Introduction: The COVID-19 pandemic has disproportionately affected individuals residing in Long-Term Care Facilities (LTCFs), necessitating tailored strategies to manage outbreaks. This study examines the outcomes of the ILPI BH project, a collaborative effort between the Municipal Health Department and the Hospital das Clínicas of the Federal University of Minas Gerais, designed to mitigate COVID-19 spread within LTCFs. Methods: Prospective cohort of secondary data: 1,794 old residents in 99 long-term care facilities of Belo Horizonte, Brazil, were followed from May 2020 to January 2021. The study analyzed the prevention strategies, residents’ clinical data, and the characteristics of the long-term care facilities, correlating these variables with the number of infections, hospitalizations, and deaths from COVID-19. It checked absolute numbers and rates of incidence, hospitalization, mortality, and lethality. Results: There have been 58 COVID-19 outbreaks in long-term care facilities. There were 399 cases among residents, 96 hospitalizations for COVID-19 and 48 deaths from COVID-19 (2.7 % of the cohort), with a case fatality rate of 12 %. After multivariate analysis, the intrinsic variables to residents associated with higher mortality risk were higher degree of frailty (OR=1.08; p = 0.004) and the fact of living in a long-term care facility with a considerable proportion of residents’ coverage by health plans (OR = 1.01; p = 0.028). Early geriatric follow-up showed an association with a reduction in the number of hospitalizations due to COVID-19. Conclusion: The correct classification of the degree of frailty of institutionalized older people seems to have been relevant for predicting mortality from COVID-19. The extensive assistance by private health plans, contrary to what is supposed, did not result in better health protection. Early geriatric follow-up was beneficial and may be an attractive strategy in the face of health emergencies that affect long-term care facilities to reduce hospital admissions.

Objetivo: caracterizar a vulnerabilidade clinico funcional e as condições de saúde de idosos com Parkinson de um serviço de atenção especializada de um município de grande porte brasileiro. Método: trata-se de um estudo documental, transversal, realizado com 230 idosos com diagnóstico de Doença de Parkinson. Foram avaliados os prontuários dos pacientes com enfoque nos instrumentos: Índice de Vulnerabilidade Clínico Funcional-20 e o Plano de Cuidados da avaliação multidimensional. As análises estatísticas foram realizadas utilizando o software R. Resultados: predomínio de idosos entre 80 e 89 anos, sexo feminino e baixa escolaridade. Apresentaram como principais marcadores de vulnerabilidade clinico funcional o comprometimento da mobilidade, incontinência urinária, sarcopenia, marcha/quedas, distúrbio de humor, comorbidade múltipla e polifarmácia. Conclusão: os achados mostraram alta prevalência de comprometimento funcional na doença de Parkinson. Acredita-se que os resultados possam contribuir na melhoria do cuidado a essa população, possibilitando aos profissionais da enfermagem atuarem na promoção do autocuidado.

ABSTRACT Objectives: to analyze the sociodemographic and clinical factors associated with frailty in the elderly with Parkinson’s disease in the context of a Specialized Health Care Service for the Elderly. Methods: documentary, analytical, correlational, cross-sectional study carried out with 230 elderly people with Parkinson’s. The medical records of the patients were evaluated with a focus on two instruments: Clinical-Functional Vulnerability Index - 20; and the Multidimensional Assessment Care Plan. Results: predominance of elderly people between 80 and 89 years old; women; with low education; classified as fragile. There was a correlation between frailty and the variables: sex; age; age group; schooling; motor manifestations; non-motor manifestations; Hoehn and Yahr staging and time to diagnosis. Conclusions: high simultaneity of frailty conditions and Parkinson’s disease was observed. It is believed that the results will be able to help the structuring of protocols for nursing performance in a systematic way in the promotion of self-care of these individuals.

OBJETIVO: Compreender as experiências de cuidadores familiares com sobrecarga e desconforto emocional, ao cuidarem de idosos dependentes no domicilio. MÉTODOS: Pesquisa realizada com o método da grounded theory, da qual participaram nove cuidadores, considerando-se a saturação teórica. Os dados foram coletados em visita domiciliar por entrevista aberta e analisados pelos processos de codificação, aberta, axial e seletiva. RESULTADOS: Substantivamente, obteve-se que o idoso dependente com necessidades de cuidados no domicílio estimulou movimento na família, pelo qual um familiar tornou-se cuidador. Com a vivência deste papel, desenvolveu sobrecarga, desgaste emocional, repercutindo na qualidade de vida, demandando atenção, apoio e capacitação. Produziu saberes e experiências de cuidado ao persistir no desempenho do papel. CONCLUSÕES: Cuidadores familiares necessitam de recursos assistenciais do sistema de saúde, de suporte emocional e de aprendizagem de processos. São necessárias pesquisas sobre planos de cuidados interprofissionais aos cuidadores familiares no âmbito de políticas e serviços de atenção domiciliar.

Objective: to describe the sociodemographic characteristics associated with depressive symptoms in hospitalized elderly. Methods: cross-sectional study carried out in a public teaching hospital. The sample was composed of 96 individuals aged ≥60 years, hospitalized in a clinical and surgical unit. The data collection was performed through a semi-structured interview with the instrument Geriatric Depression Scale. The variables were described through frequencies, percentages, means and standard deviations. The Fisher’s exact test was used to assess the association with depressive symptoms. Results: a total of 45.8% of the elderly presented mild to moderate depressive symptoms and 1.0% had severe symptoms. There was a predominance of women, between 60 and 79 years old, living with a partner, with low level of education (one to seven years of schooling), Catholics, retirees or pensioners, and with a family income of up to three minimum wages. Conclusion: the labor situation presented a statistically significant association with depressive symptoms in the hospitalized elderly.

Methods: this is a qualitative research with a contribution in the Theory Grounded in Data. There were home visits for observation and semi-structured interviews with nine relatives of dependent elderly in self-care. Results: with the coding and analysis of empirical data, one can understand the daily cares in the care relationship with their elderly dependent relatives. The consolidated experiences underlie on positive experiences, such as solidarity by the established interaction and the maintenance of self-esteem, and negative as changes in daily routine and health, with stress identification related to the caregiver role. Conclusion: in the understanding of the family, their experiences as a caregiver in the home context varied between positive and negative aspects, which respectively minimize and maximize the feeling of burden and emotional distress.

Objetivo: compreender os significados, para idosos dependentes, do cuidado familiar no domicílio. Método: pesquisa qualitativa apoiada nos pressupostos teórico-metodológicos da Teoria Fundamentada nos Dados. Foram sujeitos seis idosos com cognição preservada. Na coleta de dados, realizada no domicílio, entre junho e outubro de 2012, utilizou-se entrevista semiestruturada. Resultados: as codificações dos dados permitiram classificar as necessidades cotidianas de cuidados, a percepção e os sentimentos dos idosos nos cuidados que recebem e a construção de um esquema conceitual sobre o fenômeno cuidado domiciliar sob a ótica de idosos dependentes. Expressaram sentimentos ambivalentes de segurança e gratidão, por terem um familiar à frente dos cuidados, e de impotência e tristeza, por necessitarem de uma pessoa para realizar as atividades que antes realizavam sem ajuda. Conclusão: apesar das dificuldades e dos problemas enfrentados no cuidado domiciliar, os idosos dependentes sentem satisfação em serem cuidados em casa, perto da família, por um cuidador familiar.Descritores: Assistência Domiciliar; Idoso; Cuidadores.

OBJETIVO: Avaliar o efeito do óleo de copaíba nos níveis séricos de uréia e creatinina em ratos submetidos a síndrome de isquemia e reperfusão renal. MÉTODOS: Foram utilizados 18 ratos (Rattus norvegicus albinus),da linhagem Wistar, fêmeas, adultas, entre 90 e 120 dias de idade, pesando ente 200g e 250g, distribuídos em dois grupos: Isquemia e Reperfusão (GIR), e Isquemia e Reperfusão Copaíba (GIRC). Os animais dos dois grupos foram submetidos à isquemia renal, de ambos os rins, por 50 minutos, seguida de reperfusão por 24, 48 e 72 horas, com posterior coleta de sangue e análise dos níveis séricos de uréia e creatinina. No GIRC, realizou-se, além da isquemia e reperfusão, a administração diária do óleo de copaíba na dose de 0,63 ml/kg, por gavagem, sete dias antes do procedimento de isquemia renal. RESULTADOS: Foi observada uma diminuição estatisticamente significante dos níveis séricos de uréia no GIRC em 24 e 48 horas de reperfusão renal e uma diminuição do nível sérico de creatinina no GIRC em 48 horas de reperfusão renal quando comparados com o grupo Controle. CONCLUSÃO: Segundo os procedimentos aplicados, o óleo de copaíba diminuiu os níveis séricos de uréia em 24 horas e 48 horas e os de creatinina nas 48 horas após o procedimento de isquemia e reperfusão renal em ratos.PURPOSE: To evaluate the copaiba oil effect on urea and creatinine levels in rats submitted to kidney ischemia and reperfusion syndrome. METHODS: Eighteen Wistar rats (Rattus norvegicus albinus), aged between 90 and 120 days, weight between 200g and 250g, were allocated in 2 groups (n=9) and submitted to 50 minutes of renal ischemia and reperfusion and treated or not with copaiba oil (0,63ml/kg daily seven days before ischemia). The nitrogen excrements were assessed at 24, 48 and 72 hours after ischemia period. RESULTS: The urea serum level was smaller (p d" 0,05) at 24 and 48 hours, and the creatinine serum level was smaller at 48 hours in animals treated with copaiba oil (GIRC) than the GIR. CONCLUSION: The copaiba oil decreased significantly the urea serum level at 24 and 48 hours and the creatinine level at 48 hours after kidney ischemia and reperfusion in rats.

A possible mechanism of susceptibility to superinfection with simian-human immunodeficiency virus (SHIV)-1157ipd3N4 was explored in twelve SHIVSF162P3-infected Chinese rhesus macaques. Based on the kinetics of viral replication for the second infecting virus following SHIV-1157ipd3N4 inoculation, the monkeys were divided into two groups: those relatively resistant to superinfection (SIR) and those relatively sensitive to superinfection (SIS). We found that superinfection-resistant macaques had high primary viremia, whereas superinfection-sensitive macaques had low primary viremia, suggesting that primary SHIVSF162P3 infection with a high viral-replication level would repress superinfection with a heterologous SHIV-1157ipd3N4. Although no correlation of protection against superinfection with virus-specific CD4(+) T cell or CD8(+) T cell immune responses from gut was observed prior to superinfection, superinfection susceptibility was strongly correlated with CD4(+) Tcm cells from gut both prior to the second infecting virus inoculation and on day 7 after superinfection, but not with CD4(+) Tem cells from gut or with CD4(+) Tcm cells from peripheral blood and lymph node. These results point to the important roles of gut-derived CD4(+) Tcm cells for the study of the mechanisms of protection against superinfection and the evaluation of the safety and efficacy of vaccines and therapies against acquired immune deficiency syndrome (AIDS).

ABSTRACT Objective: To analyze the experiences of family caregivers of dependent older adults, who show performance overload and emotional distress, using the Theory of Culture Care. Method: Qualitative study with nine caregivers of home care dependent older adults, based on Grounded Theory. Results: The findings allowed the identification of potentialities and frailties in the context of family home care and subsidizing the construction of a theoretical scheme resulting from the analysis of possibilities of the nursing care practice according to the culture, through the three modes of action: maintenance, adjustment and repatterning of cultural care. Final considerations: Respecting the cultural values and family beliefs, the nurse can help to institute mutually established changes, promoting a better quality in the nursing care relationship and a relief to the strain of the role of the caregiver.

ABSTRACT Objective: To determine the self-perceived health status and clinical-functional vulnerability of the elderly attended at a Reference Center of Minas Gerais, Brazil and to evaluate the association between these variables through the Clinical-Functional Vulnerability Index (IVCF-20) instrument. Method: This is an epidemiological, retrospective study of 311 medical records. Statistical analyses were performed using the Stata program; the evaluations were by Pearson’s Chi-square test and Poisson regression models. Results: The majority of the elderly presented negative self-perceived health status(70.10%); there was statistical significance between negative self-perceived health and the variables of mood and recent hospitalization. Conclusion: Perceived health status influences the morbidity and mortality of the elderly. Mood disorders and recent hospitalizations directly interfere with active aging.
