BackgroundFear of disease progression (FoP) is among the most prevalent and major psychological burdens breast cancer patients encounter. Excessive FoP may result in serious adverse effects for patients. FoP in breast cancer patients has gained attention recently; however, its prevalence in China is unknown.ObjectivesThis meta-analysis and systematic review aimed to assess the overall FoP among Chinese breast cancer patients to make recommendations for treatment and care.MethodsSystematic search databases included PubMed, EMbase, The Cohrane Library, Web of Science, CINAHL, PsycINFO and 4 Chinese databases (Wan Fang Data, CBM, VIP and CNKI). The retrieval time ranged from the database’s establishment to March 20, 2023. After two researchers independently evaluated the literature, retrieved information, and assessed the risk of bias for the included literature, Stata 15.1 software was used to conduct a meta-analysis.ResultsA total of 37 moderate or high-quality studies involving 9,689 breast cancer patients were included. Meta-analysis showed that the pooled mean score of FoP for Chinese breast cancer patients was 33.84 [95% CI (31.91, 35.77)], prediction interval (21.57 ~ 46.11). The subgroup study found that FoP levels varied among breast cancer patients of different regions, ages, educational levels, marital statuses, residences, illness stages, and disease statuses.ConclusionBreast cancer patients have higher FoP scores. Healthcare workers should be concerned. We expect that more relevant research will be undertaken and more effective interventions will be developed. Austin was first isolated as a novel polyisoprenoid mycotoxin from Aspergillus ustus in 1976. Subsequently, some new austin-type meroterpenoids (ATMTs) have been continually found. This review attempts to give a comprehensive summary of progress on the isolation, chemical structural features, biological activities, and fungal biodiversity of 104 novel ATMTs from 5 genera of terrestrial- and marine-derived fungi reported from October 1976 to January 2023. The genera of Penicillium and Aspergillus are the two dominant producers, producing 63.5% and 30.8% of ATMTs, respectively. Moreover, about 26.9% of ATMTs display various pronounced bioactivities, including insecticidal, anti-inflammatory, cytotoxicity, antibacterial, and PTP1B inhibitory activities. The chemical diversity and potential activities of these novel fungal ATMTs are reviewed for a better understanding, and a relevant summary focusing on the source fungi and their taxonomy is provided to shed light on the future development and research of austin-type meroterpenoids.

BackgroundPOD places a heavy burden on the healthcare system as the number of elderly people undergoing surgery is increasing annually because of the aging population. As a large country with a severely aging population, China's elderly population has reached 267 million. There has been no summary analysis of the pooled incidence of POD in the elderly Chinese population.MethodsSystematic search databases included PubMed, Web of Science, EMBASE, Cochrane Library Databases, China Knowledge Resource Integrated Database (CNKI), Chinese Biomedical Database (CBM), WanFang Database, and Chinese Science and Technology Periodicals (VIP). The retrieval time ranged from the database's establishment to February 8, 2023. The pooled incidence of delirium after non-cardiac surgery was calculated using a random effects model. Meta-regression, subgroup, and sensitivity analyses were used to explore the source of heterogeneity.ResultsA total of 52 studies met the inclusion criteria, involving 18,410 participants. The pooled incidence of delirium after non-cardiac surgery in the elderly Chinese population was 18.6% (95% CI: 16.4–20.8%). The meta-regression results revealed anesthesia method and year of publication as a source of heterogeneity. In the subgroup analysis, the gender subgroup revealed a POD incidence of 19.6% (95% CI: 16.9–22.3%) in males and 18.3% (95% CI: 15.7–20.9%) in females. The year of publication subgroup analysis revealed a POD incidence of 20.3% (95% CI: 17.4–23.3%) after 2018 and 14.6 (95% CI: 11.6–17.6%) in 2018 and before. In the subgroup of surgical types, the incidence of hip fracture surgery POD was 20.7% (95% CI: 17.6–24.3%), the incidence of non-cardiac surgery POD was 18.4% (95% CI: 11.8–25.1%), the incidence of orthopedic surgery POD was 16.6% (95% CI: 11.8–21.5%), the incidence of abdominal neoplasms surgery POD was 14.3% (95% CI: 7.6–21.1%); the incidence of abdominal surgery POD was 13.9% (95% CI: 6.4–21.4%). The anesthesia methods subgroup revealed a POD incidence of 21.5% (95% CI: 17.9–25.1%) for general anesthesia, 15.0% (95% CI: 10.6–19.3%) for intraspinal anesthesia, and 8.3% (95% CI: 10.6–19.3%) for regional anesthesia. The measurement tool subgroup revealed a POD incidence of 19.3% (95% CI: 16.7–21.9%) with CAM and 16.8% (95% CI: 12.6–21.0%) with DSM. The sample size subgroup revealed a POD incidence of 19.4% (95% CI: 16.8–22.1%) for patients ≤ 500 and 15.3% (95% CI: 11.0–19.7%) for patients > 500. The sensitivity analysis suggested that the pooled incidence of postoperative delirium in this study was stable.ConclusionOur systematic review of the incidence of delirium after non-cardiac surgery in elderly Chinese patients revealed a high incidence of postoperative delirium. Except for cardiac surgery, the incidence of postoperative delirium was higher for hip fracture surgery than for other types of surgery. Abstract The present study aimed to investigate the mechanisms underlying the antidepressant-like effects of puerarin via the chronic unpredictable stress (CUS) procedure in rats. Similar to Sertraline (Ser), Chronic treatment of puerarin (60 and 120 mg/kg, i.g) elicited the antidepressant-like effects by reversing the decreased sucrose preference in sucrose preference test (SPT), by blocking the increased latency to feed in novelty-suppressed feeding test (NSFT) and the increased immobility time in forced swimming test (FST) without affecting locomotor activity. However, acute puerarin treatment did not ameliorate the antidepressant- and anxiolytic- like effects in FST and NSFT, respectively. In addition, enzyme linked immunosorbent assay (ELISA) and high performance liquid chromatography-electrochemical detection (HPLC-ECD) showed that chronic treatment of puerarin (60 and 120 mg/kg, i.g) reversed the decreased levels of progesterone, allopregnanolone, serotonin (5-HT) and 5-Hydroxyindoleacetic acid (5-HIAA) in prefrontal cortex and hippocampus of post-CUS rats. Furthermore, puerarin (60 and 120 mg/kg, i.g) blocked the increased corticotropin releasing hormone (CRH), corticosterone (Cort) and adrenocorticotropic hormone (ACTH). Collectively, repeated administration of puerarin alleviated the behavioral deficits induced by chronic stress which was associated with the biosynthesis of neurosteroids, normalization of serotonergic system and preventing HPA axis dysfunction.

Based on the hyperbolic curve tangent modulus method and the wedge stress theory, this paper proposes a calculation method for the settlement of a vertically loaded single pile in sloping ground. By establishing the relationship between the initial tangent modulus and the compression modulus of the slope soil, the tangent modulus of the slope soil is obtained combining with Mindlin-Geddes solution and Hansen formula, and the solution of the pile settlement in sloping ground is derived. Then, a series of numerical analyses are carried out to examine the feasibility of the proposed method. Finally, the effect of parameters is discussed in detail, including the slope angle, the distance of the pile from the slope crest, and the soil properties around the pile. The results show that the pile settlement will increase by increasing the slope angle or decreasing the distance of the pile from the slope crest, and the effect of the two parameters on the pile settlement is coupling-related. Besides, the compression modulus, cohesion, and internal friction angle of the soil around the pile are negatively related to the pile settlement and it is found that the compression modulus of the soil is the more influential parameter.

In this paper, a method was developed to assess the vertical response of piles considering crushing characteristics. A load transfer model of a pile is built and the relationship between the crushing characteristics and load transfer function of a pile was analyzed. Based on the analysis results, the transfer equation of load function considering the crushing characteristics was derived using the finite difference method. The ideal side resistance and the load transfer mechanism of a pile can be obtained by the transfer equation of load function. The predicted response of piles with the proposed method is in good agreement with the response observed in some pile tests reported in the literature. Further study was conducted to discuss the effect of the parameters related to the load transfer model considering crushing characteristics on the load-settlement response of a single pile.

An analytical solution for the karst water inflow into a lined tunnel in an infinite plane is derived based on conformal mapping. The new solution considers the center distance between the tunnel and the cavern, the radii of the tunnel and the cavern, and the property of the lining, such as the permeability coefficient as well as the lining radius. Numerical models are established and calculated using the finite difference software FLAC3D to compare with the analytical solution of inversion transformation, and a good agreement is found. Then, the parameters of effect are discussed in detail. The results indicate that the karst water inflow shows a curve relationship as the radius of tunnel increase and increases as the lining becomes thinner or the permeability coefficient of the lining increases. Moreover, the pressure head decreases as the tunnel radius and the center distance between the tunnel and the cavern increase.

A series of three-dimensional finite element analyses were performed to study the behavior of piles in sloping ground under undrained lateral loading conditions. The analyses have been conducted for slopes with different angles and two loading directions. The obtained results show that as the slope increases, it can cause greater lateral displacement and internal force of the pile. In addition, the increase of the slope ratio will cause the position of the maximum bending moment and soil resistance zero point of the pile to move downward, further increasing the pile deflection. Furthermore, when the pile distance from slope crest B < 7D, the displacement and internal force development of the pile under toward loading is more obvious. When the pile distance from the slope crest exceeds 7D, the effect of loading direction on the pile can be neglected.

An analytical solution for the seepage field in water-filled karst tunnel is derived based on the inversion of complex function and groundwater hydraulics theory. The solution considers the distance between the tunnel and the cavern, the size of the cavern, and the properties of the lining structure, such as the permeability coefficient as well as the radius of the grouting ring. This paper also performed numerical simulations for two cases: the application of gravity and the absence of gravity. The numerical solution was obtained to verify the analytical solution, and a good agreement was found. Then, the effect of parameters is discussed in detail, including the distance between the tunnel and the cavern, the radius of the cavern, the grouting ring, and the initial support. The results show that when the radius of the cavern is constant, the pressure head and seepage flow decrease as the distance between the tunnel and the cavern increases. When the distance is constant, the pressure head and seepage flow increase with the increase of the radius of the cavern. In addition, the pressure head and the seepage flow decrease with the increase of the thickness of the grouting ring and decrease with the decrease of the permeability coefficient. As the thickness of the initial support increases, the pressure head gradually increases and the percolation decreases. Furthermore, due to the great influence of the grouting ring and initial support on the pressure head and seepage flow, the thickness and permeability coefficient of the grouting ring and initial support should be taken into account carefully during construction.

A meta‐analysis examination was implemented to review diabetic foot wound ulcer (DFWU) management by laser therapy (LT). A broad literature examination until May 2023 was done and 1357 interconnected examinations were revised. The 26 elected examinations, enclosed 1067 personals with DFWU were in the utilized examinations 'starting point, 540 of them were utilizing LT, and 527 were utilizing control. Odds ratio (OR) and 95% confidence intervals (CIs) were utilized to appraise the DFWUs management by LT by the dichotomous and continuous approach and a fixed or random model. LT had significantly higher ulcer size decreases (MD, 17.04; 95% CI, 12.48–21.59, p < 0.001) with high heterogeneity (I2 = 99%), and complete healing rate (OR, 2.88; 95% CI, 1.89–4.37, p < 0.001) with no heterogeneity (I2 = 0%) compared with control in personals with DFWU. LT had significantly higher ulcer size decreases, and complete healing rate compared to control in personals with DFWU. Nevertheless, exercise caution while interacting with its values since all the chosen examinations were found with a low sample size for the comparisons in the meta‐analysis. [ABSTRACT FROM AUTHOR]
