Abstract

Background and aims: Endoscopy and the use of fecal calprotectin (fecal CP) are among the least favored methods for assessing disease activity by inflammatory bowel disease (IBD) patients; the handling/processing of fecal samples is also impractical. Therefore, we sought to develop a novel neo-epitope serum calprotectin ELISA, CPa9-HNE, with the aim of quantifying neutrophil activity and NETosis and proposing a non-invasive method for monitoring disease activity in IBD patients. Methods: In vitro cleavage was performed by mixing calprotectin (S100A9/S100A8) with human neutrophil elastase (HNE), and a novel HNE-derived calprotectin neo-epitope (CPa9-HNE) was identified by mass spectrometry for ELISA development. The CPa9-HNE ELISA was quantified in supernatants from ex vivo activated neutrophils and serum samples from patients with ulcerative colitis (UC, n = 43), Crohn’s disease (CD, n = 93), and healthy subjects (HS, n = 23). Abstract

A simultaneous comparison between the effects of dopamine (D) drugs on gastric and pancreatic lesions (as yet known only from separate data) was carried out in Wistar rats (200 g) subjected to 6 h or 48 h restraint stress (RS). Investigated agents (mg/kg b.w.) (D-agonists (AG) bromocriptine 10, amantadine 20, D-antagonists (AN) haloperidol 5, domperidone 5) were given i.p. 1 h before RS. Unstressed (healthy control) and saline preatreated RS-rats were used for comparison. Previously described methods (Sikiric et al., EJP 147, 321, 1988 ; Sikiric et al., EJP 158, 61, 1988) were used for gastric/pancreatic lesions assesment. Results. Time-dependent worseing of stomach and pancreas lesions were noted in RS. 6 h. Stomach. We demonstrated a strong protection for DAG, and for DAN an aggravation inhibited by coadministration of DAG. Pancreas. Domperidone did aggravate the present lesions. No aditional influence of any investigated agent was observed. 48 h. Stomach. Pancreas. No influence of D-drugs was noted. Conclusion. Therefore, based on this parallel and simoultaneous comparison as well known actions of investigated agents as DAG or DAN, these data do not provide the clear evidence for general involvement of D-mechanism in both gastric and pancreatic lesions development in RS. However, the harmful effect of domperidone (6 h-RS), as rather specific peripheral DAN, consistently with our results on bille duct-ligated rats Sikiric et al., EJP 147, 321, 1988), suggests the need for further investigation in RS.

Abstract

Peptic ulcer bleeding is one of the most common emergency situations in medicine. Combined pharmacological and endoscopic therapy together with emerging interventional radiological procedures are successfully treating peptic ulcer disease, reserving surgical procedures for only a small portion of patients unresponsive to 'conventional' therapy. Technological advancement has seen a great improvement in the field of endoscopic treatment in the form of various methods of hemostasis. However, pharmacological therapy with proton pump inhibitors still plays the central role in the peptic ulcer bleeding treatment algorithm. Copyright © 2011 S. Abstract

Background/objectives: Janus kinase inhibitors open new horizons for small-molecule drugs in treating inflammatory bowel disease, with ritlecitinib demonstrating significant efficacy in clinical trials for ulcerative colitis and Crohn's disease. Ritlecitinib, a second-generation JAK3 inhibitor, is a novel therapeutic agent for alopecia areata and other autoimmune conditions., Methods: A new stability-indicating UHPLC-DAD-MS/MS method was developed, validated, and applied for a forced degradation study of ritlecitinib under ICH guidelines., Results: The method demonstrated high specificity, sensitivity (LOD: 0.04 µg/mL; LOQ: 0.14 µg/mL), precision (RSD ≤ 0.15%), and accuracy (99.9-100.3%). Forced degradation studies under acidic, basic, oxidative, thermal, and photolytic conditions revealed four novel degradation products. Basic degradation followed second-order kinetics, while oxidative degradation followed zero-order kinetics., Conclusions: The validated method reliably characterized ritlecitinib's stability and degradation products, providing essential data for optimizing formulation, determining proper storage conditions, anticipating drug-excipient interactions, and ensuring quality control. The eco-friendliness and applicability of the developed forced degradation procedure were evaluated using various green and blue metric tools. Incorporating green analytical principles underscores its potential for sustainable pharmaceutical analysis.
