1. Evaluation of a Diabetes Remote Monitoring Program Facilitated by Connected Glucose Meters for Patients With Poorly Controlled Type 2 Diabetes: Randomized Crossover Trial

Abstract

BackgroundPatients with poorly controlled type 2 diabetes (T2D) experience increased morbidity, increased mortality, and higher cost of care. Self-monitoring of blood glucose (SMBG) is a critical component of diabetes self-management with established diabetes outcome benefits. Technological advancements in blood glucose meters, including cellular-connected devices that automatically upload SMBG data to secure cloud-based databases, allow for improved sharing and monitoring of SMBG data. Real-time monitoring of SMBG data presents opportunities to provide timely support to patients that is responsive to abnormal SMBG recordings. Such diabetes remote monitoring programs can provide patients with poorly controlled T2D additional support needed to improve critical outcomes. ObjectiveTo evaluate 6 months of a diabetes remote monitoring program facilitated by cellular-connected glucose meter, access to a diabetes coach, and support responsive to abnormal blood glucose recordings greater than 400 mg/dL or below 50 mg/dL in adults with poorly controlled T2D. MethodsPatients (N=119) receiving care at a diabetes center of excellence participated in a two-arm, 12-month randomized crossover study. The intervention included a cellular-connected glucose meter and phone-based diabetes coaching provided by Livongo Health. The coach answered questions, assisted in goal setting, and provided support in response to abnormal glucose levels. One group received the intervention for 6 months before returning to usual care (IV/UC). The other group received usual care before enrolling in the intervention (UC/IV) for 6 months. Change in hemoglobin A1c (HbA1c) was the primary outcome, and change in treatment satisfaction was the secondary outcome. Abstract

I explore the roots of ontological thinking in the late thought of Georg Lukács via the development of the nature of praxis in German Idealism and the thought of Marx. I contend that the thesis of spontaneous, self-creation as well as social relatedness are both core themes in German Idealism that achieve definitive form in Marx’s thought. In effect, I argue that the human capacities for relatedness and the formation of relations with others paired with the teleological structure of human practical agency constitute a crucial ontological ground for critical theory. It is this that Lukács focuses on in his later ontological writings and which can be used to re-orient contemporary critical theory and critical philosophy which has been dominated by post-metaphysical and neo-Idealist currents in recent decades.

24. The fat made us do it : understanding events that entangle people and vibrant matter

Abstract

This research examines a design-art practice consisting of a programme of interdisciplinary, participatory events. The events employ building an island of fat - the Fatberg - as a form of conceptual-analytical inquiry into relations with a socio-cultural, biological substance. What is striking about the practice is that fat functions not only as subject and construction material but is given an elevated status, what I call synthetic agency whose expressions purportedly direct proceedings. The result is a practice that follows and inherent logic for engaging with vibrant matter (to use Bennett's phrase) that encourages unpredictability. Identifying and critiquing the features and methods that compose Fatberg activities, the research examines the role of material as a participant and collaborator within participatory events. An introductory discourse analysis reveals three features that shape the research enquiry: a dialogic space for fat, fat as a construction material, and fat as an agent. Each represents a distinct motivation for practice, affecting the relations between designer-artist, participants, and material. They also motivate various methods, theories, and practices drawn from art, design, literature, and philosophy that help situate and interpret the practice. Huizinga's theory on the function of play in human culture offers a starting point to establish the basis of creative experimentation. I consider the ecology of play, identifying the characteristics and dimensions that define play as an activity distinct from ordinary life. From there, a path is traced to artist Allan Kaprow's improvised happenings, which expand upon Huizinga's conceptualisation of play to shape a methodology for creative practice. I then look to artist Joseph Beuys's expanded notions of art and material, which positions artistic production as the trigger for social, political, and environmental change, to examine play's transgressive potential. Examining this body of knowledge, I compile a working vocabulary supporting narrative and analytical accounts of practice. The practice-based component of the research centres on a triptych of events performed in summer 2017 and elaborates upon the three practice features to examine their practical effects on people and material. I argue that while Fatberg events appear susceptible to and arguably encourage chance disturbances and divergences, this essential to nurturing mindful engagements with vibrant material. The primary contribution of the research is that it advances a holistic notion of participation that grapples with the real and imagined agency and contributions of material. Consequently, the thesis submits evidence of a learning process comprising practical resources and empirical accounts that helped navigate and comprehend the multifaceted, unpredictable goings-on in events centred on playful engagements with vibrant matter.

Abstract

A growing body of literature critical of ethics review boards has drawn attention to the processes used to determine the ethical merit of research. Citing criticism on the bureaucratic nature of ethics review processes, this literature provides a useful provocation for (re)considering how the ethics review might be enacted. Much of this criticism focuses on how ethics review boards "deliberate," with particular attention given to the lack of transparency and opportunities for researcher recourse that characterise ethics review processes. Centered specifically on the conduct of ethics review boards convened within university settings, this paper draws on these inherent criticisms to consider the ways that ethics review boards might enact more communicative and deliberative practices. Outlining a set of principles against which ethics review boards might establish strategies for engaging with researchers and research communities, this paper draws attention to how "Deliberative communication," "Engagement with researchers" and the "Distribution of responsibility" for the ethics review might be enacted in the day-to-day practice of the university human ethics review board. This paper develops these themes via a conceptual lens derived from Habermas' (The theory of communicative action. Volume 1: Reason and the rationalization of society, 1984) articulation of 'communicative action' and Fraser's (Social Text, 25(26), 56-80, 1990) consideration of 'strong publics' to cast consideration of the role that human ethics review boards might play in supporting university research cultures. "Deliberative communication," "Engagement with researchers" and the "Distribution of responsibility" provide useful conceptual prompts for considering how ethics review boards might undertake their work.

Abstract

Optimizing thermal anneals of Si-implanted β-Ga2O3 is critical for low resistance contacts and selective area doping. We report the impact of annealing ambient, temperature, and time on the activation of room temperature ion-implanted Si in β-Ga2O3 at concentrations from 5 × 1018 to 1 × 1020 cm−3, demonstrating full activation (>80% activation, mobilities >70 cm2/V s) with contact resistances below 0.29 Ω mm. Homoepitaxial β-Ga2O3 films, grown by plasma-assisted molecular beam epitaxy on Fe-doped (010) substrates, were implanted at multiple energies to yield 100 nm box profiles of 5 × 1018, 5 × 1019, and 1 × 1020 cm−3. Anneals were performed in an ultra-high vacuum-compatible quartz furnace at 1 bar with well-controlled gas compositions. To maintain β-Ga2O3 stability, pO2 must be greater than 10−9 bar. Anneals up to pO2 = 1 bar achieve full activation at 5 × 1018 cm−3, while 5 × 1019 cm−3 must be annealed with pO2 ≤ 10−4 bar, and 1 × 1020 cm−3 requires pO2 < 10−6 bar. Water vapor prevents activation and must be maintained below 10−8 bar. Activation is achieved for anneal temperatures as low as 850 °C with mobility increasing with anneal temperatures up to 1050 °C, though Si diffusion has been reported above 950 °C. At 950 °C, activation is maximized between 5 and 20 min with longer times resulting in decreased carrier activation (over-annealing). This over-annealing is significant for concentrations above 5 × 1019 cm−3 and occurs rapidly at 1 × 1020 cm−3. Rutherford backscattering spectrometry (channeling) suggests that damage recovery is seeded from remnant aligned β-Ga2O3 that remains after implantation; this conclusion is also supported by scanning transmission electron microscopy showing retention of the β-phase with inclusions that resemble the γ-phase.
