Abstract

The welfare aspects of intra-European migration remain an important and controversial topic of academic and political debates. These discussions touch upon the classical 'welfare magnet' or 'welfare tourism' hypothesis. Transcending the politicised concept of 'benefit tourism,' our paper examines how welfare-state considerations in relation to migration decisions vary across the life course. Relying on micro-level qualitative research focusing on Spanish intra-EU movers, the paper probes deeper into how individuals perceive welfare systems, analysing the subtle and nuanced meanings of different aspects of the welfare for their migration decisions. We focus more specifically on welfare provisions in terms of health care, compulsory education, child support and other care responsibilities, unemployment and pensions and retirement. Our research indicates that, in studies on the migration-welfare nexus, it is necessary to move beyond the current narrow focus on the welfare magnet hypothesis and to examine how diverse welfare arrangements continuously and dynamically set the context for migration decisions at various stages of an individual's life. The results of our research show how features of the Spanish welfare system, in comparison to those of potential destination countries, might act as both a trigger and/or a barrier to migration. As such, we get a 'thicker description' of the role which welfare might play in shaping individuals' eventual migratory aspirations and decisions.

Abstract

Study Question: Does the immune response to coronavirus disease 2019 (COVID-19) infection or the BNT162b2 mRNA vaccine involve the ovarian follicle, and does it affect its function?Summary Answer: We were able to demonstrate anti-severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) IgG in follicular fluid (FF) from both infected and vaccinated IVF patients, with no evidence for compromised follicular function.What Is Known Already: No research data are available yet.Study Design, Size, Duration: This is a cohort study, composed of 32 consecutive IVF patients, either infected with COVID-19, vaccinated or non-exposed, conducted between 1 February and 10 March 2021 in a single university hospital-based IVF clinic.Participants/materials, Setting, Methods: A consecutive sample of female consenting patients undergoing oocyte retrieval was recruited and assigned to one of the three study groups: recovering from confirmed COVID-19 (n = 9); vaccinated (n = 9); and uninfected, non-vaccinated controls (n = 14). Serum and FF samples were taken and analyzed for anti-COVID IgG as well as estrogen, progesterone and heparan sulfate proteoglycan 2 concentration, as well as the number and maturity of aspirated oocytes and day of trigger estrogen and progesterone measurements. Main outcome measures were follicular function, including steroidogenesis, follicular response to the LH/hCG trigger, and oocyte quality biomarkers.Main Results and the Role Of Chance: Both COVID-19 and the vaccine elicited anti-COVID IgG antibodies that were detected in the FF at levels proportional to the IgG serum concentration. No differences between the three groups were detected in any of the surrogate parameters for ovarian follicle quality.Limitations, Reasons For Caution: This is a small study, comprising a mixed fertile and infertile population, and its conclusions should be supported and validated by larger studies.Wider Implications Of the Findings: This is the first study to examine the impact of SARS-Cov-2 infection and COVID-19 vaccination on ovarian function and these early findings suggest no measurable detrimental effect on function of the ovarian follicle.Study Funding/competing Interest(s): The study was funded out of an internal budget. Abstract

OBJECTIVE--To compare the ability of angiotensin converting enzyme inhibitors and beta blockers to slow the development of end stage renal failure in non-diabetic patients with chronic renal failure. DESIGN--Open randomised multicentre trial with three year follow up. SETTING--Outpatient departments of six French hospitals. PATIENTS--100 hypertensive patients with chronic renal failure (initial serum creatinine 200-400 mumol/l. 52 randomised to enalapril and 48 to beta blockers (conventional treatment). INTERVENTIONS--Enalapril or beta blocker was combined with frusemide and, if necessary, a calcium blocker or centrally acting drug in patients whose diastolic pressure remained above 90 mm Hg. RESULTS--17 patients receiving conventional treatment and 10 receiving enalapril developed end stage renal failure. The cumulative renal survival rate was significantly better in the enalapril group than in the conventional group (P < 0.05). The slope of the reciprocal serum creatinine concentration was steeper in the conventionally treated patients (-6.89 x 10(-5)l/mumol/month) than in the enalapril group (-4.17 x 10(-5)l/mumol/month; P < 0.05). No difference in blood pressure was found between groups. CONCLUSION--In hypertensive patients with chronic renal failure enalapril slows progression towards end stage renal failure compared with beta blockers. This effect was probably not mediated through controlling blood pressure.
