Purpose In the accompanying article, “Survey of Fertility Preservation Options Available to Patients With Cancer Around the Globe,” we showed that specific fertility preservation services may not be offered at various sites around the world because of cultural and legal barriers. We assessed global and regional experiences as well as the legal status of third-party reproduction and adoption to serve as a comprehensive international data set and resource for groups that wish to begin oncofertility interventions. Methods We provide data on the legalities of third-party assisted reproductive technologies and other family-building options in the 28 oncofertility-practicing countries surveyed. Results We found regional and country differences that will be important in the development of tailored resources for physicians and for patient brochures that are sensitive to these local restrictions and cultural norms. Conclusion Because many patients first consult Web-based materials, the formal assessment of the availability of these options provides members of the global oncofertility community with data to which they might otherwise not have ready access to better serve their patients.

PURPOSEOncofertility practice continues to grow in developing countries despite the lack of health care services, especially those related to cancer care. The purpose of this study is to further explore oncofertility practice in these countries and identify opportunities for field-wide coalescence.METHODSWe generated a survey to learn more about oncofertility practice in nine developing countries within our Oncofertility Consortium Global Partners Network—Mexico, Colombia, Guatemala, Argentina, Chile, Nigeria, South Africa, Saudi Arabia, and India. Their responses were collected, reviewed, and discussed.RESULTSSurveyed centers from the nine developing countries continue to experience a similar set of common challenges, including a lack of awareness among providers and patients, cultural and religious constraints, lack of insurance coverage and funding to help to support oncofertility programs, and high out-of-pocket costs for patients. Despite these barriers, many opportunities exist and there is great potential for the future.CONCLUSIONThe current need is to unify the new technologies and best practices that emerge from rural communities and developing countries with those in large metropolitan cities, both domestically (US based) and abroad, into a functional unit: the Oncofertility Professional Engagement Network. The Oncofertility Professional Engagement Network will bridge the gap between domestic and international programs to establish a strong global network in which members share resources, methodologies and experiences and further build cultural competency.

Purpose Oncofertility focuses on providing fertility and endocrine-sparing options to patients who undergo life-preserving but gonadotoxic cancer treatment. The resources needed to meet patient demand often are fragmented along disciplinary lines. We quantify assets and gaps in oncofertility care on a global scale. Methods Survey-based questionnaires were provided to 191 members of the Oncofertility Consortium Global Partners Network, a National Institutes of Health–funded organization. Responses were analyzed to measure trends and regional subtleties about patient oncofertility experiences and to analyze barriers to care at sites that provide oncofertility services. Results Sixty-three responses were received (response rate, 25%), and 40 were analyzed from oncofertility centers in 28 countries. Thirty of 40 survey results (75%) showed that formal referral processes and psychological care are provided to patients at the majority of sites. Fourteen of 23 respondents (61%) stated that some fertility preservation services are not offered because of cultural and legal barriers. The growth of oncofertility and its capacity to improve the lives of cancer survivors around the globe relies on concentrated efforts to increase awareness, promote collaboration, share best practices, and advocate for research funding. Conclusion This survey reveals global and regional successes and challenges and provides insight into what is needed to advance the field and make the discussion of fertility preservation and endocrine health a standard component of the cancer treatment plan. As the field of oncofertility continues to develop around the globe, regular assessment of both international and regional barriers to quality care must continue to guide process improvements.

Infertilidad. Inseminación artificial. Estimulación ovárica. ,Gynecology and obstetrics ,RG1-991 - Abstract

BackgroundBurnout interventions are limited by low use. Understanding resident physician preferences for burnout interventions may increase utilization and improve the assessment of these interventions. ObjectiveThis study aims to use an econometric best-worst scaling (BWS) framework to survey internal medicine resident physicians to establish help-seeking preferences for burnout and barriers to using wellness supports by quantifying selections for 7 wellness support options and 7 barriers. MethodsInternal medicine resident physicians at our institution completed an anonymous web-based BWS survey during the 2020-2021 academic year. This cross-sectional study was analyzed with multinomial logistic regression and latent class modeling to determine the relative rank ordering of factors for seeking support for burnout and barriers to using wellness supports. Analysis of variance with Tukey honest significant difference posthoc test was used to analyze differences in mean utility scores representing choice for barriers and support options. ResultsOf the 163 invited residents, 77 (47.2% response rate) completed the survey. Top-ranking factors for seeking wellness supports included seeking informal peer support (best: 71%; worst: 0.6%) and support from friends and family (best: 70%; worst: 1.6%). Top-ranking barriers to seeking counseling included time (best: 75%; worst: 5%) and money (best: 35%; worst: 21%). ConclusionsOverall, our findings suggest that low utilization of formal mental health support is reflective of resident preferences to seek help informally and that increasing utilization will require addressing pragmatic barriers of time and cost. Assessing physician preferences for wellness-related initiatives may contribute to understanding the low utilization of formal mental health services among physicians, which can be determined using a BWS framework.

In the mid 80's, procedures were introduced to eliminate the entire thickness of the endometrium as treatment of heavy menstrual bleeding. Although in the world, the number and popularity of these techniques, known as endometrial ablation has increased considerably, in our country remains a little known and underutilized tool. This article critically discusses the rationale, indications and complications of the endometrial ablation in the management of heavy menstrual bleeding, considering their clinical utility over other alternative treatments.

Objective: To reports and describes the results of the paraaortic lymphadenectomy technique in the mana-gement of endometrial and ovarían cáncer. Methods: We report a prospective cohort of patients with diagnosis of endometrial and ovarían cáncer that required a systematic paraaortic lymphadenectomy, between August 2005 and March 2008, describing the surgical technique and the main results. Results: During this period seventy-six patients carrying endometrial or ovarían cáncer are submitted to a systematic paraaortic lymphadenectomy. The average age was 57 years; 40 patients (52.6%) diagnosed with an endometrial cáncer and 35 (46%) with an ovarían one. The average surgical time was 55 minutes, with an estimated bleeding of 120 mi. We obtained an average of 25.4 lymph nodes. Two patients deceased in this cohort and one was reintervened for a hemoperitoneum. Conclusión: The described surgical technique is feasible, with minimal complications and allows an appropriate exposure of the retroperitoneum to obtain an adequate number of lymph nodes for endometrial and ovarían cáncer staging.

To describe the results of open laparoscopy to confirm the diagnosis and to evalúate resectability in patients with advanced ovarían cáncer (AOC). Methods: Analysis of medical records of all the patients with suspected AOC between October 2005 and February 2007, initially managed with open laparoscopy. Results: In 14 patients we confirmed the diagnosis of AOC. We considered feasibility of optimal cytoreduc-tion in 10 cases and it was possible in 9 of them. The others 4 cases were judged to be inoperable and were derived to neoadjuvant chemotherapy. Conclusions: We believe than open laparoscopy in suspected AOC's patients could be and important tool in the evaluation of patients with ovarían cáncer, to confirm the diagnosis and avoid unnecessary laparotomies in inoperable patients in which neoadjuvant chemotherapy and interval cytoreduction surgery may improve survival

Uterine rupture is an uncommon obstetric event, but still potentially lethal for the mother and the fetus. It presents mainly during the second orthird trimester and is associated with several riskfactors, being a pre-vious uterine scarthe most important of them. Its presentation during the first trimester is exceptional, and it constitutes a medical challenge, because of its difficult differential diagnosis and controversial management. A case of a multiparous, previous triple scar woman is presented, with a spontaneous uterine rupture at 10 weeks of pregnancy. In this case the diagnosis was made during surgery, after the patient's hemodynamic decompensation. The management included primary closure of the lesión and observation; when fetal death was confirmed, dilatation and curettage of the cavity had to be performed. Because of the failure of conser-vative management, an obstetric hysterectomy was carried out. Histopathology reported placenta accreta.

No se presentaron complicaciones en los recien nacidos atribuibles a la cirugíaThe central nervous system malformations are the second more frequent malformation in new born. The election of the way of delivery is controversial. Our center has adopted the cesarean section to resolve such cases, using either a Pfannenstiel laparotomy or a longitudinal infraumbilical section. The observation of some difficulties in both techniques carried us to adapt, a oncoginecological technique, to the obstetric practice. We present 7 cases with central nervous malformations resolved satisfactorily using a Giordano-Cherney laparotomy. The surgery was carried out in elective form, with an average time of 72 minutes. No complications were registered intra or post surgery. The pain management was carried out in routine form. All patients evolved satisfactorily. No complications in new born where attributed to surgery

Creemos que es necesario un seguimiento mayor para validar en forma definitiva esta técnica, aún cuando en nuestro estudio este procedimiento resulta superior al que utiliza el "puente mucoso" para reparar la pared posteriorWe present 50 patients carrying later genital prolapse who went under repair with a prolene mesh and a posterior intravaginal sling as a prophylaxis of the posterior vaginal prolapsed wall, entered to the service of Gynecology of the Carlos Van Buren Hospital between December 2004 and February 2006. The preoperating diagnosis was complete genital prolapse in 38 women, prolapse of previous and later vaginal wall in 8, prolapse of the cupula in 3 and prolapse of the later vaginal wall in one. In 30 patients urinary incontinencia to effort was associated in addition. 92% of the patients evolve in satisfactory form in the intra and postoperative pursuit with restoration of the anatomy of the pelvian floor and without greater complications. We believe that a greater pursuit is necessary to validate this technique completely, even though in our present study this procedure is superior to which uses the "mucous bridge" to repair the later wall

We present here a new algorithm based on a random model for simulating efficiently large brain neuronal networks. Model parameters (mean firing rate, number of neurons, synaptic connection probability and postsynaptic duration) are easy to calibrate further on real data experiments. Based on time asynchrony assumption, both computational and memory complexities are proved to be theoretically linear with the number of neurons. These results are experimentally validated by sequential simulations of millions of neurons and billions of synapses in few minutes on a single processor desktop computer.

The spatial structure of mobility flows is a key element in order to understand the functional reorganisation of the Provence-Alpes-Côte d’Azur region (France). Mobility flows can be used to define urban networks and to follow their evolution over time. This paper analyses inter-municipal dominant flows of daily and residential mobility derived from census data over the 1980s and the 1990s. On the whole, regional urban networks grow stronger and become more complex over time. They shape two metropolitan networks showing different network configurations. Other differences can be observed between the spatial extent of commuting dominant flows, which are linked to the daily functioning of metropolitan areas, and that of residential mobility dominant flows, which are more linked to household life cycles.

We present a new algorithm to efficiently simulate random models of large neural networks satisfying the property of time asynchrony. The model parameters (average firing rate, number of neurons, synaptic connection probability, and postsynaptic duration) are of the order of magnitude of a small mammalian brain or of human brain areas. Through the use of activity tracking and procedural connectivity (dynamical regeneration of synapses), computational and memory complexities of this algorithm are proved to be theoretically linear with the number of neurons. These results are experimentally validated by sequential simulations of millions of neurons and billions of synapses running in a few minutes using a single thread of an equivalent desktop computer. Purpose: The state of limited resource settings that Coronavirus (COVID-19) pandemic has created globally should be taken seriously into account especially in healthcare sector. In oncofertility, patients should receive their fertility preservation treatments urgently even in limited resource settings before initiation of anticancer therapy. Therefore, it is very crucial to learn more about oncofertility practice in limited resource settings such as in developing countries that suffer often from shortage of healthcare services provided to young patients with cancer. Methods: As an extrapolation during the global crisis of COVID-19 pandemic, we surveyed oncofertility centers from 14 developing countries (Egypt, Tunisia, Brazil, Peru, Panama, Mexico, Colombia, Guatemala, Argentina, Chile, Nigeria, South Africa, Saudi Arabia, and India). Survey questionnaire included questions on the availability and degree of utilization of fertility preservation options in case of childhood cancer, breast cancer, and blood cancer. Results: All surveyed centers responded to all questions. Responses and their calculated oncofertility scores showed different domestic standards for oncofertility practice in case of childhood cancer, breast cancer, and blood cancer in the developing countries under limited resource settings. Conclusions: Medical practice in limited resource settings has become a critical topic especially after the global crisis of COVID-19 pandemic. Understanding the resources necessary to provide oncofertility treatments is important until the current COVID-19 pandemic resolves. Lessons learned will be valuable to future potential worldwide disruptions due to infectious diseases or other global crises.

Social Sciences - Abstract

PURPOSE Oncofertility practice continues to grow in developing countries despite the lack of health care services, especially those related to cancer care. The purpose of this study is to further explore oncofertility practice in these countries and identify opportunities for field-wide coalescence. METHODS We generated a survey to learn more about oncofertility practice in nine developing countries within our Oncofertility Consortium Global Partners Network—Mexico, Colombia, Guatemala, Argentina, Chile, Nigeria, South Africa, Saudi Arabia, and India. Their responses were collected, reviewed, and discussed. RESULTS Surveyed centers from the nine developing countries continue to experience a similar set of common challenges, including a lack of awareness among providers and patients, cultural and religious constraints, lack of insurance coverage and funding to help to support oncofertility programs, and high out-of-pocket costs for patients. Despite these barriers, many opportunities exist and there is great potential for the future. CONCLUSION The current need is to unify the new technologies and best practices that emerge from rural communities and developing countries with those in large metropolitan cities, both domestically (US based) and abroad, into a functional unit: the Oncofertility Professional Engagement Network. The Oncofertility Professional Engagement Network will bridge the gap between domestic and international programs to establish a strong global network in which members share resources, methodologies and experiences and further build cultural competency.

Background Though the phenotype of anxiety about medical illness has long been recognized, there continues to be debate as to whether it is a distinct psychiatric disorder and, if so, to which diagnostic category it belongs. Objective Our objective was to investigate the pattern of psychiatric comorbidity in hypochondriasis (HC) and to assess the relationship of health anxiety to anxiety, depressive, and somatoform disorders. Methods Data were collected as part of a clinical trial on treatment methods for HC. In all, 194 participants meeting criteria for Diagnostic and Statistical Manual of Mental Disorders, 4th Edition (DSM-IV) HC were assessed by sociodemographic variables, results of structured diagnostic interviews, and validated instruments for assessing various symptom dimensions of psychopathology. Results Most of the individuals with HC had comorbid psychiatric illness; the mean number of comorbid diagnoses was 1.4, and 35.1% had HC as their only diagnosis. Participants were more likely to have only comorbid anxiety disorders than only comorbid depressive or somatoform disorders. Multiple regression analysis of continuous measures of symptoms revealed the strongest correlation of health anxiety with anxiety symptoms, and a weaker correlation with somatoform symptoms; in multiple regression analysis, there was no correlation between health anxiety and depressive symptoms. Conclusion Our findings suggest that the entity of health anxiety (HC in DSM-IV and illness anxiety disorder in DSM-5) is a clinical syndrome distinct from other psychiatric disorders. Analysis of comorbidity patterns and continuous measures of symptoms suggest that its appropriate classification is with anxiety rather than somatoform or mood disorders.

A preliminary version of this article was presented at ECTQG 2015 - XIX European Colloquium on Theoretical and Quantitative Geography, Bari (Italy), September 3rd-7th 2015; International audience; A Bayesian Network and a Possibilistic Network are used to produce trend scenarios of social polarization in the metropolitan area of Marseille (France). Both scenarios are based on uncertain knowledge of relationships among variables and produce uncertain evaluations of future social polarization. We show that probabilistic models should not be used just to infer most probable outcomes, as these would give a fallacious impression of certain knowledge. The possibilistic model produces more uncertainty-laden results which are coherent with model uncertainties and respect elicited values of possibilities. Results of the two models converge when probability values are "degraded".

One of the most important housing research issues is the interplay of socio-cultural factors (lifestyles) and of objective constraints (increasingly important in tense metropolitan housing markets) in shaping households' housing preferences. This paper presents a modelling application for interview/survey data analysis on home ownership within metropolitan areas. The goal of the application is to develop an exploratory methodological protocol to analyse the interplay of lifestyles and objective constraints in home ownership decisions, as well as the spatial inscription of residential trajectories and of mobility behaviours. Its final result is the identification of coherent socio-geographic profiles capable of segmenting new home owners within the study areas of Provence and of the French Riviera. Socio-geographic profiles are identified and characterized by a new five step clustering protocol using Bayesian Networks (BN). BN modelling can integrate both qualitative and quantitative data, and characterise profiles in a probabilistic framework. Socially appreciated suburban and as well as urban lifestyles are present in Provence and on the French Riviera, shaping particularly similar socio-geographic profiles.

We describe different optimization techniques to perform the assembly of finite element matrices in Matlab and Octave, from the standard approach to recent vectorized ones, without any low level language used. We finally obtain a simple and efficient vectorized algorithm able to compete in performance with dedicated software such as FreeFEM++. The principle of this assembly algorithm is general, we present it for different matrices in the P1 finite elements case and in linear elasticity.

PURPOSE: Oncofertility practice continues to grow in developing countries despite the lack of health care services, especially those related to cancer care. The purpose of this study is to further explore oncofertility practice in these countries and identify opportunities for field-wide coalescence. METHODS: We generated a survey to learn more about oncofertility practice in nine developing countries within our Oncofertility Consortium Global Partners Network—Mexico, Colombia, Guatemala, Argentina, Chile, Nigeria, South Africa, Saudi Arabia, and India. Their responses were collected, reviewed, and discussed. RESULTS: Surveyed centers from the nine developing countries continue to experience a similar set of common challenges, including a lack of awareness among providers and patients, cultural and religious constraints, lack of insurance coverage and funding to help to support oncofertility programs, and high out-of-pocket costs for patients. Despite these barriers, many opportunities exist and there is great potential for the future. CONCLUSION: The current need is to unify the new technologies and best practices that emerge from rural communities and developing countries with those in large metropolitan cities, both domestically (US based) and abroad, into a functional unit: the Oncofertility Professional Engagement Network. The Oncofertility Professional Engagement Network will bridge the gap between domestic and international programs to establish a strong global network in which members share resources, methodologies and experiences and further build cultural competency.

In this paper, we extend the existence results obtained in [1], for a dynamic unilateral contact problem with nonlocal friction between a Kelvin-Voigt viscoelastic body and a rigid obstacle, to the contact between two viscoelastic bodies and to a cracked viscoelastic body. We give classical and primal variational formulations for the problems. Penalized formulations are investigated by using an abstract existence and uniqueness result. Several estimates on the penalized solutions are established which allow to pass to the limit and to prove the existence of solutions for these dynamic unilateral contact problems with friction.

Wildland fires are driven by vegetation which is a medium that can be very heterogeneous at several scales. These heterogeneities from shoot to stand levels are likely to affect fire behavior. In the context of theoretical fire modelling, we investigated the effects of departure from randomness in radiative transfer. A methodology derived from approach in other turbid media for solar radiation was adapted to our problem. From several Mediterranean fuel type calculations, conditions on vegetation parameters that provide heterogeneity effects were exhibited. Fuel description was done from field measurements and plant architecture modelling. Results show that both a strong spatial variation in fuel distribution and vegetation dense enough were required to provide heterogeneity effects. The combined effect of fuel surface area, cover fraction and heterogeneity size was studied. Radiative transfer in heterogeneous maritime pines and dense shrub stands is shown to be very significantly affected by heterogeneity. On the contrary, lower effects are expected from Aleppo pine and light shrub fuel distributions. Further work will determine the appropriate resolution scale for vegetation description of each species and a way to take into account heterogeneity when no detailed information is available on fuel distribution. © 2006 Published by Elsevier B.V.

FERTILITY preservation ,CANCER patients ,REPRODUCTIVE technology ,MEDICAL care ,REPRODUCTIVE health - Abstract
Purpose In the accompanying article, “Analysis of Fertility Preservation Options Available to Patients With Cancer Around the Globe,” we showed that specific fertility preservation services may not be offered at various sites around the world because of cultural and legal barriers. FERTILITY preservation ,CANCER patient care ,MEDICAL care ,CANCER treatment ,CANCER patients - Abstract
Symptoms of dissociation, including dissociative amnesia, depersonalization, and derealization, commonly develop in individuals subject to chronic and repeated trauma during development. This includes the trauma of environmental inability to facilitate development of adequate cognitive strategies for coping with strong negative emotions. Dissociation likely involves dysregulated balance of prefrontal inhibition of limbic structures and inadequate regulation of attentional bias by both prefrontal and limbic systems. There is currently no established psychopharmacologic treatment for dissociative symptoms. Here the case of a woman with severe dissociative symptoms that were markedly improved with the administration of mixed amphetamine salts is discussed. Potential neurobiologic mechanisms for dissociative symptom improvement with psychostimulants are discussed. [ABSTRACT FROM PUBLISHER]
