Pharmacological management is a necessary treatment for this disease. Therefore, semaglutide is an essential tool to achieve the treatment targets. The present study aimed to evaluate the semaglutide effects on a cohort with type 2 diabetes mellitus (T2DM) in Colombia.Materials and Methods: The cohort included 49 patients with T2DM that have been treated in a specialized care center. Their glycemic outcomes, weight, renal function, and adverse events were evaluated through a 3-, 6- and 12-month follow-up.Results: Significant differences were observed in the outcome evaluation: reduction of glycated hemoglobin levels (MD − 2.74 CI − 1.95 to − 3.52 in 6 months), fasting plasma glucose levels, body weight (MD − 7.11 CI − 5.97 to − 8.24), and the albumin-to-creatinine ratio. The results were maintained throughout the treatment period. The adverse event rate was 16.3%, predominating gastrointestinal events.Conclusion: This real-world evidence shows the efficacy of semaglutide in achieving treatment goals in patients with T2DM.Keywords: semaglutide, diabetes mellitus, glycated hemoglobin A, obesity, glycemic control

Abstract

The authors aim to present an updated protocol for mandibular reconstruction in nongrowing patients with Pruzansky/Kaban type IIb/III congenital craniofacial microsomia with customized temporomandibular joint (TMJ) prosthesis to reduce facial nerve (FN) damage and improve surgical accuracy. This is illustrated (using 3 cases) and is based on preoperative mapping of the FN using MRI for better virtual surgical planning of custom-made TMJ prosthesis. Intraoperative FN mapping and monitoring, as well as verification of the final result with intraoperative cone-beam computed tomography (CBCT) and 3D-reconstructed images is also achieved. All 3 patients presented mild transient postoperative facial palsy due to surgical soft tissue stretching which resolved within 2 months of surgery. All patients presented proper occlusion and mouth opening without pain, with an average incisal opening of 38.8 mm (range 35.5–42 mm) at two months of follow-up. Moreover, superposition of intraoperative and preoperative 3D reconstruction images ensured surgical accuracy and avoided the need for a potential reintervention. In conclusion, the proposed surgical protocol for mandibular reconstruction with customized alloplastic TMJ prosthesis in nongrowing patients with type IIb/III Pruzansky-Kaban congenital mandibular hypoplasia may reduce FN morbidity, improve surgical accuracy and final outcomes. Abstract

The xenobiotic transporter ABCC4/MRP4 is highly expressed in pancreatic ductal adenocarcinoma (PDAC) and correlates with a more aggressive phenotype and metastatic propensity. Here, we show that ABCC4 promotes epithelialmesenchymal transition (EMT) in PDAC, a hallmark process involving the acquisition of mesenchymal traits by epithelial cells, enhanced cell motility, and chemoresistance. Modulation of ABCC4 levels in PANC-1 and BxPC-3 cell lines resulted in the dysregulation of genes present in the EMT signature. Bioinformatic analysis on several cohorts including tumor samples, primary patient-derived cultured cells, patient-derived xenografts, and cell lines, revealed a positive correlation between ABCC4 expression and EMT markers. We also characterized the ABCC4 cistrome and identified four candidate clusters in the distal promoter and intron one that showed differential binding of proepithelial FOXA1 and pro-mesenchymal GATA2 transcription factors in low ABCC4-expressing HPAF-II and high ABCC4-expressing PANC-1 xenografts. HPAF-II xenografts showed exclusive binding of FOXA1, and PANC-1 xenografts exclusive binding of GATA2, at ABCC4 clusters, consistent with their low and high EMT phenotype respectively. Our results underscore ABCC4/MRP4 as a valuable prognostic marker and a potential therapeutic target to treat PDAC subtypes with prominent EMT features, such as the basal-like/squamous subtype, characterized by worse prognosis and no effective therapies. Abstract

Since 2020, the Pampas ecoregion, a highly productive area of Argentina, has endured its most severe drought in the past century. Our understanding of the impact of droughts on physicochemical changes in lotic environments remains limited, and this knowledge gap becomes even more pronounced when considering the concurrent urbanisation and industrialisation that alter land-use patterns and pose threats to water quality. This study aims to analyse, on a short-term basis, how hydrodynamics and physicochemical characteristics of streams, exposed to different land uses, are affected by drought. During the winter of 2022, we conducted three samplings in streams within the Samborombón River basin. We selected three streams influenced by rural land use and three influenced by urban development. Our data collection encompassed meteorological information, the multivariate El Niño Southern Oscillation (ENSO) index and the Palmer drought severity index. We measured hydrological and physicochemical variables at each sampling site. Our analysis identified an extreme drought period coinciding with 'La Niña' conditions during the sampling period. The annual accumulated precipitation for this period was notably lower, at 622 mm, compared with the historical average (1958–2022) of 1006 mm for this region. Hydrological variables exhibited significant differences between urban and rural streams. Urban streams maintained a consistent water flow, while rural streams were predominantly characterised by disconnected pools. Additionally, urban streams exhibited significantly greater Secchi depth and higher nutrient concentrations, whereas rural streams displayed elevated pH, conductivity and turbidity levels. These findings underscore the crucial role of urban effluents in sustaining a base flow during extreme drought conditions. 46. Clinical and thyroid profile in patients with COVID-19 hospitalized in an intensive care unit.

Abstract

OBJECTIVE: This study aimed to summarize the epidemiological and clinical features of thyroid function in COVID-19 patients in the intensive care unit (ICU) of Civil Fray Antonio Alcalde Hospital in Mexico. PATIENTS AND METHODS: This is a cross-sectional study that included 63 ICU patients with COVID-19 from August 2021 to December 2021. Thyroid function was evaluated through the TSH, T4, T3, and FT3 measures. Comorbidities such as diabetes mellitus type 2 (T2DM), arterial hypertension (HT), body mass index (BMI), and biochemical biomarkers, including procalcitonin (PCT) and C reactive protein (CRP), were also analyzed. RESULTS: A total of 63 patients with COVID-19 were hospitalized in the ICU; 42 (67%) were male, and 21 (33%) were female, with a mean age of 47 (range of 26-76 years). A total of 49 (78%) patients were non-vaccinated, 5 (8%) had an incomplete vaccination schedule, and 4 (6%) had completed the vaccination schedule. Regarding BMI, 10 (16%) were overweight, and 26 (40%) reported obesity. When assessing thyroid function, 8 (13%) patients were euthyroid, and 55 (87%) showed alterations on the thyroid hormonal axis, mainly a low concentration of TSH (0.56±0.79; p=0.0001) and FT3 (2.34±0.52; p=0.0006). In addition, increased PCT concentrations were associated with a higher risk to decease (1.22 vs. 8.21; p=0.0001) in this group of patients. CONCLUSIONS: Based on our findings, it appears that COVID-19 patients with low TSH and FT3 levels, who have not been vaccinated against SARS-CoV-2, are overweight or obese, and exhibit high levels of PCT are more likely to experience a poor prognosis and even mortality.
