The increasing integration of artificial intelligence (AI) in healthcare, particularly in vascular and interventional radiology (VIR), has opened avenues for enhanced efficiency and precision. This narrative review delves into the potential applications of large language models (LLMs) in VIR, with a focus on Chat Generative Pre-Trained Transformer (ChatGPT) and similar models. LLMs, designed for natural language processing, exhibit promising capabilities in clinical decision-making, workflow optimization, education, and patient-centered care. The discussion highlights LLMs' ability to analyze extensive medical literature, aiding radiologists in making informed decisions. Moreover, their role in improving clinical workflow, automating report generation, and intelligent patient scheduling is explored. This article also examines LLMs' impact on VIR education, presenting them as valuable tools for trainees. Additionally, the integration of LLMs into patient education processes is examined, highlighting their potential to enhance patient-centered care through simplified and accurate medical information dissemination. Despite these potentials, this paper discusses challenges and ethical considerations, including AI over-reliance, potential misinformation, and biases. The scarcity of comprehensive VIR datasets and the need for ongoing monitoring and interdisciplinary collaboration are also emphasized. In this paper, we present a novel Artificial Intelligence (AI) -empowered system that enhances large language models and other machine learning tools with rules to provide primary care diagnostic advice to patients. Specifically, we introduce a novel methodology, represented through a process diagram, which allows the definition of generative AI processes and functions with a focus on the rule-augmented approach. Our methodology separates various components of the generative AI process as blocks that can be used to generate an implementation data flow diagram. Building upon this framework, we utilize the concept of a dialogue process as a theoretical foundation. This is specifically applied to the interactions between a user and an AI-empowered software program, which is called "Med|Primary AI assistant" (Alpha Version at the time of writing), and provides symptom analysis and medical advice in the form of suggested diagnostics. By leveraging current advancements in natural language processing, a novel approach is proposed to define a blueprint of domain-specific knowledge and a context for instantiated advice generation. Our approach not only encompasses the interaction domain, but it also delves into specific content that is relevant to the user, offering a tailored and effective AI–user interaction experience within a medical context. In recent years, social networks have changed the way people communicate, while the dissemination and obtaining of information has become fast and convenient. However, it is a double-edged sword, providing both a breeding ground for disinformation and a medium for its rapid dissemination. Meanwhile, falsehoods are proliferating and becoming a global risk, and deepfake technology makes it harder to discern. Therefore, this paper proposed a large language model-based algorithm to detect disinformation with generated videos and photos, which enhances the ability to discriminate fake news by implanting alignment and task specific instructions on the Llama model. Eventually, the result indicated that the proposed method has the potential to achieve superior performance and aligned with people's judgment of things. Furthermore, the paper delves into practical discussions concerning fine-tuning the model within the constraints of limited computational power, highlighting the challenges and potential solutions in optimizing the algorithm for applications. Multimodal medical question answering (MMQA) is a vital area bridging healthcare and Artificial Intelligence (AI). This survey methodically examines the MMQA research published in recent years. We collect academic literature through Google Scholar, applying bibliometric analysis to the publications and datasets used in these studies. Our analysis uncovers the increasing interest in MMQA over time, with diverse domains such as natural language processing, computer vision, and large language models contributing to the research. The AI methods used in multimodal question answering in the medical domain are a prominent focus, accompanied by applicability of MMQA to the medical field. MMQA in the medical field has its unique challenges due to the sensitive nature of medicine as a science dealing with human health. The survey reveals MMQA research to be in an exploratory stage, discussing different methods, datasets, and potential business models. Future research is expected to focus on application development by big tech companies, such as MedPalm. The survey aims to provide insights into the current state of multimodal medical question answering, highlighting the growing interest from academia and industry. Background: Recent advancements in Artificial Intelligence (AI) contributed significantly to suicide assessment, however, our theoretical understanding of this complex behavior is still limited. Objective: This study aimed to harness AI methodologies to uncover hidden risk factors that trigger or aggravate suicide behaviors. Methods: The primary dataset included 228,052 Facebook postings by 1,006 users who completed the gold-standard Columbia Suicide Severity Rating Scale. This dataset was analyzed using a bottom-up research pipeline without a-priory hypotheses and its findings were validated using a top-down analysis of a new dataset. This secondary dataset included responses by 1,062 participants to the same suicide scale as well as to well-validated scalesmeasuring depression and boredom. Results: An almost fully automated, AI-guided research pipeline resulted in four Facebook topics that predicted the risk of suicide, of which the strongest predictor was boredom. A comprehensive literature review using APA PsycInfo revealed that boredom is rarely perceived as a unique risk factor of suicide. A complementing top-down path analysis of the secondary dataset uncovered an indirect relationship between boredom and suicide, which was mediated by depression. An equivalent mediated relationship was observed in the primary Facebook dataset as well. However, here, a direct relationship between boredom and suicide risk was also observed. Conclusion: Integrating AI methods allowed the discovery of an under-researched risk factor of suicide. The study signals boredom as a maladaptive 'ingredient' that might trigger suicide behaviors, regardless of depression. The benefits of ideation for both industry and academia alike have been outlined by countless studies, leading to research into various approaches attempting to add new ideation methods or examine how the quality of the ideas and solutions created can be measured. Although AI-based approaches are being researched, there is no attempt to provide the ideation participants with information that inspire new ideas and solutions in real time. Our proposal presents a novel and intuitive approach that supports users in real time by providing them with relevant information as they conduct ideation. By analyzing their ideas within the respective ideation sessions, our approach recommends items of interest with high contextual similarity to the proposed ideas, allowing users to skim through, for example, publications and inspire new ideas quickly. The recommendations also evolve in real time. As more ideas are written during the ideation session, the recommendations become more precise. This real-time approach is instantiated with various ideation methods as a proof of concept, and various models are evaluated and compared to identify the best model for working with ideas. Online social networks can be used for mental healthcare monitoring using Artificial Intelligence and Machine Learning techniques for detecting various mental health disorders and corresponding risk assessment. Recent research in this domain has primarily been focused on leveraging deep neural networks and various Transformer based Large Language Models, which have now become state-of-the-art for most natural language processing and computational linguistic tasks due to their unmatched prediction accuracy. Unlike conventional machine learning algorithms, these deep neural networks are black box architectures, where it is difficult to interpret and explain their predicted outcome. However, a black box classification outcome is insufficient for healthcare applications. Such systems will not be widely adopted and trusted by healthcare practitioners if they are not able to understand and explain the reasoning behind the predicted decisions made by an AI and ML based healthcare diagnostic system. The key objective of our research is to demonstrate the applications of model agnostic, post hoc surrogate XAI techniques for providing explainability to classification decisions of pretrained LLMs (Transformers) based mental healthcare diagnostic systems fine-tuned (or trained) to detect depressive and suicidal behavior using UGC from online social networks. For this, we have used the two most recent and popular techniques, SHAP and LIME. We have conducted extensive and in-depth experiments with four datasets and six pretrained LLMs, three of which have already been domain-adapted using mental health related datasets. We have also performed Few Shot Learning experiments with these three pretrained mental health domain-adapted LLMs. The results of qualitative and descriptive data analysis in this paper demonstrate that in order to build a comprehensive understanding of a person's psychological state, emotion, and behavior and to discover the causes, symptoms, and triggers of mental health issues, it is essential to utilize e X pl AI nable (XAI) techniques with Transformer based LLMs (supervised). Alternatively, Transformer based unsupervised topic modeling technique BERTopic may be used for mental health risk monitoring and cause or symptom extraction when supervised training of LLMs is not feasible due to dataset annotation or availability challenges. The recent past has witnessed a growing interest in technologies for creating chatbots. Advances in Large Language Models for natural language processing are underpinning rapid progress in chatbot development, and experts predict revolutionary changes in the labour market as many manual tasks are replaced by virtual assistants in a range of business functions. As the new technology becomes more accessible and advanced, more companies are exploring the possibilities of implementing virtual assistants to automate routine tasks and improve service. This article reports on qualitative inductive research undertaken within a chatbot development team operating in a major international enterprise. The findings identify critical success factors for chatbot projects, and a model is developed and validated to support the planning and implementation of chatbot projects. The presented model can serve as an exemplary guide for researchers and practitioners working in this field. It is flexible and applicable in a wide range of business contexts, linking strategic business goals with execution steps. It is particularly applicable for teams with no experience in chatbot implementation, reducing uncertainty and managing decisions and risks throughout the project lifecycle, thereby increasing the likelihood of project success. With the advent of large language models (LLMs), the artificial intelligence revolution in medicine and radiology is now more tangible than ever. Every day, an increasingly large number of articles are published that utilize LLMs in radiology. To adopt and safely implement this new technology in the field, radiologists should be familiar with its key concepts, understand at least the technical basics, and be aware of the potential risks and ethical considerations that come with it. In this review article, the authors provide an overview of the LLMs that might be relevant to the radiology community and include a brief discussion of their short history, technical basics, ChatGPT, prompt engineering, potential applications in medicine and radiology, advantages, disadvantages and risks, ethical and regulatory considerations, and future directions. This study investigates self-assessment tendencies in Large Language Models (LLMs), examining if patterns resemble human cognitive biases like the Dunning–Kruger effect. LLMs, including GPT, BARD, Claude, and LLaMA, are evaluated using confidence scores on reasoning tasks. The models provide self-assessed confidence levels before and after responding to different questions. The results show cases where high confidence does not correlate with correctness, suggesting overconfidence. Conversely, low confidence despite accurate responses indicates potential underestimation. The confidence scores vary across problem categories and difficulties, reducing confidence for complex queries. GPT-4 displays consistent confidence, while LLaMA and Claude demonstrate more variations. Some of these patterns resemble the Dunning–Kruger effect, where incompetence leads to inflated self-evaluations. While not conclusively evident, these observations parallel this phenomenon and provide a foundation to further explore the alignment of competence and confidence in LLMs. As LLMs continue to expand their societal roles, further research into their self-assessment mechanisms is warranted to fully understand their capabilities and limitations. BackgroundNatural language processing tools are becoming increasingly adopted in multiple industries worldwide. They have shown promising results however their use in the field of surgery is under-recognised. Many trials have assessed these benefits in small settings with promising results before large scale adoption can be considered in surgery. This study aims to review the current research and insights into the potential for implementation of natural language processing tools into surgery.MethodsA narrative review was conducted following a computer-assisted literature search on Medline, EMBASE and Google Scholar databases. Papers related to natural language processing tools and consideration into their use for surgery were considered.ResultsCurrent applications of natural language processing tools within surgery are limited. From the literature, there is evidence of potential improvement in surgical capability and service delivery, such as through the use of these technologies to streamline processes including surgical triaging, data collection and auditing, surgical communication and documentation. Additionally, there is potential to extend these capabilities to surgical academia to improve processes in surgical research and allow innovation in the development of educational resources. Despite these outcomes, the evidence to support these findings are challenged by small sample sizes with limited applicability to broader settings.ConclusionWith the increasing adoption of natural language processing technology, such as in popular forms like ChatGPT, there has been increasing research in the use of these tools within surgery to improve surgical workflow and efficiency. This review highlights multifaceted applications of natural language processing within surgery, albeit with clear limitations due to the infancy of the infrastructure available to leverage these technologies. There remains room for more rigorous research into broader capability of natural language processing technology within the field of surgery and the need for cross-sectoral collaboration to understand the ways in which these algorithms can best be integrated.

Objective: Clinical deep phenotyping and phenotype annotation play a critical role in both the diagnosis of patients with rare disorders as well as in building computationally-tractable knowledge in the rare disorders field. These processes rely on using ontology concepts, often from the Human Phenotype Ontology, in conjunction with a phenotype concept recognition task (supported usually by machine learning methods) to curate patient profiles or existing scientific literature. With the significant shift in the use of large language models (LLMs) for most NLP tasks, we examine the performance of the latest Generative Pre-trained Transformer (GPT) models underpinning ChatGPT as a foundation for the tasks of clinical phenotyping and phenotype annotation. Materials and methods: The experimental setup of the study included seven prompts of various levels of specificity, two GPT models (gpt-3.5-turbo and gpt-4.0) and two established gold standard corpora for phenotype recognition, one consisting of publication abstracts and the other clinical observations. Results: The best run, using in-context learning, achieved 0.58 document-level F1 score on publication abstracts and 0.75 document-level F1 score on clinical observations, as well as a mention-level F1 score of 0.7, which surpasses the current best in class tool. Without in-context learning, however, performance is significantly below the existing approaches. Conclusion: Our experiments show that gpt-4.0 surpasses the state of the art performance if the task is constrained to a subset of the target ontology where there is prior knowledge of the terms that are expected to be matched. While the results are promising, the non-deterministic nature of the outcomes, the high cost and the lack of concordance between different runs using the same prompt and input make the use of these LLMs challenging for this particular task.
