Preparing special education preservice teachers (PTs) to serve students with autism includes understanding theoretical foundations of teaching strategies and developing competency in applying the resulting knowledge and strategies within teaching settings. The current pilot study explored the Internet-Based Parent-implemented Communication Strategies (i-PiCS) program, targeting evidence-based teaching practices, as an alternative training method in an autism field-experience course conducted online during the COVID-19 lockdown. The purpose of the pilot study was to explore changes in PTs' knowledge, confidence, and implementation of naturalistic communication teaching strategies in a pre- and postfield-experience course. The participants were nine females enrolled in a 3rd-year special education teacher preparation program. A multimethod assessment was utilized, including self-rating questions, knowledge quizzes, video-model analysis, and videotaped teaching simulations. Quantitative data were graphically displayed and compared. Teaching simulations were analyzed qualitatively. The results showed that all PTs improved the identification of instructional steps. Self-rating of knowledge and confidence level improved as well. Five PTs improved the accuracy of evaluating the instructional steps, albeit the level of fidelity of implementation improved only mildly. We conclude that i-PiCS could be useful for training PTs in a special education program. The development of nursing evidence within hemodialysis is challenged by the vast numbers of specialized tasks clinicians undertake. Developing an evidence base is complex and multi-faceted, requiring a collaborative and well-defined process. Commonly, a process of quality improvement is initially undertaken, whereby nurses develop a plan, undertake action, study outcomes, and review results. However, when reviewing current processes to identify and develop evidence-based practices across various health services, a more detailed and planned methodology is required. This article provides nurses with knowledge to apply a well-defined framework (the Iowa Model-Revised of Evidence-Based Practice to Promote Excellence in Health Care) to transform a quality improvement project into the development of evidence-based practice. The updated Integrated Framework for Clinical Decision Making responds to changes in evidence, policy, and practice since the publication of the first version in 2008. The original framework was proposed for persons with neurological health conditions, whereas the revised framework applies to persons with any health condition across the lifespan. In addition, the revised framework (1) updates patient-centered concepts with shared clinical decision-making; (2) frames the episode of care around the patient's goals for participation; (3) explicitly describes the role of movement science; (4) reconciles movement science and International Classification of Function language, illustrating the importance of each perspective to patient care; (5) provides a process for movement analysis of tasks; and (6) integrates the movement system into patient management. Two cases are used to illustrate the application of the framework: (1) a 45-year-old male bus driver with low back pain whose goals for the episode of care are to return to work and recreational basketball; and (2) a 65-year-old female librarian with a fall history whose goals for the episode of care are to return to work and reduce future falls. The framework is proposed as a tool for physical therapist education and to guide clinical practice for all health conditions across the lifespan. Objective: To describe the cost of using evidence-based implementation strategies for sustained behavioral health integration (BHI) involving population-based screening, assessment, and identification at 25 primary care sites of Kaiser Permanente Washington (2015-2018). Data Sources/Study Setting: Project records, surveys, Bureau of Labor Statistics compensation data. Study Design: Labor and nonlabor costs incurred by three implementation strategies: practice coaching, electronic health records clinical decision support, and performance feedback. Data Collection/Extraction Methods: Personnel time spent on these strategies was estimated for five broad roles: (a) project leaders and administrative support, (b) practice coaches, (c) clinical decision support programmers, (d) performance metric programmers, and (e) primary care local implementation team members. Principal Finding: Implementation involved 286 persons, 18 131 person-hours, costing $1 587 139 or $5 per primary care visit with screening or $38 per primary care visit identifying depression, suicidal thoughts and/or alcohol or substance use disorders, in a single year. The majority of person-hours was devoted to project leadership (35%) and practice coaches (34%), and 36% of costs were for the first three sites. Conclusions: When spread across patients screened in a single year, BHI implementation costs were well within the range for commonly used diagnostic assessments in primary care (eg, laboratory tests). This suggests that implementation costs alone should not be a substantial barrier to population-based BHI. Objective. The aim of this systematic review was to determine what percentages of physical therapists provide interventions that are of high value, low value, or unknown value for cardiorespiratory conditions, neurological conditions, or conditions requiring acute hospital care. Whether an intervention was considered high or low value was determined by reference to guidelines or systematic reviews. Methods. Searches of numerous databases were performed by combining terms synonymous with 'practice patterns' and 'physical therapy' until April 2018. Studies that investigated what interventions physical therapists provide for any cardiorespiratory condition, neurological condition, or condition requiring acute hospital care through surveys and audits of clinical notes were included. Through the use of medians and interquartile ranges, the percentages of physical therapists who provided interventions that were of high value, low value, or unknown value were summarized. Results. Twenty-six studies were included. The median percentages of physical therapists who provided interventions of high, low, and unknown value for chronic obstructive pulmonary disease ranged from 78% to 96%, 67% to 100%, and 56% to 91%, respectively. These percentages ranged from 61% to 97%, 87% to 98%, and 83% to 98% for adults who were critically ill in intensive care units; 70% to 93%, 38% to 50%, and 8% to 95% before or after cardiac/thoracic surgery; 25% to 96%, 23% to 84%, and 96% for acute stroke; and 11% (high value) and 13% (unknown value) for Parkinson disease, respectively. Conclusions. This review found patterns of physical therapist practice for cardiorespiratory conditions, neurological conditions, and conditions requiring acute hospital care that were both evidence based and not evidence based. A concern is that a substantial percentage of physical therapists provided interventions that were of low or unknown value despite the availability of high-value interventions. Impact. This systematic review is the first, to our knowledge, to summarize the percentage of physical therapist treatment choices that were high versus low value for cardiorespiratory conditions, neurological conditions, and conditions requiring acute hospital care. The findings highlight areas of practice where low-value care could be replaced with high-value care--such as in the management of patients who have chronic obstructive pulmonary disease or who are in intensive care--and identify an urgent need to develop and test strategies to ensure that patients with these conditions receive the interventions most likely to improve their outcomes., High- and low-value health care is a concept receiving substantial attention in many fields of medicine. This study analyzed PRISM unit applications to identify key characteristics of units that did and did not achieve recognition. Results may provide unit leaders with a roadmap for achieving excellence in medical-surgical patient care. Aim Analyze PRISM award applications for key characteristics of medical-surgical units awarded PRISM designation. Characteristics assessed in award applicants included six domains: leadership, recruitment and retention, evidence-based practice, quality patient outcomes, work environment, and lifelong learning. Method The sample consisted of 75 applications from 50 unique units representing 27 applications of awarded and 23 applications of non-awarded units. Content analysis was used to analyze applications for themes within each of the application categories. Results Six themes (support and empowerment, a welcoming environment, a foundation for professional practice, providing quality for patients and families, a positive place to work, commitment to professionalism) emerged consistent with the question prompts representing six domains from the PRISM application. Saturation was reached with 13 applications. Limitations and Implications Themes were influenced highly by application question prompts. Few differences were identified among awarded and non-awarded applications. The major difference was in depth of description and examples illustrating clinical nurses in leadership roles in awarded units.
