Abstract

Simple Summary: Current recommendations suggest setting two parallel cytogenetic cultures with 12-O-tetradecanoly-phorpol-13-acetate (TPA) and IL2+DSP30 as a mitogen to detect complex karyotypes (CKs) in chronic lymphocytic leukemia (CLL). However, studies comparing CK detection concordance between both methods in the same cohort are lacking. Herein, we evaluated the performance of two parallel cultures in a CLL cohort of 255 patients, specifically comparing CK detection (globally and in each individual patient). The CK detection rates and their prognostic impacts were similar for both mitogens. However, nearly one-third of CKs were only identified in one culture, mainly due to the detection of a normal karyotype or no metaphases in the other. In summary, the assessment of parallel cytogenetic cultures is the best strategy to detect CKs in CLL. Nonetheless, as IL2+DSP30 achieved the best performance, it should be prioritized above TPA if a single analysis is required to optimize cytogenetic assessment in routine practice. Current CLL guidelines recommend a two parallel cultures assessment using TPA and IL2+DSP30 mitogens for complex karyotype (CK) detection. Studies comparing both mitogens for CK identification in the same cohort are lacking. We analyzed the global performance, CK detection, and concordance in the complexity assessment of two cytogenetic cultures from 255 CLL patients. IL2+DSP30 identified more altered karyotypes than TPA (50 vs. 39%, p = 0.031). Moreover, in 71% of those abnormal by both, IL2+DSP30 identified more abnormalities and/or abnormal metaphases. CK detection was similar for TPA and IL2+DSP30 (10% vs. 11%). However, 11/33 CKs (33%) were discordant, mainly due to the detection of a normal karyotype or no metaphases in the other culture. Patients requiring treatment within 12 months after sampling (active CLL) displayed significantly more CKs than those showing a stable disease (55% vs. 12%, p < 0.001). Disease status did not impact cultures' concordance (κ index: 0.735 and 0.754 for stable and active). Although CK was associated with shorter time to first treatment (TTFT) using both methods, IL2+DSP30 displayed better accuracy than TPA for predicting TTFT (C-index: 0.605 vs. 0.580, respectively). In summary, the analysis of two parallel cultures is the best option to detect CKs in CLL. Nonetheless, IL2+DSP30 could be prioritized above TPA to optimize cytogenetic assessment in clinical practice. [ABSTRACT FROM AUTHOR]

Abstract

According to the Survey on the equipment and use of information technologies and communication in the home, 2009 (Statistics National Institute, INE) 53,8% of internet information searches’ were about health. Although Internet is a simple and flexible tool, patients and general public have information needs totally different from health professionals. This dynamism and growth of internet information suggests that users will have a greater autonomy to search information on health topics, but also makes evidence the need to adequately site’s design and organize services offered. The aim of this paper is to provide health information for this group through a proposal to develop a virtual library for patients. Methodology: Web pages specialized in health information for patients have been selected. These pages are: Medgle (beta version, with a powerful search engine and a live chat with a nurse, to ask for information); Patientslikeme (organized in communities of patients, it allows information exchange); NHS choices (National Health Service platform for patients); Scienceroll (it searches in some well-known databases about drug information, illnesses and others). Selection criteria have been: innovative approaches; usefulness for patients. These criteria have been recovered from recognized pages that gather information for patients, like NOAH or NHS Choices to guarantee contrasted quality criteria. They help us to analyze chosen resources to offer them organized. These webpages were analyzed in order to incorporate similar tools in the virtual library for patients. Results: The present work presents a structure with the contents that should be offered in a page of a public health system’s virtual library for patients. In the example of the Galician Region (Spain, NO), the starting point was the website Espazo da Cidadanía (Space for Citizens), where resources such as health legislation, medical appointments, healthy life advices, information on waiting lists and others are offered to patients. After analysing this gateway, we recommend to create more informative spaces like directories, chat rooms, or chat lists. Conclusions: - Virtual libraries must begin to integrate selected and assessed information for patients. - This information must not be limited to AZ links to pathology directories for patients. - Services like forums, live chats or tools where patients can communicate between them and with health professionals; information about health services use, functioning and provision; health advices; waiting lists; virtual offices to carry out procedures such as medical appointments or physician changes, health card or electronic prescriptions and patients and health professionals association directories must be included in this virtual library for patients. - Librarian professionals must explorer the web and analyzed the resources in order to offer it organized, their work warrant pertinence and updated resources.

Abstract

The Internet is the ideal medium for sharing information. The enormous advantage, which has resulted in the development and dissemination of articles, reports, papers, etc., must be accompanied by changes or adjustments in the style of writing and presenting the content. Our goal was to write basic information about "open access" to the physicians in a hospital on two formats, paper and the Web, according to the rules of legibility and readability of each type. Although the information is the same, the ways of writing content, design, typography, style, structure and organization, are very different and can be seen in the two "leaflets" to which the printed pamphlet and homepage have led. If an informational text reaches a good level of readability and comprehensibility and it is adapted to be read in the media in which it will be viewed, it will achieve the following things: 1. Doubling the number of readers 2. It will read in half the time 3. Readers will be able to understand and remember better.

Abstract

Objective: To reflect the situation of the websites of the hospital libraries in Spain in 2003. Material and methods: The websites of the hospital libraries have been located through C-17 directory, various hospital directories and searching Google, Altavista and Alltheweb. It has been applied the questionnaire of evaluation of websites with pondered punctuation by Lluís Codina. Visibility indicators have been obtained as well as accessibility according TAW. Results: From the C-17 catalogue have been retrieved 176 hospital libraries, of which 13 have a website; we added to these other 19 websites found through other directories and search engines. Of those 32 websites, 2 were not active; the remaining 30 were analyzed. Five of them passed the Codina's test of level I. In the TAW priority level 1, the problems detected automatically oscillated between 0 and 77. Conclusions: The results show the low number of hospital libraries with websites on the internet. The websites quality should be improved following the recommendations of the checkup lists to evaluate digital resources on the internet. The scarce visibility of these webs, hinders greatly its localization. On the other hand, the low accessibility rate of these webs blocks their visualization to handicapped people.

Abstract

Persistent house price increases are a likely candidate for consideration in fertility decisions. Theoretically, higher housing prices will cause renters to desire fewer additional children, but home owners to desire more children if they already have sufficient housing and low substitution between children and other “goods”, and fewer children otherwise. In this paper, the authors combine longitudinal data from the Canadian Survey of Labour Income and Dynamics (SLID) and averaged housing price data from the Canadian Real Estate Association to estimate the effect of housing prices on fertility in a housing market that has historically been less volatile and more conservative than its American counterpart has. They ask whether changes in lagged housing price affect the marginal fertility of homeowner and renter women aged 18–45. They present results both excluding and including those who move outside their initial real estate board area, using initial area housing prices as an instrument in the latter case. For homeowners, but not renters, the authors predominantly find evidence that lagged housing prices have a positive effect on marginal fertility and possibly on completed fertility. These pro-natal effects are confined to non-movers.

Abstract

Objective: The COVID-19 pandemic Increased the risk of impairing the mother-infant bonding. The objectives of this study were to evaluate the early bond established between mother and infant and postpartum depression (PPD) in pregnancies that occurred during the pandemic period, to identify the factors that may have influenced these outcomes and to verify if there was an association between bonding and probable PPD. Methods: This is a cross-sectional study of postpartum women from a public maternity hospital in the city of São Paulo conducted from February to June 2021, involving 127 mother-baby dyads. The initial data were collected in the immediate postpartum period and between 21–45 days after birth, using a semi-structured questionnaire on sociodemographic characteristics, gestational and birth conditions, and baby characteristics; the Edinburgh Postnatal Depression Scale (EPDS) and Postpartum Bonding Questionnaire (PBQ) were used to evaluate PPD and bonding, respectively. Results: The presence of probable PPD and unplanned pregnancies were associated with higher PBQ score and risk to impaired bonding (p = 0.001 and p = 0.004, respectively). EPDS showed a high prevalence of PPD (29.1%) and was not associated with any Studied variable. Probably, this high prevalence of probable PPD was due to the context of insecurity secondary to the pandemic. Conclusions: We observed an increase in the prevalence of probable PPD and unplanned pregnancies during the first 18 months of the pandemic, which were associated with worse scores in mother-infant bonding. The impaired bond can affect the future development of children born during this period. RESUMO Objetivo: A pandemia do COVID-19 aumentou o risco de o vínculo entre mãe-bebê ser prejudicado. Os objetivos deste estudo foram avaliar o vínculo mãe-bebê estabelecido precocemente e a depressão puerperal (DP) em gestações que ocorreram durante a pandemia, identificar os fatores que podem ter influenciado esses resultados e verificar se houve associação entre o vínculo e a provável depressão puerperal. Métodos: Foi realizado um estudo transversal com puérperas de uma maternidade pública da cidade de São Paulo, no período de fevereiro a junho de 2021, envolvendo 127 díades mãe-bebê. Os dados foram obtidos no puerpério imediato e 21 a 45 dias após o parto, utilizando-se um questionário semiestruturado sobre as características sociodemográficas, as condições de gestação e nascimento e características do recém-nascido; a escala de depressão puerperal de Edimburgo (EPDS) e o questionário de vínculo pós-parto (PBQ) foram utilizados para avaliar a presença de provável DP e o vínculo mãe-bebê, respectivamente. Resultados: A presença de provável DP e as gestações não planejadas foram associadas a maiores escores no PBQ e no risco de vínculo prejudicado (p = 0,001 e p = 0,004, respectivamente). A escala de Edimburgo mostrou alta prevalência de risco de depressão puerperal (29,1%) e não foi associada a nenhuma variável estudada. Provavelmente, a alta prevalência de provável DP foi secundária ao contexto de insegurança decorrente da pandemia. Conclusões: Observamos aumento na prevalência da provável DP e das gestações não planejadas durante os primeiros 18 meses da pandemia, os quais estiveram associados a piores escores no vínculo mãe-bebê. O prejuízo no vínculo pode afetar o desenvolvimento futuro das crianças geradas durante esse período.

39. Carbon Emissions in Transportation: A Synthesis Framework.

Abstract

With the growing concern worldwide regarding greenhouse gas (GHG) emissions and their impacts on human health and the environment, transportation has become a central theme in their mitigation, responsible for 15% of anthropogenic GHG, 23% of global energy-related, and 8.7 Gt CO2-eq emissions. This study's objective was to comprehensively review the current state of carbon mitigation in the transportation sector. This was conducted through a systematic literature review based on the multi-level perspective of socio-technical transition theory and structural contingency theory. In total, 30 review papers covering 3561 original articles were selected for full-text examination. The main findings were related to the fact that in order to build resilience against climate change, transportation services must adapt to the current scenario and act quickly to avert future changes. Enablers, barriers, benefits, disadvantages, and metrics in carbon emission reduction were identified. A comprehensive framework and a dynamic co-word analysis emphasised the interrelationships among the dimensions of sustainability transition in transportation. Important trade-offs among the transition dimensions are context-dependent and should be adapted to different countries and transport modes to succeed. Abstract

The assessment of mood disorders and addiction linked to the practice of chemsex is of interest given the psychoactive substances used. The aim of this study was to assess risky sexual and addictive behavior to chemsex and related anxiety/depression symptoms in individuals receiving HIV pre‐exposure prophylaxis (PrEP). In this cross‐sectional study, all adults presenting for PrEP renewal at French sexual health centers were enrolled from January 2018 to March 2019. Participants completed a questionnaire on chemsex (i.e., the use of psychoactive substances before/during sex), including adapted Alcohol, Smoking and Substance Involvement Screening Test (ASSIST) to chemsex addiction (questions of ASSIST were modified to focus on chemsex). Anxiety/depression was assessed with the Hospital Anxiety and Depression Scale. In the last 3 months before enrollment, 39.8% (94/236) of participants reported chemsex. The main psychoactive substances consumed during chemsex were cathinones (74.6%), gamma‐hydroxybutyrate (66.3%), and other psychostimulants (60%). The median score of the chemsex‐focused ASSIST was 8 [IQR25–75: 3–15]; 72.2% of participants had a score justifying at least a brief intervention (>4). In multivariate analyses, anxiety and cathinones consumption were associated with an ASSIST score >4: OR 13.65 (95%CI 1.68–662.7), P = 0.0062, and OR 8.468 (95%CI 2.066–43.059), P = 0.0014, respectively. The level of addiction to the practice of chemsex can be difficult to estimate for the user, and the ASSIST makes it possible to evaluate this addiction and to direct the subjects toward specialized consultations of addictology, sexual health, or PrEP renewals. 46. The Effect of Literacy on Immigrant Earnings

Abstract

We examine the impact of literacy on immigrant earnings and the sources of lower returns to education and experience among immigrants. We find that the native-born literacy distribution dominates that for immigrants. However, the two groups obtain similar returns to literacy skills, contrary to discrimination-based explanations for immigrant-native-born earnings differentials. Among the university-educated, literacy differences account for about two-thirds of the earnings gap. However, low returns to foreign experience have a larger impact on this differential. Among the less educated, literacy differences and differences in the returns to experience have similar effects on the earnings differential.
