Sir, A healthy, six-year-old boy presented with a slowly grown dome-shaped nodule on the mandibular angle region present for two years. The patient’s past medical and family history were unremarkable. A physical examination revealed a solitary, 1.3 × 1 cm, firm, painless, flesh-colored tumor (Fig. 1). Dermoscopy showed branching, serpentine vessels on a pink background (Fig. 2a). These features disappeared when slight pressure was exerted on the dermoscope and the tumor exhibited a central, white, structureless area (Fig. 2b). An excisional biopsy was performed. A microscopic examination showed a well-circumscribed, paucicellular dermal tumor composed of eosinophilic collagen bundles separated by clefts and forming a storiform pattern. Scattered fibroblasts were found among the collagen bundles. The overlying epidermis was slightly flattened (Fig. 3). The diagnosis of solitary sclerotic fibroma was established. Sclerotic fibroma (SF), also known as storiform collagenoma, is a rare benign skin tumor. It usually manifests itself as an asymptomatic, slowly growing, white-to-skin-colored papule or nodule [1]. It was first described in patients with Cowden’s disease, yet may also occur sporadically [2]. There were no mucocutaneous features of Cowden’s disease (tricholemmomas, oral fibromas, acral keratoses, palmar pits, and gingival and palatal papules) in the patient and her family members. Dermatofibroma, the main differential diagnosis of SF, usually exhibits hyperplastic changes of the epidermis instead of atrophy, and the boundaries of the lesion are unclear [2]. Only two papers have been published describing the dermoscopic findings of SF, consisting of a white background with peripheral arborizing vessels [3]. A white background may be related to an increased dermal collagen density. It is also described in dermatofibroma, typically with a peripheral pigmentation network. Although dermoscopy may improve the clinical diagnosis of SF, histopathological analysis is required.

Alopecia is a common dermatological complaint. Affected patients are often distressed and attempt to arrest the hair loss by taking various over the counter nutritional supplements containing vitamins and minerals. The evidence supporting their efficacy however is limited. Moreover, there are toxicity reports. We reviewed the literature about the normal levels and the daily dietary needs of the most common micronutrients, their role in the hair follicle cycle as well as their use in the hair loss treatment. 4 independent researchers reviewed a total of 119 papers, and 92 articles published in the English language within the last 30 years were included. Telogen effluvium and alopecia areata have been associated with lower iron, zinc and vitamin D levels. Androgenetic alopecia has been associated with lower iron and vitamin D levels. Both lower and increased vitamin A levels can result in telogen effluvium, but lower levels are associated also with hair breakage. Vitamin C insufficiency results in hair shaft abnormality (cork screw hairs). No data exist about hair loss associated with abnormal biotin levels. The role of micronutrients for the hair follicle function is not completely understood. Empiric treatments of hair loss with micronutrients without confirmed deficiencies have not shown utility.

Introduction: Exfoliative cheilitis, one of a spectrum of diseases that affect the vermilion border of the lips, is uncommon and has no known cause. It is a chronic superficial inflammatory disorder of the vermilion borders of the lips characterized by persistent scaling; it can be a difficult condition to manage. The diagnosis is now restricted to those few patients whose lesions cannot be attributed to other causes, such as contact sensitization or light. Case Report: We present a 17 year-old male presented to the out clinic in Baghdad with the chief complaint of a persistent scaly on his lower lips. The patient reported that the skin over the lip thickened gradually over a 3 days period and subsequently became loose, causing discomfort. Once he peeled away the loosened layer, a new layer began to form again. Conclusion: The lack of specific treatment makes exfoliative cheilitis a chronic disease that radically affects a person’s life. The aim of this paper is to describe a case of recurrent exfoliative cheilitis successfully treated with intralesional corticosteroids and to present possible hypotheses as to the cause.

Calcinosis cutis is a term used to describe a group of disorders in which calcium deposits form in the skin and may be classified as dystrophic, metastatic, idiopathic or iatrogenic calcification, and calciphylaxis. Idiopathic calcinosis cutis occurs without any underlying tissue damage or metabolic disorder. In this paper, the authors report a new case of idiopathic calcinosis involving the medial canthus of the left eye that was mistaken for milia. An 18-year-old previously healthy male patient, presented with an asymptomatic whitish solitary tumour of the medial canthus of the left eye. The patient had no systemic or trauma history, and the serum levels of calcium and phosphorous were normal. An excisional biopsy was performed and histopathologic examination revealed subepidermal calcinosis. Calcinosis cutis is a rare condition that should be included in the differential diagnosis of a benign-appearing lesion of the face. While it can occur in patients with a history of inflammation, trauma, or hypercalcemia, its etiology can also be idiopathic.
