Study Design: Retrospective review of private neuromonitoring databases. Objectives: To review neuromonitoring alerts in a large series of patients undergoing lateral lumbar interbody fusion (LLIF) and determine whether alerts occurred more frequently when more lumbar levels were accessed or more frequently at particular lumbar levels. Methods: Intraoperative neuromonitoring (IONM) databases were reviewed and patients were identified undergoing LLIF between L1 and L5. All cases in which at least one IONM modality was used (motor evoked potentials (MEP), somatosensory evoked potentials (SSEP), evoked electromyography (EMG)) were included in this study. The type of IONM used and incidence of alerts were collected from each IONM report and analyzed. The incidence of alerts for each IONM modality based on number of levels at which at LLIF was performed and the specific level an LLIF was performed were compared. Results: A total of 628 patients undergoing LLIF across 934 levels were reviewed. EMG was used in 611 (97%) cases, SSEP in 561 (89%), MEP in 144 (23%). The frequency of IONM alerts for EMG, SSEP and MEPs did not significantly increase as the number of LLIF levels accessed increased. No EMG, SSEP, or MEP alerts occurred at L1-L2. EMG alerts occurred in 2-5% of patients at L2-L3, L3-L4, and L4-L5 and did not significantly vary by level (P =.34). SSEP and MEP alerts occurred more frequently at L4-L5 versus L2-L3 and L3-L4 (P <.03). Conclusions: IONM may provide the greatest utility at L4-L5, particularly MEPs, and may not be necessary for more cephalad LLIF procedures such as at L1-L2. [ABSTRACT FROM AUTHOR]

Resumen: Introducción: La instrumentación con tornillos pediculares (TP) puede comprometer las estructuras neurales adyacentes. La electromiografía evocada (tEMG) es una modalidad del neuromonitoreo intraoperatorio utilizada para asistir la colocación de estos. El objetivo del presente estudio es evaluar la confiabilidad de esta herramienta. Método: Revisión retrospectiva de pacientes intervenidos de artrodesis lumbar posterolateral instrumentada desde enero del 2017 hasta diciembre del 2019. Se correlacionaron las imágenes de TC postoperatorias (TC-postop) con los resultados de la tEMG para cada TP con un umbral de 10 mA, y se establecieron la sensibilidad y especificidad de esta herramienta. Background Context: Total removal of spinal schwannomas is ideal but it sometimes requires tumor-involved root resection, which increases the risk of postoperative motor deterioration (PMD). Therefore, it is important for clinicians to predict the impact of tumor-involved root resection on motor function in spinal schwannomas.Purpose: To investigate the role of intraoperative electromyographic (EMG) techniques in decision-making of tumor-involved root resection for treating spinal schwannomas.Study Design: A retrospective analysis PATIENT SAMPLE: Sixty-eight patients with spinal schwannomas arising from C5-T1 or L3-S1 roots underwent total resection of schwannoma, including tumoral root.Outcome Measures: Nerve root activation threshold, free-running EMG signals, visual analogue scale, and American Spinal Injury Association scale.Methods: During evoked EMG, nerve root activation threshold for tumoral root stimulation was recorded from muscles anatomically corresponding to tumoral root. During free-running EMG, abnormal EMG signals were identified as irregularly recurrent, monomorphic signals, low frequency (<5 Hz) or absent discharges recorded from muscles innervated by tumoral root. Clinical assessments were performed before, 3 to 5 day's and six months' after operation.Results: Sixteen (16 of 68, 23.5%) patients showed PMD, and muscle strength improved or was not affected in the other 52 patients. Absent myogenic responses were observed in 19 patients with non-PMD, and nerve root activation threshold in non-PMD group was higher than that in PMD group (p<.05). Receiver operating characteristic curve revealed that cut-off value of nerve root activation threshold for distinguishing functional and nonfunctional roots was 11.8 mA. A larger number of patients without PMD than with PMD showed abnormal free-running EMG signals (p<.05). At postoperative 6-months' follow-up, ten patients with muscle weakness after tumor-involved root resection showed functional recovery (full vs. partial recovery: 5 vs. 5), and intraoperative nerve root activation threshold in these patients was higher than that in the other patients without functional recovery (p<.05). Furthermore, there is negative relationship between the duration for full recovery and nerve root activation threshold (p<.05).Conclusions: Both evoked and free-running EMG can be used as supplementary tests for differentiating functional and nonfunctional tumoral roots in spinal schwannomas, and nerve root activation threshold may be also related to prognosis of patients with muscle weakness caused by tumor-involved root resection. Objectives/Hypothesis: In‐office recurrent laryngeal nerve conduction studies (NCSs) are a technique that can potentially provide information about laryngeal innervation. NCS is essential in the management of other neuropathies including carpal tunnel syndrome and spinal cord injury. We hypothesize that laryngeal NCS may have similar utility in managing patients with vocal fold paralysis, atrophy, and neurodegenerative disease. NCSs are technically challenging because they require transcervical stimulation of the recurrent laryngeal nerve (RLN). This study combines radiographic data with cadaveric dissection to describe the anatomic parameters for optimal RLN stimulation. Study Design: Radiographic and Cadaveric Study. Methods: Fifty computed tomography scans were reviewed to determine the dimensions for ideal needle electrode placement. These values were compared to measurements from 12 fresh human cadaveric neck dissections. Ultrasound imaging was utilized in select cases. The neck was dissected to assess the accuracy of electrode placement. Results: Radiographically, the mean transcervical depth to the RLN was 33.2 mm ± 8.3 mm in males versus 29.4 mm ± 9.4 mm in females. The working space between the lateral trachea and carotid artery was 15.3 mm ± 3.6 mm on the right and 14.1 mm ± 2.9 mm on the left. After placement of stimulating electrodes into the cadaveric neck, the electrode tips were consistently within 8 mm of the RLN. Ultrasound guidance improved placement accuracy of the stimulating electrode. Conclusions: Laryngeal NCSs can provide detailed and objective information about laryngeal innervation that could dramatically improve the management of various neuropathies. In‐office NCSs require technical precision, and this study describes anatomic factors that may affect the feasibility of performing this technique. Study Design: Retrospective observational study.Objective: It has been shown that pedicle screw stimulation thresholds less than or equal to 8 mA yield a very high diagnostic accuracy of detecting misplaced screws in spinal surgery. In our study, we determined clinical implications of low stimulation thresholds.Summary Of Background Data: Posterior lumbar spinal fusions (PSF), using pedicle screws, are performed to treat many spinal pathologies, but misplaced pedicle screws can result in new postoperative neurological deficits.Methods: Patients with pedicle screw stimulation testing who underwent PSF between 2010 and 2012 at the University of Pittsburgh Medical Center (UPMC) were included in the study. We evaluated the sensitivity, specificity, and diagnostic odds ratio (DOR) to determine how effectively low pedicle screw responses predict new postoperative lower extremity neurological deficits.Results: One thousand one hundred seventy nine eligible patients underwent 8584 pedicle screw stimulations with lower extremity somatosensory evoked potentials (LE SSEP) monitoring for lumbar fusion surgery. One hundred twenty one of these patients had 187 pedicle screws with a stimulation response at a threshold less than or equal to 8 mA. Smoking had a significant correlation to pedicle screw stimulation less than or equal to 8 mA (P = 0.012). A threshold of less than or equal to 8 mA had a sensitivity/specificity of 0.32/0.90 with DOR of 4.34 [1.83, 10.27] and an area under the ROC curve (AUC) of 0.61 [0.49, 0.74]. Patients with screw thresholds less than or equal to 8 mA and abnormal baselines had a DOR of 9.8 [95% CI: 2.13-45.17] and an AUC of 0.73 [95% CI: 0.50-0.95].Conclusion: Patients with pedicle screw stimulation thresholds less than or equal to 8 mA are 4.34 times more likely to have neurological clinical manifestations. Smoking and LE deficits were shown to be significantly correlated with pedicle screw stimulation thresholds less than or equal to 8 mA. Low stimulation thresholds result in a high specificity of 90%. Pedicle screw stimulation less than or equal to 8 mA can serve as an accurate rule in test for postoperative neurological deficit, warranting reevaluation of screw placement and/or replacement intraoperatively.Level of Evidence: 3. Summary Objectives Pedicle screw testing is a widely used technique in the field of neuromonitoring for spinal surgery. It was designed by Calancie et al. (1992) in order to detect pedicle breach, one of the major complications of pedicle screw fixation, which can lead to neurological impairment. However, numerous false negative and equivocal results led to its clinical relevance being questioned. We aimed to clarify these discrepancies and characterize electrical parameters underlying this technique. Methods In this setting, our study is divided into two parts: (1) a clinical part assessing the difference between direct pedicle hole via the pedicle perforator stimulation and indirect stimulation via the implanted screw; (2) an in vitro study testing the electrical properties (resistivity and conductivity) on a sample of different commonly used pedicle screws. Results We showed that there were discrepancies between direct perforator stimulation and pedicle screws, especially at high threshold values. These might be attributed to electrical contact discontinuity. In vitro testing revealed that the previously described resistivity variability and the instability of measures are due to the result of slight changes of position of the recording contacts. Conclusion Electrical continuity is crucial in the pedicle screw test technique. This parameter cannot actually be fully ensured and can lead to discrepancies and potentially false negative results. Therefore, we recommend the use of both direct stimulation of the pedicle hole and control testing of the screw. Time-variant muscle responses under electrical stimulation (ES) are often problematic for all the applications of neuroprosthetic muscle control. This situation limits the range of ES usage in relevant areas, mainly due to muscle fatigue and also to changes in stimulation electrode contact conditions, especially in transcutaneous ES. Surface electrodes are still the most widely used in noninvasive applications.Electrical field variations caused by changes in the stimulation contact condition markedly affect the resulting total muscle activation levels. Fatigue phenomena under functional electrical stimulation (FES) are also well known source of time-varying characteristics coming from muscle response under ES. Therefore it is essential to monitor the actual muscle state and assess the expected muscle response by ES so as to improve the current ES system in favour of adaptive muscle-response-aware FES control. To deal with this issue, we have been studying a novel control technique using evoked electromyography (eEMG) signals to compensate for these muscle time-variances under ES for stable neuroprosthetic muscle control. In this perspective article, I overview the background of this topic and highlight important points to be aware of when using ES to induce the desired muscle activation regardless of the time-variance. I also demonstrate how to deal with the common critical problem of ES to move toward robust neuroprosthetic muscle control with the Evoked Electromyographically Controlled Electrical Stimulation paradigm.

Abstract: Background: Muscles innervated by the facial nerve show different sensitivities to muscle relaxants than muscles innervated by somatic nerves, especially in the presence of facial nerve injury. We compared the evoked electromyography (EEMG) response of orbicularis oris and gastrocnemius in with and without a non-depolarizing muscle relaxant in a rabbit model of graded facial nerve injury. Methods: Differences in EEMG response and inhibition by rocuronium were measured in the orbicularis oris and gastrocnemius muscles 7 to 42 d after different levels of facial nerve crush injuries in adult rabbits. Results: Baseline EEMG of orbicularis oris was significantly smaller than those of the gastrocnemius. Gastrocnemius was more sensitive to rocuronium than the facial muscles (P < 0.05). Baseline EEMG and EEMG amplitude of orbicularis oris in the presence of rocuronium was negatively correlated with the magnitude of facial nerve injury but the sensitivity to rocuronium was not. No significant difference was found in the onset time and the recovery time of rocuronium among gastrocnemius and normal or damaged facial muscles. Conclusions: Muscles innervated by somatic nerves are more sensitive to rocuronium than those innervated by the facial nerve, but while facial nerve injury reduced EEMG responses, the sensitivity to rocuronium is not altered. Partial neuromuscular blockade may be a suitable technique for conducting anesthesia and surgery safely when EEMG monitoring is needed to preserve and protect the facial nerve. This paper proposed a closed-loop torque control strategy of functional electrical stimulation (FES) with the aim of obtaining an accurate, safe, and robust FES system. Generally, FES control systems are faced with the challenge of how to deal with time-variant muscle dynamics due to physiological and biochemical factors (such as fatigue). The degraded muscle force needs to be compensated in order to ensure the accuracy of the motion restored by FES. Another challenge concerns the fact that implantable sensors are unavailable to feedback torque information for FES in humans. As FES-evoked electromyography (EMG) represents the activity of stimulated muscles, and also enables joint torque prediction as presented in our previous studies, here we propose an EMG-feedback predictive controller of FES to control joint torque adaptively. EMG feedback contributes to taking the activated muscle state in the FES torque control system into account. The nature of the predictive controller facilitates prediction of the muscle mechanical response and the system can therefore control joint torque from EMG feedback and also respond to time-variant muscle state changes. The control performance, fatigue compensation and aggressive control suppression capabilities of the proposed controller were evaluated and discussed through experimental and simulation studies. Unequivocally, pedicle screw instrumentation has evolved as a primary construct for the treatment of both common and complex spinal disorders. However, an inevitable and potentially major complication associated with this type of surgery is misplacement of a pedicle screw(s) which may result in neural and vascular complications, as well as impair the biomechanical stability of the spinal instrumentation resulting in loss of fixation. In light of these potential surgical complications, critical reviews of outcome data for treatment of chronic, low-back pain using pedicle screw instrumentation concluded that "pedicle screw fixation improves radiographically demonstrated fusion rates; " however, the expense and complication rates for such constructs are considerable in light of the clinical benefit (Resnick et al. 2005a). Currently, neuromonitoring using free-run and evoked (triggered) electromyography (EMG) is widely used and advocated for safer and more accurate placement of pedicle screws during open instrumentation procedures, and more recently, guiding percutaneous placement (minimally invasive) where the pedicle cannot be easily inspected visually. The latter technique, evoked or triggered EMG when applied to pedicle screw instrumentation surgeries, has been referred to as the pedicle screw stimulation technique. As concluded in the Position Statement by the American Society of Neurophysiological Monitoring (ASNM), multimodality neuromonitoring using free-run EMG and the pedicle screw stimulation technique was considered a practice option and not yet a standard of care (Leppanen 2005). Subsequently, the American Association of Neurological Surgeons/Congress of Neurological Surgeons (AANS/CNS) Joint Section on Disorders of the Spine and Peripheral Nerves published their "Guidelines for the Performance of Fusion Procedures for Degenerative Disease of the Lumbar Spine" (Heary 2005, Resnick et al. 2005a, Resnick et al. 2005b). It was concluded that the "primary justification " of intraoperative neuromonitoring "... is the perception that the safety and efficacy of pedicle screw fixation are enhanced..." (Resnick et al. 2005b). However, in summarizing a massive (over 1000 papers taken from the National Library of Medicine), contemporary, literature review spanning nearly a decade (1996 to 2003), this invited panel (Resnick et al. 2005b) recognized that the evidence-based documents contributing to the parts related to pedicle screw fixation and neuromonitoring were "... full of potential sources of error..." and lacked appropriate, randomized, prospective studies for formulating rigid standards and guidelines. Nevertheless, current trends support the routine use and clinical utility of these neuromonitoring techniques. In particular, free-run and triggered EMG have been well recognized in numerous publications for improving both the accuracy and safety of pedicle screw implantation. Currently, treatment with pedicle screw instrumentation routinely involves all levels of the spine - lumbosacral, thoracic, and cervical. Significant historical events, various neuromonitoring modalities, intraoperative alarm criteria, clinical efficacy, current trends, and caveats related to pedicle screw stimulation along the entire vertebral column will be reviewed. Summary: We have proposed a hypothetical mechanism to involuntarily sustain the effective eyelid retraction, which consists of not only voluntary but also reflexive contractions of the levator palpebrae superior muscle (LPSM). Voluntary contraction of fast-twitch fibres of the LPSM stretches the mechanoreceptors in Mueller''s muscle to evoke trigeminal proprioception, which induces continuous reflexive contraction of slow-twitch fibres of the LPSM through the trigeminal proprioceptive nerve fibres innervating the mechanoreceptors in Mueller''s muscle via the oculomotor neurons, as a tonic trigemino-oculomotor reflex. In the common skeletal mixed muscles, electrical stimulation of the proprioceptive nerve, which afferently connects the mechanoreceptors in muscle spindles to the motoneurons, induces the electromyographic response as the Hoffmann reflex. To verify the presence of the trigemino-oculomotor reflex, we confirmed whether intra-operative electrical simulation of the transverse trigeminal proprioceptive nerve on the proximal Mueller''s muscle evokes an electromyographic response in the LPSM under general anaesthesia in 12 patients. An ipsilateral, phasic, short-latency response (latency: 2.8±0.3ms) was induced in the ipsilateral LPSM in 10 of 12 subjects. As successful induction of the short-latency response in the ipsilateral LPSM corresponds to the Hoffmann reflex in the common skeletal mixed muscles, the present study is the first electromyographic verification of the presence of the monosynaptic trigemino-oculomotor reflex to induce reflexive contraction of the LPSM. The presence of the trigemino-oculomotor reflex may elucidate the unexplainable blepharoptosis due to surgery, trauma and tumour, all of which may damage the trigeminal proprioceptive nerve fibres to impair the trigemino-oculomotor reflex. The purpose of this study was to evaluate the ability of electroneuronography (ENOG), also called evoked electromyography (EEMG), and facial nerve latency testing (FNLT) to assess the prognosis of facial nerve palsy, using the House-Brackmann facial nerve grading system as criterion. From 1988 to 1994 these tests were employed at the ORL Clinic of the University of Ioannina in 250 patients with idiopathic facial nerve palsy. The ENOG test results indicated that when the amplitude of the compound muscle action potentials ranged from 51% to 95% of the normal value, 97% of the patients achieved complete functional recovery (grade 1) within at least 2 months. When the muscle action potential decreased to a value below 51% of normal values, prognosis for recovery was considerably worse. FNLT test results indicated that as the latency time extended, the recovery grade of the facial nerve worsened. When latency time was within the normal range (group A patients), about 92% of patients had complete functional recovery. In contrast all patients having either a very extended latency time or unable to be monitored (groups C and D) demonstrated incomplete functional recoveries that ranged from grade 11 to grade VI. Comparing each test with the House-Brackmann facial nerve grading system, we ascertained that the percent accuracy for ENOG was 97.6%, and that for FNLT was 94.4%. [ABSTRACT FROM AUTHOR]

We report our results from the application of evoked electromyography (EEMG) and facial nerve latency testing (FNLT) in 30 children aged 4-14 years with idiopathic facial palsy. Our aim was to define the value of these tests as prognostic tools in Bell's palsy. From the EEMG results we ascertained that, when the amplitude of the compound muscle action potential varied between 51% and 95% of the normal value, the neuronal damage is slight (neurapraxia). When the percentage value of muscle response decreases, the prognosis is worse. The results of the FNLT showed that, when the latency is within normal limits, the damage to the nerve is slight (neurapraxia). If the latency is prolonged, the prognosis is worse. Evaluation of our results and comparison with the index of facial nerve functional recovery showed that those two tests have a high percentage of correctness (100% for the EEMG and 96.7% for the FNLT) and a low percentage of error. We conclude that these tests are excellent for predicting the outcome of facial nerve palsy in childhood and we suggest that young patents undergo both so that the determination of the lesion may be as correct as possible.
