This paper explores whether there has been a change in International Monetary Fund (IMF) policy advice and conditions in its loan programmes and Article IV surveillance by examining the 148 country reports for IMF programmes in 2020, in the context of significant shifts in its global macroeconomic policy framework during the COVID-19 pandemic. It documents the policy recommendations made in these reports and finds that the IMF has supported increased expenditure on health care and cash transfer programmes, often on a temporary basis, even when it meant higher fiscal deficit and public debt. However, it also finds that the IMF has supported fiscal consolidation and reduction of public debt even more frequently, in 129 of the 148 reports examined. This seems to corroborate the findings of a number of recent studies. Given the pronounced gaps in social protection coverage, comprehensiveness and adequacy across all countries, it is essential that the measures taken to cope with the emergency do not remain a mere stopgap response, but progressively lead to the establishment or strengthening of rights-based national social protection systems, including floors. To do so, countries can and should pursue diverse financing options that are equitable in order to mobilize the financial resources needed for social investments, including investments in social protection systems and quality public services. Neighborhood renewal programs are a type of place-based policy that aim to revive underperforming localities. The literature on place-based policies has found mixed results regarding their effects on local labor market outcomes, but there are relatively few studies of policies that aim to improve local labor supply. In this paper, we examine the labor market effects of the Neighbourhood Renewal Fund, which targeted eighty-eight of the most deprived areas in England during the early 2000s as part of the Labour Government's National Strategy for Neighbourhood Renewal. The fund disbursed almost £3 billion for spending on community safety, education, health care and worklessness, with supply-side interventions making up the bulk of the program's spending on worklessness. Using a difference-in-differences approach, we find statistically significant impacts on local employment and out-of-work benefit claimants. Africa is said to be one of the most diseased regions in the world. With mere 13% of the world's population, Africa carries 24% of the global burden of diseases as there are more people living with HIV/AIDS in Africa than anywhere in the world. Malaria also kills more children than anywhere in the world as there are no less than half a million children younger than the age of five that die from malaria every year. Cardiovascular diseases are beginning to kill more people in Africa more than ever before. It is crystal clear that the rate at which diseases are killing people in Africa is alarming and catastrophic and this has attracted the attention of health policy makers in the continent. Africa has adopted and implemented different health policies to tackle health challenges ravaging the continent. However, many of these policies have failed perhaps, owing to the fact that the designers of health policies failed to recognize the role of culture in their design as well as implementation. In this paper, we make a case for the recognition of cultural factors in the design and implementation of health policies in Africa. This paper examines the geographic variation in Medicare and non-Medicare health spending and finds little support for the view that most of the variation is attributable to differences in practice styles. Instead, I find that socioeconomic factors that affect the need for medical care, as well as interactions between the Medicare system, Medicaid, and private health spending, can account for most of the variation in Medicare health spending. Furthermore, I find that the health spending of the non-Medicare population is not well correlated with Medicare spending, suggesting that Medicare spending is not a good proxy for average health spending by state. Finally, there is a negative correlation between the level and growth of Medicare spending; lowspending states are not low-growth states and are thus unlikely to provide the key to curbing excess cost growth in Medicare. The paper also explores the econometric differences between controlling for health attributes at the state level (the method used in this paper) and controlling for them at the individual level (the approach used by the Dartmouth group.) I show that a state-level approach is likely to explain more of the state-level variation associated with omitted health attributes than the individuallevel approach, and argue that this econometric differences likely explains most of the difference between my results and those of the Dartmouth group. More broadly, the paper shows that the geographic variation in health spending does not provide a useful measure of the inefficiencies of our health system. States where Medicare spending is high are very different in multiple dimensions from states where Medicare spending is low, and thus it is difficult to isolate the effects of differences in health spending intensity from the effects of the differences in the underlying state characteristics. I show, for example, that the relationships between health spending, physician composition and quality are likely the result of omitted factors rather than the result of causal relationships. This paper evaluates the effects of possible demographic changes on medical spending for the elderly. Standard forecasts highlight the potential for greater life expectancy to increase costs. The authors suggest that medical costs generally increase with age. They discuss two factors that work in the opposite direction. Increases in life expectancy mean that a smaller share of the elderly will be in their last year of life, when medical costs are generally high. Disability rates have decreased and the reductions, if sustained, reduce the average medical spending of the elderly. The authors suggest that effects are not as extensive as the estimated rise in medical spending stemming from increases in overall medical costs. The end result is that technological changes in medicine will continue to result in significant public sector burden for medical costs. This paper investigates the geographic variation in Medicare spending. The authors suggest that a considerable amount of the variation can be explained by variations across areas in the health of the elderly population. This indicates that some of the geographic variation in Medicare spending is efficient. The authors explain some of the remaining variations after accounting for differences in the health of the population. The paper deals with the impact of the global financial crisis on public service delivery - mainly education and healthcare - in Belarus. The pre-crisis period of 2003-2008 was the most prosperous in recent history. These trends resulted in a pretty good fiscal performance. Nevertheless, the share of expenditures on education and healthcare in GDP was decreasing during the 2000s, which was a consequence of demographic trends and a number of reforms in these sectors. The global crisis hurt the Belarusian economy considerably. However, macro-indicators of the Belarusian economy looked pretty good in comparison to other countries, and the deterioration of public finance was limited. Thus, expenditures on both education and healthcare were mainly part of long-term trends and were able to avoid shock adjustments during the crisis. However, there are a number of medium and long-term threats to these public service sectors associated with the crisis agenda. After years of stagnation and political cataclysms, Georgia tried to recover by launching radical economic and political reforms starting in 2004. The results of the reforms appeared to be impressive. The country's GDP has more than doubled; the total volume of bank deposits is five times what it used to be. Key international indices (Doing Business Index, Economic Freedom Index, Corruption Perception Index) have also reflected the success of the reforms. The occupation of the Georgian territories by Russia in August 2008 and the global financial crisis have significantly changed the current macroeconomic environment in Georgia. The August conflict undermined investor and consumer confidence, put pressure on public finances, damaged physical and other infrastructure and undermined the banking system with a large volume of deposit withdrawals. The deepening of the international financial crisis put further pressure on currency and foreign investments. The purpose of this paper is to consider the nature and magnitude of the impact of the global financial crisis on Georgia's social services sector and on the country's economy as a whole. The global financial crisis had a sharp impact on the most disadvantaged members of the society. The main objectives of the paper are to describe to what extent the education and healthcare sectors were affected in Georgia and to investigate how government policies have addressed the problems which arose due to the financial crisis. A long theoretical literature has analyzed optimal patent policy design, yet there is very little empirical evidence on a key parameter needed to apply these models in practice: the relationship between patent strength and research investments. I argue that the dearth of empirical evidence on this question reflects two key challenges: the difficulty of measuring specific research investments and the fact that finding variation in patent protection is difficult. I then summarize the findings of two recent studies that have made progress in starting to overcome these empirical challenges by combining new data sets measuring biomedical research investments with novel sources of variation in the effective intellectual property protection provided to different inventions. The first study, Budish, Roin, and Williams (2015a), documents evidence consistent with patents affecting the rate and direction of research investments in the context of cancer drug development. The second study, Williams (2013), documents evidence that one form of intellectual property rights on the human genome had quantitatively important impacts on follow-on scientific research and commercial development. Retirement saving and financial planning have become more important for achieving financial security during retirement. After three decades of growth, employer-sponsored defined contribution (DC) plans and tax-favored Individual Retirement Accounts (IRAs) have become two important retirement income sources for many Americans. About 52% of U.S. households had retirement assets in DC plans or IRAs in 2016. In the past few decades, the major responsibility of retirement investing and planning has shifted from the employer to the American worker. With DC plans and IRAs, people typically need to make decisions about how much to contribute each year, how to invest their retirement wealth over a lifetime, and how to withdraw their funds in retirement without outliving their assets.Making these decisions may require complicated calculations involving uncertainty about future conditions, such as expectations about inflation and market returns, estimates about health care expenditures, and projections about family members’ longevity.Some people may find it difficult to navigate the various options and decisions. American families with different income levels and different access to retirement savings vehicles may have different retirement savings needs.  Households with adequate savings in retirement accounts or on track for adequate retirement income may include employees with high incomes and existing retirement plans, such as those who have employer sponsored pensions or who diligently fund their retirement plans. Retirement savings policies for those families may focus on protecting their investment wealth by regulating investment disclosure and management, facilitating investment choices, and regulating to ensure appropriate investment advice.  Some middle-income families with little or no retirement savings or retirement income beyond Social Security may not be on track for adequate retirement income. Some of those families may find saving for retirement difficult for financial reasons. Others may not be eligible to participate in an employer sponsored retirement savings plan, making saving for retirement more challenging. Families who may have difficulty maintaining their preretirement living standards during retirement may benefit from policies to increase participation and contributions in retirement plans.  For some lowest-income families, Social Security and Supplemental Security Income (SSI) generally replace a substantial proportion of preretirement earned income. Income from DC plans and IRAs is typically a small proportion of their retirement income. If these families are not on track for adequate retirement income, policy changes in SSI and other government income support programs may be an appropriate way to help these families meet basic needs in retirement. Behavioral research suggests that humans tend to have biases in rather predictable patterns. For example, the number, order, and structure of options, as well as the process around the choice, can change decisions for many people. Although consumers might not be aware of these biases when making financial decisions,research suggests that these biases can be used to encourage consumers to save more for retirement and make better retirement decisions. Using best practices from behavioral research, policymakers may consider how to best structure retirement accounts and retirement planning decisions to help more people achieve retirement security. Common policy options around saving for retirement tend to relate to (1) retirement account features, (2) investment policies, and (3) financial literacy. For example, some retirement account features that may help consumers save more for retirement include making payroll-deductible retirement savings accounts available to every citizen, automatic enrollment of participants into retirement savings accounts, and automatic escalation of participants’ contributions.In addition, how retirement account information and investment options are disclosed may impact consumer decision making. People may also find retirement planning challenging because of limited financial management, knowledge, and skills (or financial literacy). In 2019, Congress passed the Setting Every Community Up for Retirement Enhancement Act (SECURE Act; P.L. 116-94, Division O). One of the act’s goals was to encourage retirement savings, for example, by expanding Americans’ DC plan access through employers and making changes to DC plans to try to make retirement decision making easier. We study the effect of the Massachusetts health care reform on the uninsured rate and the self-employment rate in the state. The reform required all individuals to obtain health insurance, required most employers to offer health insurance to their employees, formed a private marketplace that offered subsidized health insurance options and expanded public insurance. We examine data from the Current Population Survey (CPS) for 1994-2012 and its Annual Social and Economic (ASEC) Supplement for 1996-2013. We show that the reform led to a dramatic reduction in the state's uninsured rate due to increased enrollment in both public and private health insurance. Estimation results from difference-in-differences models and the synthetic control method indicate that the aggregate self-employment rate was higher in the state after the implementation of the reform. We conclude that easier access to health insurance encouraged self- employment in Massachusetts. There are many similarities between the Massachusetts health care reform and the national health care reform, the Patient Protection and Affordable Care Act (PPACA). Based on Massachusetts' experience, the PPACA will lower the national uninsured rate and may lead to a higher self-employment rate in the nation. [ABSTRACT FROM AUTHOR]
