Abstract Objectives To investigate whether utilizing a convolutional neural network (CNN)-based arterial input function (AIF) improves the volumetric estimation of core and penumbra in association with clinical measures in stroke patients. Methods The study included 160 acute ischemic stroke patients (male = 87, female = 73, median age = 73 years) with approval from the institutional review board. Transforaminal lumbar interbody fusion (TLIF) is a commonly used technique for treating lumbar degenerative diseases. In this study, we developed a fully computer-supported pipeline to predict both the cage height and the degree of lumbar lordosis subtraction from the pelvic incidence (PI-LL) after TLIF surgery, utilizing preoperative X-ray images. The automated pipeline comprised two primary stages. First, the pretrained BiLuNet deep learning model was employed to extract essential features from X-ray images. Subsequently, five machine learning algorithms were trained using a five-fold cross-validation technique on a dataset of 311 patients to identify the optimal models to predict interbody cage height and postoperative PI-LL. LASSO regression and support vector regression demonstrated superior performance in predicting interbody cage height and postoperative PI-LL, respectively. For cage height prediction, the root mean square error (RMSE) was calculated as 1.01, and the model achieved the highest accuracy at a height of 12 mm, with exact prediction achieved in 54.43% (43/79) of cases. In most of the remaining cases, the prediction error of the model was within 1 mm. Additionally, the model demonstrated satisfactory performance in predicting PI-LL, with an RMSE of 5.19 and an accuracy of 0.81 for PI-LL stratification. In conclusion, our results indicate that machine learning models can reliably predict interbody cage height and postoperative PI-LL.

Globally, sustainable development goals (SDGs), which need to be achieved using the sharing economy, are the focus of many recent studies. This article investigates the impact of sharing economy features such as core competence and resources on achieving SDGs in the tourism industry in the BRICS countries (Brazil, Russia, India, China, and South Africa). It examines the mediating impact of distinctive competitive advantage between the core competence and core resource features of a sharing economy, and achievement of the SDGs. The study also examines the moderating role of institutional support on the nexus of distinctive competitive advantage and achievement of SDGs in the BRICS countries. Questionnaires are used to collect primary data and Smart-PLS is used to analyse the data. The results indicate that the core competence and core resource features have a positive association with the achievement of SDGs, and that distinctive competitive advantage significantly mediates the association between core competence, core resources, and achievement of the SDGs. The outcomes reveal that institutional support significantly moderates the nexus of distinctive competitive advantage and achievement of SDGs. The research provides guidelines for regulators establishing policies related to the achievement of SDGs using the sharing economy.

The aim of this study was to propose a finite element method based numerical approach for evaluating various hallux valgus treatment strategies. We developed three-dimensional hallux valgus deformity models, with different metatarsal osteotomy methods and Kirschner wire fixation strategies, under two types of standing postures. Ten Kirschner wire fixations were analyzed and compared. The fixation stability, bone stress, implant stress, and contact pressure on the osteotomy surface were calculated as the biomechanical indexes. The results showed that the biomechanical indexes of the osteotomy and Kirschner wire fixations for hallux valgus deformity could be effectively analyzed and fairly evaluated. The distal metatarsal osteotomy method provided better biomechanical indexes compared to the proximal metatarsal osteotomy method. This study proposed a finite element method based numerical approach for evaluating various osteotomy and Kirschner wire fixations for hallux valgus deformity before surgery.

The incidence of humerus greater tuberosity (GT) fractures is about 20% in patients with proximal humerus fractures. This study aimed to investigate the biomechanical performances of the humerus GT fracture stabilized by a locking plate with rotator cuff function for shoulder rehabilitation activities. A three-dimensional finite element model of the GT-fracture-treated humerus with a single traction force condition was analyzed for abduction, flexion, and horizontal flexion activities and validated by the biomechanical tests. The results showed that the stiffness calculated by the numerical models was closely related to that obtained by the mechanical tests with a correlation coefficient of 0.88. Under realistic rotator cuff muscle loading, the shoulder joint had a larger displacement at the fracture site (1.163 mm), as well as higher bone stress (60.6 MPa), higher plate stress (29.1 MPa), and higher mean screw stress (37.3 MPa) in horizontal flexion rehabilitation activity when compared to that abduction and flexion activities. The horizontal flexion may not be suggested in the early stage of shoulder joint rehabilitation activities. Numerical simulation techniques and experimental designs mimicked clinical treatment plans. These methodologies could be used to evaluate new implant designs and fixation strategies for the shoulder joint.

A numerical approach is one of feasible ways to discover the biomechanics of hallux valgus deformity with various osteotomy and fixation strategies. In the present study, two types of finite element models for analyzing the biomechanical performances of hallux valgus treatment with plate fixations were developed including the single first metatarsal bone model and the musculoskeletal lower extremity model. There are four types of plate fixations that were used to correct the deformity of hallux valgus. The strengths and limitations of both the single first metatarsal bone model and the musculoskeletal lower extremity model were evaluated and discussed. The results revealed that the single metatarsal bone models can be used to quickly predict the biomechanical performances of different hallux valgus treatments. Additionally, the musculoskeletal lower extremity models can be used to predict the biomechanical performances of different hallux valgus treatments under a physiological loading. The plate fixations with the insertion of all locking screws revealed better osteotomy fixation stability and lower risk of the implant failure compared to the other fixations. Additionally, the plate fixations with the insertion of six bone screws had lower risk of the metatarsal bone failure compared to the plate fixations with the insertion of four bone screws. The six holes plate with the insertion of six locking screws was the best treatment among the four treatment strategies. The numerical models and simulation techniques developed in the present study can provide useful information for understanding the biomechanics of hallux valgus treatments.

Plate or nail fixations have been applied to the repair of clavicle fractures. However, it is quite difficult to fairly evaluate the different clavicle fixation techniques owing to variations in the bone anatomy, bone quality, and fracture pattern. The purpose of this study was to investigate the biomechanical performances of different fixation techniques applied to a clavicle fracture using the finite element method. A simplified single-clavicle model and a complete human upper-body skeleton model were developed in this study. Three types of plate fixations, namely, superior clavicle plate, anterior clavicle plate, and clavicle anatomic spiral fixations, and one nail fixation, a titanium elastic nail fixation, were investigated and compared. The plate fixation techniques have a better fixation stability compared to the nail fixation technique. However, the nail fixation technique shows lower bone stress and can reduce the risk of a peri-implant fracture compared to the plate fixation techniques. Increasing the number of locking screws for the clavicle plate system can reduce the implant stress. Insertion of the bone plate into the anterior site of the clavicle or a multi-plane fixation is recommended to achieve the required biomechanical performance. A plate fixation revealed a relatively better fixation stability, and a nail fixation showed a lower risk of a peri-implant fracture.

Evaluations are vital to quantify the functionalities of athletic footwear, such as the performance of slip resistance, shock absorption, and rebound. Computational technology has progressed to become a promising solution for accelerating product development time and providing customized products in order to keep up with the competitive contemporary footwear market. In this research, the effects of various tread pattern designs on traction performance in a normal gait were analyzed by employing an approach that integrated computational simulation and gait analysis. A state-of-the-art finite element (FE) model of a shoe was developed by digital sculpting technology. A dynamic plantar pressure distribution was automatically applied to interpret individualized subject conditions. The traction performance and real contact area between the shoe and the ground during the gait could be characterized and predicted. The results suggest that the real contact area and the structure of the outsole tread design influence the traction performance of the shoe in dry conditions. This computational process is more efficient than mechanical tests in terms of both cost and time, and it could bring a noticeable benefit to the footwear industry in the early design phases of product development.

Purpose: Anterior cervical discectomy and fusion is a common surgical procedure performed to remove a degenerative or herniated disc in cervical spine. Unfortunately, clinical complications of anterior cervical plate (ACP) systems still occur, such as weak fixation stability and implant loosening. Previous researchers have attempted to ameliorate these complications by varying screw orientations, but the screw orientations are mainly determined according to the investigator's experiences. Thus, the aim of this study was to discover the optimal screw orientations of ACP systems to achieve acceptable fixation stability using finite element simulations and engineering algorithms. Methods: Three-dimensional finite element models of C3-T2 multi-level segments with an ACP system were first developed to analyze the fixation stability using ANSYS Workbench 14.5. Then, artificial neural networks were applied to create one objective function, and the optimal screw orientations of an ACP system were discovered by genetic algorithms. Finally, the numerical models and the optimization study were validated using biomechanical tests. Results: The results showed that the optimal design of the ACP system had highest fixation stability compared with other ACP designs. The neuro-genetic algorithm has effectively reduced the time and effort required for discovering for the optimal screw orientations of an ACP system. Conclusions: The optimum screw orientation of the ACP system could be successfully discovered, and it revealed excellent fixation stability for the treatment of cervical degenerative disc disease. Anterior lumbar plate (ALP) systems have been widely used as an effective interbody fusion device for treating spinal cord compression. However, clinical complications, such as implant loosening and breakage, still occur. Past studies have investigated the effects of the screw orientation on the interfacial strength, but these studies were inconsistent. The purpose of this study was to identify an ALP system with excellent interfacial strength by varying the screw orientation. Three-dimensional finite element models of L4-L5 segments with an ALP system were first constructed. A neurogenetic algorithm, which combines artificial neural networks and genetic algorithms, was subsequently developed to discover the optimum plate design. Finally, biomechanical tests were conducted to validate the results of the finite element models and the engineering algorithm. The results indicated that the interfacial strength of the optimum plate design obtained using the neurogenetic algorithm was excellent compared with the other designs and that all of the locking screws should be inserted divergently. Abstract??TheUniversal Mobile Telecommunications System(UMTS) adopts the WCDMA technology as the radio access interface to provide variable transmission rate services. There are four classes of connections identified in UMTS, which are the conversational, streaming, interactive, and background connections. To efficiently utilize radio bandwidth, the shared channel approach is proposed to deliver the packets for the interactive and background connections. This paper proposes a ?Shared-Channel Assignment and Scheduling? (SCAS) algorithm to periodically allocate shared channels to serve interactive and background connections. We conduct formal mathematical proofs and simulation experiments to investigate the performance of the SCAS algorithm. We formally prove that with SCAS, a shared channel can be fully utilized (i.e., the utilization of a shared channel can be up to 100%) to serve the interactive connections. Our analysis indicates that compared with the previously proposed shared channel allocation and scheduling algorithms, there are less computation and communication overheads introduced in the SCAS algorithm. The results of the simulation experiments indicate that it is preferred to set up the Transmission Time Interval (TTI; that is, the unit of time interval for shared channel allocation) smaller to optimize the performance of the SCAS algorithm, including the shared channel utilization and the average waiting time of a connection before getting transmission service. In this paper, we propose a new novel polling-based medium access control protocol, named UPCF (Unified Point Coordination Function), to provide power conservation and quality-of-service (QoS) guarantees for multimedia applications over wireless local area networks. Specifically, UPCF has the following attractive features. First, it supports multiple priority levels and guarantees that high-priority stations always join the polling list earlier than low-priority stations. Second, it provides fast reservation scheme such that associated stations with real-time traffic can get on the polling list in bounded time. Third, it employs dynamic channel time allocation scheme to support CBR/VBR transportation and provide per-flow probabilistic bandwidth assurance. Fourth, it employs the power management techniques to let mobile stations save as much energy as possible. Fifth, it adopts the mobile-assisted admission control technique such that the point coordinator can admit as many newly flows as possible while not violating QoS guarantees made to already-admitted flows. The performance of UPCF is evaluated through both analysis and simulations. Simulation results do confirm that, as compared with the PCF in IEEE 802.11, UPCF not only provides higher goodput and energy throughput, but also achieves lower power consumption and frame loss due to delay expiry. Last but not least, we expect that UPCF can pass the current Wi-Fi certification and may coexist with the upcoming IEEE 802.11e standard. [ABSTRACT FROM AUTHOR]

The mesh-connected multiprocessor has become popular for its simple and regular structure. In this paper, a new data structure, the R-array, is proposed to represent the mesh at first. The element in the R-array stores the statistical information about the occupied conditions of the mesh. The statistical information of the R-array can direct the allocation process to jump to the processors that can serve as a base of a free submesh. Based on a simple and reasonable assumption, we develop a stochastic process to analyze the behaviors of the proposed scheme. The proposed scheme is the first one whose probabilities of locating free submeshes under different workloads are precisely computed. These results can be applied to each full-recognition scheme. In addition, the execution costs of the proposed scheme can also be accurately calculated. Finally, the simulations are performed.
