Abstract

The improvement of children's mental health has been a governmental priority over recent years (DH & NHS, 2015; DHSC & DfE, 2017;2018), with increasing numbers of children and young people experiencing mental health difficulties (Sadler et al., 2018, PHE, 2016; WHO, 2014). Guidance positions schools as providing a key platform for the improvement of children's mental health, through approaches the seek to identify and prevent problems, provide quicker access to support and promote resilience. Despite parents being in a prime position to promote children's resilience and support mental health, evidence suggests that there are challenges in the way that the parent role is currently conceptualised within the agenda to improve children's mental health. Research indicates that parents are one of the most likely persons to refer their child to a professional for support with their mental health (Crenna-Jennings & Hutchinson, 2019; Frith, 2017), this in light of rising referrals to child and adolescent mental health services (CAMHs), stretched services and increased waiting times points towards the need to explore and consider how the parent role might be supported within this context. This research adopted a constructivist grounded theory (Charmaz, 2014) approach to explore collaboratively with 11 parents of primary school aged children, how they understood their role in the promotion of their children's positive mental health and in preventing possible mental health problems. The findings indicated that the parents saw themselves as holding the responsibility for their children's mental health and were motivated to work alongside school in order to fulfil this role. In implementing actions to promote and support their children's mental health, the parents experienced empowerment, disempowerment and uncertainty, drawing attention towards the need for parentally inclusive approaches that recognise and empower them within their role. The research finds implications for schools and for Educational Psychologists (EPs) in light of their unique and distinctive role working with schools, children and parents (Beaver, 2011; Cameron, 2005).

3. Contact, Cigarette and Flame Burns in Physical Abuse: A Systematic Review.

Abstract

This systematic review identifies features of intentional non-scald burns in physical abuse. Fifteen bibliographic databases of international literature (1950-2011) were searched to identify potential studies that were critically appraised using standardised methods. Primary studies with confirmed intentional non-scald burns in children aged < 18 years were included to provide a descriptive analysis of the causation, appearance and distribution of burns. Twenty studies were included: one cross-sectional, one case control and 18 case studies/small case series representing 73 children with intentional non-scald burns. The majority were contact burns from household items: the agents included cigarettes (18), irons (9), electric fires/heaters/radiators (10), cigarette lighters (2), hairdryers (7), curling tongs (3), chemicals (3), microwaves (2) flame burns (7), miscellaneous (7) and burns of unknown cause (5). The burns occurred throughout childhood. The majority of children were older than three years. The characteristic features were of clearly demarcated contact burns or scars in shapes that mirrored the agent. They were predominantly recorded on the limbs, trunks and the backs of hands; they were frequently multiple and co-existed with additional signs of abuse. Abstract

Retinal haemorrhages (RH) in a young child suggest child abuse, yet other causes merit consideration. This systematic review addresses non-traumatic causes of RH in children with one or more overlapping clinical features of abuse (bruising, fractures, intracranial haemorrhage). Searching 12 databases (1950-2009) identified 338 studies for review by two reviewers. Inclusion criteria included: children < 15 years examined by an ophthalmologist detailing retinal findings, confirmed organic cause and exclusion of abuse. Exclusion criteria included: diagnosed coagulopathy, ocular mass lesion and post-mortem cases. Twenty included studies identified nine relevant conditions (metabolic diseases, bony dysplasias, bleeding disorders and vascular malformations). Where detailed, the RH were bilateral and involved only one area of the retina (the posterior pole). Among proposed confounders of RH, no child with a cough (100 children) or acute life-threatening event (184 cases) had RH. Just two of 217 children with seizures had posterior pole RH, one unilateral and one bilateral. One cardiopulmonary resuscitation (CPR) study (43 cases) was included where a child with co-existent clotting abnormalities had tiny (bilateral dot) RH after 60 minutes' resuscitation. Child abuse remains the commonest cause of RH in young children, although rarer disorders merit consideration. There are insufficient data to conclude that CPR is a confounder in children with RH. Abstract

Context and Objective: To determine which combinations of clinical features assist in distinguishing abusive head trauma (AHT) from nonabusive head trauma., Methods: Individual patient data from 6 comparative studies of children younger than 3 years with intracranial injury were analyzed to determine the association between AHT and combinations of apnea; retinal hemorrhage; rib, skull, and long-bone fractures; seizures; and head and/or neck bruising. An aggregate analysis of data from these studies used multiple imputation of combined clinical features using a bespoke hotdeck imputation strategy, which accounted for uncertainty arising from missing information., Results: Analyzing 1053 children (348 had AHT), excluding nonsignificant variables (gender, age, skull fractures), for a child with an intracranial injury and 1 or 2 of the 6 features, the positive predictive value (PPV) of AHT varies from 4% to 97% according to the different combinations. Although rarely recorded, apnea is significantly associated with AHT (odds ratio [OR]: 6.89 [confidence interval: 2.08-22.86]). When rib fracture or retinal hemorrhage was present with any 1 of the other features, the OR for AHT is >100 (PPV > 85%). Any combination of 3 or more of the 6 significant features yielded an OR of >100 (PPV for AHT > 85%)., Conclusions: Probabilities of AHT can be estimated on the basis of different combinations of clinical features. The model could be further developed in a prospective large-scale study, with an expanded clinical data set, to contribute to a more refined tool to inform clinical decisions about the likelihood of AHT.
