Abstract

Background: Printed participant information about randomised controlled trials is often long, technical and difficult to navigate. Improving information materials is possible through optimisation and user-testing, and may impact on participant understanding and rates of recruitment. Methods: A study within a trial (SWAT) was undertaken within the CASPER trial. Potential CASPER participants were randomised to receive either the standard trial information or revised information that had been optimised through information design and user testing. Results: A total of 11,531 patients were randomised in the SWAT. Rates of recruitment to the CASPER trial were 2.0% in the optimised information group and 1.9% in the standard information group (odds ratio 1.027; 95% CI 0.79 to 1.33; p=0.202). Conclusions: Participant information that had been optimised through information design and user testing did not result in any change to rate of recruitment to the host trial. Abstract

Community-based screening and treatment of women age 70–85 years at high fracture risk reduced fractures; moreover, the screening programme of fracture risk in older women had an effect that was cost-saving. The results support a case for a screening programme of fracture risk in older women in the UK.Purpose The SCOOP (screening for prevention of fractures in older women) randomised controlled trial investigated whether community-based screening could reduce fractures in women age 70–85 years. The objective of this study was to estimate the long-term cost-effectiveness of screening for fracture risk in a UK primary care setting compared with usual management, based on the SCOOP study.Methods:A health economic Markov model was used to predict the life-time consequences in terms of costs and quality of life of the screening programme compared with the control arm. The model was populated with costs related to drugs, administration and screening intervention derived from the SCOOP study. Fracture risk reduction in the screening arm compared with the usual management arm was derived from SCOOP. Modelled fracture risk corresponded to the risk observed in SCOOP. Results: Screening saved 9 hip fractures and 20 non-hip fractures over the remaining lifetime (mean 14 years) of 1,000 patients compared with usual management. In total, the screening arm saved costs (£286) and gained 0.015 QALYs/patient in comparison with usual management arm. Conclusions:This analysis suggests that a screening programme of fracture risk in older women in the UK would gain quality of life and life years, and reduce fracture costs to more than offset the cost of running the programme.

Summary In the large community-based SCOOP trial, systematic fracture risk screening using FRAX® led to greater use of AOM and greater adherence, in women at high fracture risk, compared with usual care. Introduction In the SCreening of Older wOmen for Prevention of fracture (SCOOP) trial, we investigated the effect of the screening intervention on subsequent long-term self-reported adherence to anti-osteoporosis medications (AOM). Methods SCOOP was a primary care–based UK multicentre trial of screening for fracture risk. A total of 12,483 women (70–85 years) were randomised to either usual NHS care, or assessment using the FRAX® tool ± dual-energy X-ray absorptiometry (DXA), with medication recommended for those found to be at high risk of hip fracture. Abstract

Objective. The primary aims of this study were to explore: (1) manifestations of socio-environmental models of allied health support provision in the disability sector; and (2) narrative experiences of individual allied health professionals in the disability sector. Methods. A narrative qualitative study using interviews from a purposive sample of two allied health professionals working in the disability sector explored manifestations of socio-environmental models of allied health support provision and their experiences from case examples. The key informants had more than 10 years of experience in the disability support services setting. Results. Seven key themes exploring manifestations of socio-environmental models of allied health professional practice in the disability sector emerged: (1) dignity of risk; (2) models of care; (3) considerations when working in the supported person's environment; (4) goal-oriented work; (5) informed choice and informed consent; (6) reactive and flexible plans; and (7) training and education role. Conclusions. Socio-environmental models of allied health support provision in the disability sector focus on empowering people with disability to achieve their goals. Measurement capabilities of five meteor radars are assessed and compared to determine how well radars having different transmitted power and antenna configurations perform in defining mean winds, tidal amplitudes, and gravity wave (GW) momentum fluxes. The five radars include two new-generation meteor radars on Tierra del Fuego, Argentina (53.8 deg S) and on King George Island in the Antarctic (62.1 deg S) and conventional meteor radars at Socorro, New Mexico (34.1 deg N, 106.9 deg W), Bear Lake Observatory, Utah (approx 41.9 deg N, 111.4 deg W), and Yellowknife, Canada (62.5 deg N, 114.3 deg W). Our assessment employs observed meteor distributions for June of 2009, 2010, or 2011 for each radar and a set of seven test motion fields including various superpositions of mean winds, constant diurnal tides, constant and variable semidiurnal tides, and superposed GWs having various amplitudes, scales, periods, directions of propagation, momentum fluxes, and intermittencies. Radars having higher power and/or antenna patterns yielding higher meteor counts at small zenith angles perform well in defining monthly and daily mean winds, tidal amplitudes, and GW momentum fluxes, though with expected larger uncertainties in the daily estimates. Conventional radars having lower power and a single transmitting antenna are able to describe monthly mean winds and tidal amplitudes reasonably well, especially at altitudes having the highest meteor counts. They also provide qualitative estimates of GW momentum fluxes at the altitudes having the highest meteor counts; however, these estimates are subject to uncertainties of approx 20 to 50% and uncertainties rapidly become excessive at higher and lower altitudes. Estimates of all quantities degrade somewhat for more complex motion fields.
