22. Thromboelastography in Long-Term Antiplatelet Therapy for Patients Diagnosed with Benign Prostate Hyperplasia Undergoing Holmium Laser Enucleation of the Prostate: A Retrospective Study.

Abstract

To compare low- vs high-power HoLEP effects on coagulation in patients on antiplatelet (AP) therapy via thromboelastography (TEG). Methods: 210 patients was retrospectively analyzed and stratificated into three discrete groups, specifically: Group A (AP therapy, high-power HoLEP, n = 72); Group B (AP therapy, low-power HoLEP, n=73); Group C (no AP therapy, low-power HoLEP, n = 65). Baseline characteristics and coagulation profiles via TEG were compared. Univariate and multivariate analyses were conducted to identify independent risk factors associated with hematuria. Furthermore, parameters such as IPSS, Qmax, post-void residual volume V2 and PSA levels were recorded during 1year follow-up. Results: No differences in terms of baseline characteristics across all groups. Significant differences were observed in the duration of enucleation, morcellation, bladder irrigation, post-operative catheterization, length of hospital stay and the extent of hemoglobin reduction (F = 54.06, 8.54, 6.68, 9.24, 17.06, 5.97, p <  0.05). No differences were noted in postoperative hematuria, urine retention, transfusion rates, and SUI (x12 = 1.082 ; x22 =  0.197,; x32 =  3.981;x42 =  0.816, p >  0.05). Univariate and multivariate analyses revealed that prostate volume emerged as an independent risk factor for hematuria (OR 1.080, 95% CI: 1.007– 1.158, p =  0.031). Clinical outcomes including Qmax, IPSS, V2, and PSA demonstrated significant enhancement during 1 year follow-up. Conclusion: Compared to HP-HoLEP, LP-HoLEP effectively reduces surgical and subsequent processing times, decreases hospital stay duration, and diminishes hemoglobin decline, offering a viable option without discontinuing AP therapy. Abstract

Predicting wake morphology during debris flow when passing a cylindrical obstacle is vital for disaster assessment, early warning, evacuation planning, engineering design, and ecologic conservation. It can provide a scientific foundation for pertinent decision-making processes, diminishing the risks and impacts of debris flow disasters. This study extracts the morphological characteristics of debris flow cylindrical flow traces through the steady-state motion of debris flow observed in a flume during cyclical tests. It introduces a theoretical prediction formula and compares it to empirical data. The results indicated that the morphology of debris flow cylindrical flow traces can be described as a wall-jet-like bow wave (a bow wave formed by an upward wall jet on the obstacle upstream face). The primary upstream inflow is predominantly discharged through the wall and lateral jets. Formulas for three crucial parameters that determine the morphology of the traces are derived by combining the aerodynamics theory and extant literature. Abstract

As a debris-flow control and mitigation countermeasure, flexible net barriers can effectively mitigate debris flows. The control and interception characteristics of flexible net barrier play an important role in engineering design. Many influencing factors exist in debris-flow flexible net barrier control and interception processes. In order to explore the most important factors, a flume-based experimental study was carried out by selecting the four main factors, i.e., the flume slope, debris flow bulk density, net barrier interval and relative volume. The purpose of the study is to analyze the influencing degree of the four factors. Moreover, the debris-flow interception ratio, blocking ratio, velocity reduction ratio and bulk density attenuation ratio are taken as the evaluation index. Based on the theory of orthogonal experimental design, the experiment results were analyzed in detail by range analysis and variance analysis. The research results indicated the following: in regard to the interception ratio, blocking ratio, and velocity reduction ratio, the net barrier interval exerted the most notable impact, followed by the bulk density. In regard to the bulk density attenuation ratio, the flume slope and bulk density were the first and second most important influencing factors, respectively. The form of interception ratio with maximum value was as follows: the flume slope was 9°, the net barrier interval was 18 mm, the bulk density was 21 kN/m3 and the relative volume was 2/3. The form of blocking ratio with minimum value was as follows: the flume slope was 6°, the net barrier interval was 50 mm, the bulk density was 12 kN/m3 and the relative volume was 1/2. The form of velocity reduction ratio with maximum value was as follows: the flume slope was 12°, the net barrier interval was 18 mm, the bulk density was 17 kN/m3 and the relative volume was 1. The form of bulk density attenuation ratio with maximum value was as follows: the flume slope was 12°, the net barrier interval was 30 mm, the bulk density was 17 kN/m3 and the relative volume was 1/3.
