Over the last several decades, radio detection of air showers has been widely used to detect ultra-high-energy cosmic rays. We developed an experiment under controlled laboratory conditions at SLAC with which we measured the radio-frequency radiation from a charged particle shower produced by bunches of electrons as primaries with known energy. The shower took place in a target made of High Density Polyethylene located in a strong magnetic field. The experiment was designed so that Askaryan and magnetically-induced components of the radio emission could be measured independently. At the same time, we performed a detailed simulation of this experiment to predict the radio signal using two microscopic formalisms, endpoint and ZHS. In this paper, we present the simulation scheme and make a comparison with data characteristics such as linearity with magnetic field and amplitude. The simulations agree with the measurements within uncertainties and present a good description of the data. In particular, reflections within the target that accounted for the largest systematic uncertainties are addressed. The prediction of the amplitude of Askaryan emission agrees with measurements to within 5% for the endpoint formalism and 11% for the ZHS formalism. The amplitudes of magnetically-induced emission agree to within 5% for the endpoint formalism and less than 1% for the ZHS formalism. The agreement of the absolute scale of emission gives confidence in state-of-the-art air shower simulations which are based on the applied formalisms.

As for hypertension, chronic pain, epilepsy and other disorders with particular symptoms, a commonly accepted and unambiguous definition provides a common ground for researchers and clinicians to study and treat the problem. The WHO's ICD11 definition only mentions tinnitus as a nonspecific symptom of a hearing disorder, but not as a clinical entity in its own right, and the American Psychiatric Association's DSM-V doesn't mention tinnitus at all. Here we propose that the tinnitus without and with associated suffering should be differentiated by distinct terms: “Tinnitus” for the former and “Tinnitus Disorder” for the latter. The proposed definition then becomes “Tinnitus is the conscious awareness of a tonal or composite noise for which there is no identifiable corresponding external acoustic source, which becomes Tinnitus Disorder “when associated with emotional distress, cognitive dysfunction, and/or autonomic arousal, leading to behavioural changes and functional disability.”. In other words “Tinnitus” describes the auditory or sensory component, whereas “Tinnitus Disorder” reflects the auditory component and the associated suffering. Whereas acute tinnitus may be a symptom secondary to a trauma or disease, chronic tinnitus may be considered a primary disorder in its own right. If adopted, this will advance the recognition of tinnitus disorder as a primary health condition in its own right. The capacity to measure the incidence, prevalence, and impact will help in identification of human, financial, and educational needs required to address acute tinnitus as a symptom but chronic tinnitus as a disorder. © 2021 Elsevier B.V.

RNO is the mid-scale discovery instrument designed to make the first observation of neutrinos from the cosmos at extreme energies, with sensitivity well beyond current instrument capabilities. Background: Maintenance therapy following autologous stem cell transplantation (ASCT) can delay disease progression and prolong survival in patients with multiple myeloma. Ixazomib is ideally suited for maintenance therapy given its convenient once-weekly oral dosing and low toxicity profile. In this study, we aimed to determine the safety and efficacy of ixazomib as maintenance therapy following ASCT. Methods: The phase 3, double-blind, placebo-controlled TOURMALINE-MM3 study took place in 167 clinical or hospital sites in 30 countries in Europe, the Middle East, Africa, Asia, and North and South America. Eligible participants were adults with a confirmed diagnosis of symptomatic multiple myeloma according to International Myeloma Working Group criteria who had achieved at least a partial response after undergoing standard-of-care induction therapy followed by high-dose melphalan (200 mg/m2) conditioning and single ASCT within 12 months of diagnosis. Patients were randomly assigned in a 3:2 ratio to oral ixazomib or matching placebo on days 1, 8, and 15 in 28-day cycles for 2 years following induction, high-dose therapy, and transplantation. The initial 3 mg dose was increased to 4 mg from cycle 5 if tolerated during cycles 1–4. Randomisation was stratified by induction regimen, pre-induction disease stage, and response post-transplantation. The primary endpoint was progression-free survival (PFS) by intention-to-treat analysis. Safety was assessed in all patients who received at least one dose of ixazomib or placebo, according to treatment actually received. This trial is registered with ClinicalTrials.gov, number NCT02181413, and follow-up is ongoing. Findings: Between July 31, 2014, and March 14, 2016, 656 patients were enrolled and randomly assigned to receive ixazomib maintenance therapy (n=395) or placebo (n=261). With a median follow-up of 31 months (IQR 27·3–35·7), we observed a 28% reduction in the risk of progression or death with ixazomib versus placebo (median PFS 26·5 months [95% CI 23·7–33·8] vs 21·3 months [18·0–24·7]; hazard ratio 0·72, 95% CI 0·58–0·89; p=0·0023). No increase in second malignancies was noted with ixazomib therapy (12 [3%] patients) compared with placebo (eight [3%] patients) at the time of this analysis. 108 (27%) of 394 patients in the ixazomib group and 51 (20%) of 259 patients in the placebo group experienced serious adverse events. During the treatment period, one patient died in the ixazomib group and none died in the placebo group. Interpretation: Ixazomib maintenance prolongs PFS and represents an additional option for post-transplant maintenance therapy in patients with newly diagnosed multiple myeloma. HF-induced Descending Artificial Plasma Layers (DAPLs) are artificially ionized plasma layers with plasma density in excess of that of the F2-peak. They were discovered during HF heating experiments at HAARP where they descended up to 70 km from the initial O mode wave reflection height. The DAPLs were attributed to the ionization of the neutral gas by high-energy electrons accelerated by the artificial ionospheric turbulence. Recently, DAPL formation was reported during the HF-heating experiment at Arecibo [Bernhardt et al., 2017]. This result was unexpected since Arecibo has the Effective Radiated Power (ERP) 4 – 5 times lower than that at HAARP, and since the experiment at Arecibo also has an unfavorable geometry, where the HF beam is directed vertically while the inclination of the geomagnetic field is 43.5o, allowing the fast electrons to escape the volume where their interaction with the artificial plasma turbulence occurs. However, the presence of photoelectrons due to the UV radiation from the Sun at the low latitude of Arecibo could magnify the flux of hot electrons. A model of artificial plasma layers created by the Arecibo HF facility is presented. It shows that Langmuir turbulence due to the HF heating can accelerate part of the ambient photoelectrons to energies above the ionization threshold of the neutral gas, leading to the formation of DAPLs. The present model results are in quantitative agreement with the experiments of Bernhardt et al. [2017].
