Abstract For the last two decades the Internet of Things (IoT) has been a subject of growing global interest. Particularly dynamic industries such as the healthcare service sector have just begun to understand the benefits of the IoT for the provision of a new, more advanced type of services. However, whilst the healthcare service industry is yet to fully grasp the benefits of information systems for its practitioners and managers, and for patients and families, there is a need for a better understanding of the challenges and opportunities associated to IoT-based healthcare systems as another disruptive wave of technologies. In particular, research on the relevance of users' skills for adoption of IoT-based healthcare services has been limited. Using the current Internet-based healthcare service landscape as a platform for the formulation and testing of its hypotheses, this paper explores the relationship between patients' capabilities for effective use of information and communication technologies and the success of IoT-based healthcare services. The resulting theoretical model for effective use of information and communication technologies and the success of IoT-based healthcare services was then validated. The validation was based on data collected from a randomly selected sample of 256 users of Internet-based healthcare services provided by the public healthcare system of the Region of Murcia in Spain. The findings of this research inform future strategies for the implementation of new generations of health and well-being services based on IoT technologies. Purpose – The purpose of this paper is to examine the impact of an organization's unlearning context and information systems (IS) capabilities on the organization's ability to challenge basic beliefs and to implement processes that are explicitly or tacitly helpful in the reception of new ideas (absorptive capacity). The authors also seek to examine the relationship between absorptive capacity and the existence and enhancement of innovativeness. Design/methodology/approach – These relationships are examined through an empirical investigation of 54 doctors and 62 nurses belonging to 44 hospital-in-the-home units (HHU) in Spain. Findings – The results show that absorptive capacity is an important dynamic determinant for developing a HHU's innovativeness. Moreover, this relationship is best explained with two related constructs. Firstly, the HHU's unlearning context plays a key role in managing the tension between potential absorptive capacity and realized absorptive capacity. Secondly, the results also shed light on a tangible means for health managers to enhance their units' innovativeness (quality of service) through IS capabilities. Research limitations/implications – The cross-sectional design does not allow observation of the short- and long-term impact of absorptive capacity on the unlearning context, information systems capability and HHU's innovativeness. Although the model presented here proposes sequenced relationships between absorptive capacities (PACAP and RACAP), the unlearning context and IS capability, the authors measure all these constructs at one point in time. Practical implications – This sequential model presented in this paper provides practical steps for managers interested in organizational structures that support organizational innovativeness. Background With the passing of time, knowledge like other resources can become obsolete. Thus, people in a healthcare system need to update their knowledge in order to keep pace with the ongoing changes in their operational environment. Information technology continually provides a great amount of new knowledge which can lead to healthcare professionals becoming overloaded with knowledge. This overloading can be alleviated by a process of unlearning which enables the professional to retain just the relevant and critical knowledge required to improve the quality of service provided by them. Objective This paper shows some of the tools and methods that Hospital-in-the-Home Units (HHUs) have used to update the physician–patient knowledge and the technology knowledge of the HHUs' personnel. Design A survey study was carried out in the HHU in Spanish health system in 2010. Setting Fifty-five doctors and 62 nurses belonging to 44 HHUs. Interventions None. Results Three hypotheses are presented and supported, which suggest that technology and physician–patient knowledge is related to the unlearning context and the unlearning context impacts positively on the quality of health services provided. Conclusion The key benefits of the unlearning context for the quality of service provided in HHUs are clear: it enables them to identify and replace poor practices and also avoids the reinvention of the wheel (e.g.: by minimizing unnecessary work caused by the use of poor methods) and it reduces costs through better productivity and efficiency (improving services to patients). Many researchers who have investigated health care organizations have indicated that health care professionals are replete with outdated knowledge, and some researchers go even further to argue that without the presence of a context that facilitates unlearning (forgetting) practitioners may lose the ability to recognize relevant changes with respect to knowledge pertaining to all aspects of the health care sector and they may decide to rely on potentially out-of-date knowledge and inappropriate ways of interpreting data with attendant loss of decision quality and attendant risks. This article presents an analysis and develops a model of the factors that influence unlearning which is focused on the health care industry and is comprised of three constituent components: (1) a framework characterizing the lens through which individuals view situations; (2) a framework for characterizing how individual habits change; and (3) a framework for characterizing the manner in which emergent understandings are consolidated into existing knowledge and knowledge structures. The model was developed and analyzed using qualitative data from the Hospital-in-the-Home Unit of a Spanish Regional Hospital. From a practical perspective the article provides for the identification of factors that influence the nature and effectiveness of the unlearning context in Hospital-in-the-Home-Units in regional hospitals. This not only valuably adds to the knowledge of the way these units function but also may enable actions to be taken to improve the learning processes associated with such units, resulting in an improvement in the quality of knowledge used in day-to-day decision making. • Big data is projected to grow in all industries and in particular the health industry. • Telemedicine is a growing concept facilitating the integration of data and information technology-based concepts. • However, the uptake of these technologies has not been widely effective. • An ambidexterity context for the successful adoption of telemedicine technologies is investigated. • Effective enquiry and dialogue is likely to improve the patients' levels of utilisation of telemedicine technology. The evidence to demonstrate the effectiveness of the uptake of telemedicine technologies for the management of health by users is still lacking. Hence, this research explores the relevance of an ambidexterity context to balance explorative and exploitative learning processes to support the adoption of telemedicine technology. Survey data obtained from 252 healthcare end users was analysed using the ADANCO software version 2.0.1. The results derived from this empirical study support the observation that explorative and exploitative processes mediate the relationship between an ambidexterity context and the utilisation of telemedicine technology. Prior research has revealed that it is fundamentally important for organisations to be able to exploit and explore external sources of knowledge. However, for the uptake of telemedicine technologies, there has been limited research that aids in developing an adequate framework to analyse the role played by explorative and exploitative learning processes with patients. In order to address this issue, this study is done from the point of view of either home health patients or their carers. This paper supports the use of an ambidexterity context that facilitates an environment where effective balance between explorative and exploitative learning processes is likely to improve the patients' levels of utilisation of telemedicine technology. In an attempt to enhance their effectiveness, public health institutions are making a great effort to develop online health care services targeted to all citizens. The initial acceptance of these services by patients is important but their success depends on its continued use, that is, on patients' electronic loyalty (eLoyalty). In this research, an integrated model combining the Technology Acceptance Model (TAM) and the Seddon's Model is developed to test online health care services success from a sample of 256 healthcare end users. The results obtained suggest that quality perception dimensions (i.e. information quality and system quality) are antecedents of perceptual dimensions of individual benefits (i.e., perceived usefulness and satisfaction) and of organizational impact dimensions (i.e. eLoyalty).
