The septum magnet bends the injection beam before the kicker magnet. The septum magnet is located near the storage ring in the Taiwan Photon Source (TPS). As the space from the magnet wall to the vacuum chamber of the ring is less than 3 mm, the leakage field of the septum magnet thus influences the stability of the electron beam in the storage ring. The main field and the leakage field of a septum of the direct-drive type are compared to one of the eddy-current type. The effects of the vacuum chamber and \mu-metal are simulated with full-sine and half-sine current pulses with the OPERA-2D TR analysis module; measurement results are presented in this paper. Objectives: To estimate the cost and determinants of morbidity from work related disabilities.Methods: 114 people who lived in metropolitan Taipei and who had received disability compensation from the Labor Insurance Bureau from March to June, 1991 were randomly selected. There were 77 workers interviewed through a questionnaire that inquired about possible loss of productivity including the duration of morbidity, the ability to return to work, and any change in monthly income upon returning to work.Results: The mean (SD) duration of a stay in hospital was 29 (39) days, median: 15 days. The average duration between discharge from the hospital and returning to work was 111 (146) days, median: 45 days. The main determinants of the duration of the stay in hospital were the number of stays in hospital and the severity of the injury. A multivariate linear analysis showed that old age and the severity of injury determine the durations of morbidity. An ordinal logistic regression analysis showed that the severity of injury, size of the factory, and age determined the magnitude of future productivity loss. Based on these models, it was estimated that the total duration of morbidity (in hospital and at home) due to occupational disability was 660,000 person-days each year. When the percentage of the decrease in income because of permanent disability was converted into a loss of work days, the annual morbidity costs were about 19,000-26,000 person-years between 1985-1990.Conclusion: The morbidity cost was about five times as high as the lump sum payment that a worker usually received for disability compensation. We conclude that morbidity cost should be evaluated carefully in the future for the establishment of accurate and fair disability compensation payments. [ABSTRACT FROM AUTHOR]

Background: Incisional recurrence after thoracoscopic surgery has been reported infrequently. In recent years, several reports of port-site recurrence after laparoscopic oncologic procedures have been published. This study evaluates the incidence of incisional recurrence among patients with intrathoracic malignancy after diagnostic and therapeutic thoracoscopy.Methods: The medical records of all patients with intrathoracic malignancies who underwent thoracoscopic procedures between 1992 and 1998 at Chang Gung Memorial Hospital Linkou Medical Center were reviewed. Information includes preoperative tumor status, thoracoscopic findings, primary tumor location, tumor pathology, procedures performed, and perioperative complications were recorded.Results: A total of 1,069 patients with known intrathoracic malignancies underwent thoracoscopy. The mean follow-up time was 17.1 months (range, 1-68 months). Two recurrences at the incision were identified (0.19%). Both patients with incision-site recurrence had advanced intrathoracic disease at the time of thoracoscopy. The one patient had a malignant pleural effusion (T4), and the other had diffuse pleural metastasis.Conclusion: The incidence of incisional recurrence after thoracoscopic oncologic surgery is very low. When recurrence occurs at the incision, it is associated most commonly with advanced intrathoracic disease. Additional patients and a longer follow-up evaluation are required, however, to confirm this observation. [ABSTRACT FROM AUTHOR]

Deleterious health sequelae caused by licit and illicit substance use is a serious problem in our society. Adolescent students and particularly those who are prone to substance use are of special concern in the prevention of drug abuse. The purpose of this study was to investigate the prevalence of alcohol consumption, tobacco smoking and illicit drug use and to identify the risk factors related to these habits among adolescent students. A total of 1358 adolescent students aged 16–18 y old were recruited into this study via stratified random sampling from a vocational school in Kaohsiung city. Students were asked to complete a structured questionnaire anonymously and a 96.7% response rate was achieved. Prevalence of substances use was estimated as follows: alcohol drinking, 70.7% (boys 75.1%, girls 51.4%); tobacco smoking, 56% (boys 61.8%, girls 30.2%); illicit drug use 6.4% (boys 6.6%, girls 5.6%). Significant risk factors that emerged as common correlates with substances use were behaviour problems, non-negative attitude toward parent’s substance use, and peer influence. A dose–response relationship was found between the prevalence of drinking, smoking, illicit drug use and the number of risk factors adolescents were exposed to. Prevention of adolescent substance abuse should be attempted and risk factors should be reduced. An educational approach is essential not only to gain/impart knowledge of substance abuse, but also to develop an effective program for health and social development. [ABSTRACT FROM AUTHOR]

Background: The prognostic impact of perineural invasion (PNI) in patients with esophageal cancer who receive neoadjuvant chemoradiotherapy (nCRT) remains unclear., Methods: A thorough pathological review of PNI was performed on post-nCRT esophagectomy specimens obtained from non-ypT0 patients with esophageal squamous cell carcinoma (ESCC). When PNI was identified, it was classified according to the presence or absence of penetration through the nerve sheath (i.e., PNI surrounding the nerve sheath [PNI-SS] versus PNI penetrating through the nerve sheath [PNI-TS]). The impact of PNI on overall survival (OS) was assessed in combination with clinical and pathological risk factors., Results: A total of 177 eligible patients were identified between 1998 and 2008. PNI was identified in 43.5% (77/177) of participants. Of them, 33 and 44 had PNI-SS and PNI-TS, respectively. The 5-year OS rate of patients with PNI-TS was significantly lower (6.7%) than that observed in those without PNI (30.6%, P < 0.001). However, the 5-year OS observed in the latter group did not differ significantly from that of patients with PNI-SS (26%, P = 0.68). Multivariate analysis identified PNI-TS (hazard ratio [HR] = 1.965, P = 0.02), LVI (HR = 1.514, P = 0.048), and ypN2 stage (HR = 2.39, P = 0.007) as independent adverse prognostic factors for OS., Conclusions: The presence of PNI-TS after nCRT is associated with poor survival. Objective: To identify the risk factors for catheter migration and demonstrate possible mechanisms of this migration., Design: Retrospective study., Setting: Chang Gung Memorial Hospital, a tertiary medical centre in Taiwan., Patients: Patients who underwent implantation of intravenous ports via the superior vena cava (SVC)., Interventions: Procedures involving catheter placement and re-intervention for catheter migration., Main Outcome Measures: The anatomic location of the catheter tip was confirmed by plain chest X-rays (postero-anterior view). From these plain radiographs, the distance (in cm) between the carina and catheter tip and the angle (in degrees) between the locking nut and catheter were measured., Methods: A total of 1542 procedures related to intravenous port implantation were retrospectively reviewed but only procedures involving implantation via the SVC were included in the analysis. The study group was composed of 31 interventions because of catheter migration, while the control group consisted of 1475 implantation and re-intervention procedures except those involving catheter migrations., Results: Shallow catheter-tip location (p < 0.0001) and the presence of lung cancer (p = 0.006) were risk factors for catheter migration., Conclusions: Shallow catheter-tip location and the presence of lung cancer are risk factors for catheter migration. To introduce our preliminary experience of treating locally and regionally recurrent Head and Neck cancer patients at Tsing Hua Open-Pool Reactor in Taiwan, four patients (M/F=3/1, median age 68 Y/O) were enrolled. BNCT with BPA (400 mg/kg) injected in 2 phases and prescription dose of 12-35 Gy (Eq.)/fraction for 2 fractions at 30 day interval can be given with sustained blood boron concentration and tolerable early toxicities for recurrent H & N cancer., (Copyright © 2011 Elsevier Ltd. In this article, we will consider several core configurations and run the core calculation with MCNP to obtain the neutrons distribution at THOR. The thermal neutron flux inside the vertical tubes (VT-B-VT-E) and the fast neutron flux in the first row facing to the boron neutron capture therapy (BNCT) facility (I3-I5) were tallied for indication. Based on these simulation results, the fuel elements were rearranged during the annual repair period in 2007. The epithermal neutron flux at the center of BNCT beam exit in air was measured again, and the results showed that the beam intensity increased by 50%. Comparing the neutron intensities both in reactor core and at the BNCT beam exit for several core configurations, the results show that the BNCT beam intensity can be increased without decreasing the neutron intensity in core.

Background: We evaluated the efficacy and safety of the extra-long Montgomery T tube for the management of major airway obstruction in tertiary care patients in Taiwan., Method: Eleven patients with major airway stenosis treated with an extra-long Montgomery T tube between April 2004 and December 2006 were retrospectively reviewed. Five patients had tracheostomy stenosis, two had intubation stenosis, one had traumatic stenosis, one had corrosive stenosis, one had laser burn stenosis and one had tubercular stenosis. All patients underwent three-dimensional airway reconstruction and endoscopic evaluation of airway stenosis. After determining the severity and location of airway stenosis, rigid bronchotherapy and Montgomery T tube placement were performed by rigid bronchoscopy., Results: The overall procedural success rate was 100 per cent. Three (27 per cent) patients were weaned from artificial ventilation, and all patients exhibited improved respiratory and functional status. No major post-operative complications or mortality were observed. At follow up (mean, 21.5 months), the decannulation rate was 27 per cent, and eight (73 per cent) patients had stable T tube ventilation. In four patients, granulation over the end of the T tube was controlled by endoscopic procedures. Three patients with stents above the vocal folds showed aspiration and required further intervention (i.e. one nasogastric feeding tube for nutrient supplement, one feeding jejunostomy and one stent shortening to decrease aspiration)., Conclusion: The extra-long Montgomery T tube is an effective and safe method for treating major airway obstruction in the supra-glottic to lower tracheal region.

Objectives: To determine the association between printing works and nasopharygeal carcinoma as well as other diseases., Methods: Demographic data were obtained for those who had worked in a particular newspaper company since its establishment in 1950. Through access to the data bank of the hospital records of the Labor Insurance Bureau for 1985-94, all workers were identified who had been admitted to hospital during their employment in the newspaper company. Multiple logistic regressions were performed to estimate the adjusted morbidity odds ratio (OR) for various diseases among the printing workers with cardiovascular diseases as the reference diseases. Biopsy specimens from patients with nasopharyngeal carcinoma were all subjected to in situ hybridisation of Epstein-Barr virus (EBV), and colocalisation of EBV and secretor component protein., Results: Of the 1564 people who had worked in this company, 579 of them were admitted to hospital at least once. Five out of 144 printing workers admitted to hospital were diagnosed with nasopharyngeal carcinoma compared with none of the other 435 non-printing workers admitted to hospital. The morbidity OR for nasopharyngeal carcinoma in printing workers was 57.0 (95% confidence interval (95% CI) 2.8 to 1155.3). The morbidity OR for benign skin tumours was 28.0 (95% CI 2.7 to 293.1). Chronic pharyngitis or sinusitis also showed significant relations with printing works with a morbidity OR 29.4 (95% CI 1.7 to 514.7). Using all other diseases as the reference diseases for calculation of morbidity ORs still showed a similar trend. In situ hybridisation of EBV encoded small nuclear RNA-1 (EBER-1) showed tumour cells free of the EBV in each biopsy specimen. Colocalisation of EBER-1 and secretor component showed that some tumour cells contained both secretor component and EBV signal in each case., Conclusion: Printing works are associated with an increased risk of nasopharyngeal carcinoma, benign skin tumours, chronic pharyngitis or sinusitis, chronic liver diseases, and mechanical injuries. Induction of the development of nasopharyngeal carcinoma is probably not related to EBV infection in these patients.

Background: To study the differences in the health status of rural and urban ambulatory elderly in Taipei County., Method: Non-compulsory general health check-up for elderly people over 65 years old in rural and urban areas. The content of the health examination included past medical history, health behavior, physical examination, laboratory examination, electrocardiogram and x-ray. Chi square test, t-test and logistic regression were applied for analysis. Risk factors relating to the cardiovascular system were included in the study. Gender differences affecting the prevalence of diseases and health behavior were also considered in the analysis., Results: Significantly higher proportions of the rural elderly men smoked, drank alcohol, and had hypertension and impaired renal function. On the other hand, higher proportions of rural elderly women were obese and had diabetes, hypertension and renal impairment. The mean plasma glucose level of newly-diagnosed diabetic patients in the rural area was significantly higher than that in the urban area (p < 0.05). Diabetes, obesity, hypercholesterolemia and smoking were significantly associated with hypertension. The odds ratio for hypertension between rural and urban areas was 1.45 (p < 0.0001). The cardiovascular risk-rating score of rural elderly was statistically higher than that of urban elderly (p < 0.001)., Conclusion: There were some minor differences in health status between urban and rural elderly. Health promotion should be varied according to the needs of various communities and various risk groups. Further studies should concentrate on prospective cohort research with well-defined determinants to evaluate whether cost-effective biopsychosocial intervention is necessary.

Aim: This study compared the cost and effectiveness of long-term institutional care and home care for stroke patients with severe physical disabilities., Background: Whether home care is more economical or effective than institutional care for patients with chronic illnesses remains controversial when the cost of family labour is considered. Thus, decisions concerning the appropriate type of care setting for patients with severe chronic illness remain difficult., Methods: From November 1995 to March 1996, 313 hospitalized stroke patients with severe physical disabilities treated at one of five hospitals in the Taipei metropolitan area were followed from the day of hospital discharge until the third month after discharge. These 313 patients were divided into four groups as follows: (1) 106 who were admitted to a chronic care unit in a hospital, (2) 60 who were admitted to nursing homes, (3) 60 who received professional home nursing care and (4) 87 who returned home without receiving professional care. The change of physical functional status in the patient was examined as the difference between activities of daily living (ADL) scores measured at discharge and at the end of the third month after discharge., Results: Information on family costs for caregiving, including pay for long-term services utilized, labour costs for caregiving and out-of-pocket expenditures for miscellaneous materials was obtained during a weekly telephone interview. The results indicated that caring for patients in their own homes was not only more expensive but was also less effective in improving ADL scores than caring for patients in nursing homes and in chronic care units of hospitals., Conclusions: The results suggest that caring for patients with severe physical disabilities in institutions is more appropriate than caring of them at home.

A prospective study was carried out in Taiwan to measure the family cost of caregiving at home for 289 patients newly diagnosed with dementia (106 with senile-type dementia, 171 with vascular-type dementia). Their families were first interviewed in the outpatient department and then followed for 1 month after the dementia diagnosis was made. Simultaneously, the costs for providing nursing home care to patients with dementia were calculated using accounting data reported from six nursing homes. Comparisons were made between the amount and type of cost encountered by families and by nursing homes to determine whether family-based care was cheaper and more appropriate than nursing home care for dementia patients with different functional levels. The results showed that higher costs were encountered for caregiving at home when the patients had severe dependence. This tendency was more evident for patients with senile dementia than for patients with vascular-type dementia. The cost of labour was an important expense for families caring for patients at home (at least 85% of family costs). Our findings suggest that, when family labour cost is considered, nursing home care is less expensive than family-based care for long-term care, especially for dementia patients with severe or moderate dependence. Nursing home care is also a better choice when patients have great need for multiple health services.

The rapid increase of breast cancer in Taiwan has prompted the authors to evaluate the predictive factors of early menarche among contemporary Taiwanese girls. A total of 895 four-grade girls from eight elementary schools in Taipei City and County were identified as a closed cohort from the first semester of 1993. Data were collected from self-administered questionnaires and school records. A total of 799 girls who had not menstruated in the first year remained in the group through 1994. The effects of potential predictive factors were assessed by logistic regression. Among the 799 girls followed, 69 (8.6%) had first menstruation between the fourth and fifth grades. Height, weight, body mass index and maternal early onset of menarche were positively related to the onset of menarche within the preceding year. Energy consumption during exercise showed only moderate association after being adjusted for age and weight. Calorie intake from junk food was not associated with early menarche within the preceding year. Poor interpersonal family relationships and stressful life events also showed a moderate association with early menarche. The data obtained supported the hypothesis that height, weight, body mass index and maternal early menarche are positive predictive factors of early menarche. The effects of exercise and childhood stress are less prominent.
