14. Dental implant treatment in patients with cancer on high‐dose antiresorptive medication: A prospective feasibility study with 2 years follow‐up.

Abstract

Objectives: Dental implant treatment is considered contraindicated in patients with cancer on high‐dose antiresorptive medication (HDAR). The aim of this prospective, feasibility study was to evaluate implant treatment in patients with cancer on HDAR, in terms of implant survival, implant success, and oral health‐related quality of life (OHLQoL) after 2 years of loading. Materials and Methods: Implants were inserted in three groups of HDAR patients: (1) Previous tooth extraction, no medication‐related osteonecrosis of the jaw (MRONJ), (2) Previous MRONJ, now healed, (3) Existing MRONJ, planned surgical resection. Implants were placed without adjunctive bone or soft tissue argumentation. Abutment operation was performed after ≥12 weeks. Prosthetic treatment was initiated ≥14 weeks. Survival and success rate were determined, and OHLQoL was analyzed with OHIP‐49 and QLQ‐H&N35 questionnaires. Patients were seen for 6 months, 1‐ and 2 years follow‐up. Results: Twenty‐two patients, 39 implants, completed the implant‐based prosthetic treatment. Implant‐supported crowns and overdentures were fabricated. Thirteen patients (59%) with 23 implants (59%) completed 2 years follow‐up. Overall implant survival and success rate after 2 years of loading were 100% and 97.4%, respectively. OHLQoL for the patients increased in all groups after the treatment, a substantial increase was seen in group 3. Two patients developed MRONJ, but not related to the implant treatment. Conclusion: Dental implant treatment, with high survival and success rate and increased post‐treatment OHLQoL, is feasible in HDAR patients after 2 years of loading. 24. Sensitivity of Lillgrund Wind Farm Power Performance to Turbine Controller

Abstract

The impact of the choice of the individual wind turbine controller on the power production of a wind farm is studied by means of large eddy simulations (LES) combined with aero-servo-elastic actuator disc representations of the rotors. The investigation includes both two typical controllers for pitch-regulated variable speed wind turbines and a simplified control strategy based on tabulated rotor speed and pitch as a function of the average velocity at the rotor. The simulations show that the power output of the individual wind turbines is strongly affected by choosing either a standard or simplified control strategy when they operate in the deep wake of upstream turbines. On the other hand, the sensitivity to control strategy is less significant for wind directions where the effective stream-wise distance between the turbines is larger.

25. RANS-AD based ANN surrogate model for wind turbine wake deficits

Abstract

Inside wind farms, wake effects are the primary source of turbine interactions, and as such, they constitute one of the most important aspects of wind farm operations. The two most widespread methods for calculating the wind farm wake flow are computational fluid dynamics (CFD) methods and engineering models. Both methods have drawbacks; CFD methods can be very accurate but are computationally expensive. Vice versa, engineering models sacrifice accuracy by simplifying the physics, thereby improving computational efficiency. In cases where many evaluations of the flow are needed, this trade-off is a hindrance. One such case is wind farm layout optimization problems. It has been shown that the estimation of wake flows can be improved by surrogate modeling. Recently, Artificial Neural Networks (ANN) have been demonstrated to predict accurate wakes over a mesh. In this work, a new mesh-free ANN-based wake model is proposed. This new model can predict the flow everywhere in the domain, and as it employs smooth activation functions, it is suitable for gradient-based optimization. Two ANNs were trained with data generated by Reynolds-Averaged Navier-Stokes with Actuator Disc simulations for several yaw angles. The first ANN predicts streamwise wake velocity induction/deficit, the second ANN predicts added wake turbulence intensity. Both ANNs predict with a low error.

26. Cross-code verification of non-neutral ABL and single wind turbine wake modelling in LES

Abstract

Large Eddy Simulations (LES) of atmospheric boundary layer (ABL) flow with the actuator disc (AD) for turbine modelling is a widely used method of simulating wind farm flows. Hence, it is important to understand the requirements for achieving a good comparison between ABL flow and turbine wakes between different research group setups, despite unavoidable differences which include LES numerical framework, sub-grid scale (SGS) model, and turbine modelling, and at grid resolutions achievable in large wind farm simulations. In this work, conventionally neutral (CNBL), stable (SBL) and convective (CBL) boundary layers, and single turbine wakes under these different conditions, are compared between the EPFL pseudo-spectral code WIRE LES and the DTU finite-volume code EllipSys3D. ABL profiles largely agree well with hub height velocity magnitudes agreeing to 0.7%, 3.8% and 0.5% for the CNBL, SBL and CBL respectively. The scale-dependent SGS model of WIRE LES results in reduced grid dependency, while EllipSys3D required higher grid resolution in the SBL. Wake flows show improved wake recovery and greater added turbulence intensity with increasing grid resolution, and good agreement is achieved with a radius R to cell size ratio of R/(dxdydz)1/3 ≥ 6.5. Trends in wake flow with different stability conditions, such as the influence of inflow turbulence intensity or shear, are well replicated between codes. Likewise, wake deficit and added TI profiles, and distributions of turbine power and thrust also agree well. Mean power output predictions match to 4.3%, 7.2% and 3.8% in the CNBL, SBL and CBL respectively between the two codes. Overall, these results demonstrate that good agreement is possible with aligned turbine data and sufficient grid resolution.

27. Stochastic wind farm flow generation using a reduced order model of LES

Abstract

Large eddy simulations are computationally expensive, which typically limits the simulation time and hence the statistics useful for validation. Here, a reduced order model with a stochastic engine is used to generate stochastic inflow to 48 wind turbines in the Lillgrund wind farm. The stochastic flow realization captures the power production and more importantly the correct cross-correlations in power and blade root flapwise bending moments over the large spatiotemporal region, which the wind farm covers. The analysis show how the cross-correlation increase further into the wind farm, where the highly turbulent flow becomes increasingly self-organized and how the dynamic interaction across the entire wind farm is captured by the first eight modes. Overall, the reduced order model captures the correct dynamics at a fraction of the computational cost, effectively generalizing the LES results beyond a relatively short simulation period.

30. Prevalence of survey-based criteria for fibromyalgia and impact on hospital burden: a 7 year follow-up study from an outpatient clinic.

Abstract

To evaluate the prevalence of survey-based criteria for fibromyalgia (FM) among newly referred patients in a rheumatic outpatient clinic, and to compare the use of secondary healthcare services between survey-based FM and non-FM cases. Newly referred patients to an outpatient clinic were screened for the fulfilment of the 2011 FM survey criteria during a 6 month period in 2013 in this observational cohort study. Demographic data were obtained at baseline. Patients' medical files were evaluated and comparisons between groups were made regarding the use of hospital healthcare facilities during the 7 year observation period. Out of 300 invited patients, 248 (83%) completed the questionnaire; 90 patients (36%) fulfilled survey-based criteria for FM at enrolment. FM cases were primarily women (80% vs 54% of non-FM cases), and received more medications (median 4 vs 3 drugs) and public economic support (62% vs 20%). At the 7 year follow-up, crude analyses showed that FM cases had a higher number of hospital courses (median 10 vs 8) and had undergone more invasive procedures (78% vs 60%). Neurologists (42% vs 28%), gastroenterologists (30% vs 13%), endocrinologists (40% vs 21%), pain specialists (13% vs 3%), psychiatrists (20% vs 7%), and abdominal surgeons (43% vs 30%) were consulted more often by FM than by non-FM cases. Fulfilment of FM survey criteria among newly referred patients to a rheumatic outpatient clinic is frequent. Our study findings show that FM continues to present a challenge for healthcare professionals as well as for patients. ● Fulfilment of FM survey criteria among newly referred patients to a rheumatic outpatient clinic is frequent. ● The burden on the secondary healthcare system for these patients is significant. ● This study suggests the need for increased awareness about the diagnosis of FM among certain medical and surgical specialties. Abstract

Reliable quantitative vegetation reconstructions for Europe during the Holocene are crucial to improving our understanding of landscape dynamics, making it possible to assess the past effects of environmental variables and land-use change on ecosystems and biodiversity, and mitigating their effects in the future. We present here the most spatially extensive and temporally continuous pollen-based reconstructions of plant cover in Europe (at a spatial resolution of 1° × 1°) over the Holocene (last 11.7 ka BP) using the 'Regional Estimates of VEgetation Abundance from Large Sites' (REVEALS) model. This study has three main aims. First, to present the most accurate and reliable generation of REVEALS reconstructions across Europe so far. This has been achieved by including a larger number of pollen records compared to former analyses, in particular from the Mediterranean area. Second, to discuss methodological issues in the quantification of past land cover by using alternative datasets of relative pollen productivities (RPPs), one of the key input parameters of REVEALS, to test model sensitivity. Finally, to validate our reconstructions with the global forest change dataset. The results suggest that the RPPs.st1 (31 taxa) dataset is best suited to producing regional vegetation cover estimates for Europe. Abstract

Wind farm Annual Energy Production (AEP) calculations are required to design energy efficient wind farm layouts. We investigate methods that can reduce the computational effort of AEP calculations using Reynolds-averaged Navier-Stokes simulations of an idealized atmospheric wind farm setup. In addition, we introduce a generalized wind turbine model that compares well with wind turbine aerodynamic data covering a large range of wind turbine sizes. We apply the general wind turbine model to reduce the computational effort of the AEP calculations by decreasing the number of independent wind speed flow cases. Furthermore, we apply Reynolds-number similarity to compute the wind speed flow cases faster and we show how wind farm layout mirror- and rotational-symmetry can reduce the number of independent wind direction flow cases.

37. Influence of shielding gas flow on the uniformity of additively manufactured martensitic stainless steel

Abstract

Shielding gas flow is essential to the additive manufacturing (AM) process, and the effects of argon shielding gas flow variation on the macroscopic homogeneity of additive manufactured stainless steel parts have been studied using an open-architecture AM system. Such a variation manifests itself layer-by-layer within one part and part-by-part across the build plate. Within one build, a combination of balling behavior, conduction melting and keyhole melting is observed across the build plate using the same laser parameters. Quantitative characterization of the melt pool shapes show that the melt pool width and the penetration depth exhibit the largest variations. Possible relations between the gas flow condition and macroscopic structure variations are discussed and guidelines for improved design of a gas flow system as well as future research directions are suggested for achieving macroscopically uniform metal AM.

38. Impact of Turbulent Time Scales on Wake Recovery and Operation

Abstract

Enhancing wake recovery behind wind turbines has the potential to significantly improve the power production and efficiency of large wind farms. Rather than investigating turbine control strategies, floater motion or global turbulent quantities such as turbulence intensity, this work aims to study wake stability and recovery through a focus on the turbulent scales of the inflow. Using Large Eddy Simulations of a single turbine, sinusoidal streamwise forcing is applied to the inflow with a constant amplitude and mean flow velocity, but differing time scales between 80s and 140s. For all applied time scales the turbine wake is characterised by the rolling-up of the near wake into the periodic shedding of vortex rings, and an excitation of the applied forcing frequency resulting in velocity fluctuations in the wake several times larger than that at the inflow. For shorter time scales (80s - 90s) a more aggressive and earlier wake roll-up led to a shorter near wake region, faster overall recovery and significantly improved the expected power output from 6R downstream onwards. An inflow time period of 80s gave rise to more than a 50% increase in power output of a fictive downstream turbine placed at 14R downstream, compared to an inflow time scale of 140s.

39. Closing the gap:the interface between animal law and animal ethics

Abstract

A recent paper in PlosOne has reported that on average 85% of all Danish egg-layers in commercial production systems, whether caged or non-caged, have keel bone fractures (KBF). Based on this case, the paper analyses the discrepancy between animal law and animal ethics taking as a point of departure the difference between the law regulating the use of animals for production and a production system that relies on animals bred and held in a way that leads to the majority of them living with serious physical injuries. According to the fundamental norm laid down in section 2 of the Danish Animal Welfare Act, animals must be treated properly and protected as far as possible from pain, suffering, fear, permanent injury and severe distress. This seems impossible to reconcile with the current situation for egg-layers and one therefore would think that the production would be shut down immediately after the publication of the research. Reality, however, is different. The industry has promised to investigate the problem and a timeframe ranging from 10-20 years for rectifying the situation has been presented. This situation illustrates how the ethical intentions behind animal law come up short when faced with the economic logic of animal production. This leads us to ask whether the interface between animal law and animal ethics, where the language of rights is typically used to ‘transfer’ ethical considerations into regulation, ought to be framed differently by approaching the issue from other philosophical perspectives such as consequentialism, virtue ethics and ethics of care. Furthermore, we will carry out a preliminary analysis of the ramifications of such approaches.

Abstract

Back pain is a common and debilitating disorder with largely unknown underlying biology. Here we report a genome-wide association study of back pain using diagnoses assigned in clinical practice; dorsalgia (119,100 cases, 909,847 controls) and intervertebral disc disorder (IDD) (58,854 cases, 922,958 controls). We identify 41 variants at 33 loci. The most significant association (ORIDD = 0.92, P = 1.6 × 10−39; ORdorsalgia = 0.92, P = 7.2 × 10−15) is with a 3’UTR variant (rs1871452-T) in CHST3, encoding a sulfotransferase enzyme expressed in intervertebral discs. The largest effects on IDD are conferred by rare (MAF = 0.07 − 0.32%) loss-of-function (LoF) variants in SLC13A1, encoding a sodium-sulfate co-transporter (LoF burden OR = 1.44, P = 3.1 × 10−11); variants that also associate with reduced serum sulfate. Genes implicated by this study are involved in cartilage and bone biology, as well as neurological and inflammatory processes. © 2022, The Author(s).

Abstract

Objective: Streptococcus pneumoniae is the most frequent bacterial pathogen isolated in subjects with Community-acquired pneumonia (CAP) worldwide. Limited data are available regarding the current global burden and risk factors associated with drug-resistant Streptococcus pneumoniae (DRSP) in CAP subjects. We assessed the multinational prevalence and risk factors for DRSP-CAP in a multinational point-prevalence study. Design: The prevalence of DRSP-CAP was assessed by identification of DRSP in blood or respiratory samples among adults hospitalized with CAP in 54 countries. Prevalence and risk factors were compared among subjects that had microbiological testing and antibiotic susceptibility data. Multivariate logistic regressions were used to identify risk factors independently associated with DRSP-CAP. Results: 3,193 subjects were included in the study. The global prevalence of DRSP-CAP was 1.3% and continental prevalence rates were 7.0% in Africa, 1.2% in Asia, and 1.0% in South America, Europe, and North America, respectively. Macrolide resistance was most frequently identified in subjects with DRSP-CAP (0.6%) followed by penicillin resistance (0.5%). Subjects in Africa were more likely to have DRSP-CAP (OR: 7.6; 95% CI: 3.34-15.35, p < 0.001) when compared to centres representing other continents. Conclusions: This multinational point-prevalence study found a low global prevalence of DRSP-CAP that may impact guideline development and antimicrobial policies. Abstract

We present a formalism for dissipation-optimized decomposition of the strain rate tensor (SRT) of turbulent flow data using Proper Orthogonal Decomposition (POD). The formalism includes a novel inverse spectral SRT operator allowing the mapping of the resulting SRT modes to corresponding velocity fields, which enables a complete dissipation-optimized reconstruction of the velocity field. Flow data snapshots are obtained from a direct numerical simulation of a turbulent channel flow with friction Reynolds number R e τ = 390. The lowest dissipation-optimized POD (d-POD) modes are compared to the lowest conventional turbulent kinetic energy (TKE)-optimized POD (e-POD) modes. The lowest d-POD modes show a richer small-scale structure, along with traces of the large-scale structure characteristic of e-POD modes, indicating that the former captures structures across a wider range of spatial scales. Profiles of both TKE and dissipation are reconstructed using both decompositions, and reconstruction convergences are compared in all cases. Both TKE and dissipation are reconstructed more efficiently in the dissipation-rich near-wall region using d-POD modes, and in the TKE-rich bulk using e-POD modes. Lower modes of either decomposition tend to contribute more to either reconstructed quantity. Separating each term into eigenvalues and factors relating to the inherent structures in each mode reveals that higher e-POD modes tend to encode more dissipative structures, whereas the structures encoded by d-POD modes have roughly constant inherent TKE content, supporting the hypothesis that structures encoded by d-POD modes tend to span a wide range of spatial scales.
