working with semantic technologies and other forms of digital objects. Antimicrobial resistance has become a critical global health problem due to the abuse of conventional antibiotics and the rise of multi-drug-resistant microbes. Antimicrobial peptides (AMPs) are a group of natural peptides that show promise as next-generation antibiotics due to their low toxicity to the host, broad spectrum of biological activity, including antibacterial, antifungal, antiviral, and anti-parasitic activities, and great therapeutic potential, such as anticancer, anti-inflammatory, etc. Most importantly, AMPs kill bacteria by damaging cell membranes using multiple mechanisms of action rather than targeting a single molecule or pathway, making it difficult for bacterial drug resistance to develop. However, experimental approaches used to discover and design new AMPs are very expensive and time-consuming. In recent years, there has been considerable interest in using in silico methods, including traditional machine learning (ML) and deep learning (DL) approaches, to drug discovery. While there are a few papers summarizing computational AMP prediction methods, none of them focused on DL methods. In this review, we aim to survey the latest AMP prediction methods achieved by DL approaches. First, the biology background of AMP is introduced, then various feature encoding methods used to represent the features of peptide sequences are presented. We explain the most popular DL techniques and highlight the recent works based on them to classify AMPs and design novel peptide sequences. The paper reviews the entire spectrum of Artificial Intelligence (AI) in mental health and its positive role in mental health. AI has a huge number of promises to offer mental health care and this paper looks at multiple facets of the same. The paper first defines AI and its scope in the area of mental health. It then looks at various facets of AI like machine learning, supervised machine learning and unsupervised machine learning and other facets of AI. The role of AI in various psychiatric disorders like neurodegenerative disorders, intellectual disability and seizures are discussed along with the role of AI in awareness, diagnosis and intervention in mental health disorders. The role of AI in positive emotional regulation and its impact in schizophrenia, autism spectrum disorders and mood disorders is also highlighted. The article also discusses the limitations of AI based approaches and the need for AI based approaches in mental health to be culturally aware, with structured flexible algorithms and an awareness of biases that can arise in AI. The ethical issues that may arise with the use of AI in mental health are also visited.

Cardiovascular diseases (CVDs) are the leading cause of premature death and disability globally, leading to significant increases in healthcare costs and economic strains. Artificial intelligence (AI) is emerging as a crucial technology in this context, promising to have a significant impact on the management of CVDs. A wide range of methods can be used to develop effective models for medical applications, encompassing everything from predicting and diagnosing diseases to determining the most suitable treatment for individual patients. This literature review synthesizes findings from multiple studies that apply AI technologies such as machine learning algorithms and neural networks to electrocardiograms, echocardiography, coronary angiography, computed tomography, and cardiac magnetic resonance imaging. A narrative review of 127 articles identified 31 papers that were directly relevant to the research, encompassing a broad spectrum of AI applications in cardiology. These applications included AI models for ECG, echocardiography, coronary angiography, computed tomography, and cardiac MRI aimed at diagnosing various cardiovascular diseases such as coronary artery disease, hypertrophic cardiomyopathy, arrhythmias, pulmonary embolism, and valvulopathies. The papers also explored new methods for cardiovascular risk assessment, automated measurements, and optimizing treatment strategies, demonstrating the benefits of AI technologies in cardiology. In conclusion, the integration of artificial intelligence (AI) in cardiology promises substantial advancements in diagnosing and treating cardiovascular diseases.

This paper is written by a group of Ph.D. students pursuing their work in different areas of ICT, outside the direct area of Information Quantum Technologies IQT. An ambitious task was undertaken to research, by each co-author, a potential practical influence of the current IQT development on their current work. The research of co-authors span the following areas of ICT: CMOS for IQT, QEC, quantum time series forecasting, IQT in biomedicine. This paper will discuss the European funded iToBoS project, tasked by the European Commission to develop an AI diagnostic platform for the early detection of skin melanoma. The paper will outline the project, provide an overview of the data being processed, describe the impact assessment processes, and explain the AI privacy risk mitigation methods being deployed. Following this, the paper will offer a brief discussion of some of the more complex aspects: (1) the relatively low population clinical trial study cohort, which poses risks associated with data distinguishability and the masking ability of the applied anonymisation tools, (2) the project's ability to obtain informed consent from the study cohort given the complexity of the technologies, (3) the project's commitment to an open research data strategy and the additional privacy risk mitigations required to protect the multi-modal study data, and (4) the ability of the project to adequately explain the outputs of the algorithmic components to a broad range of stakeholders. The paper will discuss how the complexities have caused tension which are reflective of wider tensions in the health domain. A project level solution includes collaboration with a melanoma patient network, as an avenue for fair and representative qualification of risks and benefits with the patient stakeholder group. However, it is unclear how scalable this process is given the relentless pursuit of innovation within the health domain, accentuated by the continued proliferation of artificial intelligence, open data strategies, and the integration of multi-modal data sets inclusive of genomics.

AbstractObjective Human error estimating food intake is a major source of bias in nutrition research. Artificial intelligence (AI) methods may reduce bias, but the overall accuracy of AI estimates is unknown. This study was a systematic review of peer-reviewed journal articles comparing fully automated AI-based (e.g. deep learning) methods of dietary assessment from digital images to human assessors and ground truth (e.g. doubly labelled water).Materials and Methods Literature was searched through May 2023 in four electronic databases plus reference mining. Eligible articles reported AI estimated volume, energy, or nutrients. Independent investigators screened articles and extracted data. Potential sources of bias were documented in absence of an applicable risk of bias assessment tool.Results Database and hand searches identified 14,059 unique publications; fifty-two papers (studies) published from 2010 to 2023 were retained. For food detection and classification, 79% of papers used a convolutional neural network. Common ground truth sources were calculation using nutrient tables (51%) and weighed food (27%). Included papers varied widely in food image databases and results reported, so meta-analytic synthesis could not be conducted. Relative errors were extracted or calculated from 69% of papers. Average overall relative errors (AI vs. ground truth) ranged from 0.10% to 38.3% for calories and 0.09% to 33% for volume, suggesting similar performance. Ranges of relative error were lower when images had single/simple foods.Conclusions Relative errors for volume and calorie estimations suggest that AI methods align with – and have the potential to exceed – accuracy of human estimations. However, variability in food image databases and results reported prevented meta-analytic synthesis. The field can advance by testing AI architectures on a limited number of large-scale food image and nutrition databases that the field determines to be adequate for training and testing and by reporting accuracy of at least absolute and relative error for volume or calorie estimations.

This survey paper attempts to cover a broad range of topics related to computational biomedicine. The field has been attracting great attention due to a number of benefits it can provide the society with. New technological and theoretical advances have made it possible to progress considerably. Traditionally, problems emerging in this field are challenging from many perspectives. In this paper, we considered the influence of big data on the field, problems associated with massive datasets in biomedicine and ways to address these problems. Breast cancer is considered the most commonly diagnosed cancer among women worldwide. Several studies have shown that mammography screening could significantly decrease breast cancer mortality. Despite other screening modalities, such as MRI and ultrasound (US), mammography plays a vital role in detecting cancer and following up on it, due to its qualities and properties. The aim of this literature review is to look at recent studies that use AI with different medical imaging mammograms, MRI, and US, in detecting breast lesions. A literature search was carried out using Google Scholar, Semantic Scholar, medRxiv, and PubMed databases for a period of the last four years. The search terms were "breast lesion," "breast imaging," and "breast cancer" combined with "machine learning," "deep learning," and "artificial intelligence." Among these studies, only the medical imaging related to breast lesions with AI was selected. A total of 25 articles were extracted from the following databases: 4 Google Scholar, 3 Semantic Scholar, 4 medRxiv, and 14 PubMed. Only papers related to breast lesions with medical imaging modalities were extracted, and all duplications were removed. In this study, the papers were reviewed by medical imaging professionals. This literature review summarizes the most recent articles on utilizing AI in detecting breast lesions for different imaging modalities: mammogram, ultrasound, and MRI. Reviewed studies showed that AI performance in detecting lesions was significant, associated with high accuracy, sensitivity, and specificity for these modalities.

Background Using reports of randomised trials of smoking cessation interventions as a test case, this study aimed to develop and evaluate machine learning (ML) algorithms for extracting information from study reports and predicting outcomes as part of the Human Behaviour-Change Project. It is the first of two linked papers, with the second paper reporting on further development of a prediction system. Methods Researchers manually annotated 70 items of information (‘entities’) in 512 reports of randomised trials of smoking cessation interventions covering intervention content and delivery, population, setting, outcome and study methodology using the Behaviour Change Intervention Ontology. These entities were used to train ML algorithms to extract the information automatically. The information extraction ML algorithm involved a named-entity recognition system using the ‘FLAIR’ framework. The manually annotated intervention, population, setting and study entities were used to develop a deep-learning algorithm using multiple layers of long-short-term-memory (LSTM) components to predict smoking cessation outcomes. Results The F1 evaluation score, derived from the false positive and false negative rates (range 0-1), for the information extraction algorithm averaged 0.42 across different types of entity (SD=0.22, range 0.05-0.88) compared with an average human annotator’s score of 0.75 (SD=0.15, range 0.38-1.00). The algorithm for assigning entities to study arms (e.g., intervention or control) was not successful. This initial ML outcome prediction algorithm did not outperform prediction based just on the mean outcome value or a linear regression model. Conclusions While some success was achieved in using ML to extract information from reports of randomised trials of smoking cessation interventions, we identified major challenges that could be addressed by greater standardisation in the way that studies are reported. Outcome prediction from smoking cessation studies may benefit from development of novel algorithms, e.g., using ontological information to inform ML (as reported in the linked paper (1)).

Abstract Background There is growing enthusiasm for the application of machine learning (ML) and artificial intelligence (AI) techniques to clinical research and practice. However, instructions on how to develop robust high-quality ML and AI in medicine are scarce. In this paper, we provide a practical example of techniques that facilitate the development of high-quality ML systems including data pre-processing, hyperparameter tuning, and model comparison using open-source software and data. Methods We used open-source software and a publicly available dataset to train and validate multiple ML models to classify breast masses into benign or malignant using mammography image features and patient age. We compared algorithm predictions to the ground truth of histopathologic evaluation. We provide step-by-step instructions with accompanying code lines. Findings Performance of the five algorithms at classifying breast masses as benign or malignant based on mammography image features and patient age was statistically equivalent (P > 0.05). Area under the receiver operating characteristics curve (AUROC) for the logistic regression with elastic net penalty was 0.89 (95% CI 0.85 – 0.94), for the Extreme Gradient Boosting Tree 0.88 (95% CI 0.83 – 0.93), for the Multivariate Adaptive Regression Spline algorithm 0.88 (95% CI 0.83 – 0.93), for the Support Vector Machine 0.89 (95% CI 0.84 – 0.93), and for the neural network 0.89 (95% CI 0.84 – 0.93). Interpretation Our paper allows clinicians and medical researchers who are interested in using ML algorithms to understand and recreate the elements of a comprehensive ML analysis. Following our instructions may help to improve model generalizability and reproducibility in medical ML studies.

Personalized medicine transforms healthcare by adapting interventions to individuals’ unique genetic, molecular, and clinical profiles. To maximize diagnostic and/or therapeutic efficacy, personalized medicine requires advanced imaging devices and sensors for accurate assessment and monitoring of individual patient conditions or responses to therapeutics. In the field of biomedical optics, short-wave infrared (SWIR) techniques offer an array of capabilities that hold promise to significantly enhance diagnostics, imaging, and therapeutic interventions. SWIR techniques provide in vivo information, which was previously inaccessible, by making use of its capacity to penetrate biological tissues with reduced attenuation and enable researchers and clinicians to delve deeper into anatomical structures, physiological processes, and molecular interactions. Combining SWIR techniques with machine learning (ML), which is a powerful tool for analyzing information, holds the potential to provide unprecedented accuracy for disease detection, precision in treatment guidance, and correlations of complex biological features, opening the way for the data-driven personalized medicine field. Despite numerous biomedical demonstrations that utilize cutting-edge SWIR techniques, the clinical potential of this approach has remained significantly underexplored. This paper demonstrates how the synergy between SWIR imaging and ML is reshaping biomedical research and clinical applications. As the paper showcases the growing significance of SWIR imaging techniques that are empowered by ML, it calls for continued collaboration between researchers, engineers, and clinicians to boost the translation of this technology into clinics, ultimately bridging the gap between cutting-edge technology and its potential for personalized medicine.

Given the increasingly important role that the use of artificial intelligence algorithms is taking on in the medical field today (especially in oncology), the purpose of this systematic review is to analyze the main reports on such algorithms applied for the prognostic evaluation of patients with head and neck malignancies. The objective of this paper is to examine the currently available literature in the field of artificial intelligence applied to head and neck oncology, particularly in the prognostic evaluation of the patient with this kind of tumor, by means of a systematic review. The paper exposes an overview of the applications of artificial intelligence in deriving prognostic information related to the prediction of survival and recurrence and how these data may have a potential impact on the choice of therapeutic strategy, making it increasingly personalized. This systematic review was written following the PRISMA 2020 guidelines.

Abstract Background Over the last decade, computational sciences have contributed immensely to characterization of phenotypes of airway diseases, but it is difficult to compare derived phenotypes across studies, perhaps as a result of the different decisions that fed into these phenotyping exercises. We aim to perform a systematic review of studies using computational approaches to phenotype obstructive airway diseases in children and adults. Methods and analysis We will search PubMed, Embase, Scopus, Web of Science, and Google Scholar for papers published between 2010 and 2020. Conferences proceedings, reference list of included papers, and experts will form additional sources of literature. We will include observational epidemiological studies that used a computational approach to derive phenotypes of chronic airway diseases, whether in a general population or in a clinical setting. Two reviewers will independently screen the retrieved studies for eligibility, extract relevant data, and perform quality appraisal of included studies. A third reviewer will arbitrate any disagreements in these processes. Quality appraisal of the studies will be undertaken using the Effective Public Health Practice Project quality assessment tool. We will use summary tables to describe the included studies. We will narratively synthesize the generated evidence, providing critical assessment of the populations, variables, and computational approaches used in deriving the phenotypes across studies Conclusion As progress continues to be made in the area of computational phenotyping of chronic obstructive airway diseases, this systematic review, the first on this topic, will provide the state of the art on the field and highlight important perspectives for future works. Today, artificial intelligence (AI) and machine learning (ML) have dramatically advanced in various industries, especially medicine. AI describes computational programs that mimic and simulate human intelligence, for example, a person's behavior in solving problems or his ability for learning. Furthermore, ML is a subset of artificial intelligence. It extracts patterns from raw data automatically. The purpose of this paper is to help researchers gain a proper understanding of machine learning and its applications in healthcare. In this paper, we first present a classification of machine learning-based schemes in healthcare. According to our proposed taxonomy, machine learning-based schemes in healthcare are categorized based on data pre-processing methods (data cleaning methods, data reduction methods), learning methods (unsupervised learning, supervised learning, semi-supervised learning, and reinforcement learning), evaluation methods (simulation-based evaluation and practical implementation-based evaluation in real environment) and applications (diagnosis, treatment). According to our proposed classification, we review some studies presented in machine learning applications for healthcare. We believe that this review paper helps researchers to familiarize themselves with the newest research on ML applications in medicine, recognize their challenges and limitations in this area, and identify future research directions. Topic modeling—a text‐mining technique often used to uncover thematic structures in large collections of texts—has been increasingly frequently used in the context of the analysis of scholarly output. In this study, we construct a corpus of 19,488 texts published since 1971 in seven leading journals in the field of bioethics and philosophy of medicine, and we use a machine learning algorithm to identify almost 100 topics representing distinct themes of interest in the field. On the basis of intertopic correlations, we group the content‐based topics into eight clusters, thus providing a novel, fine‐grained intellectual map of bioethics and philosophy of medicine. Moreover, we conduct a number of diachronic analyses, examining how the "prominence" of different topics has changed across time. In this way, we are able to observe the distinct patterns in which bioethics and philosophy of medicine have evolved and changed their focus over the past half a century. [ABSTRACT FROM AUTHOR]

Artificial intelligence (AI) is becoming increasingly important, especially in the medical field. While AI has been used in medicine for some time, its growth in the last decade is remarkable. Specifically, machine learning (ML) and deep learning (DL) techniques in medicine have been increasingly adopted due to the growing abundance of health-related data, the improved suitability of such techniques for managing large datasets, and more computational power. ML and DL methodologies are fostering the development of new “intelligent” tools and expert systems to process data, to automatize human–machine interactions, and to deliver advanced predictive systems that are changing every aspect of the scientific research, industry, and society. The Italian scientific community was instrumental in advancing this research area. This article aims to conduct a comprehensive investigation of the ML and DL methodologies and applications used in medicine by the Italian research community in the last five years. To this end, we selected all the papers published in the last five years with at least one of the authors affiliated to an Italian institution that in the title, in the abstract, or in the keywords present the terms “machine learning” or “deep learning” and reference a medical area. We focused our research on journal papers under the hypothesis that Italian researchers prefer to present novel but well-established research in scientific journals. We then analyzed the selected papers considering different dimensions, including the medical topic, the type of data, the pre-processing methods, the learning methods, and the evaluation methods. As a final outcome, a comprehensive overview of the Italian research landscape is given, highlighting how the community has increasingly worked on a very heterogeneous range of medical problems.

According to the Pan American Health Organization, cardiovascular disease is the leading cause of death worldwide, claiming an estimated 17.9 million lives each year. This paper presents a systematic review to highlight the use of IoT, IoMT, and machine learning to detect, predict, or monitor cardiovascular disease. We had a final sample of 164 high-impact journal papers, focusing on two categories: cardiovascular disease detection using IoT/IoMT technologies and cardiovascular disease using machine learning techniques. For the first category, we found 82 proposals, while for the second, we found 85 proposals. The research highlights list of IoT/IoMT technologies, machine learning techniques, datasets, and the most discussed cardiovascular diseases. Neural networks have been popularly used, achieving an accuracy of over 90%, followed by random forest, XGBoost, k-NN, and SVM. Based on the results, we conclude that IoT/IoMT technologies can predict cardiovascular diseases in real time, ensemble techniques obtained one of the best performances in the accuracy metric, and hypertension and arrhythmia were the most discussed diseases. Finally, we identified the lack of public data as one of the main obstacles for machine learning approaches for cardiovascular disease prediction.

The practice of medicine is changing with the development of new Artificial Intelligence (AI) methods of machine learning. Coupled with rapid improvements in computer processing, these AI-based systems are already improving the accuracy and efficiency of diagnosis and treatment across various specializations. The increasing focus of AI in radiology has led to some experts suggesting that someday AI may even replace radiologists. These suggestions raise the question of whether AI-based systems will eventually replace physicians in some specializations or will augment the role of physicians without actually replacing them. To assess the impact on physicians this research seeks to better understand this technology and how it is transforming medicine. To that end this paper researches the role of AI-based systems in performing medical work in specializations including radiology, pathology, ophthalmology, and cardiology. Increasing emphasis on the use of real-world evidence (RWE) to support clinical policy and regulatory decision-making has led to a proliferation of guidance, advice, and frameworks from regulatory agencies, academia, professional societies, and industry. A broad spectrum of studies use real-world data (RWD) to produce RWE, ranging from randomized trials with outcomes assessed using RWD to fully observational studies. Yet, many proposals for generating RWE lack sufficient detail, and many analyses of RWD suffer from implausible assumptions, other methodological flaws, or inappropriate interpretations. The Causal Roadmap is an explicit, itemized, iterative process that guides investigators to prespecify study design and analysis plans; it addresses a wide range of guidance within a single framework. By supporting the transparent evaluation of causal assumptions and facilitating objective comparisons of design and analysis choices based on prespecified criteria, the Roadmap can help investigators to evaluate the quality of evidence that a given study is likely to produce, specify a study to generate high-quality RWE, and communicate effectively with regulatory agencies and other stakeholders. This paper aims to disseminate and extend the Causal Roadmap framework for use by clinical and translational researchers; three companion papers demonstrate applications of the Causal Roadmap for specific use cases.

Abstract Background and Aim Schizophrenia and bipolar disorder (BD) are critical and high‐risk inherited mental disorders with debilitating symptoms. Worldwide, 3% of the population suffers from these disorders. The mortality rate of these patients is higher compared to other people. Current procedures cannot effectively diagnose these disorders because it takes an average of 10 years from the onset of the first symptoms to the definitive diagnosis of the disease. Machine learning (ML) techniques are used to meet this need. This study aimed to summarize information on the use of ML techniques for predicting schizophrenia and BD to help early and timely diagnosis of the disease. Methods A systematic literature search included articles published until January 19, 2020 in 3 databases. Two reviewers independently assessed original papers to determine eligibility for inclusion in this review. PRISMA guidelines were followed to conduct the study, and the Prediction Model Risk of Bias Assessment Tool (PROBAST) to assess included papers. Results In this review, 1243 papers were retrieved through database searches, of which 15 papers were included based on full‐text assessment. ML techniques were used to predict schizophrenia and BDs. The main algorithms applied were support vector machine (SVM) (10 studies), random forests (RF) (5 studies), and gradient boosting (GB) (3 studies). Input and output characteristics were very diverse and have been kept to enable future research. RFs algorithms demonstrated significantly higher accuracy and sensitivity than SVM and GB. GB demonstrated significantly higher specificity than SVM and RF. We found no significant difference between RF and SVM in terms of specificity. Conclusion ML can precisely predict results and assist in making clinical decisions‐concerning schizophrenia and BD. RF often performed better than other algorithms in supervised learning tasks. This study identified gaps in the literature and opportunities for future psychological ML research.

Background: During the process of the treatment of COVID-19 hospitalized patients, physicians still face a lot of unknowns and problems. Despite the application of the treatment protocol, it is still unknown why the medical status of a certain number of patients worsens and ends with death. Many factors were analyzed for the prediction of the clinical outcome of the patients using different methods. The aim of this paper was to develop a prediction model based on initial laboratory blood test results, accompanying comorbidities, and demographics to help physicians to better understand the medical state of patients with respect to possible clinical outcomes using neural networks, hypothesis testing, and confidence intervals. Methods: The research had retrospective-prospective, descriptive, and analytical character. As inputs for this research, 12 components of laboratory blood test results, six accompanying comorbidities, and demographics (age and gender) data were collected from hospital information system in Sarajevo for each patient from a sample of 634 hospitalized patients. Clinical outcome of the hospitalized patients, survival or death, was recorded 30 days after admission to the hospital. The prediction model was designed using a neural network. In addition, formal hypothesis tests were performed to investigate whether there were significant differences in laboratory blood test results and age between patients who died and those who survived, including the construction of 95% confidence intervals. Results: In this paper, 11 neural networks were developed with different threshold values to determine the optimal neural network with the highest prediction performance. The performances of the neural networks were evaluated by accuracy, precision, sensitivity, and specificity. Optimal neural network model evaluation metrics are: accuracy = 87.78%, precision = 96.37%, sensitivity = 90.07%, and specificity = 62.16%. Significantly higher values (P < 0.05) of blood laboratory result components and age were detected in patients who died. Conclusion: Optimal neural network model, results of hypothesis tests, and confidence intervals could help to predict, analyze, and better understand the medical state of COVID-19 hospitalized patients and thus reduce the mortality rate.

The paper also discusses about the implementation opportunities various AI technologies like Machine Learning, Deep Learning, Reinforcement Learning, Natural Language Processing, Computer Vision provide in different fields of healthcare and clinical workflows and how Artificial Intelligence systems will boost the capabilities of healthcare professionals in restoring the human touch in patient-physician encounters. A physician’s intuition and judgement will always remain better suited since each case, each health condition, and each person is unique in its own way, but AI methods can help enhance the accuracy of diagnosis, assist physicians in making improved and precise clinical decisions.

We present in this paper some of the existing machine learning and sound processing techniques involved in the medical process and show how they can be applied in the context of a vocal disease detection task. The machine learning techniques are the usual, ranging from decision trees to neural networks, and when it comes to the sound processing approaches some scientists probably are familiar with them from the speech processing or music analysis areas. However, the techniques are adapted to the particularities of the medical field -- data collection, verification, etc. Through the concrete example of the vocal disease detection task we worked on, we made some interesting observations, both on the classification ability of the model and on the influence of the data.

Abstract Freak or rogue waves are a danger to ships, offshore infrastructure, and other maritime equipment. Reliable rogue wave forecasts could mitigate this risk for operations at sea. While the occurrence of oceanic rogue waves at sea is generally acknowledged, reliable rogue wave forecasts are unavailable. In this paper, the authors seek to overcome this shortcoming by demonstrating how rogue waves can be predicted from field measurements. An extensive buoy data set consisting of billions of waves is utilized to parameterize neural networks. This network is trained to distinguish waves prior to an extreme wave from waves which are not followed by an extreme wave. With this approach, three out of four rogue waves are correctly predicted 1 min ahead of time. When the advance warning time is extended to 5 min, it is found that the ratio of accurate predictions is reduced to seven out of ten rogue waves. Another strength of the trained neural networks is their capabilities to extrapolate. This aspect is verified by obtaining forecasts for a buoy location that is not included in the networks’ training set. Furthermore, the performance of the trained neural network carries over to realistic scenarios where rogue waves are extremely rare.

Abstract Objectives In a time of exponential growth of new evidence supporting clinical decision-making, combined with a labor-intensive process of selecting this evidence, methods are needed to speed up current processes to keep medical guidelines up-to-date. This study evaluated the performance and feasibility of active learning to support the selection of relevant publications within medical guideline development and to study the role of noisy labels. Design We used a mixed-methods design. Two independent clinicians’ manual process of literature selection was evaluated for 14 searches. This was followed by a series of simulations investigating the performance of random reading versus using screening prioritization based on active learning. We identified hard-to-find papers and checked the labels in a reflective dialogue. Main outcome measures Inter-rater reliability was assessed using Cohen’s Kappa (ĸ). To evaluate the performance of active learning, we used the Work Saved over Sampling at 95% recall (WSS@95) and percentage Relevant Records Found at reading only 10% of the total number of records (RRF@10). We used the average time to discovery (ATD) to detect records with potentially noisy labels. Finally, the accuracy of labeling was discussed in a reflective dialogue with guideline developers. Results Mean ĸ for manual title-abstract selection by clinicians was 0.50 and varied between − 0.01 and 0.87 based on 5.021 abstracts. WSS@95 ranged from 50.15% (SD = 17.7) based on selection by clinicians to 69.24% (SD = 11.5) based on the selection by research methodologist up to 75.76% (SD = 12.2) based on the final full-text inclusion. A similar pattern was seen for RRF@10, ranging from 48.31% (SD = 23.3) to 62.8% (SD = 21.20) and 65.58% (SD = 23.25). The performance of active learning deteriorates with higher noise. Compared with the final full-text selection, the selection made by clinicians or research methodologists deteriorated WSS@95 by 25.61% and 6.25%, respectively. Conclusion While active machine learning tools can accelerate the process of literature screening within guideline development, they can only work as well as the input given by human raters. Noisy labels make noisy machine learning.

Abstract Background The demand for high-quality systematic literature reviews (SRs) for evidence-based medical decision-making is growing. SRs are costly and require the scarce resource of highly skilled reviewers. Automation technology has been proposed to save workload and expedite the SR workflow. We aimed to provide a comprehensive overview of SR automation studies indexed in PubMed, focusing on the applicability of these technologies in real world practice. Methods In November 2022, we extracted, combined, and ran an integrated PubMed search for SRs on SR automation. Full-text English peer-reviewed articles were included if they reported studies on SR automation methods (SSAM), or automated SRs (ASR). Bibliographic analyses and knowledge-discovery studies were excluded. Record screening was performed by single reviewers, and the selection of full text papers was performed in duplicate. We summarized the publication details, automated review stages, automation goals, applied tools, data sources, methods, results, and Google Scholar citations of SR automation studies. Results From 5321 records screened by title and abstract, we included 123 full text articles, of which 108 were SSAM and 15 ASR. Automation was applied for search (19/123, 15.4%), record screening (89/123, 72.4%), full-text selection (6/123, 4.9%), data extraction (13/123, 10.6%), risk of bias assessment (9/123, 7.3%), evidence synthesis (2/123, 1.6%), assessment of evidence quality (2/123, 1.6%), and reporting (2/123, 1.6%). Multiple SR stages were automated by 11 (8.9%) studies. The performance of automated record screening varied largely across SR topics. In published ASR, we found examples of automated search, record screening, full-text selection, and data extraction. In some ASRs, automation fully complemented manual reviews to increase sensitivity rather than to save workload. Reporting of automation details was often incomplete in ASRs. Conclusions Automation techniques are being developed for all SR stages, but with limited real-world adoption. Most SR automation tools target single SR stages, with modest time savings for the entire SR process and varying sensitivity and specificity across studies. Therefore, the real-world benefits of SR automation remain uncertain. Standardizing the terminology, reporting, and metrics of study reports could enhance the adoption of SR automation techniques in real-world practice.

Abstract Accurately estimating Battery State of Charge (SOC) is essential for safe and optimal electric vehicle operation. This paper presents a comparative assessment of multiple machine learning regression algorithms including Support Vector Machine, Neural Network, Ensemble Method, and Gaussian Process Regression for modelling the complex relationship between real-time driving data and battery SOC. The models are trained and tested on extensive field data collected from diverse drivers across varying conditions. Statistical performance metrics evaluate the SOC prediction accuracy on the test set. Gaussian process regression demonstrates superior precision surpassing the other techniques with the lowest errors. Case studies analyse model competence in mimicking actual battery charge/discharge characteristics responding to changing drivers, temperatures, and drive cycles. The research provides a reliable data-driven framework leveraging advanced analytics for precise real-time SOC monitoring to enhance battery management.

Abstract Energy consumption of constructed educational facilities significantly impacts economic, social and environment sustainable development. It contributes to approximately 37% of the carbon dioxide emissions associated with energy use and procedures. This paper aims to introduce a study that investigates several artificial intelligence-based models to predict the energy consumption of the most important educational buildings; schools. These models include decision trees, K-nearest neighbors, gradient boosting, and long-term memory networks. The research also investigates the relationship between the input parameters and the yearly energy usage of educational buildings. It has been discovered that the school sizes and AC capacities are the most impact variable associated with higher energy consumption. While 'Type of School' is less direct or weaker correlation with 'Annual Consumption'. The four developed models were evaluated and compared in training and testing stages. The Decision Tree model demonstrates strong performance on the training data with an average prediction error of about 3.58%. The K-Nearest Neighbors model has significantly higher errors, with RMSE on training data as high as 38,429.4, which may be indicative of overfitting. In contrast, Gradient Boosting can almost perfectly predict the variations within the training dataset. The performance metrics suggest that some models manage this variability better than others, with Gradient Boosting and LSTM standing out in terms of their ability to handle diverse data ranges, from the minimum consumption of approximately 99,274.95 to the maximum of 683,191.8. This research underscores the importance of sustainable educational buildings not only as physical learning spaces but also as dynamic environments that contribute to informal educational processes. Sustainable buildings serve as real-world examples of environmental stewardship, teaching students about energy efficiency and sustainability through their design and operation. By incorporating advanced AI-driven tools to optimize energy consumption, educational facilities can become interactive learning hubs that encourage students to engage with concepts of sustainability in their everyday surroundings.

Abstract Early detection of the adenocarcinoma cancer in colon tissue by means of explainable deep learning, by classifying histological images and providing visual explainability on model prediction. Considering that in recent years, deep learning techniques have emerged as powerful techniques in medical image analysis, offering unprecedented accuracy and efficiency, in this paper we propose a method to automatically detect the presence of cancerous cells in colon tissue images. Various deep learning architectures are considered, with the aim of considering the best one in terms of quantitative and qualitative results. As a matter of fact, we consider qualitative results by taking into account the so-called prediction explainability, by providing a way to highlight on the tissue images the areas that from the model point of view are related to the presence of colon cancer. The experimental analysis, performed on 10,000 colon issue images, showed the effectiveness of the proposed method by obtaining an accuracy equal to 0.99. The experimental analysis shows that the proposed method can be successfully exploited for colon cancer detection and localisation from tissue images.

Abstract Renewable microgrids enhance security, reliability, and power quality in power systems by integrating solar and wind sources, reducing greenhouse gas emissions. This paper proposes a machine learning approach, leveraging Gaussian Process (GP) and Krill Herd Algorithm (KHA), for energy management in renewable microgrids with a reconfigurable structure based on remote switching of tie and sectionalizing. The method utilizes Gaussian Process (GP) for modeling hybrid electric vehicle (HEV) charging demand. To counteract HEV charging effects, two scenarios are explored: coordinated and intelligent charging. A novel optimization method inspired by the Krill Herd Algorithm (KHA) is introduced for the complex problem, along with a self-adaptive modification to tailor solutions to specific situations. Simulation on an IEEE microgrid demonstrates efficiency in both scenarios. The predictive model yields a remarkably low Mean Absolute Percentage Error (MAPE) of 1.02381 for total HEV charging demand. Results also reveal a reduction in microgrid operation cost in the intelligent charging scenario compared to coordinated charging.

Abstract Leptospirosis is a global disease that impacts people worldwide, particularly in humid and tropical regions, and is associated with significant socio-economic deficiencies. Its symptoms are often confused with other syndromes, which can compromise clinical diagnosis and the failure to carry out specific laboratory tests. In this respect, this paper presents a study of three algorithms (Decision Tree, Random Forest and Adaboost) for predicting the outcome (cure or death) of individuals with leptospirosis. Using the records contained in the government National System of Aggressions and Notification (SINAN, in portuguese) from 2007 to 2017, for the state of Pará, Brazil, where the temporal attributes of health care, symptoms (headache, vomiting, jaundice, calf pain) and clinical evolution (renal failure and respiratory changes) were used. In the performance evaluation of the selected models, it was observed that the Random Forest exhibited an accuracy of 90.81% for the training dataset, considering the attributes of experiment 8, and the Decision Tree presented an accuracy of 74.29 for the validation database. So, this result considers the best attributes pointed out by experiment 10: time first symptoms medical attention, time first symptoms ELISA sample collection, medical attention hospital admission time, headache, calf pain, vomiting, jaundice, renal insufficiency, and respiratory alterations. The contribution of this article is the confirmation that artificial intelligence, using the Decision Tree model algorithm, depicting the best choice as the final model to be used in future data for the prediction of human leptospirosis cases, helping in the diagnosis and course of the disease, aiming to avoid the evolution to death.

Abstract Identifying lithologies in meteorite impact craters is an important task to unlock processes that have shaped the evolution of planetary bodies. Traditional methods for lithology identification rely on time-consuming manual analysis, which is costly and limits the efficiency of rapid decision-making. This paper utilizes different machine learning algorithms namely Random Forest, Decision Tree, K Nearest Neighbors, and Logistic Regression with Grid Search to classify rock lithologies using data from the Bosumtwi impact crater in Ghana. A repeated stratified k-fold cross-validation method is applied to Grid Search to select the best combination of hyperparameters. The findings demonstrate that the Random Forest algorithm achieves the most promising results in classifying lithologies in the meteorite impact crater with an accuracy score of 86.89%, a recall score of 84.88%, a precision score of 87.21%, and an F1 score of 85.48%. The findings also suggest that more high-quality data has the potential to further increase the accuracy scores of the machine learning algorithm. In conclusion, this study demonstrates the significant potential of machine learning techniques to revolutionize lithology identification in meteorite impact craters, thus paving the way for their influential role in future space exploration endeavors.
