Summary: Lassa fever continues to be a major public health burden in West Africa, yet effective therapies or vaccines are lacking. The isolation of protective neutralizing antibodies against the Lassa virus glycoprotein complex (GPC) justifies the development of vaccines that can elicit strong neutralizing antibody responses. However, Lassa vaccine candidates have generally been unsuccessful at doing so, and the associated antibody responses to these vaccines remain poorly characterized. Here, we establish an electron microscopy-based epitope mapping workflow that enables high-resolution structural characterization of polyclonal antibodies to the GPC. By applying this method to rabbits vaccinated with a recombinant GPC vaccine and a GPC-derived virus-like particle, we reveal determinants of neutralization that involve epitopes of the GPC-A competition cluster. Furthermore, by identifying undescribed immunogenic off-target epitopes, we expose the challenges that recombinant GPC vaccines face. By enabling detailed polyclonal antibody characterization, our work ushers in a next generation of more rational Lassa vaccine design.

The leather industry is a complex system with multiple actors that faces a fundamental transition toward more sustainable chemistry. To support this process, this article analyzes challenges of the industry and consumers’ roles as a nexus of transition-relevant developments. We present findings of an empirical study (N = 439) among consumers on their perception of leather, related knowledge, and purchasing behavior. We found that participants perceived leather as natural, robust, and of high quality. Knowledge about the manufacturing of leather products was overall limited but varied. Applying a psychological behavior theory, we found that being aware of environmental and health consequences from conventional manufacturing of leather products was positively associated with a personal norm to purchase leather products that are less harmful to environment and health. The perceived ease of buying such products was positively associated with their purchase. Our findings shed light on consumers’ roles in the current leather system and their support of niche innovations toward more sustainable chemistry. Against this backdrop, we discuss implications for product design, consumer information, and needs for traceability along supply chains.

A growing body of literature mainly in the context of consumer research indicates that the formal-aesthetic and conceptual design of objects can influence users’ thoughts, emotions and even behavioural patterns. While there is strong evidence regarding these effects on actual purchasing decisions, evidence on the effect of aesthetic design features (e.g., haptics, colour) on health-related mental concepts and intentions for health behaviour change is scarce. Based on insights from material and conceptual priming, this article illustrates the research-driven and evidence-based design process of two design primes and comprises pre-tests and an experiment in two settings on the effect of design on health behaviour focusing i.a. on intention for health behaviour change. In an evidence-based and research-driven process, two lecterns were designed to work as primes, i.e., to have a positive vs. negative influence on several mental constructs (sense of control, sense of coherence, resiliency, self-efficacy) and health-related intention. The lecterns differed mainly in terms of aesthetic appearance (e.g., material, colour, proportion, steadiness). They were tested in (a.) a university setting with students (n = 83) and (b.) a clinical setting with orthopaedic rehabilitation patients (n = 38). Participants were asked to perform an unrelated task (evaluation of an unrelated product) while standing at and using the lecterns. Overall, t-tests and Mann–Whitney-U tests show no significant differences but differing tendencies in a mentioning task. When asked to name health-promoting activities, in the clinical setting, participants using the “positive” prime (i.e., the steady lectern, n = 13) mentioned more sport-related aspects on average and a higher portion of sport-related aspects of their answers than participants using the “negative” prime (n = 11). In the university setting (positive: n = 36; negative n = 38), no such differences emerged. This finding gives reason to believe that the prime might be specifically effective in the clinical setting as it relates to physical activity being the most relevant topic of the patients’ pathology.

Summary: Lassa fever is an acute hemorrhagic fever caused by the zoonotic Lassa virus (LASV). The LASV glycoprotein complex (GPC) mediates viral entry and is the sole target for neutralizing antibodies. Immunogen design is complicated by the metastable nature of recombinant GPCs and the antigenic differences among phylogenetically distinct LASV lineages. Despite the sequence diversity of the GPC, structures of most lineages are lacking. We present the development and characterization of prefusion-stabilized, trimeric GPCs of LASV lineages II, V, and VII, revealing structural conservation despite sequence diversity. High-resolution structures and biophysical characterization of the GPC in complex with GP1-A-specific antibodies suggest their neutralization mechanisms. Finally, we present the isolation and characterization of a trimer-preferring neutralizing antibody belonging to the GPC-B competition group with an epitope that spans adjacent protomers and includes the fusion peptide. Our work provides molecular detail information on LASV antigenic diversity and will guide efforts to design pan-LASV vaccines.

With roughly half of the global population living in cities, urban environments become central to public health often perceived as health risk factors. Indeed, mental disorders show higher incidences in urban contexts compared to rural areas. However, shared urban environments also provide a rich potential to act as a resource for mental health and as a platform to increase mental health literacy. Based on the concepts of salutogenesis and restorative environments, we propose a framework for urban design interventions. It outlines (a) an output level, i.e., preventive and discursive potentials of such interventions to act as biopsychosocial resources, and (b) a process level, i.e., mechanisms of inter- and transdisciplinary collaboration of researchers and citizens in the design process. This approach aims at combining evidence-based, salutogenic, psychosocially-supportive design with a focus on mental health. Implementing low-threshold, resource-efficient options in the existing urban context brings this topic to the public space. Implications for the implementation of such interventions for citizens, researchers, and municipality stakeholders are discussed. This illustrates new directions of research for urban person-environment interactions, public health, and beyond.

RESUMO Objetivo: Avaliar as modificações nas características das paradas cardíacas no hospital após a implantação de um Time de Resposta Rápida. Métodos: Este foi um estudo observacional prospectivo de paradas cardíacas ocorridas no hospital entre janeiro de 2013 e dezembro de 2017. O critério de exclusão foi parada cardíaca na unidade de terapia intensiva, na emergência ou na sala cirúrgica. O Time de Resposta Rápida foi introduzido no hospital do estudo em julho de 2014. Os pacientes foram classificados em dois grupos: Pré-Time de Resposta Rápida (parada cardíaca no hospital antes da implantação do Time de Resposta Rápida) e Pós- Time de Resposta Rápida (parada cardíaca no hospital após a implantação do Time de Resposta Rápida). Os pacientes foram seguidos até a alta hospitalar ou óbito. Resultados: Ocorreram 308 paradas cardíacas (64,6 ± 15,2 anos; 60,3% homens; 13,9% com ritmo inicial chocável). Houve diminuição de 4,2 para 2,5 no índice de parada cardíaca no hospital por 1.000 admissões após o início da atuação do Time de Resposta Rápida, além de cerca de 124 chamados por 1.000 admissões. A parada antes da implantação do Time de Resposta Rápida se associou com hipóxia (29,4 versus 14,3%; p = 0,006) e alteração da frequência respiratória (14,7 versus 4,2%; p = 0,004) em comparação aos dados referentes à parada cardíaca após a implantação do Time de Resposta Rápida. Parada cardíaca por hipóxia foi mais comum antes da implantação do Time de Resposta Rápida (61,2 versus 38,1%; p < 0,001). Na análise multivariada, o retorno à circulação espontânea se associou com ritmo chocável (RC 2,97; IC95% 1,04 - 8,43) e parada cardíaca testemunhada (RC 2,52; IC95% 1,39 - 4,59) mas não com a implantação do Time de Resposta Rápida (RC 1,40; IC95% 0,70 - 2,81) ou sinais premonitórios (RC 0,71; IC95% 0,39 - 1,28). Na análise multivariada, a mortalidade hospitalar se associou com ritmo não chocável (RC 5,34; IC95% 2,28 - 12,53) e idade (RC 1,03; IC95% 1,01 - 1,05), porém não com a implantação do Time de Resposta Rápida (RC 0,89; IC95% 0,40 - 2,02). Conclusão: Apesar de a implantação de um Time de Resposta Rápida se associar com redução na incidência de parada cardíaca no hospital, ela não se associou com a redução da mortalidade das vítimas de parada cardíaca no hospital. Observou-se significante diminuição nas paradas cardíacas devidas a causas respiratórias após a implantação do Time de Resposta Rápida.

OBJETIVO: Identificar infecções bacterianas, virais e fúngicas nos primeiros 20 pacientes pediátricos submetidos a transplante de fígado no HCPA. PACIENTES E MÉTODOS: 21 transplantes foram realizados em 20 crianças e adolescentes, no período de março de 1995 a setembro de 1997, no HCPA. Todos os transplantes foram de doador cadavérico, do mesmo grupo sangüíneo ABO. Nove transplantes foram de fígado inteiro e 11, de fígado reduzido. O diagnóstico de infecção bacteriana foi feito quando havia evidências clínico-laboratoriais e/ou hemocultura e/ou outros culturais positivos. Os vírus pesquisados foram citomegalo e Epstein Barr. Fungos eram pesquisados através de hemoculturas e culturas de secreções, drenos e coleções, cateteres e urina. RESULTADOS: Dos 20 pacientes transplantados, dois morreram nas primeiras 24-48 horas e apenas quatro não apresentaram infecção e/ou culturais positivos, clinicamente significativos. Quatorze pacientes apresentaram infecção bacteriana, sendo que nove pacientes apresentaram mais do que um episódio infeccioso. Os organismos mais freqüentes foram Staphylococus aureus e epidermidis e Xantomonas maltophilia. Cinco receptores positivaram antigenemia para CMV, sendo que apenas um apresentava sorologia negativa no pré-transplante. Infecção fúngica foi diagnosticada em dois pacientes e um terceiro paciente apresentou cultura do dreno biliar positiva. CONCLUSÕES: Dos 20 pacientes transplantados, quatro foram ao óbito por complicações infecciosas. Um controle cuidadoso e medidas profiláticas e terapêuticas adequadas podem diminuir infecções e suas conseqüências após transplante hepático.

Background: Lassa virus (LASV) can cause severe acute systemic infection in humans. No approved antiviral drugs or vaccines are currently available. Antibody-based therapeutics are considered a promising treatment strategy in the management of LASV disease. Methods: We used chimeric Ifnar−/− C57BL/6 (Ifnar−/− Bl6) mice, a lethal LASV mouse model, to evaluate the protective efficacy of polyclonal antibodies purified from sera of rabbits hyperimmunized with virus-like particles displaying native-like LASV glycoprotein GP spikes. Results: Polyclonal anti-LASV GP antibodies provided 100% protection against lethal LASV infection in a pre- and post-exposure treatment setting and prevented LASV disease. Treatment also significantly lowered viremia level and virus load in organs. When treatment was initiated at the onset of symptoms, the hyperimmune antibodies provided partial protection and increased the survival rate by 80%. Conclusions: Our findings support the consideration of animal-derived hyperimmune antibodies targeting GP as an effective treatment option for highly pathogenic LASV.

Soybean plants cultivated with 50, 100 and 200 mM of NaCl, revealed that root growth was less affected by salinity than shoots. Salinity led to a reduction in leaf area and an increase in water content of the roots. These factors could contribute to the adaptation of the plant, improving its hydration. Although nitrate and free amino acid levels were reduced by salt treatment in roots, protein content of leaves was not altered. Salinity led to alterations in xylem amino acid composition, with increases in Ser, Ala, Gaba and Pro and a decrease in Asn. Similar changes were seen for Asn and Ser in roots together with a much stronger increase in Gaba. It is suggested that the decline in Asn reflects its conversion to Ala and Gaba (via Glu) in the roots while the increase in Pro and Gaba could be related to the adaptation of the plant to salinity.

O líquen plano é uma desordem comum do epitélio escamoso estratificado que acomete as mucosas oral e genital, a pele, as unhas e o couro cabeludo. O líquen plano oral (LPO) afeta mulheres de meiaidade e apresenta padrões e distribuição característicos, como estriações brancas, pápulas ou placas brancas, eritema, erosões e bolhas, que podem estar associadas a medicações e/ou materiais dentários no paciente. O diagnóstico clínico somente poderá ser feito se a doença apresentar padrões clássicos, como lesões concomitantes na mucosa oral e na pele. O diagnóstico laboratorial por meio do exame histopatológico se caracteriza pela presença de projeções do epitélio em forma de dentes de serra e corpos de Civatte, e possibilita excluir condições de displasia e malignidade. A imunofluorescência direta é utilizada em suspeita de outras doenças, como pênfigo e penfigoide. O LPO é tratado com agentes anti-inflamatórios, principalmente, corticosteroides tópicos, e novos agentes e técnicas têm-se demonstrado eficazes. A transformação maligna do LPO e sua incidência exata permanecem controversas. Este trabalho tem como objetivo apresentar, com base na revisão da literatura, a etiopatogenia, o diagnóstico clínico, exames complementares e complicações do LPO.Lichen planus is a common disorder of the stratified squamous epithelium that affects oral and genital mucous membranes, skin, nails, and scalp. Oral Lichen Planus (OLP) affects middle-aged women and shows distribution patterns and characteristics such as white striations, white plaques or papules, erythema, blisters and erosions, and may be associated with medication and/or dental materials used by the patient. The clinical diagnosis can only be made if the disease presents classical patterns such as concomitant lesions in the oral mucosa and skin. The laboratory diagnosis is histopathologically characterized by the presence of projections of the epithelium in the form of sawtooth and Civatte bodies and allows the exclusion of dysplasia and malignancy. Direct immunofluorescence is used when there is suspicion of other diseases, such as pemphigus and pemphigoid. OLP is treated with anti-inflammatory agents, particularly topical corticosteroids; new agents and techniques have proved effective. The malignant transformation of OLP and its exact incidence remain controversial. This work aims at presenting, through literature review, the etiopathogenesis, clinical diagnosis, laboratory tests, and complications of OLP.

O Dermatofibrosarcoma protuberans (DFSP) é uma neoplasia maligna rara em ginecologia com probabilidade de recidiva local e baixo risco para metástases. Paciente com 34 anos com endometriose tratada apresentou dores na região inguinal esquerda. Reportava sensação de aumento gradual em nódulo palpável indolor e de difícil mobilização. Exérese da lesão foi realizada com hipótese diagnóstica de endometriose. A microscopia revelou neoplasia mesenquimal com proliferação de células fusiformes monomórficas e aspecto estoriforme caracterizando o DFSP. DFSP deve ser lembrado no diagnóstico diferencial das afecções da região inguinal em ginecologia, pois, apesar de ser raramente encontrado, representa uma neoplasia maligna.Dermatofibrosarcoma protuberans (DFSP) is a rare malignant neoplasia in gynecology, with probability of recurrence and low possibilities of metastasis. A 34-year-old patient after endometriosis treatment presented pain in the left inguinal region. She reported sensation of gradual increase in a solid nodule, painless at palpation, with difficult mobilization. Exeresis was proceeded with diagnostic hypothesis of endometriosis. Microscopy revealed a mesenchymal neoplasia characterized by proliferation of monomorphic fusiform cells and storiform aspect characterizing the DFSP.DFSP must be remembered in the differential diagnosis of the affections of the inguinal region in gynecology because it represents a malignant neoplasia, although it's rare.

O mieloma múltiplo é neoplasia de células plasmáticas que infiltram a medula óssea; as lesões cutâneas no mieloma múltiplo são raras, inespecíficas ou específicas, sendo estas últimas primárias ou secundárias. Lesões específicas secundárias de pele em pacientes com mieloma ocorrem por extensão direta para a pele, a partir de lesões ósseas subjacentes, ou como plasmocitomas extramedulares metastáticos. Os autores relatam caso de paciente de 59 anos, que apresentou diagnóstico de mieloma múltiplo e, após sete meses de evolução, desenvolveu plasmocitomas cutâneos, caracterizados por nódulos eritêmato-violáceos nas extremidades.Multiple myeloma is a malignant proliferation of plasma cells which infiltrate bone marrow; cutaneous involvement with plasma cell neoplasms is uncommon, and may be nonspecific or specific. The specific dermatologic lesions arise either as a direct extension from the underlying bone lesions or by metastatic accumulation in the form of nodules and plaques. The authors report the case of a 59-year-old male patient with multiple myeloma, who developed cutaneous plasmacytomas (violaceous nodules on the limbs) seven months after the disease diagnosis.

Os autores relatam um caso de ceratodermia palmoplantar hereditária de Unna-Thost, de apresentação clínica atípica, bem como complicação do tipo estrangulamento ainhumóide no quinto dedo de ambos os pés. Os autores citam ainda outras ceratodermias palmoplantares hereditárias que podem cursar com pseudo-ainhum, bem como os critérios clínicos básicos que diferenciam essas manifestações.The authors report a case of hereditary palmoplantar keratoderma, Unna-Thost type, demonstrating some atypical features when compared to descriptions found in the literature. The case study also includes pseudoainhum on both fifth toes. The authors describe other hereditary palmoplantar keratodermas that may also present with pseudoainhum, and demonstrate the basic clinical criteria that they share.

Paquidermoperiostose é genodermatose autossômica dominante rara, que afeta pele e ossos, porém sua prevalência e incidência permanecem desconhecidas. Ocorre predominantemente em homens, que também mostram fenótipo mais grave da doença. Descrevese o caso de paciente do sexo masculino, de 60 anos.Pachydermoperiostosis is a rare autosomal dominant genodermatosis characterized by bone and cutaneous involvement, even though its prevalence and incidence are ignored. It occurs predominantly in men, which present a more serious clinical presentation. We report a case of a 60-year-old man with this disease.

We present a case of subcutaneous hyalohyphomycosis due to Acremonium recifei, a species whose habitat is probably the soil, first identified in 1934 by Arêa Leão and Lobo in a case of podal eumycetoma with white-yellowish grains and initially named Cephalosporium recifei. A white immunocompetent female patient from the state of Bahia, Brazil, with a history of traumatic injury to the right hand is reported. The lesion was painless, with edema, inflammation and the presence of fistulae. Seropurulent secretion with the absence of grains was present. Histopathological examination of material stained with hematoxylin-eosin showed hyaline septate hyphae. A culture was positive for Acremonium recifei. Treatment with itraconazole, 200 mg/day, for two months led to a favorable course and cure of the process. We report for the first time in the literature a case of subcutaneous hyalohyphomycosis due to Acremonium recifei in a immunocompetent woman. Treatment with itraconazole 200 mg/day, for two months, resulted in cure.Os autores registram caso de hialo-hifomicose subcutânea por Acremonium recifei em paciente branca, imunocompetente, natural da Bahia, com história de traumatismo no dorso da mão direita. A lesão era indolor, com edema, inflamação, presença de fístulas, secreção seropurulenta, e ausência de grãos. O exame histopatológico mostrou hifas septadas hialinas pela hematoxilina-eosina. Cultura positiva para Acremonium recifei, espécie identificada em 1934, pela primeira vez, por Arêa Leão & Lobo, com o nome de Cephalosporium recifei, de um caso de eumicetoma podai por grãos branco-amarelados. Tratamento com itraconazol, 200 mg ao dia, com evolução favorável e cura do processo. No Brasil, trata-se do primeiro registro de hialohifomicose provocado por esta espécie, cujo habitat deve ser o solo.

Stringently controlled conditional expressing systems are crucial for the functional characterization of genes. Currently, screening of multiple clones to identify the tightly controlled ones is necessary but time-consuming. Here, we describe a system fusing Tet (tetracycline)-inducible elements, BAC (bacterial artificial chromosome) and Gateway technology together to allow tight control of gene expression in BAC-transfected eukaryotic bulk cell cultures. Recombinase cloning into the shuttle vector and the BAC facilitates vector construction. An EGFP (enhanced green fluorescent protein) allows FACS (fluorescence activated cell sorting) and the BAC technology ensures tight control of gene expression that is independent of the integrating site. In the current first application, our gene of interest encodes a beta-catenin-ERalpha fusion protein. Tested by luciferase assay and western blotting, in HTB56 lung cancer cells the final BAC E11-IGR-beta-catenin-ERalpha vector demonstrated sensitive inducibility by Tet or Dox (doxycycline) in a dose-dependent manner with low background, and the EGFP was an effective selection marker by FACS in bulk culture HTB56 and myeloblastic 32D cells. This is a highly efficient tool for the rapid generation of stringently controlled Tet-inducible systems in cell lines.

An approach for the estimate of the uncertainty in measurement considering the individual sources related to the different steps of the method under evaluation as well as the uncertainties estimated from the validation data for the determination of mercury in seafood by using thermal decomposition/amalgamation atomic absorption spectrometry (TDA AAS) is proposed. The considered method has been fully optimized and validated in an official laboratory of the Ministry of Agriculture, Livestock and Food Supply of Brazil, in order to comply with national and international food regulations and quality assurance. The referred method has been accredited under the ISO/IEC 17025 norm since 2010. The approach of the present work in order to reach the aim of estimating of the uncertainty in measurement was based on six sources of uncertainty for mercury determination in seafood by TDA AAS, following the validation process, which were: Linear least square regression, Repeatability, Intermediate precision, Correction factor of the analytical curve, Sample mass, and Standard reference solution. Those that most influenced the uncertainty in measurement were sample weight, repeatability, intermediate precision and calibration curve. The obtained result for the estimate of uncertainty in measurement in the present work reached a value of 13.39%, which complies with the European Regulation EC 836/2011. This figure represents a very realistic estimate of the routine conditions, since it fairly encompasses the dispersion obtained from the value attributed to the sample and the value measured by the laboratory analysts. From this outcome, it is possible to infer that the validation data (based on calibration curve, recovery and precision), together with the variation on sample mass, can offer a proper estimate of uncertainty in measurement. A method for the determination of total mercury in fresh fish and shrimp samples by solid sampling thermal decomposition/amalgamation atomic absorption spectrometry (TDA AAS) has been validated following international foodstuff protocols in order to fulfill the Brazilian National Residue Control Plan. The experimental parameters have been previously studied and optimized according to specific legislation on validation and inorganic contaminants in foodstuff. Linearity, sensitivity, specificity, detection and quantification limits, precision (repeatability and within-laboratory reproducibility), robustness as well as accuracy of the method have been evaluated. Linearity of response was satisfactory for the two range concentrations available on the TDA AAS equipment, between approximately 25.0 and 200.0 μg kg−1(square regression) and 250.0 and 2000.0 μg kg−1(linear regression) of mercury. The residues for both ranges were homoscedastic and independent, with normal distribution. Correlation coefficients obtained for these ranges were higher than 0.995. Limits of quantification (LOQ) and of detection of the method (LDM), based on signal standard deviation (SD) for a low-in-mercury sample, were 3.0 and 1.0 μg kg−1, respectively. Repeatability of the method was better than 4%. Within-laboratory reproducibility achieved a relative SD better than 6%. Robustness of the current method was evaluated and pointed sample mass as a significant factor. Accuracy (assessed as the analyte recovery) was calculated on basis of the repeatability, and ranged from 89% to 99%. The obtained results showed the suitability of the present method for direct mercury measurement in fresh fish and shrimp samples and the importance of monitoring the analysis conditions for food control purposes. Abstract  The nature of mitochondrial DNA heteroplasmy is still unclear. It could either be caused by two mitochondrial DNA (mtDNA) haplotypes coexisting within a single cell or by an admixture of homoplasmic cells, each of which contains only one type of mtDNA molecule. To address this question, single lymphocytes were separated by flow cytometry assisted cell sorting and analyzed by cycle sequencing or minisequencing. To attain the required PCR sensitivity, the reactions were carried out on the surface of chemically structured glass slides in a reaction volume of 1–2 μl. In this study, blood samples from two healthy donors showing mitochondrial point heteroplasmy in direct sequencing (195Y and 234R, respectively) were analyzed. Nearly 96% of single lymphocytes tested were found to be in a homoplasmic state, but heteroplasmic cells were also detected. These results suggest that mitochondrial point heteroplasmy in blood may well be mainly due to the mixture of homoplasmic cells. [ABSTRACT FROM AUTHOR]

Abstract Background There is a shortage of registered nurses in the European Union (EU), and job dissatisfaction and perceived high work–family conflict have been identified as causes of nursing staff turnover. Reducing work hours is an organisational intervention that could have a positive effect on nurses’ and assistant nurses’ job satisfaction, work–life balance, and willingness to stay in the job. An orthopaedic surgery department at a large hospital in Sweden introduced reduced work hours for nurses and assistant nurses in order to improve the working situation. The aim of the study was to investigate the experiences of reduced work hours and no lunch breaks among nurses and assistant nurses at an orthopaedic surgery department at a hospital in Sweden, with a particular focus on recovery and psychosocial working environment. Methods A qualitative design was used in the study. Eleven nurses and assistant nurses working at the particular orthopaedic department took part in the study, and semi-structured interviews were used to collect data. The interviews were analysed by interpretative phenomenological analysis. Results Four main themes were developed in the analysis of the data: A more sustainable working situation, Improved work–life balance, Consequences of being part of a project, and Improved quality of care. Each theme consisted of subthemes. Conclusions Overall, reduced work hours appeared to have many, mainly positive, effects for the participants in both work and home life.

Pemphigus vulgaris is an autoimmune bullous disease whose therapy is based on systemic corticosteroids, with or without immunosuppressants. Rituximab is a chimeric monoclonal antibody of the IgG class, directed at a specific CD20 B cell surface antigen, used in pemphigus vulgaris empirically since 2002, with success in 90% of the cases and long periods of remission. Male patient, 33 years old, diagnosed with pemphigus vulgaris, confirmed by histopathology and direct immunofluorescence. He was treated for seven months with numerous treatments, including immunosuppressive drugs, with an unsatisfactory response, until he had complete remission with the use of rituximab. During a 34-month follow-up period, the patient presented a slight clinical relapse, which was successfully controlled with prednisone in a daily dose of 120mg, soon reduced to 20mg.

The authors report a case of exuberant giant condyloma acuminatum of Buschke-Loewenstein in a male patient, slow-growing, progressive and with locally destructive behavior in the inguinal, body of the penis, scrotum, perineal and perianal regions. After surgery he showed no signs of recurrence in 20 months of follow-up. The identification of HPV types 6 and 11 was performed using in situ hybridization.Os autores relatam um caso exuberante de condiloma acuminado gigante de Buschke-Lowenstein, em paciente do sexo masculino, de crescimento lento e progressivo e de comportamento destrutivo das regiões inguinal, corpo do pênis, escroto, perineal e perianal. Após tratamento cirúrgico, não apresentou sinais de recidiva em 20 meses de seguimento. A identificação dos HPVs, tipos 6 e 11, foi realizada através da técnica de hibridização in situ.

O granuloma actínico de O'Brien é uma doença cutânea rara. Há muitas controvérsias na sua caracterização, podendo representar uma entidade nosológica distinta ou uma forma de granuloma anular em áreas expostas. A patogênese é desconhecida; no entanto, acredita-se que a radiação solar seja um fator desencadeante. Os autores relatam o caso de um paciente de 78 anos, branco, que apre sentava há 10 anos pápulas eritematosas e infiltradas na fronte e hemiface à esquerda. O diagnóstico de granuloma actínico de O'Brien foi estabelecido a partir do exame histopatológico, uma vez que o quadro clínico exuberante apresentado pelo paciente diferia do de casos anteriormente observados.O'Brien's actinic granuloma is a rare skin disease. Controversy continues over whether it should be considered a specific condition or a form of granuloma annulare located in sun-exposed areas. Its pathogenesis is unknown; however, the most widely accepted hypothesis suggests that solar radiation is the triggering factor. This paper describes the case of a 78-year old, fair-skinned male, who presented with a 10-year history of an infiltrate of annular erythematous papules on his forehead and left malar region. The diagnosis of O'Brien actinic granuloma was established from histopathology, since the clinical condition of the patient was extensive, unlike cases reported in the literature.

FUNDAMENTOS: O risco da ocorrência, de Melanoma Cutâneo (MC), no Nevo Melanocítico Congênito Pequeno (NMCP) (< 3 cm), não tem sido motivo de estudos freqüentes ou mesmo recentes. A conduta a respeito de tais lesões ainda não apresenta nenhum consenso. OBJETIVOS: o propósito deste estudo foi avaliar a freqüência do melanoma cutâneo originado em Nevo Melanocítico Congênito Pequeno, e discutí-lo em relação a literatura, com propósitos de conduta prática. CASUÍSTICA E MÉTODOS: de um total de 204 pacientes, com Melanoma Cutâneo, foram selecionados aqueles cuja doença foi originada de um Nevo Melanocítico Congênito Pequeno. O critério de inclusão foi clínico, baseado na informação do paciente. RESULTADOS: Do total de 204 pacientes, 44(21,6%) com Intervalo de Confiança (IC) de 12,2 a 28,0% , resultaram da transformação de um Nevo Congênito Melanocítico Pequeno. Nenhum caso ocorreu antes dos vinte anos e não houve uma faixa etária preferencial de transformação. CONCLUSÕES: O Nevo Congênito Melanocítco Pequeno é precursor do Melanoma Cutâneo; a transformação, provavelmente, não ocorre antes da puberdade; o risco de transformação, após a puberdade, parece existir de forma homogênea por toda a vida. Sugestão: a excisão deve ser realizada no início da puberdade.

A osteodistrofia hereditária de Albright é caracterizada por calcificações cutâneas, obesidade, baixa estatura, braquidactilia associada ao pseudo-hipoparatireoidismo do tipo IA entre outras alterações hormonais como hipotireoidismo e hipogonadismo. O diagnóstico é baseado no quadro clínico associado aos achados de hipocalcemia e níveis elevados de hormônio da paratireóide. Os autores relatam caso em que a avaliação dermatológica foi de grande contribuição para o diagnóstico.Albright hereditary osteodystrophy is characterized by subcutaneous calcification, obesity, short stature, brachydactyly and pseudohypoparathyroidism type IA. Hypothyroidism and hypogonadism may be present. The diagnosis is based on clinical characteristics associated with hypocalcemia and high levels of parathyroid hormone. The authors report a case in which the dermatological evaluation contributed to diagnosis.

Gelatin industry residues are increasingly used as fertilizer and soil conditioner. However, correct residue dosage is critical for grass development and minimizing environmental impacts. This randomized block design study determined adequate dosage of mixed gelatin sludge (MGS) for Marandu grass production in wet/dry seasons in Brazil. Five MGS levels (0–200% of required nitrogen) were compared to mineral fertilizer. Agronomic/productivity characteristics, bromatological composition, macro/micronutrient composition of leaves, and soil chemical attributes were evaluated. Agronomic/productivity characteristics were influenced by MGS dose in both dry/rainy seasons, except for leaf blade pseudostem ratio and percentage of leaves/pseudostem. Bromatological composition was influenced by MGS doses in dry/rainy seasons except for dry/mineral material quantities. Marandu leaf tissue chemical composition was significantly influenced by MGS dose, except for potassium, boron, and iron. Chemical composition of four soil layers between 0 and 50 cm influenced MGS dose, except for pH, organic matter, magnesium, copper, manganese, and zinc. GMS dose for Marandu production should be 200% of nitrogen requirement. MGS application increased productivity/quality of Marandu grass. Macronutrients (nitrogen, phosphorus) and micronutrients (calcium, magnesium, sulfur, copper, and zinc) increased in Marandu grass and in the soil (calcium, sulfur, and sodium). The increased sodium level was not limiting.

We report a case of subcutaneous pheohyphomycosis observed in a male patient presenting pulmonary sarcoidosis and submitted to corticosteroid treatment. He presented nodular erythematous-violaceous skin lesions in the dorsum of the right hand. Histopathological examination of the biopsied lesion revealed dematiaceous hyphae and yeast-like cells, with a granulomatous tissual reaction. The isolated fungus was identified as Phoma cava. A review of the literature on fungal infection caused by different Phoma species, is presented. The patient healed after therapy with amphotericin B, followed by itraconazoleO presente trabalho registra um caso de feo-hifomicose subcutânea em paciente do sexo masculino com o diagnóstico de sarcoidose pulmonar, submetido à terapêutica por corticosteróides quando apresentou no dorso da mão direita lesões cutâneas nodulares, eritêmato-violáceas, de aspecto infiltrado, exigindo biópsia para o diagnóstico. O exame histopatológico revelou processo granulomatoso, com a presença de hifas e células arredondadas demácias. Cultivo positivo para fungo identificado com Phoma cava. Os Autores fizeram revisão da literatura sobre as infecções fúngicas provocadas por diversas espécies de Phoma, mostrando a raridade desta observação. A evolução foi favorável com a administração da anfotericina B (via venosa) seguida do itraconazol (via oral)

The inorganic fraction of arsenic species, iAs = ∑[As(III) + As(V)] present in fish samples can be quantified in the presence of other arsenic species also found in fishes, such as: monomethylarsonic acid (MMA), dimethylarsinic acid (DMA) and arsenobetaine (AsB). The toxic arsenic fraction was selected taking into account the dissociation constants of these arsenic species in different hydrogen ions concentration leading to the arsine formation from iAs compounds detected as As(III) by HG AAS. For thus, a microwave assisted extraction was carried out using HCl 1 mol L −1 in order to maintain the integrity of the arsenic species in this mild extraction media. Recovery experiments were done for iAs fraction, in the presence of other arsenic species. The recovery values obtained for iAs fraction added were quantitative about 87–107% (for N = 3, RSD ⩽ 3%). The limit of detection (LOD), and the limit of quantification (LOQ), were 5 μg kg −1 and 16 μg kg −1 respectively. [ABSTRACT FROM AUTHOR]

Relata-se a ocorrência de um pequeno surto de leishmaniose tegumentar americana, na Serra da Cantareira, São Paulo, SP, Brasil. Foram feitas considerações sobre a persistência de nichos de leishmaniose no Estado de São Paulo.A microepidemic outbreak of american mucocutaneous leishmaniasis occurred at the beginning of 1978 in the "Serra da Cantareira", S. Paulo, Brazil. The persistence of some of the niche is commented on.

CONTEXT AND OBJECTIVE: Cutaneous melanoma represents around 3% of all skin tumors. About 20% of such patients will have advanced disease and will die before reaching five years of survival. The aim of this paper was to describe the clinical and histopathological variables and their correlations. DESIGN AND SETTING: Retrospective, descriptive, epidemiological study at the Melanoma Unit, Dermatological Clinic, Irmandade da Santa Casa de Misericórdia, São Paulo. METHODS: Records from 364 cases between May 1993 and January 2006 were analyzed. The frequencies of all study variables and their 95% confidence intervals were determined. The chi-squared test was used to evaluate associations among the variables, adopting a significant level of 0.05. RESULTS: Females predominated, with 1.4 women for each man. The patients’ mean age was 58.9 years. Nonwhite patients represented 13.7% of the sample. The prevalent anatomical sites for cutaneous melanoma were the trunk and feet, for both men and women. Acral lentiginous melanoma represented 22.3% of the cohort. In situ primary lesions were observed in few cases and a high percentage of thick cutaneous melanoma was detected. Ulceration was found in 13.4% of the thin tumors (< 1.0 mm). Thicker and ulcerated lesions predominated in male patients (p = 0.011 and p < 0.001 respectively) and in elderly patients (p = 0.021 and p = 0.015). CONCLUSIONS: The cohort mostly presented thick and ulcerated tumors, denoting late diagnosis and bad prognosis. Also, the sample was characterized by considerable prevalence of female patients, nonwhite patients, limb lesions and acral lentiginous melanoma.

This research aimed to evaluate the physiological quality of pepper (Capsicum chinense Jacq.) var. Bode Vermelha seeds, obtained from fruits at different ripening stages. The fruits of C. Chinese were separated according to the ripening stages, based on pericarp color (green fruits – stage 1, orange fruits – stage 2, light red fruits – stage 3, red fruits – stage 4, and carmine-red fruits – stage 5). To evaluate the ripeness and the physiological quality of the seeds were determined the biometric characteristics of the fruits and seeds, mass of thousand seeds, seeds water content, electrical conductivity, standard germinations test, speed of germination index, percentage of normal seedlings, speed of seedlings formation, accelerated aging, and seedling length. The characters length, diameter, weight, and number of seeds of the fruits and length, width, thickness, and mass of the seeds were analyzed by the estimated parameters using descriptive statistics. For the other variables was used the completely randomized design. The results were submitted to variance analysis and comparison of means by the Scott-Knott test at 5% probability. The ripeness stage affects the physiological quality of C. chinense Jacq. var. Bode Red seeds, being the seeds from red (stage 4) to carmine-red (stage 5) fruits those with the best performance thus, they are physiologically ripe, being the most indicated to seeds obtaining.

Abstract: The objective of this study was to evaluate the spatial variability in soybean seeds quality and in soil chemical attributes of a production field. Data were collected at 138 georeferenced points of a soybean production property located in Santo Antônio de Leverger - MT. Soil related variables, such as phosphorus, potassium, calcium, magnesium, and organic matter (OM) contents, pH, cation exchange capacity (CEC) and base saturation (V%) were evaluated. On the other hand, yield, one thousand seed mass, size, germination, emergence in seedbed, electrical conductivity, accelerated aging and tetrazolium reaction were evaluated as seed variables. The data were submitted to descriptive and geostatistical analysis, and the fit semivariogram parameters were used to elaborate spatial distribution maps of the analyzed variables. After the analysis, it was possible to conclude that there was spatial variability in the evaluated attributes for both seeds and soil related variables, indicating that the soybean seed production area can be divided into management zones, which allows the definition of areas to be harvested or discarded within a field of seed production.
