Objective: Periodontitis is a type of prevalent chronicinflammatory disorder resulting in a failure in the function oftissues supporting the tooth, like gingiva, alveolar bone, andperiodontal ligament. Although antibiotic therapy is a commontherapy for periodontitis cases, this approach can cause someadverse effects in these patients. Thus, finding an effectivecurative option with low side effects is still a puzzle.Materials and Methods: This narrative review was conducted onthe effects of herbal and nano-based herbal medicine againstperiodontitis by searching different databases such as GoogleScholar, PubMed, Scopus, Web of Science, Science Direct, andScientific Information Databases.Results: According to published studies, some popular herbalformulations, such as Aloe vera, curcumin, Melaleuca alternifolia,and Scutellaria baicalensis Georgi, can be effective in periodontitistreatment. However, these herbal products may be accompaniedby some pharmacological limitations, such as poor bioavailability,instability, and weak water solubility. On the other hand,harnessing nano-based herbal formulations can elevate thebioavailability, diminish toxicity, and omit repeated administrationof drugs.Conclusion: Herbal and nano-based herbal products can create agood chance to treat periodontitis efficiently.

Abstract Stem cells are a promising solution for repaired damage tissue. Isolation of this cell from tissue, proliferation in vitro, and implanting them in damaged tissue microenvironment are the basic strategy of Regenerative medicine. Mesenchymal stem cells (MSCs) have unique therapeutics function such as homing to target tissue and stimulating host cell regeneration, repairing damaged cells, autocrine/paracrine effects, immune modulation and several lineages differentiation. For this reason, MSCs are appropriate choice for tissue engineering and cell based therapy. Umbilical cord as a commonly discarded tissue, which are obtained non-invasively. Umbilical cord contains Wharton’s jelly (WJ), a gelatin substance surrounding umbilical cord vessels. WJ, a rich source of self-renewal MSCs with extended proliferation and multilineage differentiation potential can be a best option for Regenerative medicine based on celltherapy. In this chapter, we will discuss an outline of the recent findings related to Wharton’s jelly Mesenchymal stem cells (WJ- MSCs) and its therapeutic effects and possible advantage they possess.

Background: This study aimed to evaluate the predicting factors affecting sperm retrieval. We prospectively assessedthe relationship between sonographic and microdissection testicular sperm extraction (mTESE) findings in Klinefeltersyndrome (KS).Materials and Methods: In this prospective study, 44 azoospermic men with 47, XXY karyotypes participated in thisstudy. In order to evaluate the amount of blood supply in different parts of testicular tissue, a doppler ultra-sonographicwas performed. Also, for the detection of sperm in this group mTESE technique was performed.Results: The age average of positive mTESE and negative mTESE groups was 29.4 and 33.6 years, respectively. Bycomparing the testicle volume (based on the data obtained from the clinical examinations conducted by the urologist)it was determined that there is no significant difference between mTESE positive and negative groups. Folliclestimulatinghormone (FSH) levels in men with negative mTESE (P=0.03) and testosterone levels in men with positivemTESE significantly increased (P=0.017). The overall rate of testis vascularity was significantly higher in the positivemTESE group than in the negative mTESE group. The clinical pregnancy rate in positive mTESE men was 9% percycle, 16.6% per embryos were transferred (ET), and 12.5% per cycle.Conclusion: Totally, our observation indicated that there is not a significant relationship between sonographic and mTESEresults in KS patients. However, more investigations with bigger sample Size can be useful to validate our results.

Background: The axillary arch is a rare axillary abnormality described as a slim muscular slip extending from the latissimus dorsi to the pectoralis major muscles. This arch has been indicated as the main reason for hyper-abduction syndrome and neurovascular compression in the axillary region. Case presentation: During the dissection of the right axillary cavity of a 74-year-old man for medical students, a case of the axillary arch was found, arising from the lateral border of the latissimus dorsi muscle and terminating in the pectoralis major tendon and the fascia covering the coracobrachialis muscle. Conclusion: Since this variation has not been reported in our country so far, this study can help surgeons to have a better understanding of anatomy and the prevalence of this structure. This muscle can be clinically important in axillary surgeries; due to its influences on the brachial plexus and axillary vessels, covering lymph nodes and altering local anatomy.

Objective: Destruction of pancreatic beta-cells induces an insulin deficiency and causes type 1 diabetes. The role of autophagy in inducing insulin-secreting cells (ISCs) from adipose-derived mesenchymal stem cells (AMSCs) was investigated in the current study. Materials and Methods: In this experimental study, the isolated AMSCs were characterization and exposed to a cocktail differentiation medium (CDM) in the absence or presence of 3-methyladenine (3MA), an autophagy inhibitor. The differentiation of ISCs was confirmed by the evaluation of the expression of beta-cell-specific genes including pancreatic and duodenal homeobox 1 (PDX1), musculoaponeurotic fibrosarcoma oncogene homolog A (MAF-A), Nk class of homeodomain-encoding genes 6.1 and 2.2 (NKX6-1 and NKX2.2), Glucose transporter 2 (GLUT-2) and INSLIN. Using Newport Green (NG), insulin-positive cells were identified. Insulin secretion in response to various glucose concentrations was measured. Autophagy was evaluated by Acridine orange (AO) staining. Also, expression of autophagy-associated genes, including autophagy-related gene 5 (ATG-5), autophagy-related gene 7 (ATG-7), BECLIN-1, and mammalian target of rapamycin (mTOR), was evaluated by Real-time polymerase chain reaction (PCR) method. Autoimmune disease, caused by unwanted immune responses to self-antigens, affects millions of people each year and poses a great social and economic burden to individuals and communities. In the course of autoimmune disorders, including rheumatoid arthritis, systemic lupus erythematosus, type 1 diabetes mellitus, and multiple sclerosis, disturbances in the balance between the immune response against harmful agents and tolerance towards self-antigens lead to an immune response against self-tissues. In recent years, various regulatory immune cells have been identified. Disruptions in the quality, quantity, and function of these cells have been implicated in autoimmune disease development. Therefore, targeting or engineering these cells is a promising therapeutic for different autoimmune diseases. Regulatory T cells, regulatory B cells, regulatory dendritic cells, myeloid suppressor cells, and some subsets of innate lymphoid cells are arising as important players among this class of cells. Here, we review the roles of each suppressive cell type in the immune system during homeostasis and in the development of autoimmunity. Moreover, we discuss the current and future therapeutic potential of each one of these cell types for autoimmune diseases.

It has been explained that how gender affects angiotensin-converting enzyme 2 (ACE2), which is a basic factor for the COVID-19 pathogenesis introducing the sex diversities in platelet function, immune reactions and how sex hormones affect immune functions, also the effect of androgens on transmembrane protein serine protease 2 (TMPRSS2) receptor in COVID-19 patients was investigated.

Seminal infections were considered to be an etiologic factor of male infertility. Semen washing is used as an appropriate technique in artificial reproductive technology (ART) for serodiscordant couples when the partner of male is contaminated. The aim of this study was to investigate the role of semen washing methods in reducing the transmission of viral infections in ART. In this narrative review we investigated accessible information from Google Scholar, PubMed, Scopus, Web of Science, IranMedex, Irandoc, Magiran, SID, MEDLIB and Scientific Information Database until 2019. The MeSH terms of semen washing, HIV, assisted reproduction, serodiscordant, and virus transmission were used. According to the search strategy, 89 articles were discovered. After checking the titles, abstracts and manuscripts, a collection of 45 papers were chosen pursuant to the suitability indexes. The studies were performed around different categories of semen washing (swim up, density gradient) in serodiscordant couples. Some studies highlighted that semen washings can completely prevent viral transmission, but a few do not believe this claim. However, scientific researchers have revealed that sperm washing may diminish the possibility of infection in serodiscordant couples where the male partner is infected. Swim up in combination with density gradient is a suitable method to diminish the transmission of infections.

Background: In the process of human reproduction, spermatogenesis is one of the most important stages, which is controled by special genes on Y chromosome. Previous studies show that some infertile men have microdeletions on Y chromosome, which cause the reduction of sperm count. Three prominent spermatogenesis loci have been identified on the Y chromosome and entitled “azoospermia factors” (AZFa, b, and c). Hereby, this review article aimed to investigate the content of the Y chromosome microdeletions and their importance in male fertility. Methods: Data and information were collected on English-language articles from PubMed and MEDLINE databases. For Persian articles, Persian-language databases, including SID Scientific Database, IranMedex Medical Articles Database, IranDoc (Iran Scientific Information and Documents Research Institute), Magiran Publication Information, and MedLib were investigated. More than 50 articles on Y chromosome microdeletions and infertility published during 2000-2020 were studied. Results: Previous studies implicated that Y chromosome microdeletions in AZFa, AZFb, and AZFc regions are accompanied by defect in spermatogenesis, leading to oligo / azoospermia. Patients with AZFa and AZFb microdeletions present secretory azoospermia and do not have sperm in their seminiferous tubules. Complete AZFc deletion involves region b2/b4, which contains a total of 12 genes. Incomplete deletion of AZFc includes b1/b3, b2/b3 and gr/gr. The most common of which are gr/gr. In men with gr/gr deletion, sperm count and motility were lower than control group. Conclusion: Y chromosomal microdeletions emerged as the most frequent structural chromosome anomaly associated with the quantitative reduction of sperm. The development of assisted reproductive techniques (ART) like intra-cytoplasmic sperm injection (ICSI) and testicular sperm extraction (TESE) helps to bypass the natural barriers of fertilization.

Background: Approximately 40-50% of infertilities is related to males. Abnormal sperm chromatin structure is suggested as a significant cause of infertility. Protamines constitute a significant component of the sperm chromatin, and they play a vital role in the proper packaging of chromatin. Numerous studies have shown that protamine deficiency in sperm is associated with low sperm quality and infertility. Given the importance of protamine infertility, the purpose of this article is to investigate the content of protamine and its biological importance in male fertility. Materials and Methods: In this study, data and information collected on English-language articles from PubMed and MEDLINE databases. For Persian articles, Persian-language databases, including SID Scientific Database, IranMedex Medical Articles Database, IranDoc (Iran Scientific Information and Documents Research Institute), Magiran Publication Information and MedLib investigated. Results: Based on previous studies about protamine and its role in spermatogenesis, any disruption in protamine genes, including PRM1 and PMR2, can disrupt histone/protamine ratio, leading to abnormal spermatozoa. Conclusion: Most the previous studies approved any mutation in protamine genes correlates with infertility in men. These results can be a potential subject for future researches into infertility treatment in men.

Ovarian cancer (OCa) is characterized as one of the common reasons for cancer-associated death in women globally. This gynecological disorder is chiefly named the “silent killer” due to lacking an association between disease manifestations in the early stages and OCa. Because of the disease recurrence and resistance to common therapies, discovering an effective therapeutic way against the disease is a challenge. According to documents, some popular herbal formulations, such as curcumin, quercetin, and resveratrol, can serve as an anti-cancer agent through different mechanisms. However, these herbal products may be accompanied by some pharmacological limitations, such as poor bioavailability, instability, and weak water solubility. On the contrary, using nano-based material, e.g., nanoparticles (NPs), micelles, liposomes, can significantly solve these limitations. Therefore, in the present study, we will summarize the anti-cancer aspects of these herbal and-nano-based herbal formulations with a focus on their mechanisms against OCa.

Abstract:Some failures in ovary function, like folliculogenesis and oogenesis, can give rise to various infertility-associated problems, including polycystic ovary syndrome (PCOS) and premature ovarian insufficiency (POI). PCOS influences 8 to 20% of women; while POI occurs in at least 1% of all women. Regrettably, the current therapies for these diseaseshave not sufficiently been effective, and finding a suitable strategy is still a puzzle. One of the helpful strategies for managing and treating these disorders is understanding the contributing pathogenesis and mechanisms. Recently, it has been declared that abnormal expression of microRNAs (miRNAs), as a subset of non-coding RNAs, is involved in thepathogenesis of reproductive diseases. Among the miRNAs, the roles of miRNA-21 in the pathogenesis of PCOS and POI have been highlighted in some documents; hence, the purpose of this mini-review was to summarize the evidence in conjunction with the functions of this miRNA and other effective microRNAs in the normal or abnormal functions of the ovary (i.e., PCOS and POI) with a mechanistic insight.
