BACKGROUND: Hypertension is a serious global public health problem that affects a large part of the Brazilian adult population and can cause limitations and losses of quality of life. OBJECTIVE: The objective of this study was to analyze the association of hypertension and its correlated limitations, with sociodemographic and epidemiological factors. DESIGN AND SETTING: Cross-sectional study analyzing information on 44,271 adults (30 years or older) from the Brazilian National Health Survey of 2013. METHODS: The prevalence of hypertension and the degree of limitation of the patients’ activities associated with hypertension, according to sociodemographic characteristics, anthropometric measurements and lifestyles, were calculated for both sexes. To analyze the strength of association, bivariate and multivariate Poisson regression were used. RESULTS: Hypertension was the most prevalent risk factor among Brazilian adults aged 30 years or older (40.7%). It was strongly associated with the aging process (prevalence ratio, PR 3.51), obesity (PR 1.73), heart disease (PR 1.67) and stroke (PR 1.86). Furthermore, limitations associated with hypertension were more prevalent among those with comorbidities from noncommunicable diseases relating to hypertension complications (stroke PR 1.47; heart disease PR 1.69) and with incomplete elementary education (PR 1.19). CONCLUSIONS: This study showed sociodemographic inequality in the prevalence of hypertension, especially in the population with some degree of limitation associated with hypertension. It showed that improvements in access to primary care services for controlling hypertension at its initial stages are essential in order to avoid comorbidities of greater severity and limitations and losses of quality of life, especially among socially disadvantaged people.

RESUMO: Objetivo: Comparar as mudanças de estilos de vida durante a pandemia COVID-19, segundo a presença ou não de doenças crônicas não transmissíveis (DCNT) em adultos brasileiros. Métodos: Estudo transversal, com dados da pesquisa ConVid - Pesquisa de Comportamentos, realizada entre abril e maio de 2020. Avaliaram-se as variáveis estilo de vida e presença de uma ou mais DCNT (diabetes, hipertensão, doença respiratória, doença do coração e câncer). As características sociodemográficas foram usadas como ajuste. Calcularam-se as frequências relativas e os intervalos de confiança (IC) de 95% das variáveis antes da e durante a pandemia. Para a comparação de grupos, sem ou com DCNT, estimaram-se as prevalências e razões de prevalência bruta e ajustada (RPa) utilizando a regressão de Poisson. Resultados: Houve redução da prática de atividade física (60% nos sem DCNT e 58% nos com DCNT) e do consumo de hortaliças (10,8% nos sem DCNT e 12,7% nos com DCNT). Verificou-se aumento no tempo de uso de televisão e computador/tablet (302 e 43,5% nos sem DCNT e 196,5 e 30,6% nos com DCNT, respectivamente); consumo de congelados (43,6% nos sem DCNT e 53,7% com DCNT), salgadinhos (42,3% sem DCNT e 31,2% com DCNT) e chocolate (14,8% sem DCNT). Durante a pandemia, portadores de DCNT apresentaram menor prática de atividade física suficiente (RPa = 0,77; IC95% 0,65 - 0,92), maior hábito de assistir à televisão (RPa = 1,16; IC95% 1,08 - 1,26) e menor consumo de hortaliças (RPa = 0,88; IC95% 0,81 - 0,96). Conclusão: Evidenciou-se que adultos com DCNT tiveram seus estilos de vida mais alterados durante a pandemia de COVID-19.

OBJECTIVES To estimate the healthy life expectancy at 60 years by sex and Federated States and to investigate geographical inequalities by socioeconomic status. METHODS Healthy life expectancy was estimated by the Sullivan method, based on the information of the National Survey on Health, 2013. Three criteria were adopted for the definition of “unhealthy state”: self-assessment of bad health, functionality for performing the activities of daily living, and the presence of noncommunicable disease with intense degree of limitation. The indicator of socioeconomic status was built based on the number of goods at household and educational level of the head of household. To analyze the geographical inequalities and socioeconomic level, inequality measures were calculated, such as the ratio, the difference, and the angular coefficient. RESULTS Healthy life expectancy among men ranged from 13.8 (Alagoas) to 20.9 (Espírito Santo) for the self-assessment criterion of bad health. Among women, the corresponding estimates were always higher and ranged from 14.9 (Maranhão) to 22.2 (São Paulo). As to the ratio of inequality by Federated State, the medians were always higher for healthy life expectancy than for life expectancy, regardless of the definition adopted for healthy state. Regarding the differences per Federated State, the healthy life expectancy was seven years higher in one state than in another. By socioeconomic status, differences of three and four years were found, approximately, between the last and first fifth, for men and women, respectively. CONCLUSIONS Despite the association of the mortality indicators with living conditions, the inequalities are even more pronounced when the welfare and the limitations in usual activities are considered, showing the necessity to promote actions and programs to reduce the socio-spatial gradient. RESUMO OBJETIVO Estimar a esperança de vida saudável aos 60 anos por sexo e Unidade da Federação e investigar as desigualdades geográficas e por nível socioeconômico. MÉTODOS A esperança de vida saudável foi estimada pelo método de Sullivan, com base nas informações da Pesquisa Nacional de Saúde, 2013. Foram adotados três critérios para definição de estado “não saudável”: autoavaliação de saúde ruim, funcionalidade para realização das atividades da vida diária, e presença de doença crônica não transmissível com grau intenso de limitação. O indicador de nível socioeconômico foi construído com base no número de bens no domicílio e grau de escolaridade do responsável. Para analisar as desigualdades geográficas e por nível socioeconômico, foram calculadas medidas de desigualdade, como a razão, a diferença e o coeficiente angular. RESULTADOS A esperança de vida saudável entre os homens variou de 13,8 (Alagoas) a 20,9 (Espírito Santo) para o critério de autoavaliação de saúde ruim. Entre as mulheres, as estimativas correspondentes foram sempre mais altas e variaram de 14,9 (Maranhão) a 22,2 (São Paulo). Quanto à razão de desigualdades por Unidade da Federação, as medianas foram sempre maiores para a esperança de vida saudável do que para a esperança de vida, independentemente da definição adotada para estado saudável. Quanto às diferenças por Unidade da Federação, a esperança de vida saudável chegou a ser sete anos maior em um estado do que em outro. Por nível socioeconômico, foram encontradas diferenças de três e quatro anos, aproximadamente, entre os últimos e primeiro quintos, para homens e mulheres, respectivamente. CONCLUSÕES Além de os indicadores de mortalidade estarem associados às condições de vida, as desigualdades são ainda mais pronunciadas quando o bem-estar e as limitações nas atividades habituais são levados em consideração, mostrando a necessidade de promover ações e programas para diminuir o gradiente socioespacial.

Background The demographic shift and epidemiologic transition in Brazil have drawn attention to ways of measuring population health that complement studies of mortality. In this paper, we investigate regional differences in healthy life expectancy based on information from the National Health Survey (PNS), 2013. Methods In the survey, a three-stage cluster sampling (census tracts, households and individuals) with stratification of the primary sampling units and random selection in all stages was used to select 60,202 Brazilian adults (18 years and over). Healthy life expectancies (HLE) were estimated by Sullivan’s method according to sex, age and geographic region, using poor self-rated health for defining unhealthy status. Logistic regression models were used to investigate socioeconomic and regional inequalities in poor self-rated health, after controlling by sex and age. Results Wide disparities by geographic region were found with the worst indicators in the North and Northeast regions, whether considering educational attainment, material deprivation, or health care utilization. Life expectancy at birth for women and men living in the richest regions was 5 years longer than for those living in the less wealthy regions. Modeling the variation across regions for poor self-rated health, statistically significant effects (p < 0.001) were found for the North and Northeast when compared to the Southeast, even after controlling for age, sex, diagnosis of at least one non-communicable chronic disease, and schooling or socioeconomic class. Marked regional inequalities in HLE were found, with the loss of healthy life much higher among residents of the poorest regions, especially among the elderly. Conclusions By combining data on self-rated health status and mortality in a single indicator, Healthy Life Expectancy, this study demonstrated the excess burden of poor health experienced by populations in the less wealthy regions of Brazil. To mitigate the effects of social exclusion, the development of strategies at the regional level is essential to provide health care to all persons in need, reduce risk exposures, support prevention policies for adoption of healthy behaviors. Such strategies should prioritize population groups that will experience the greatest impact from such interventions.

OBJECTIVE To analyze the temporal evolution of the hospitalization of older adults due to ambulatory care sensitive conditions according to their structure, magnitude and causes. METHODS Cross-sectional study based on data from the Hospital Information System of the Brazilian Unified Health System and from the Primary Care Information System, referring to people aged 60 to 74 years living in the state of Rio de Janeiro, Souhteastern Brazil. The proportion and rate of hospitalizations due to ambulatory care sensitive conditions were calculated, both the global rate and, according to diagnoses, the most prevalent ones. The coverage of the Family Health Strategy and the number of medical consultations attended by older adults in primary care were estimated. To analyze the indicators’ impact on hospitalizations, a linear correlation test was used. RESULTS We found an intense reduction in hospitalizations due to ambulatory care sensitive conditions for all causes and age groups. Heart failure, cerebrovascular diseases and chronic obstructive pulmonary diseases concentrated 50.0% of the hospitalizations. Adults older than 69 years had a higher risk of hospitalization due to one of these causes. We observed a higher risk of hospitalization among men. A negative correlation was found between the hospitalizations and the indicators of access to primary care. CONCLUSIONS Primary healthcare in the state of Rio de Janeiro has been significantly impacting the hospital morbidity of the older population. Studies of hospitalizations due to ambulatory care sensitive conditions can aid the identification of the main causes that are sensitive to the intervention of the health services, in order to indicate which actions are more effective to reduce hospitalizations and to increase the population’s quality of life. OBJETIVO Analisar a evolução temporal da internação de idosos por condições sensíveis à atenção primária à saúde segundo sua estrutura, magnitude e causas. MÉTODOS Estudo transversal com base em dados do Sistema de Informação Hospitalar do Sistema Único de Saúde e do Sistema de Informação da Atenção Básica referentes a pessoas com idade entre 60 e 74 anos, residentes no estado do Rio de Janeiro. Foram calculadas a proporção e taxa de internações por condições sensíveis à atenção primária global e segundo diagnóstico mais prevalentes. Foram estimadas a cobertura da Estratégia de Saúde da Família e o número de consultas médicas realizadas por idosos na atenção primária. Para analisar o impacto dos indicadores nas internações foi realizado teste de correlação linear. RESULTADOS Encontrou-se acentuada redução das internações por condições sensíveis à atenção primária para todas as causas e grupos etários. Insuficiências cardíacas, doenças cerebrovasculares e pulmonares obstrutivas crônicas acumularam 50,0% das internações. Idosos com mais de 69 anos tiveram maior risco de internação por alguma dessas causas. Observou-se maior risco de internação entre os homens. Foi encontrada correlação negativa entre as internações e os indicadores de acesso à atenção primária. CONCLUSÕES A atenção primária em saúde no estado do Rio de Janeiro vem provocando impactos significativos na morbidade hospitalar da população idosa. Estudos das internações por condições sensíveis à atenção primária podem auxiliar na identificação das principais causas sensíveis à intervenção dos serviços de saúde, indicando quais ações são mais efetivas para a diminuição das internações e o aumento da qualidade de vida da população.

To evaluate the adequacy of the Clinical-Functional Vulnerability Index-20, a rapid triage instrument to test vulnerability in Brazilian older adults, for the use in primary health care.The study included convenience sample of 397 patients aged older than or equal to 60 years attended at Centro de Referência para o Idoso (Reference Center for Older Adults) and of 52 older adults the same age attended at the community. The results of the questionnaire, consisting of 20 questions, were compared with those of the Comprehensive Geriatric Assessment, considered a reference for identifying frail older adults. Spearman's correlation was evaluated in the Clinical-Functional Vulnerability Index-20 with the Comprehensive Geriatric Assessment; the validity was verified by the area under the ROC curve; reliability was estimated by the percentage of agreement among evaluators and by the kappa coefficient, both with quadratic weighted. The cut-off point was obtained based on the higher accuracy criterion. Cronbach's alpha, a measure of internal consistency, was estimated.The Spearman's correlation coefficient was high and positive for both groups (0.792 for older adults attended at the Reference Center and 0.305 for older adults from the community [p0.001]). The area under the ROC curve for older adults attended at the Reference Center was substantial (0.903). The cut-off point obtained was six, and older adults with scores in Clinical-Functional Vulnerability Index-20 above that value had strong possibility of being frail. For older adults from the community, the quadratic weighted agreement among evaluators was 99.5%, and the global quadratic weighted kappa coefficient was 0.94. Cronbach's alpha was high for older adults attended at the Reference Center (0.861) and those attended at the community (0.740).The Clinical-Functional Vulnerability Index-20 questionnaire, in the sample examined, turned out to be positively correlated with the Comprehensive Geriatric Assessment, in addition to the results indicating a high degree of validity and reliability. Thus, the Clinical-Functional Vulnerability Index-20 proves to be viable as a triage instrument in the primary health care that identifies frail older adults (older adults at risk of weakening and frail older adults).Avaliar a adequação do Índice de Vulnerabilidade Clínico-Funcional-20 , instrumento de triagem rápida de vulnerabilidade em idosos brasileiros, para utilização pela atenção básica. O estudo incluiu amostra de conveniência de 397 pacientes com idade maior ou igual a 60 anos atendidos em um Centro de Referência para o Idoso e de 52 idosos da mesma idade atendidos na comunidade. Os resultados do questionário, constituído por 20 perguntas, foram comparados com aqueles da Avaliação Geriátrica Ampla, considerada referência para identificação do idoso frágil. Foi avaliada a correlação de Spearman do Índice de Vulnerabilidade Clínico-Funcional-20 com a Avaliação Geriátrica Ampla; a validade foi verificada pela área sob a curva ROC; a confiabilidade foi estimada pelo percentual de concordância entre avaliadores e coeficiente kappa, ambos com ponderação quadrática. Obteve-se ponto de corte com base no critério de maior acurácia. O alfa de Cronbach, medida de consistência interna, foi calculado. O coeficiente de correlação de Spearman foi elevado e positivo em ambos os grupos (0,792 para idosos atendidos no Centro de Referência para o Idoso e 0,305 para idosos da comunidade [p0,001]). A área sob a curva ROC para idosos atendidos no Centro de Referência para o Idoso foi substancial (0,903). O ponto de corte obtido foi seis e idosos com pontuação no Índice de Vulnerabilidade Clínico-Funcional-20 acima desse valor tinham forte possibilidade de serem frágeis. Para idosos da comunidade, a concordância ponderada quadrática entre avaliadores foi 99,5% e o coeficiente kappa ponderado quadrático global, 0,94. O alfa de Cronbach foi elevado para idosos atendidos no Centro de Referência para o Idoso (0,861) e da comunidade (0,740).O questionário do Índice de Vulnerabilidade Clínico-Funcional-20, na amostra analisada, mostrou ser positivamente correlacionado com a Avaliação Geriátrica Ampla, além de os resultados indicarem alto grau de validade e confiabilidade. Assim, o Índice de Vulnerabilidade Clínico-Funcional-20 se mostra viável como instrumento de rastreio na atenção básica que identifica o idoso com fragilidade (idoso em risco de fragilização e idoso frágil).
