11. Anatomic variability of tricuspid leaflets and right ventricle in Ebstein's anomaly: Anatomic and morphogenetic considerations.

- Abstract
Ebstein's anomaly of the tricuspid valve (TV) is a congenital cardiac malformation of the tricuspid valve (TV) and right ventricle (RV) characterized by downward displacement of the functional annulus usually involving septal and inferior leaflets. Ebstein's anatomical spectrum is highly variable. This anatomical study aimed to unravel the morphogenetic mechanisms underlying this variability. Forty-two hearts exhibiting Ebstein anomaly from the Reference Center for Complex Congenital Heart Defects (CHD) collection were macroscopically analysed: 28 isolated and 14 associated with another CHD. Tricuspid leaflets, right ventricle (RV), and associated lesions were analysed. Measurements were related to fetal heart reference values according to gestational age. Specimens were classified depending on the distal insertion of the anterior leaflet: linear, hyphenated or focal. Among isolated Ebstein hearts, distal insertion of the anterior leaflet was linear in 36%, hyphenated in 29% and focal in 36%. Linear insertion was always associated with anterior TV orifice and absent inferior leaflet, with absent septal leaflet in 80%. Atrialisation (thin, paper-like, inferior RV wall) was found in 90% of the linear forms and 37.5% of the hyphenated forms, which were associated with various degrees of inferior and septal leaflets non-delaminationanomalies. In focal forms, only the septal leaflet was involved, and there was no atrialisation. Ventricular septal defect was found in 3 specimens, Uhl-like RV anterior wall in 2, including 1 with left ventricle non compaction. Associated Ebstein anomalies were predominantly found in pulmonary atresia with intact ventricular septum; there was no atrialisation and septal leaflet only was involved. Ebstein anomaly corresponds to an arrest in TV development, early in linear forms, late in focal forms, intermediate in hyphenated forms. Atrialisation was always associated with absent inferior leaflet, underlining developmental interaction between endocardial cushions and myocardium at the RV inferior wall. [ABSTRACT FROM AUTHOR]

21. Pulmonary atresia with "intact ventricular septum" associated with a ventricular septal defect: An apparent paradox?

- Abstract
Pulmonary atresia with intact ventricular septum (PA-IVS) means pulmonary valvar atresia without a ventricular septal defect (VSD). In PA-IVS, the right ventricle (RV) appears externally normal in size but the internal cavity is hypoplastic with major wall hypertrophy. Histology reveals myocardial disarray and endocardial fibroelastosis. The leaflets of the pulmonary valve are present but fused indicating an acquired lesion during fetal life. The files of all children with PA-IVS in our institution between 1981 and 2023 were reviewed, using a data warehouse to look for associated VSD. Patients with Ebstein anomaly and pulmonary atresia were excluded. When available, echocardiographic examinations were reviewed. One patient had severe coronary arterial anomaly leading to death and autopsy. The total cohort included 300 consecutive patients. Among those, we found 8 children with an associated VSD (2.6% of the cohort). According to the IPCCC-ICD11 nomenclature, the VSD was central perimembranous in 3, muscular in 4, outlet in 1. In this last patient, autopsy revealed a hypoplastic but not malaligned outlet septum, without aortic overriding, which discarded the diagnosis of tetralogy of Fallot (Fig. 1). Paradoxically, the presence of a VSD is compatible with diagnosis of PA-IVS. This rare association cannot be classified as "PA-VSD": this last term should be reserved to extreme forms of tetralogy of Fallot, associating outlet VSD, anteriorly malaligned outlet septum, overriding aorta and PA. Conversely, PA-IVS may be due to a RV myocardial disease, leading to a diminutive RV cavity, tricuspid valve hypoplasia and progressive fusion of the pulmonary leaflets leading to atresia. The anatomic type of VSD, muscular and perimembranous, and their rarity, are in favor of an incidental association. The imprecise term "PA-IVS" does not reflect the defect's anatomy and pathogenesis. Recent use of very small photon beams (down to 4 mm) in stereotactic radiotherapy requires new detectors to accurately determine the delivered dose. Diamond detectors have been presented in the literature as an attractive candidate for this application, due to their small detection volume and the diamond atomic number (Z=6) which is close to water effective atomic number (Zeff∼7.42). However, diamond exhibits a density 3.51 times greater than that of water and recent studies using Monte Carlo simulations have demonstrated the drawback of a high-density detector on small beam output factors. The current study focuses on geometrical parameters of diamond detector, namely, the diamond dimensions and the electrode geometry, in order to solve the dosimetric issues still observed in small photon beams with diamond detectors. To give better insights to these open questions, we have used both computational method and experimental analysis. This study highlighted that reducing diamond dimensions is crucial for small beam output factor measurements and to limit the influence of its high density. Furthermore, electrodes covering the whole diamond surface were essential for a dose rate independence of the diamond detector. The optimal dosimeter derived from this work presented small diamond dimensions of approximately 1-1-0.15mm
3 , with diamond-like-carbon electrodes covering the whole diamond surface. A dose rate independence of this diamond detector (better than 0.5% over a wide range of dose rates available on a stereotactic dedicated facility) was obtained due to the electrode geometry. Concerning the output factor measurements, a good agreement (better than 1.1%) was observed between this carbon material detector and two types of passive dosimeters (LiF microcubes and EBT2 radiochromic films) for all beam sizes except the smallest field of 0.6-0.6 cm2 with a deviation of 2.6%. This new study showed the high performance of this diamond detector in small photon beams, in comparison with various commercially available passive and active dosimeters. Background High rates of food insecurity are documented among Inuit households in Canada; however, data on food insecurity prevalence and seasonality for Inuit households with children are lacking, especially in city centres. This project: (1) compared food consumption patterns for households with and without children, (2) compared the prevalence of food insecurity for households with and without children, (3) compared food consumption patterns and food insecurity prevalence between seasons, and (4) identified factors associated with food insecurity in households with children in Iqaluit, Nunavut, Canada. Methods Randomly selected households were surveyed in Iqaluit in September 2012 and May 2013. Household food security status was determined using an adapted United States Department of Agriculture Household Food Security Survey Module. Univariable logistic regressions were used to examine unconditional associations between food security status and demographics, socioeconomics, frequency of food consumption, and method of food preparation in households with children by season. Results Households with children (n = 431) and without children (n = 468) participated in the survey. Food insecurity was identified in 32.9% (95% CI: 28.5–37.4%) of households with children; this was significantly higher than in households without children (23.2%, 95% CI: 19.4–27.1%). The prevalence of household food insecurity did not significantly differ by season. Demographic and socioeconomic characteristics of the person responsible for food preparation, including low formal education attainment (ORSept = 4.3, 95% CI: 2.3–8.0; ORMay = 3.2, 95% CI: 1.8–5.8), unemployment (ORSept = 1.1, 95% CI: 1.1–1.3; ORMay = 1.3, 95% CI: 1.1–1.5), and Inuit identity (ORSept = 8.9, 95% CI: 3.4–23.5; ORMay = 21.8, 95% CI: 6.6–72.4), were associated with increased odds of food insecurity in households with children. Fruit and vegetable consumption (ORSept = 0.4, 95% CI: 0.2–0.8; ORMay = 0.5, 95% CI: 0.2–0.9), as well as eating cooked (ORSept = 0.5, 95% CI: 0.3–1.0; ORMay = 0.5, 95% CI: 0.3–0.9) and raw (ORSept = 1.7, 95% CI: 0.9–3.0; ORMay = 1.8, 95% CI: 1.0–3.1) fish were associated with decreased odds of food insecurity among households with children, while eating frozen meat and/or fish (ORSept = 2.6, 95% CI: 1.4–5.0; ORMay = 2.0, 95% CI: 1.1–3.7) was associated with increased odds of food insecurity. Conclusions Food insecurity is high among households with children in Iqaluit. Despite the partial subsistence livelihoods of many Inuit in the city, we found no seasonal differences in food security and food consumption for households with children. Interventions aiming to decrease food insecurity in these households should consider food consumption habits, and the reported demographic and socioeconomic determinants of food insecurity. International audience; Purpose To investigate the optimal use of XR-RV3 GafChromic® films to assess patient skin dose in interventional radiology while addressing the means to reduce uncertainties in dose assessment. Methods XR-Type R GafChromic films have been shown to represent the most efficient and suitable solution to determine patient skin dose in interventional procedures. As film dosimetry can be associated with high uncertainty, this paper presents the EURADOS WG 12 initiative to carry out a comprehensive study of film characteristics with a multisite approach. The considered sources of uncertainties include scanner, film, and fitting-related errors. The work focused on studying film behavior with clinical high-dose-rate pulsed beams (previously unavailable in the literature) together with reference standard laboratory beams. Results First, the performance analysis of six different scanner models has shown that scan uniformity perpendicular to the lamp motion axis and that long term stability are the main sources of scanner-related uncertainties. These could induce errors of up to 7% on the film readings unless regularly checked and corrected. Typically, scan uniformity correction matrices and reading normalization to the scanner-specific and daily background reading should be done. International audience; Purpose The use of small photon fields is now an established practice in stereotactic radiosurgery and radiotherapy. However, due to a lack of lateral electron equilibrium and high dose gradients, it is difficult to accurately measure the dosimetric quantities required for the commissioning of such systems. Moreover, there is still no metrological dosimetric reference for this kind of beam today. In this context, the first objective of this work was to determine and to compare small fields output factors (OF) measured with different types of active detectors and passive dosimeters for three types of facilities a CyberKnife® system, a dedicated medical linear accelerator (Novalis) equipped with m3 microMLC and circular cones, and an adaptive medical linear accelerator (Clinac 2100) equipped with an additional m3 microMLC. The second one was to determine the kQclin,Q msrfclin,fmsr correction factors introduced in a recently proposed small field dosimetry formalism for different active detectors. Methods Small field sizes were defined either by microMLC down to 6 × 6 mm2 or by circular cones down to 4 mm in diameter. OF measurements were performed with several commercially available active detectors dedicated to measurements in small fields (high resolution diodes IBA SFD, Sun Nuclear EDGE, PTW 60016, PTW 60017; ionizing chambers PTW 31014 PinPoint chamber, PTW 31018 microLion liquid chamber, and PTW 60003 natural diamond). Two types of passive dosimeters were used LiF microcubes and EBT2 radiochromic films. Results Significant differences between the results obtained by several dosimetric systems were observed, particularly for the smallest field size for which the difference in the measured OF reaches more than 20%. For passive dosimeters, an excellent agreement was observed (better than 2%) between EBT2 and LiF microcubes for all OF measurements. Moreover, it has been shown that these passive dosimeters do not require correction factors and can then be used as reference dosimeters. Correction factors for the active detectors have then been determined from the mean experimental OF measured by the passive dosimeters. Conclusions Four sets of correction factors needed to apply the new small field dosimetry formalism are provided for several active detectors. A protocol for small photon beams OF determination based on passive dosimeters measurements has been recently proposed to French radiotherapy treatment centers. © 2013 American Association of Physicists in Medicine.

Purpose This study aims at characterising the properties of TruView™ and ClearView™ two new gel dosimeters (Modus Medical Devices Inc.) and at studying the feasibility of relative dosimetry using these dosimeters and the Vista™ Optical CT scanner to accurately evaluate dose. Methods In this work, we investigated key dosimetric aspects (dose response, energy and dose rate dependence) and stability of these radiochromic gels initiated in preliminary works (Huet et al., 2017; Colnot et al., 2017) using spectrophotometric measurements. Moreover, by mean of optical CT scanning (Vista™), their performances to measure relative depth dose (PDD) and cross profiles were analysed. Results TruView™ and ClearView™ present a linear dose response up to 20 Gy and up to 80 Gy respectively, independent of both photon beam energy (4–18 MV) and dose rate (up to 9.9 Gy/min) (Huet et al., 2017; Colnot et al., 2017). ClearView™ response proves to be stable for a week post-irradiation and uniform within the batch whereas TruView™ presents an unstable but uniform response. Optical CT scanning generates errors due to stray light that need to be corrected in order to use these gels; ClearView™ scanning particularly requires important precautions. After corrections, those gels used in combination with the Vista™ scanner show promising spatial and dosimetric precision (dose difference <5%). Finally, TruView™ is reusable and presents excellent reproducible response (maximum 3% difference) and the ClearView™ dosimeter presents good spatial stability (0.5% difference after 6 days). Conclusion This study provides important knowledge about two gel dosimeters presenting interesting dosimetric properties. A study is ongoing to benchmark those promising candidates for clinical dose verification. [ABSTRACT FROM AUTHOR]

Background: Human papillomaviruses are the most common sexually transmitted infections, and genital warts, caused by HPV-6 and 11, entail considerable morbidity and cost. The natural history of genital warts in relation to HIV-1 infection has not been described in African women. We examined risk factors for genital warts in a cohort of high-risk women in Burkina Faso, in order to further describe their epidemiology. Methods: A prospective study of 765 high-risk women who were followed at 4-monthly intervals for 27 months in Burkina Faso. Logistic and Cox regression were used to identify factors associated with prevalent, incident and persistent genital warts, including HIV-1 serostatus, CD4+ count, and concurrent sexually transmitted infections. In a subset of 306 women, cervical HPV DNA was tested at enrolment. Results: Genital wart prevalence at baseline was 1.6% (8/492) among HIV-uninfected and 7.0% (19/273) among HIV-1 seropositive women. Forty women (5.2%) experienced at least one incident GW episode. Incidence was 1.1 per 100 person-years among HIV-uninfected women, 7.4 per 100 person-years among HIV-1 seropositive women with a nadir CD4+ count >200 cells/mu L and 14.6 per 100 person-years among HIV-1 seropositive women with a nadir CD4+ count

Traits related to crop performance (sugar, grain, biomass) are complex and their genetic bases difficult to study. To take advantage, in the process of selection, of tools now provided by molecular genetics, these traits need to be 'dissected' into elemental processes, more simple genetically and less prone to GxE (genotype X environment) interactions (Dingkuhn et al. 2007). However, evaluating a component trait individually is meaningless as plant and crop performance is the dynamic result of physiological, and sometimes genetic, linkages between traits in a given environment. This represents today a great challenge in the context of plant phenotyping and selection. Modelling enables formalizing biological processes as equations with G-dependent parameters, potentially linked to genetic information (eg. QTL). The use of model parameters as phenotypic traits for genetic analyses already showed its relevance in the case of simple models (Reymond et al. 2003; Yin et al. 1999). More recently, a few studies demonstrated the interest of using more integrative plant models for discriminating genotypes, based on parameter values within component, process based modules (Lecoeur et al. 2008; Luquet et al. 2008). Among them is Ecomeristem (Luquet et al. 2006), model simulating cereals' crop performance based on a simple formalism of plant vegetative morphogenesis, i.e. of meristem activity and its GxE regulation (organ creation, size, etc). Its use for QTL detection in a rice mapping population (based on parameters related to tillering, leaf size, phyllochron) gave even promising preliminary results (Ahmadi et al. 2008). DELICAS is a project that aims to identify, for sugar cane, molecular markers associated with genes of yield formation (Nibouche et al. 2010). Crop modelling is intended to support the phenotyping of component processes. In this context, Ecomeristem is currently adapted to sugar cane in order to take advantage of its plant level approach of morphogenesis and source-sink relationships. This paper aims to present this new model version, before it is tested and compared to other modelling approach(es) for its discriminative value (Martiné et al. 2010).

Des clones issus de différents génotypes de cocotier ont été obtenus au laboratoire ORSTOM/CIRAD-CP à Montpellier, France. Cependant, le nombre de ramets regénérés par clone reste faible à cause de l'absence d'une phase de multiplication intense. Ce chapitre résume les différentes étapes du procesus qui ont permis d'aboutir à une embryogenèse somatique: - induction de cals à partir d'explants prélevés sur de jeunes feuilles ou des inflorescences immatures; - étude histologique qui a permis de mettre en évidence deux voies embryogènes; - initiation de l'embryogenèse somatique étudiée sous l'angle des besoins en éléments minéraux nutritifs et en hormones; - enfin énumération des problèmes rencontrés lors de la maturation des embryons et leur conversion en plantules

The average lifetime ofB hadrons was measured using data collected with the DELPHI detector at the LEP collider during 1991 and 1992. The measurement was performed using two different anayses. The first method was an improvement on a previous technique, which used charged particle impact parameter distributions. This analysis measured an average lifetime forB hadrons of $$\tau _B = 1.542 \pm 0.021(stat.) \pm 0.045(syst.)ps.$$ (1) The second method was based on a new technique, which used inclusively reconstructed secondary vertices to measure $$\tau _B = 1.599 \pm 0.014(stat.) \pm 0.035(syst.)ps.$$ (2) Taking into account both the statistical and systematic corelations, the results of these two methods were combined to give an average lifetime forB hadrons of $$\tau _B = 1.582 \pm 0.012(stat.) \pm 0.032(syst.)ps.$$ (3)
