Abstract

Abstract Background Whole genome sequencing is increasingly being used for the diagnosis of patients with rare diseases. However, the diagnostic yields of many studies, particularly those conducted in a healthcare setting, are often disappointingly low, at 25–30%. This is in part because although entire genomes are sequenced, analysis is often confined to in silico gene panels or coding regions of the genome. Methods We undertook WGS on a cohort of 122 unrelated rare disease patients and their relatives (300 genomes) who had been pre-screened by gene panels or arrays. Patients were recruited from a broad spectrum of clinical specialties. We applied a bioinformatics pipeline that would allow comprehensive analysis of all variant types. We combined established bioinformatics tools for phenotypic and genomic analysis with our novel algorithms (SVRare, ALTSPLICE and GREEN-DB) to detect and annotate structural, splice site and non-coding variants. Results Our diagnostic yield was 43/122 cases (35%), although 47/122 cases (39%) were considered solved when considering novel candidate genes with supporting functional data into account. Structural, splice site and deep intronic variants contributed to 20/47 (43%) of our solved cases. Five genes that are novel, or were novel at the time of discovery, were identified, whilst a further three genes are putative novel disease genes with evidence of causality. We identified variants of uncertain significance in a further fourteen candidate genes. The phenotypic spectrum associated with RMND1 was expanded to include polymicrogyria. Two patients with secondary findings in FBN1 and KCNQ1 were confirmed to have previously unidentified Marfan and long QT syndromes, respectively, and were referred for further clinical interventions. Clinical diagnoses were changed in six patients and treatment adjustments made for eight individuals, which for five patients was considered life-saving. Conclusions Genome sequencing is increasingly being considered as a first-line genetic test in routine clinical settings and can make a substantial contribution to rapidly identifying a causal aetiology for many patients, shortening their diagnostic odyssey. We have demonstrated that structural, splice site and intronic variants make a significant contribution to diagnostic yield and that comprehensive analysis of the entire genome is essential to maximise the value of clinical genome sequencing.

Summary: Inhibitory receptors (IRs) function as critical regulators of immune responses by tempering T cell activity. In humans, several persisting viruses as well as cancers exploit IR signaling by upregulating IR ligands, resulting in suppression of T cell function (i.e., exhaustion). This allows escape from immune surveillance and continuation of disease. Here, we report the design, implementation, and results of a phenotypic high-throughput screen for molecules that modulate CD8+ T cell activity. We identify 19 compounds from the ReFRAME drug-repurposing collection that restore cytokine production and enhance the proliferation of exhausted T cells. Analysis of our top hit, ingenol mebutate, a protein kinase C (PKC) inducing diterpene ester, reveals a role for this molecule in overriding the suppressive signaling cascade mediated by IR signaling on T cells. Collectively, these results demonstrate a disease-relevant methodology for identifying modulators of T cell function and reveal new targets for immunotherapy. : Discovery of pharmacologic drugs that target exhausted T cells is essential to overcome the limitations of current checkpoint blockade therapies. Abstract

The Hippo pathway, by tightly controlling the phosphorylation state and activity of the transcription cofactors YAP and TAZ is essential during development and tissue homeostasis whereas its deregulation may lead to cancer. Recent studies have linked the apicobasal polarity machinery in epithelial cells to components of the Hippo pathway and YAP and TAZ themselves. However the molecular mechanism by which the junctional pool of YAP proteins is released and activated in epithelial cells remains unknown. Here we report that the tumour suppressor ASPP2 forms an apical-lateral polarity complex at the level of tight junctions in polarised epithelial cells, acting as a scaffold for protein phosphatase 1 (PP1) and junctional YAP via dedicated binding domains. ASPP2 thereby directly induces the dephosphorylation and activation of junctional YAP. Collectively, this study unearths a novel mechanistic paradigm revealing the critical role of the apical-lateral polarity complex in activating this localised pool of YAP in vitro, in epithelial cells, and in vivo, in the murine colonic epithelium. We propose that this mechanism may commonly control YAP functions in epithelial tissues.

Abstract

Cytokine storm during respiratory viral infection is an indicator of disease severity and poor prognosis. Type 1 interferon (IFN-I) production and signaling has been reported to be causal in cytokine storm-associated pathology in several respiratory viral infections, however, the mechanisms by which IFN-I promotes disease pathogenesis remain poorly understood. Here, using Usp18-deficient, USP18 enzymatic-inactive and Isg15-deficient mouse models, we report that lack of deISGylation during persistent viral infection leads to severe immune pathology characterized by hematological disruptions, cytokine amplification, lung vascular leakage and death. This pathology requires T cells but not T cell-intrinsic deletion of Usp18. However, lack of Usp18 in myeloid cells mimicked the pathological manifestations observed in Usp18-/- or Usp18C61A mice which were dependent on Isg15. We further mechanistically demonstrate that interrupting the ISGylation/deISGylation circuit increases extracellular levels of ISG15 which is accompanied by inflammatory neutrophil accumulation to the lung. Importantly, neutrophil depletion reversed morbidity and mortality in Usp18C61A mice. In summary, we reveal that the enzymatic function of Usp18 is crucial for regulating extracellular release of ISG15. This is accompanied by altered neutrophil differentiation, cytokine amplification and mortality following persistent viral infection. Moreover, our results suggest that extracellular ISG15 may drive the inflammatory pathology observed and could be both a prospective predictor of disease outcome and a therapeutic target during severe respiratory viral infections.

Contains fulltext : 206572.pdf (Publisher’s version ) (Open Access) We delineate a KMT2E-related neurodevelopmental disorder on the basis of 38 individuals in 36 families. This study includes 31 distinct heterozygous variants in KMT2E (28 ascertained from Matchmaker Exchange and three previously reported), and four individuals with chromosome 7q22.2-22.23 microdeletions encompassing KMT2E (one previously reported). Almost all variants occurred de novo, and most were truncating. Most affected individuals with protein-truncating variants presented with mild intellectual disability. One-quarter of individuals met criteria for autism. Additional common features include macrocephaly, hypotonia, functional gastrointestinal abnormalities, and a subtle facial gestalt. Epilepsy was present in about one-fifth of individuals with truncating variants and was responsive to treatment with anti-epileptic medications in almost all. More than 70% of the individuals were male, and expressivity was variable by sex; epilepsy was more common in females and autism more common in males. The four individuals with microdeletions encompassing KMT2E generally presented similarly to those with truncating variants, but the degree of developmental delay was greater. The group of four individuals with missense variants in KMT2E presented with the most severe developmental delays. Epilepsy was present in all individuals with missense variants, often manifesting as treatment-resistant infantile epileptic encephalopathy. Microcephaly was also common in this group. Haploinsufficiency versus gain-of-function or dominant-negative effects specific to these missense variants in KMT2E might explain this divergence in phenotype, but requires independent validation. Disruptive variants in KMT2E are an under-recognized cause of neurodevelopmental abnormalities.

Abstract

Chromosomal abnormalities are implicated in a substantial number of human developmental syndromes, but for many such disorders little is known about the causative genes. The recently described 1q41q42 microdeletion syndrome is characterized by characteristic dysmorphic features, intellectual disability and brain morphological abnormalities, but the precise genetic basis for these abnormalities remains unknown. Here, our detailed analysis of the genetic abnormalities of 1q41q42 microdeletion cases identified TP53BP2, which encodes apoptosis-stimulating protein of p53 2 (ASPP2), as a candidate gene for brain abnormalities. Consistent with this, Trp53bp2-deficient mice show dilation of lateral ventricles resembling the phenotype of 1q41q42 microdeletion patients. Trp53bp2 deficiency causes 100% neonatal lethality in the C57BL/6 background associated with a high incidence of neural tube defects and a range of developmental abnormalities such as congenital heart defects, coloboma, microphthalmia, urogenital and craniofacial abnormalities. Interestingly, abnormalities show a high degree of overlap with 1q41q42 microdeletion-associated abnormalities. Abstract

SummaryRegulation of nuclear import is fundamental to eukaryotic biology. The majority of nuclear import pathways are mediated by importin-cargo interactions. Yet not all nuclear proteins interact with importins, necessitating the identification of a general importin-independent nuclear import pathway. Here, we identify a code that determines importin-independent nuclear import of ankyrin repeats (ARs), a structural motif found in over 250 human proteins with diverse functions. AR-containing proteins (ARPs) with a hydrophobic residue at the 13th position of two consecutive ARs bind RanGDP efficiently, and consequently enter the nucleus. This code, experimentally tested in 17 ARPs, predicts the nuclear-cytoplasmic localization of over 150 annotated human ARPs with high accuracy and is acquired by the most common familial melanoma-associated CDKN2A mutation, leading to nuclear accumulation of mutant p16ink4a. The RaDAR (RanGDP/AR) pathway represents a general importin-independent nuclear import pathway and is frequently used by AR-containing transcriptional regulators, especially those regulating NF-κB/p53.

10. Combined Effect of Superabsorbent Polymers and Cellulose Fibers on Functional Performance of Plasters

Abstract

Plaster has, from ancient times, been used as a decorative material. However, the advances in materials engineering such as thermal and moisture control provide new opportunities. Superabsorbent polymers (SAPs) have been found to possess passive moisture control that may find utilization in modern buildings. However, the main drawback is associated with a limited number of applicable SAPs due to mechanical strength loss. In this regard, concurrent utilization of cellulose fibers may provide additional benefits linked with the reinforcing of plaster structure and preservation of superior hygric properties. In this regard, this study investigates the combined effect of SAP and cellulose fibers on the material properties of cement-lime plaster in terms of its mechanic, thermal, and hygric properties. To access the capability of such modified plasters to control the interior moisture fluctuations, the moisture buffering value is determined. Obtained results show the effect of both applied admixtures on material performance, whilst the synergic effect was most obvious for humidity control accessed through the moisture buffer coefficient.

Abstract

Complex statistical and sensitivity analysis of principal stresses in concrete slabs of the real type of rigid pavement made from recycled materials is performed. The pavement is dominantly loaded by the temperature field acting on the upper and lower surface of concrete slabs. The computational model of the pavement is designed as a spatial (3D) model, is based on a nonlinear variant of the finite element method that respects the structural nonlinearity, enables to model different arrangement of joints, and the entire model can be loaded by thermal load. Four concrete slabs separated by transverse and longitudinal joints and the additional structural layers including soil to the depth of about 3 m are modeled. The thickness of individual layers, physical and mechanical properties of materials, characteristics of joints, and the temperature of the upper and lower surface of slabs are supposed to be random variables. The simulation technique Updated Latin Hypercube Sampling with 20 simulations is used for the reliability analysis. As results of statistical analysis, the estimates of basic statistics of the principal stresses σ1 and σ3 in 106 points on the upper and lower surface of slabs are obtained. For sensitivity analysis the sensitivity coefficient based on the Spearman rank correlation coefficient is used. As results of sensitivity analysis, the estimates of influence of random variability of individual input variables on the random variability of principal stresses σ1 and σ3 are obtained.

Abstract

Underpasses play an important role in ensuring permeability of line traffic structures (motorways, expressways) for feral species. There are many studies, articles and methodological guidebooks available describing proper design principles for technical parameters of underpasses and their placement within the terrain. Width, height and length are the main technical parameters affecting wildlife migration. Wildlife behaviour and their requirements for the mentioned technical parameters of underpasses also have been examined in depth. In practically all cases, underpass functionality grows with increasing width and height. Of course, this also results in greater construction costs. The objective should therefore be to find a balanced compromise enabling “sufficient” functionality while maintaining “reasonable” costs. Data envelopment analysis (DEA) provides a way to identify “good” solutions in the sense of “sufficient” underpass functionality with “reasonable” total costs. Underpass functionality calculations were based on a methodology for calculating migration potentials (Žák and Florian, 2013). Total costs were established according to actual construction work prices in the Czech Republic, including prices for preparations, design, construction, maintenance, and demolition. The results indicate that DEA can be used to find “good” solutions and can be of assistance in particular when planning measures to ensure motorways are permeable to wildlife.

13. Biochar for Vertical Greenery Systems

Abstract

Vertical greenery systems (VGS) are effective at solving urban heat. They can absorb noise pollution and dust, and, aesthetically, they are positively perceived. Systems using hydroponic irrigation and nutrition, in combination with mineral wool as a base, are light and effective (they are able to hold water, with a high percentage of air, and a good mechanical structure to hold the plant stable). However, the functionality of a system can be compromised if the water supply is depleted or the irrigation system fails. This deficiency can be partially remedied if a certain amount of biochar or a suitable organic fertilizer is also a part of the system. The research task consisted of verifying this assumption and determining the effective amount of the biochar. Samples with different amounts of biochar were examined under the same temperature and humidity conditions; extended drying times, additional costs, and safety tank size savings were found. Subsequently, the effective amount of the biochar was determined by the Data Envelopment Analysis (DEA) method. It has been experimentally verified that biochar has a positive effect and prolongs the drying time; the additional costs are almost offset by the benefits. It should be noted that the results are valid for central Europe, and may be modified for different climate and economic zones.

Retrofitting of historical and traditional buildings is an effective thermal protection measure. The presence of thermal insulation in the composition of building envelopes might, however, bring some shortages due to a decrease of exterior surface temperatures or possible water vapor condensation. These shortages can improve living conditions for various microorganisms on the exterior surfaces, especially in the case of interior thermal insulation systems that are typical with thermal bridges and thus supply the surface with heat to a greater extent. This paper, therefore, aims at the investigation of hygrothermal conditions in selected critical construction details and evaluates the results from the point of view of potential biofilms growth. Two-dimensional modeling of coupled heat and moisture is applied and the hygrothermal patterns are evaluated based on an adjusted isopleth growth model. The results showed that the duration of favorable conditions for biofilms growth is relatively low, accounting for less than 180 h in the worst-case scenario. It means the exterior surfaces of historical buildings provided with interior thermal insulation systems are not threatened by biofilms growth. Anyway, other negative aspects have been revealed that should be treated individually. Possible wood decay or increased hygrothermal straining are the typical examples in that respect.

Low volume roads are widely used all over the world. To improve their quality a FEM computer simulation of their behavior is proposed. The input information about mechanical properties of individual materials is crucial for obtaining results as exact as possible. Among others, the mechanical properties are generally dependent on the state of stress and on humidity conditions. For this purpose the cyclic-load triaxial machine testing of cyclic-load performance of materials seems to be a promising test method. The test specimens can be prepared with different amounts of water. Thus modulus of elasticity (Young modulus) of different materials including recycled ones can be measured under the different conditions of horizontal and vertical stresses and under the different humidity conditions. Using the proposed testing procedure the modulus of elasticity of materials used in the newly built low volume road is obtained under the different state of stress as well as humidity conditions set to standard, dry and fully saturated level. Also recycled materials which are able to replace the traditional materials in the pavement are tested. Obtained values of modulus of elasticity can be used in a FEM study of the newly built road.
