Background: Anxiety is a prevalent issue among adolescents and can adversely affect their academic performance and everyday functioning. Aims & Objectives: To estimate the prevalence of anxiety among adolescents in urban and rural areas and to study the socio-demographic correlates of anxiety among adolescents in urban and rural areas. Methods: A comprehensive survey was conducted in the field practice area of Department of Community Medicine of a Medical College in western district of Uttar Pradesh. The survey was aimed at young people between the ages of 10 and 19 who lived in the practice area of the medical college. Result: The burden of anxiety was observed to be 19.6 % (n=92). Female gender, urban residence, early adolescent age-group (10-13 yr), nuclear family, day-time somnolence and family history of mental illness were factors having statistical significance. Poor education level of parents was also statistically significant. Physical activity and substance abuse did not have significant contribution in burden of anxiety. Timely access and continuum of care in older adults with Alzheimer's Disease and Related Dementia (ADRD) is critical. This is a retrospective study on Medicare fee-for-service beneficiaries with ADRD diagnosis discharged to home with home health care following an episode of acute hospitalization. Our sample included 262,525 patients. White patients in rural areas have significantly higher odds of delay (odds ratio [OR], 1.03; 95% CI, 1.01–1.06). Black patients in urban areas (OR, 1.15; 95% CI, 1.12–1.19) and Hispanic patients in urban areas also were more likely to have a delay (OR, 1.07; 95% CI, 1.03–1.11). Black and Hispanic patients residing in urban areas had a higher likelihood of delay in home healthcare initiation following hospitalization compared to Whites residing in urban areas. Introduction: Adolescence is a stressful period due to physical, psychological, sexual changes, and presence of disorder such as depression is a matter of concern. Objectives: To study the prevalence of depression among adolescents in urban and rural areas and its socio-demographic correlates. Materials and methods: The PSU consisted all villages (n=29) and mohallas (n=23) in field-practice area. The required number of villages and mohallas, i.e., 10 each, were chosen using the basic random procedure, wherein each village and Mohalla has been assigned a unique serial number. Results: Overall, depression was observed in 110 participants (27.5%) with similar distribution in urban and rural areas. Low maternal and paternal education was a significant risk factor. Adolescents from the upper middle socioeconomic class (42.9%) and those from the lower middle socioeconomic class (33.3%) experienced the most depression. Burden was more among adolescents with low physical activity. Conclusion: This community based study of 400 adolescents residing in the urban and rural areas of Moradabad observed significant burden of depression in adolescent with poor maternal education, living in nuclear families, leading poor lifestyle and affected by any chronic illness at the time of the interview. Backgroud: Coronavirus Disease 2019 (COVID-19) has spread globally affecting all strata of people including the orthopaedic surgeons of India. We have witnessed a drastic fall in the number of patients. The aim of study was to assess the extent to which the Indian orthopaedic practice has been affected by the pandemic. Methods: We conducted an online survey amongst currently practicing Indian orthopaedic surgeons. Those currently not in practice or under training or having left clinical practice before the onset of pandemic were excluded. A total of 533 orthopaedic surgeons took part in the study amongst which, complete responses were obtained from 407 individuals. Statistical analysis was done to see the association between demographic profile of study participants and various variables of orthopaedic practice. Results: There was drastic fall in all the parameters of orthopaedic practice. Over half of the practicing surgeons witnessed fall in out-patients over 90%. Most had stopped elective surgeries (64%) and even emergency ones (21%) altogether. More than 50% of doctors had their earnings reduced by > 75%. We found a statistically highly significant association of reduction in earnings with the sector, type of set-up and duration of practice. (p-value < 0.001). Conclusion: This study suggests that orthopaedic surgeons across all sectors in different kinds of set-ups have been affected in their out-patient and operative numbers. With regards to earnings, those working in private and running their own (individual) hospitals & clinics have been most severely affected while those in government sector and medical colleges have been affected the least. Background and Aims: To assess and compare the effect of bilateral continuous rectus sheath infusion (CRSB) for postoperative analgesia with continuous thoracic epidural infusion (TEA) in patients undergoing midline incision laparotomies. Methods: A prospective, randomised study involving sixty patients with Indian Society of Anesthesiologists (ASA) grade I to III, planned for elective laparotomy were enrolled for the study. Patients were randomly allocated into two groups. In the TEA group, an epidural was sited before induction of general anaesthesia (GA), whereas in the CRSB group, bilateral ultrasound-guided RSB catheters were placed at the end of the surgical procedure, before extubation. Both groups received continuous 0.2% Ropivacaine infusion for postoperative analgesia. They were followed for two post-operative days (POD), for the opioid requirement and post-operative pain at rest, coughing, and moving. Age and body mass index (BMI) were compared using independent t-test and visual analogue scale (VAS) scores were compared by the Mann–Whitney test between the two groups. Opioid consumption, gender, and type of surgery were compared using the Chi-Square test. Statistical analysis was done using Statistical Package for Social Sciences (SPSS 21.0). Results: Opioid consumption in both groups was comparable, for the first two post-operative days with no statistically significant difference. Pain scores were comparable among the groups at all times except postoperative day (POD) 0 (4 h and 12 h postop) and POD 2 (8 AM and 12 PM), where lower pain scores were observed in CRSB Group. Context: Preoperative anxiety in surgical patients imposes stress and dissatisfaction. It results in altered neuroendocrine response and various perioperative complications. Aims: This study was conducted to determine the changes in anxiety level and need for information about the anesthetic and the surgical procedures at three different time points before surgery and evaluate the correlating factors. Settings and Design: A prospective observational study in a university hospital. Materials and Methods: Five hundred adults, American Society of Anesthesiologists Physical Status Class 1 and 2 patients were included in this study. Level of anxiety and need for information were assessed with the Amsterdam Preoperative Anxiety and Information Scale at three time points before the surgery: Evening before surgery in the ward (T1); on the day of surgery, in the preoperative holding area (T2); and in the operating room, after being positioned on the operating table (T3). T-test was applied to compare the mean between two groups, and the Chi-square test for independence of association between two categorical variables. Mann–Whitney–Wilcoxon test was applied to test the equality of distribution between two groups. Kruskal–Wallis test was applied for one-way analysis for comparing median score, and Friedman test was applied for two-way analysis of comparing score among three time points. Results: Total anxiety score recorded was significantly different over the time period (P = 0.023), with an increasing trend over the time. Need for information did not change significantly over time period. Conclusions: Preoperative anxiety continues to increase from ward to operation table. The factors responsible are nonmodifiable. Background Little is known about variation in use of rehabilitation services provided in acute care hospitals for people who have had a stroke. Objective The objective was to examine patient and hospital sources of variation in acute care rehabilitation services provided for stroke. Design This was a retrospective, cohort design. Methods The sample consisted of Medicare fee-for-service beneficiaries with ischemic stroke admitted to acute care hospitals in 2010. Medicare claims data were linked to the Provider of Services file to gather information on hospital characteristics and the American Community Survey for sociodemographic data. Chi-square tests compared patient and hospital characteristics stratified by any rehabilitation use. We used multilevel, multivariable random effect models to identify patient and hospital characteristics associated with the likelihood of receiving any rehabilitation and with the amount of therapy received in minutes. Results Among 104,295 patients, 85.2% received rehabilitation (61.5% both physical therapy and occupational therapy; 22.0% physical therapy only; and 1.7% occupational therapy only). Patients received 123 therapy minutes on average (median [SD] = 90.0 [99.2] minutes) during an average length of stay of 4.8 [3.5] days. In multivariable analyses, male sex, dual enrollment in Medicare and Medicaid, prior hospitalization, ICU stay, and feeding tube were associated with lower odds of receiving any rehabilitation services. These same variables were generally associated with fewer minutes of therapy. Patients treated by tissue plasminogen activator, in limited-teaching and nonteaching hospitals, and in hospitals with inpatient rehabilitation units, were more likely to receive more therapy minutes. Limitation The findings are limited to patients with ischemic stroke. Conclusion Only 61% of patients with ischemic stroke received both physical therapy and occupational therapy services in the acute setting. Background: The purpose of the study was to collect information from local population of Sundar Nagar, District Mandi, Himachal Pradesh, to assess their attitude toward replacement of teeth. Materials and Methods: An analytical, cross-sectional, epidemiological survey was conducted on 522 individuals randomly selected from a local population of Sundar Nagar, to determine attitude toward various treatment modalities for the replacement of teeth. A self-designed closed-ended questionnaire was prepared to assess the attitude of the patient, and collected data were analyzed statistically using the Chi-square test at the significance level of P < 0.05. Results: Around 87% of patients showed positive attitude for the replacement of missing teeth. Almost 78.2% of patients liked to replace missing teeth even although it is an expensive treatment. Around 47.1% of patients believed that they socialized less with a removable prosthesis because of poor stability and retention. Eighty-eight percent of patients of 40-59 years showed a positive attitude toward fixed prosthesis as compared to older age, who preferred removable prosthesis due to lack of interest in esthetics and low perceived need of prosthesis. Forty-seven percent of patients showed a positive attitude toward acceptance of implant. Conclusion: It is essential to evaluate attitude toward treatment as treatment expectations may influence treatment planning and perceived value by the patient and its outcome. Objectives To create a risk index (Mexican American Dementia Nomogram ( MADeN)) that predicts dementia over a 10-year period for Mexican Americans aged 65 and older. Design Retrospective cohort study with longitudinal analysis. Setting Texas, New Mexico, Colorado, Arizona, and California. Participants Hispanic Established Populations for the Epidemiologic Study of the Elderly (H- EPESE) participants (n = 1,739). Measurements Dementia was defined as a decline of three or more points per year on the Mini-Mental State Examination and inability to perform one or more daily activities. Candidate risk factors included demographic characteristics, measures of social engagement, self-reported health conditions, ability to perform daily activities, and physical activity. Results The MADeN comprised the following risk factors: age, sex, education, not having friends to count on, not attending community events, diabetes mellitus, feeling the blues, pain, impairment in instrumental activities of daily living, and unable to walk a half-mile. The area under the receiver operating characteristic curve was 0.74 (95% confidence interval ( CI) = 0.70-0.78) and a score of 16 points or higher had a sensitivity of 0.65 (95% CI = 0.59-0.72) and specificity of 0.70 (95% CI = 0.67-0.73) in predicting dementia. Conclusion The MADeN was able to predict dementia in a population of older Mexican-American adults with moderate accuracy. It has the potential to identify older Mexican-American adults who may benefit from interventions to reduce dementia risk and to educate this population about risk factors for dementia.
