The objectives of this study were to evaluate the potential of using thermophilic aerobic digestion (TAD) to hydrolyze aquaculture sludge, and to investigate the hydrolysis efficiency and changes in microbial community structure during TAD at 0, 15, and 30 practical salinity units (psu). As digestion progressed, soluble organic matter concentrations in all reactors increased to their maximum values at 6 h. The hydrolysis efficiency at 6 h decreased as salinity increased: 2.42% at 0 psu, 1.78% at 15 psu, and 1.04% at 30 psu. The microbial community compositions at the genus level prominently differed in the relative abundances of dominant bacteria between 0 psu and 30 psu. The relative abundance of genera Iodidimonas and Tepidiphilus increased significantly as salinity increased. Increase in the salinity at which thermophilic aerobic digestion of aquaculture sludge was conducted altered the microbial community structure, which in turn decreased the efficiency of organic matter hydrolysis.

Indocyanine green (ICG) has been used in clinical practice for more than 40 years and its safety and preferential accumulation in tumors has been reported for various tumor types, including colon cancer. However, reports on clinical assessments of ICG-based molecular endoscopy imaging for precancerous lesions are scarce. We determined visualization ability of ICG fluorescence endoscopy in colitis-associated colon cancer using 30 lesions from an azoxymethane/dextran sulfate sodium (AOM/DSS) mouse model and 16 colon cancer patient tissue-samples. With a total of 60 images (optical, fluorescence) obtained during endoscopy observation of mouse colon cancer, we used deep learning network to predict four classes (Normal, Dysplasia, Adenoma, and Carcinoma) of colorectal cancer development. ICG could detect 100% of carcinoma, 90% of adenoma, and 57% of dysplasia, with little background signal at 30 min after injection via real-time fluorescence endoscopy. Correlation analysis with immunohistochemistry revealed a positive correlation of ICG with inducible nitric oxide synthase (iNOS; r > 0.5). Increased expression of iNOS resulted in increased levels of cellular nitric oxide in cancer cells compared to that in normal cells, which was related to the inhibition of drug efflux via the ABCB1 transporter down-regulation resulting in delayed retention of intracellular ICG. With artificial intelligence training, the accuracy of image classification into four classes using data sets, such as fluorescence, optical, and fluorescence/optical images was assessed. Fluorescence images obtained the highest accuracy (AUC of 0.8125) than optical and fluorescence/optical images (AUC of 0.75 and 0.6667, respectively). These findings highlight the clinical feasibility of ICG as a detector of precancerous lesions in real-time fluorescence endoscopy with artificial intelligence training and suggest that the mechanism of ICG retention in cancer cells is related to intracellular nitric oxide concentration.

Background The retro superior costotransverse ligament space (RSS) block, reported as a novel target in paraspinal block, involves the spreading of local anesthetics into the thoracic paravertebral space through slits around the superior costotransverse ligament. This blocks not only the dorsal rami but also the ventral rami, achieving a reliable complete sensory blockade. Case We performed an RSS block at the T5, T7, and T9 levels on both sides for postoperative analgesia in two patients who underwent laparoscopic gastrectomy. Both patients showed complete sensory blockade from T4 to L1 on the anterior, lateral, and posterior chest walls in the recovery room. The resting and dynamic pain scores were 0 at 30 min and 6 h postoperatively. The pain score consistently remained below 3 throughout postoperative period. Conclusions The RSS block provided effective postoperative analgesia in laparoscopic gastrectomy through definitive complete sensory blockade.

A new fluorescein derivative bearing a boronic acid group was investigated as a fluorescent chemosensor for [F.sup.-]. An off-on type fluorescence enhancement was observed by the blocking of the photoinduced electron transfer mechanism, which was induced by the interaction between fluoride and boronic acid moiety.

Objectives This study aimed to investigate the epidemiology of intravenous midazolam-induced postoperative expressive aphasia (EA). Methods The incidence rate, risk ratio, and contributing factors to intravenous midazolam-induced postoperative EA were analyzed retrospectively in 6756 orthopedic patients. A telephone interview was conducted with patients with EA after surgery. Results Patients were allocated to either the midazolam group (n = 6178) or no-midazolam group (n = 578). Twelve patients developed EA in the midazolam group, with an incidence of 0.19%, and no patient developed EA in the no-midazolam group. The mean age of EA patients was 70 years, and 92% were women. Among them, 75% received general anesthesia, and the mean dose of midazolam was 1.8 mg. EA was reversed in nine of 12 (75%) patients within 4 minutes of flumazenil administration, and >60 minutes were required to reverse EA in the other three patients (25%). Conclusion Intravenous midazolam administration for preoperative sedation caused transient EA in 0.19% of patients, especially elderly women who received general anesthesia, and EA could be reversed by flumazenil.

Background: Musculoskeletal disorders are one of the most common chronic disorders and can develop from repetitive micro-traumas, which occurs often from one’s occupation. Work-related musculoskeletal disorders (WMSD) cost the United States billions of dollars annually. Many traditional therapeutic interventions, like manual therapy. electrical stimulation and hot and cold packs, are being utilized to treat WMSD however there is minimal evidence supporting the use of these interventions to treat WMSD. Therefore, ergonomic interventions (EI) has been proposed as a conservative, non-invasive, and cost-effective intervention to treat WMSD as it functions to correct the cause of repetitive micro-traumas due to one’s occupation by adjusting posture, workstations design, and product selection. Aim: The aim of this paper is to (a) briefly overview the theories of WMSD and EI (b) analyze the efficacy of traditional therapeutic interventions (c) establish the practical applications of EI (d) analyze the efficacy of EI, (e) discuss the contraindications of EI and (f) draw conclusions and discuss the future directions of EI in preventing WMSD. Results and Discussion: It was found that traditional therapeutic interventions provides only short-term pain relief for musculoskeletal disorders, prompting the need for a different approach. EI was found to have promising results in treating WMSD, however there is limited evidence in the form of randomized controlled trials (RCTs) to truly determine the efficacy of EI in addressing WMSD. Further research is needed to determine the efficacy of EI and the long term effects of this intervention in treating WMSD. [ABSTRACT FROM AUTHOR]

Background: Individuals with diabetic peripheral neuropathy (DPN) have significant mobility impairments and these impairments often represent a significant risk of falling. Considering the consequences of impaired mobility resulting from DPN, it is clear that treatment modalities must be applied to address these problems directly. Purpose: To examine the feasibility and possible effects of sonic wave vibration on gait performance in DPN patients. Methods: Eight males (57.3±2.8yrs, 171.8±3.4cm, 79.5±4.7kgs) and ten female DPN patients (61.4±2.1yrs, 162.3±3.6cm, 68.2±4.1kgs) received 12 sessions (three 4-min bouts a day, 3 days a week) of sonic wave vibration therapy (30Hz of frequency with 5 mm of amplitude). The main gait variables were cadence, ambulation, velocity, step time, step length, stride length, and the coefficient of variation for step time, step length and stride length. Results: The results showed a significant decrease in the coefficient of variation for left step time (4.25 to 2.5%), left and right step length (4.75 to 3.08% and 4.5 to 2.6%) and left and right stride length (3.9 to 2.2% and 3.5 to 2.0%) (p<0.05). No significant changes were observed in other gait variables. Conclusion: 4 weeks of sonic wave vibration therapy improved DPN patients' coefficient of variability for step time, step length and stride length. These results suggest that sonic wave vibration can be used as a valuable method to improve gait in DPN patients. Future research is warranted to explore the role of gait variability in the prevention of DPN associated falls and injury. [ABSTRACT FROM AUTHOR]
