RESUMEN La tomografía computarizada es una técnica no invasiva, de gran resolución espacial, con excelente definición del calcio y del espacio intravascular al emplear medios de contraste, que brinda la posibilidad de realizar tanto mediciones como simulaciones virtuales de intervencionismo coronario y estructural. Se ha establecido como la técnica de referencia en la selección de pacientes y la planificación de procedimientos de intervencionismo transcatéter coronario y estructural en diferentes escenarios (implante percutáneo de válvula aórtica, cierre percutáneo de orejuela izquierda, reemplazo de válvula mitral transcatéter y reemplazo de válvula tricúspide transcatéter). El presente trabajo revisa el papel de la tomografía computarizada en el intervencionismo cardiaco coronario y estructural.

ABSTRACT Computed tomography is a noninvasive imaging technique with high spatial resolution, providing excellent definition of calcium and intravascular space through the use of contrast media. This imaging modality allows both highly accurate measurements and virtual simulations for preprocedural planning in coronary and structural heart disease interventions. Computed tomography is currently the gold standard technique for patient selection and preprocedural planning in numerous scenarios, such as transcatheter aortic valve implantation, left atrial appendage occlusion, transcatheter mitral valve repair, and transcatheter tricuspid valve repair. This article reviews the role of computed tomography in transcatheter coronary and structural heart disease interventions.

RESOLUCIÓN DEL CASO El caso se presentó en sesión médico-quirúrgica y, dado el alto riesgo, la paciente fue aceptada para cierre percutáneo del seudoaneurisma1,2. El procedimiento se realizó bajo anestesia general y guiado por ecocardiografía transesofágica y fusión de imagen. Como la paciente era portadora de una prótesis mitral, se planificó una estrategia de abordaje retrógrado mediante acceso arterial femoral derecho (6 Fr). Con ayuda de un catéter AL 1 (Cordis Corporation, EE.UU.) y una guía recta Terumo (Terumo Medical Corporation, EE.UU.) se cruzó la válvula aórtica nativa, avanzando el AL 1 hacia el ventrículo izquierdo. Se realizó una ventriculografía izquierda con un catéter Pigtail (Cordis Corporation, EE.UU.), que mostró una cavidad en relación con el ventrículo izquierdo con paso de contraste hacia ella (vídeo 1 del material adicional). Tras varios intentos posicionando el catéter en el origen del defecto, finalmente se consiguió cruzar a la cavidad del seudoaneurisma con ayuda de una guía Terumo curva. En este paso fue crucial la información proporcionada por la fusión de imagen, ya que permitió marcar el origen del defecto y así facilitar el cruce de la guía a la cavidad (figura 1 y figura 2). CASE RESOLUTION The case was presented in the medical-surgical session and given the high risk involved the percutaneous closure of the pseudoaneurysm was decided.1-2 The procedure was performed under general anesthesia and guided by transesophageal echocardiography and fluoroscopic fusion imaging. Since the patient carried a prosthetic mitral valve, a retrograde approach strategy was decided via right femoral arterial access (6-Fr). With help from an AL 1 catheter (Cordis Corporation, FL, United States) and a Terumo straight guidewire (Terumo Medical Corporation, NJ, United States), the native aortic valve was crossed, and the AL 1 catheter was advanced towards the left ventricle. A Pigtail catheter was used to perform a left ventriculography (Cordis Corporation, FL, United States) that revealed a cavity in relation to the left ventricle with the passing of contrast towards such cavity (video 1 of the supplementary data). After several attempts trying to place the catheter at the origin of the defect, the pseudoaneurysm cavity was finally crossed using a curved Terumo guidewire. At this point, the information provided by fluoroscopic fusion imaging was crucial because it revealed the origin of the defect and facilitated guidewire crossing towards the cavity (figure 1 and figure 2). RESUMEN Introducción y objetivos: El cierre de la orejuela izquierda (COI) puede ser una alternativa de tratamiento eficaz para prevenir eventos cardiovasculares en pacientes con fibrilación auricular, en especial en aquellos con alto riesgo de sangrado. Sin embargo, algunos de estos pacientes en los que se realiza COI desarrollan trombosis relacionada con el dispositivo (TRD). Este estudio presenta las características del flujo sanguíneo que son clave en la formación de TDR, a partir de simulaciones personalizadas para cada paciente. Métodos: Para crear las simulaciones personalizadas se incluyeron en el estudio pacientes intervenidos de COI entre 2014 y 2019 en un único centro, de quienes se disponía de imágenes de tomografía computarizada previas al procedimiento y de seguimiento, así como de control ecocardiográfico. Para el COI se utilizaron los dispositivos Amulet. Las simulaciones se analizaron de forma ciega al diagnóstico de TRD. Resultados: En total se estudiaron 6 pacientes, de los que la mitad presentaban TRD según las imágenes del seguimiento clínico. Tras analizar los resultados de las simulaciones, los índices hemodinámicos asociados con TRD fueron la presencia de flujo estancado, las recirculaciones de sangre a velocidades bajas (< 0,20 m/s) cerca de la superficie del dispositivo y las regiones con alta complejidad de flujo y baja tensión de cizallamiento en la pared. Conclusiones: Las simulaciones de flujo personalizadas en pacientes con COI predijeron correctamente el diagnóstico clínico de TRD en todos los casos analizados. Los resultados obtenidos demuestran el potencial de los modelos personalizados para recomendar configuraciones óptimas del dispositivo y minimizar el riesgo de TRD.

ABSTRACT Introduction and objectives: Left atrial appendage occlusion (LAAO) can be an efficient treatment to prevent strokes in patients who suffer from atrial fibrillation, especially those at risk of bleeding. A non-negligible number of patients treated with LAAO develop device-related thrombosis (DRT) after device implantation. Our study aimed to identify the key blood flow characteristics leading to DRT using patient-specific flow simulations. Methods: Patients treated with LAAO between 2014 and 2019 at a single center with preoperative and follow-up computerized tomography images and ultrasound imaging (US) were used to create patient-specific flow simulations. Amulet LAAO devices were implanted in the study patients. Flow simulations were blindly assessed to discard the presence of DRT in the follow-up imaging. Results: A total of 6 patients were processed in this pivotal study, half of them with DRT at the follow-up according to the imaging analysis. After a comprehensive analysis of the simulations, the most relevant in silico indices associated with DRT were the presence of stagnant blood flow, recirculation with low flow velocities (< 0.20 m/s) next to the device surface, and regions with high flow complexity combined with low wall shear stress. Conclusions: Patient-specific flow simulations of LAAO were successfully used to predict blood flow patterns with different device configurations. The results show the potential of the present modelling and simulation approach to recommend optimal settings capable of minimizing the risk of DRT.

Sr. Editor: A pesar de que el uso del implante percutaneo de valvula aortica (TAVI) se ha extendido a pacientes de menor riesgo, la estancia hospitalaria tras TAVI sigue siendo larga, segun el registro espanol de TAVI se situa en torno a los 8 dias1. Estudios recientes confirman que el alta precoz tras un TAVI transfemoral con valvula balonexpandible resulta una opcion viable y segura2,3. La demanda sin precedentes que han soportado los servicios sanitarios durante la pandemia de COVID-19 ha obligado a redirigir los recursos y a restringir muchas intervenciones cardiovasculares para salvaguardar los limitados recursos disponibles tales como el soporte anestesico y los ventiladores, asi como las infraestructuras vinculadas a los cuidados criticos. El actual protocolo de alta precoz se desarrollo en respuesta a la pandemia de COVID-19 para valorar la seguridad y viabilidad que ofrece el alta precoz, en nuestro ambito, tras un TAVI minimamente invasivo con valvulas balon- o auto-expandibles. To the Editor, Despite the expansion of transcatheter aortic valve implantation (TAVI) to lower-risk patients, the length of stay after TAVI remains high for an average 8 days according to the Spanish TAVI registry.1 Recently published studies have demonstrated that early discharge following balloon-expandable transfemoral TAVI is feasible and safe.2,3 The unprecedented demand sustained by the healthcare services during the current COVID-19 pandemic has led to redirecting assets and restricting many cardiovascular procedures to protect the limited resources available like anesthesia support, ventilators, and critical care infrastructures. The present fast-track protocol was developed in response to the COVID-19 pandemic to assess the safety and feasibility of early discharge after minimalist TAVI with either balloon-expandable or self-expanding valves in our setting. Patients undergoing transfemoral TAVI were prospectively recruited. The inclusion criteria in the fast-track pathway were based on the 3M TAVI study: femoral access eligible for percutaneous closure, body mass index < 35, low-risk aortic annulus anatomy (coronary height > 10 mm, tricuspid valve, non-severe left ventricular outflow tract calcification), ejection fraction ≥ 30%, low anticipated risk of advanced conduction disturbances (PR interval < 240 ms, absence of right bundle branch block), familial support within the first 24-48 hours. RESUMEN La insuficiencia tricuspídea se asocia de manera independiente con un aumento de la mortalidad. Investigaciones recientes demuestran que la cirugía aislada de la válvula tricúspide presenta la mortalidad quirúrgica más alta dentro de los procedimientos valvulares. Las terapias percutáneas han surgido como una alternativa a la cirugía en pacientes seleccionados de alto riesgo quirúrgico. El dispositivo MitraClip en posición tricuspídea ha sido el más empleado. El propósito de este artículo es revisar los diferentes dispositivos transcatéter para el tratamiento de la insuficiencia tricuspídea, con especial énfasis en el MitraClip. Se describirán las vistas básicas de la exploración por imagen y el procedimiento paso a paso para la inserción del clip. Este documento pretende ser una guía para la sistematización de la evaluación de la válvula tricúspide y los pasos del implante del dispositivo para asegurar el éxito del procedimiento.

ABSTRACT Severe tricuspid regurgitation is independently associated with an increased mortality. Recent studies show that surgery for isolated tricuspid valve (TV) is still associated with the highest surgical risk of all valve procedures. Percutaneous treatments for the management of tricuspid regurgitation have become an alternative to surgery in select patients of high surgical risk. To this day, the most widely used device is MitraClip in the tricuspid position. We reviewed the different devices used today for transcatheter treatments paying special attention to MitraClip. We describe the basic standpoints on transesophageal echocardiography screening and step-by-step procedure for the implantation of the clip. This document stands as a guide to systematize tricuspid valve assessments and the steps involved in device implantation to achieve successful procedures.

Aim Our purpose was to evaluate by cardiovascular magnetic resonance (CMR) the presence of diffuse as well as focal MF in a series of high-performance veteran endurance athletes.Methods Thirty-four veteran healthy male endurance athletes, still being in regular training, with more than 10 years of training underwent a CMR. A cardiopulmonary exercise test was also performed to assess their maximal physical performance. The control group consisted in 12 non-trained normal individuals.Results We found an increase in both, right and left ventricular (LV) volumes in the athlete’s group when compared with controls. There was no increase in indexed LV myocardial mass despite of a significantly increased maximal myocardial wall thickness in comparison to controls. Native T1 values and extracellular volume (ECV) were normal in all cases. We did not find differences in native T1 values and ECV between both groups. In three athletes (9%), non-ischaemic late gadolinium enhancement (LGE) was observed. We did not find a correlation between total training volume and presence of LGE or with the ECV value.Conclusions Our results show that the majority of veteran endurance athletes present with myocardial remodelling without MF as a physiological adaptive phenomenon. In the only three athletes with focal MF, the LGE pattern observed suggests an intercurrent event not related with the remodelling phenomenon.
