Introduction and Objectives: Metabolic Dysfunction-associated Fatty Liver Disease (MAFLD) has been recognized as the hepatic component of metabolic syndrome and has become a growing public health concern. Early and accurate detection of MAFLD is crucial for implementing appropriate intervention strategies and preventing progression to serious complications such as liver cirrhosis. We aimed to describe the diagnostic performance of the Hepatic Steatosis Index (HSI) in subjects with MAFLD and compare it with the Homeostatic Model Assessment of Insulin Resistance (HOMA), Waist-to-Hip Ratio (WHR), and Visceral Fat (SECA). Materials and Methods: Retrospective design study. Bioanthropometric, clinical, and biochemical variables were collected. The HSI was calculated using the formula HSI = 8 * ALT/AST + BMI + 2 (if diabetic) + 2 (if female). ROC curves and their areas were generated. Statistical significance was determined at p < 0.05. Results: A total of 585 subjects were evaluated, of whom 279 were classified with MAFLD (65.5% females) and 306 without MAFLD (76.8% females). Subjects with MAFLD exhibited higher values of age, BMI, WHR, HOMA, CAP, and HSI (Table 1). The HSI showed a diagnostic performance with an area under the curve (AUC) of 0.80. A cutoff point of 39.9 was established for the HSI, with a sensitivity of 63%, specificity of 74%, positive predictive value (PPV) of 73%, and negative predictive value (NPV) of 64%. The HSI demonstrated superior diagnostic performance compared to visceral fat (0.70), HOMA (0.70), and WHR (0.66) (Fig 1). Conclusions: The results of this study demonstrate that HOMA, visceral fat, and ICC can be useful as screening strategies in MAFLD. However, the Hepatic Steatosis Index (HSI) showed superior diagnostic performance compared to the other evaluated biomarkers. Therefore, it is suggested that HSI be considered a useful diagnostic tool in MAFLD.

Introduction and Objectives: Leflunomide is a drug used to treat autoimmune diseases. However, despite its therapeutic benefits, adverse effects have been reported after six months of treatment, including liver damage. Materials and Patients: A 19-year-old female from Acayucan, Veracruz, without significant hereditary or family history, drug addiction denied, with a pathological history of hypothyroidism for 2 years and rheumatoid arthritis since the age of 15, treated with leflunomide 20 mg, chloroquine 150 mg, levothyroxine 100 mcg. She presented in October 2022 with nausea, vomiting, postprandial abdominal pain, jaundice. Abdominal ultrasound revealed chronic calculous cholecystitis. Laparoscopic cholecystectomy was performed in December 2022 without complications. After surgical treatment, he persisted with jaundice, alteration of liver function tests of mixed pattern that evolves to cholestatic pattern (R factor 0.2, see Table 1). Non-reactive to viral hepatitis A, B and C, HIV, negative antibodies ANA, AML, AMA, anti-DNAds, anti-Smith, anti-Rho, complement C3 145 and C4 33. Cholangiography, with hepatomegaly, diffuse hepatic steatosis without evidence of biliary lesions. Liver biopsy showed features of portal and lobular hepatitis, intracytoplasmic and intracanalicular cholestasis, macrovesicular steatosis, and F2 fibrosis according to METAVIR (Fig. 1), which were not consistent with autoimmune or viral disease. RUCAM score with possible relation to hepatoxicity (4 points), so it is considered drug-induced liver injury (leflunomide). Results: Leflunomide was discontinued and treatment was adjusted to chloroquine 150 mg and prednisone 50 mg. Clinical and biochemical improvement was observed. Conclusions: This case highlights the importance of suspecting DILI in patients treated with potentially hepatotoxic drugs and with alterations in liver biochemistry. It is a rare disease for which there are no specific markers. Therefore, liver biopsy is a useful tool for early diagnosis.

Introduction and Objectives: Metabolic-associated fatty liver disease (MAFLD) poses a significant risk for progression to advanced liver diseases, underscoring the need for early detection. This study aims to assess and compare the diagnostic efficacy of non-invasive markers (APRI, FIB-4, Hepamet, and NAFLD Score (NFS) in detecting hepatic fibrosis among MAFLD patients. Materials and Patients: A retrospective examination was performed on adults with MAFLD who had undergone transient liver elastography. Hepatic fibrosis was identified at a cut-off point of ≥8 kPa. APRI, FIB-4, Hepamet, and NFS scores were evaluated with cut-off points determined via the Youden index. Receiver Operating Characteristic (ROC) curves and their areas were computed. All participants provided informed consent. Results: Our cohort consisted of 150 MAFLD patients, the median age of 55 years (48-65), comprising 66.2% (129) females and 33.88% (66) males. The median BMI was 32.1 (28.8-35.6), kPa was 5.6 (4.6-7.8), and CAP was 310 (280-341). Hepatic fibrosis was evident in 24.7% (37) of the participants. Among the evaluated scores, APRI exhibited superior diagnostic performance, achieving an area under the curve of 0.72, followed by FIB-4 (0.66), Hepamet (0.64), and NFS (0.62). The cut-off points of 0.50 for APRI, 1.65 for FIB-4, 0.05 for Hepamet, and -0.75 for NFS yielded sensitivities of 86%, 82%, 86%, and 81%, respectively (Fig 1). Conclusions: Non-invasive scoring systems, notably APRI, demonstrate valuable potential in evaluating hepatic fibrosis among Mexican MAFLD patients. Utilization of adjusted cut-off points enhances test efficiency, thereby facilitating early detection of individuals at greater risk of disease progression.

Antecedentes: El síndrome de intestino irritable (SII) es un trastorno gastrointestinal común en el mundo, con una prevalencia de 8 a 31 % en las personas con obesidad, la cual contribuye a la dislipidemia. Objetivo: Comparar los perfiles de lípidos séricos de pacientes con SII y de controles sanos (CS) para evaluar el riesgo cardiovascular mediante diferentes índices aterogénicos. Material y métodos: Se incluyeron 75 pacientes con SII y 45 CS. Se evaluaron triglicéridos (TG), colesterol total (CT), de lipoproteínas de alta densidad (C-HDL), de baja densidad (C-LDL), de muy baja densidad (C-VLDL) y colesterol no-HDL (C-no HDL); así como índice aterogénico del plasma, índices de riesgo de Castelli I y II (IRC I y II) y coeficiente aterogénico (CA). Resultados: Los niveles séricos de C-HDL fueron más bajos en los pacientes con SII que en CS (p < 0.0001), y significativamente más bajos en pacientes con obesidad y SII (p = 0.05). Los pacientes con SII presentaron mayor riesgo de enfermedad cardiovascular (ECV) que los CS: IRC-I, p = 0.0301; IRC-II, p < 0.0001; AC, p = 0.004. Conclusiones: Dado que las puntuaciones más altas de IRC-I, IRC-II y AC se asociaron positivamente con SII, podrían ser índices predictivos sólidos e independientes de ECV en pacientes con SII.

Pacientes con enfermedad hepática crónica de cualquier etiología que se infectan con SARS-CoV-2 tienen un mayor riesgo de mortalidad en comparación con aquellos pacientes que no tienen enfermedad hepática crónica. Se llevó a cabo una revisión de la literatura en relación a lo publicado de COVID 19 y enfermedad hepática pre-existente. La proporción de pacientes con COVID-19 con función hepática anormal al ingreso osciló entre el 40 % y el 75 % y la proporción con daño hepático fue cercana al 30 %. Los estudios actuales muestran una asociación importante entre la enfermedad hepática preexistente y la COVID-19. La presencia de cirrosis es ahora un predictor independiente de gravedad para COVID-19 y hospitalización prolongada en este grupo de pacientes. Los pacientes con cirrosis tienen una mayor tasa de mortalidad y esta tasa se incrementa con el aumento de la gravedad de la enfermedad hepática.

Antecedentes: Aunque los pacientes con enfermedad celiaca (EC) tienen en su mayoría manifestaciones digestivas, algunos pueden presentarlas de índole extraintestinal (atípicas), como anemia crónica, ataxia y trastornos de la fertilidad. Objetivo: Determinar la prevalencia de anticuerpos relacionados con EC en mujeres mexicanas con trastornos de la fertilidad. Material y métodos: Estudio de casos y controles de mujeres que acudieron a valoración por trastornos de la fertilidad en un centro especializado. Se cuantificó h-tTG IgA, gliadina IgA II y gliadina IgG II; los títulos > 30 UI fueron considerados como positivos. Resultados: Se incluyeron 171 casos y 171 controles; 137 pacientes (80.1 %) tuvieron infertilidad y 34 (19.9 %), esterilidad. Ocho pacientes (4.6 %, IC 95 % = 2.3-8.9) tuvieron al menos un marcador positivo para EC comparadas con una mujer del grupo control (0.5 %, IC 95 % = 0.01-3, p = 0.04, razón de momios = 8.3). Seis de las ocho pacientes presentaron infertilidad inexplicable. Conclusiones: Hasta 4.6 % de las mujeres con infertilidad presentó al menos un marcador positivo para EC. Al igual que en otras partes del mundo, podría recomendarse el escrutinio para EC en mujeres con infertilidad, en especial en quienes padecen infertilidad inexplicable.

Introducción: Una encuesta nacional en población abierta mostró que la enfermedad por reflujo gastroesofágico (ERGE) tiene alta prevalencia en México. Objetivo: Comparar la eficacia y seguridad de dos isómeros, dexrabeprazol (10 mg) versus esomeprazol (20 mg), en el tratamiento de la ERGE durante cuatro semanas. Métodos: Ensayo clínico fase III, aleatorizado, multicéntrico, prospectivo, doble ciego, en dos grupos que incluyeron 230 pacientes. Resultados: Con ambos tratamientos se observó disminución estadísticamente significativa en la severidad de los síntomas de ERGE (pirosis, regurgitación, dolor epigástrico y disfagia), evaluados mediante una escala visual análoga. La puntuación promedio de dexrabeprazol en el Cuestionario de Carlsson-Dent a los 28 días fue de 2.12 y la de esomeprazol de 3.02. Ambos tratamientos fueron efectivos, sin diferencia estadísticamente significativa (p < 0.05). En el Cuestionario de Salud SF-36 se observó que ambos mejoraron la puntuación en la calidad de vida, sin diferencia significativa. Ambos medicamentos fueron bien tolerados y el perfil de incidencia de eventos adversos fue bajo. Conclusiones: En el tratamiento de ERGE no erosiva, el uso de 10 mg/día de dexrabeprazol es tan efectivo como 20 mg/día de esomeprazol, con la ventaja de que la dosis es menor con un adecuado perfil de seguridad.

Interactions between macrophages and adipocytes are early molecular factors influencing adipose tissue (AT) dysfunction, resulting in high leptin, low adiponectin circulating levels and low-grade metaflammation, leading to insulin resistance (IR) with increased cardiovascular risk. We report the characterization of AT dysfunction through measurements of the adiponectin/leptin ratio (ALR), the adipo-insulin resistance index (Adipo-IRi), fasting/postprandial (F/P) immunometabolic phenotyping and direct F/P differential gene expression in AT biopsies obtained from symptom-free adults from the GEMM family study. AT dysfunction was evaluated through associations of the ALR with F/P insulin-glucose axis, lipid-lipoprotein metabolism, and inflammatory markers. A relevant pattern of negative associations between decreased ALR and markers of systemic low-grade metaflammation, HOMA, and postprandial cardiovascular risk hyperinsulinemic, triglyceride and GLP-1 curves was found. We also analysed their plasma non-coding microRNAs and shotgun lipidomics profiles finding trends that may reflect a pattern of adipose tissue dysfunction in the fed and fasted state. Direct gene differential expression data showed initial patterns of AT molecular signatures of key immunometabolic genes involved in AT expansion, angiogenic remodelling and immune cell migration. These data reinforce the central, early role of AT dysfunction at the molecular and systemic level in the pathogenesis of IR and immunometabolic disorders.

Antecedentes: El cáncer colorrectal (CCR) es una de las cinco primeras causas de morbimortalidad por cáncer en nuestro país y en todo el mundo. La prueba inmunoquímica fecal (FIT, fecal immunochemical test) es una herramienta de tamizaje no invasiva que permite seleccionar a los sujetos con mayor probabilidad de lesión premaligna en la colonoscopia. Objetivo: Reportar los resultados del programa de escrutinio para CCR mediante FIT en población abierta en México. Método: Estudio multicéntrico nacional en población abierta mayor de 50 años a través de medios de difusión masiva para participar en un programa de escrutinio de CCR. Se utilizó FIT cuantitativa específica para detectar hemoglobina humana con un punto de corte de 100 ng/ml (prueba positiva). Se realizó colonoscopia a los positivos. Se tomaron biopsias dirigidas de las lesiones premalignas/cáncer para análisis histopatológico. Resultados: Se procesaron 751 FIT, de las cuales 51 (6.8%) fueron positivas, con una tasa de 15.9 lesiones premalignas por cada 1,000 sujetos evaluados, y 1.3 pacientes con CCR por cada 1,000 pacientes. Conclusiones: Nuestro estudio concuerda con lo reportado en la literatura mundial, apoyando así la iniciativa de fomentar el establecimiento de un tamizaje formal y estandarizado dentro del sector de salud pública.

Background and aims. Autoimmune hepatitis (AIH) is a chronic inflammatory condition of the liver in which the immunological mechanisms involved in tissue destruction and/or repair are still unclear. Different pro-inflammatory cytokines have been shown to play a determinant role in AIH pathogenesis. Here, we aim to compare the circulating levels of pro-and anti-inflammatory cytokines such as IL-6, TNF-α, IL-17A/F, IL-21, IL-22, IL-23, and IL-10 in patients with type 2 AIH compared to patients with type 1 AIH and healthy controls (HC). Fourty-six Mexican patients with AIH were recruited in our study. Patients were classified as type 1 or 2 AIH based on immune serological markers. Fourty-four serum samples from healthy individuals were included as controls. Serum cytokine levels were determined by ELISA technique.Results. Compared to healthy controls, serum levels of IL-17F, IL-21, IL-23, IL-10, IL-6, and TNF-α, but not IL-17A and IL-22, were significantly increased in AIH patients. When patients were grouped by aminotransferase activity, a biomarker of active disease, a positive correlation between serum IL-17F and alanine transaminase (rs: 0.4739; P = 0.0009) and aspartate transaminase (rs: 0.4984; P = 0.0004) levels was found. A cytokine signature profile associated with type 2 AIH was characterized by high serum IL-21 (type 1 AIH: 0.66 pg/mL; type 2 AIH: 331.1 pg/mL; P = 0.0042) and IL-22 (type 1 AIH: 0.1 pg/mL; type 2 AIH: 55.26 pg/mL; P = 0.0028) levels.Conclusions. We show for the first time, differential regulation of certain pro-inflammatory cytokines associated with disease progression and AIH type in Mexican patients.

Hepatoblastoma (HB) rarely occurs in adults. We report herein the unusual case of a 19-year-old, otherwise healthy woman with no history of liver disease who presented with upper abdominal pain and hepatomegaly. Tests for hepatitis B virus (HBV), hepatitis C virus (HCV) were negative, and AFP was normal. There was no evidence of liver cirrhosis. A well-demarcated solid mass of 14 cm in diameter, which was lobulated and partly necrotic, was detected in the liver by computed tomography (CT). At surgical exploration a large liver mass was detected occupying the entire right lobe. A right trisegmentectomy was performed with tumor grossly resected with microscopic residual disease (i.e positive margins). On microscopic examination the tumor was composed mainly of two components which were intermingled: epithelial and mesenchymal elements. The epithelial component was formed of small embryonal cells, grouped into nodules, scattered in cellular mesenchymal tissue. The diagnosis was mixed hepatoblastoma. The patient received 4 cycles of systemic chemotherapy with cisplatinum and adriamycin. Post-chemotherapy evaluation revealed recurrence of the hepatoblastoma in the remaining liver. She died 6 months later.

Increasing evidence suggests a microbial pathogenesis in irritable bowel syndrome (IBS) but the relationship remains elusive. Fecal DNA samples from 120 patients with IBS, 82 Mexican (IBS-C: n = 33, IBS-D: n = 24, IBS-M: n = 25) and 38 British (IBS-C: n = 6, IBS-D: n = 27, IBS-M: n = 5), were available for analysis using 16S rRNA gene sequencing. Firmicutes (mean: 82.1%), Actinobacteria (10.2%), and Bacteroidetes (4.4%) were the most abundant taxa. The analysis of all samples (n = 120), and females (n = 94) only, showed no significant differences in bacterial microbiota, but the analysis of Mexican patients (n = 82) showed several differences in key taxa (e.g., Faecalibacterium) among the different IBS subtypes. In IBS-D there were significantly higher Bacteroidetes in British patients (n = 27) than in Mexican patients (n = 24), suggesting unique fecal microbiota signatures within the same IBS subtype. These differences in IBS-D were also observed at lower phylogenetic levels (e.g., higher Enterobacteriaceae and Streptococcus in Mexican patients) and were accompanied by differences in several alpha diversity metrics. Beta diversity was not different among IBS subtypes when using all samples, but the analysis of IBS-D patients revealed consistent differences between Mexican and British patients. This study suggests that fecal microbiota is different between IBS subtypes and also within each subtype depending on geographical location.

Introduction and Objectives: Autoimmune hepatitis (AIH) is a chronic immune-mediated disease with an estimated frequency of 11 to 25 subjects per 100 000 population. It has been reported that viral hepatitis can be a trigger for chronic active hepatitis with AIH criteria. Hepatitis E virus is infrequent in our environment, but it is recognized as the cause of about 20 million new infections per year, being described in case reports as a triggering factor of AIH. Materials and Patients: We present the case of a 39-year-old woman who debuts with jaundice and general condition attack, this, 20 days after her return from Qatar, referring to have consumed raw meat during her stay, denying chronic degenerative history, alcohol and drug consumption. Liver function tests showed changes with R factor >5 compatible with hepatocellular pattern, liver ultrasound showed no liver or biliary changes. AgsVHB (-), HCV (-), VHA IgG (+), IgM (-), AC VHE IgG (-), IgM (+), immunoglobulins IgA 342, IgG 1777 U, IgM 97 U, negative ANA, AML, AMA, anti-LKM-1 antibodies were reported, which was considered probable acute hepatitis E. Cholangioresonance was performed due to the increase in bilirubin at the expense of direct bilirubin, confirming the absence of biliary tract changes. Results: The patient was stable for 4 weeks with clinical improvement and a gradual decrease in bilirubin and transaminases, without evidence of liver damage or encephalopathy. After this period, the patient presented again with an abrupt rise in transaminases of more than 20 times the LSN. In view of these findings, an ultrasound-guided liver biopsy was performed. The histopathological report was consistent with autoimmune hepatitis, and treatment with prednisone and azathioprine was initiated, to which the patient responded favourably. The patient is currently asymptomatic and stable. Conclusions: It is important to consider that acute hepatitis due to HEV is increasingly recognised, although sometimes misdiagnosed and confused with other liver diseases. It is also important to highlight that autoimmune diseases may be preceded by a viral infection due to an inadequate immune response, which forces us to highlight liver biopsy as a useful tool when serological markers are insufficient.

La dispepsia funcional (DF) y la gastroparesia (Gp) representan muchas veces un desafío para el gastroenterólogo, ya que ambas condiciones se caracterizan por síntomas gastrointestinales altos como dolor epigástrico, saciedad temprana y náuseas; con la diferencia de que la DF se diagnostica con base en criterios clínicos, mientras que la Gp requiere una medición objetiva del tiempo de vaciamiento gástrico en ausencia de obstrucción mecánica. Por otro lado, la DF se considera un trastorno de la interacción cerebro-intestino y la Gp un trastorno de la motilidad. El siguiente artículo tiene como objetivo revisar la evidencia sobre mecanismos fisiopatológicos, factores de riesgo, métodos diagnósticos y tratamiento de ambas entidades, para tratar de establecer puntos clave que ayuden a distinguirlas. Sin embargo se reconoce que cada día nos movemos hacia tratar estas enfermedades como un continuum y no como patologías aisladas.

La neurogastroenterología es una subespecialidad emergente en el área de la gastroenterología. Esta revisión explora la enseñanza tutorial en neurogastroenterología, destacando su relevancia en la formación académica y profesional. La enseñanza tutorial, una forma personalizada de educación, ofrece apoyo individualizado a los estudiantes adaptándose a sus necesidades. En neurogastroenterología, los tutores desempeñan un papel crucial al guiar a los tutorados en aspectos clínicos y de investigación, promoviendo habilidades de comunicación y empatía. La relación tutor-tutorado en neurogastroenterología se caracteriza por un aprendizaje mutuo. Los tutores comparten conocimientos, pero también aprenden de las perspectivas frescas de los tutorados, fomentando la innovación en investigación y práctica clínica. En la investigación, los tutores orientan proyectos y desarrollan habilidades investigativas en sus tutorados, facilitando colaboraciones y acceso a recursos. En la formación clínica, guían la evaluación y el tratamiento de los pacientes, incluyendo la enseñanza de procedimientos clínicos especializados. Un buen tutor en neurogastroenterología debe tener un profundo conocimiento de la disciplina, habilidades pedagógicas, empatía y ética profesional. Además, debe fomentar la investigación y mantenerse actualizado. Por otro lado, un buen tutorado debe ser comprometido, empático, poseer habilidades clínicas sólidas, pensar críticamente, trabajar en equipo y comprometerse con la mejora continua. En conjunto, esta relación tutor-tutorado en neurogastroenterología contribuye al desarrollo integral de profesionales competentes y a avances significativos en la comprensión y el tratamiento de los trastornos gastrointestinales.

La neurogastroenterología es una subespecialidad de la gastroenterología que tiene como objetivo el estudio integral de los trastornos la interacción cerebro-intestino, antes denominados «trastornos funcionales digestivos». Estos trastornos representan uno de los motivos de consulta más frecuentes para el gastroenterólogo, consumen recursos de forma notable y afectan significativamente la calidad de vida de los pacientes.

Los trastornos de la sensibilidad rectal hiposensibilidad rectal (HoR) e hipersensibilidad rectal (HeR) juegan un papel determinante en la fisiopatología de los trastornos anorrectales.

La suplementación con fibra dietaria (a través de aumentar la ingesta de alimentos ricos en fibra o el uso de suplementos) es una intervención común que se ha utilizado para el manejo de trastornos metabólicos y gastrointestinales. En esta revisión narrativa, evaluamos el efecto terapéutico de la fibra en los trastornos de la interacción intestino-cerebro (TIIC), con especial atención en el estreñimiento crónico funcional (ECF) y el síndrome del intestino irritable (SII). En el caso del ECF la evidencia recomienda su uso, en especial la fibra soluble ya que mejora el volumen de las heces y acelera el tránsito intestinal, y se emite con cautela la recomendación para la fibra insoluble por los efectos secundarios. En el caso del SII la evidencia es menor y mucho más heterogénea y si bien mejora la consistencia y frecuencia de las evacuaciones, su efecto sobre el dolor abdominal es incierto. Por lo tanto, son necesarios más estudios de calidad para poder realizar recomendaciones específicas y conocer con claridad la dosis, tipo y fuente de fibra requerida, para el manejo tanto de los síntomas en ECF, SII y otros TIIC. Por otra parte, consumo de alimentos naturales ricos en fibra y la co-administración de fibras (por ejemplo, psyllium más inulina) son opciones prometedoras que deben de explorarse.
