Abstract This study aimed to evaluate the association between ultra-processed food (UPF) and dental caries, considering muscle mass (MM), bone mineral density (BMD), and oral hygiene habits (OHH) as mediators. This study has an analytical cross-sectional design with 2,515 adolescents (18–19 years). The main exposure – the UPF intake ratio – was established using the food frequency questionnaire. The outcome was the number of decayed teeth, according to the DMFT index. The model adjustment included socio-economic status (SES), frequency of physical activity, and concurrent risk habits (CRH) as potential confounders. Three latent variables were considered: SES (family income, economic class, household head, and adolescent education), OHH (gingival bleeding on probing index and visible plaque index), and CRH (alcohol and tobacco dependence). Resumo Este artigo pretende compreender a perspectiva de empresários de Clínicas Populares de Saúde (CPS) e representantes da classe médica sobre serviços ofertados pelo setor; impactos decorrentes da pandemia covid-19; e futuro do mercado de trabalho médico. Trata-se de pesquisa qualitativa, da área de saúde coletiva, com enfoque nas representações sociais. Foram realizadas entrevistas semiestruturadas, entre março e julho de 2021, com quatro empresários locais e três representantes de classe médica de uma cidade da região Nordeste do Brasil. As CPS ofertam serviços assistenciais restritos a consultas e exames e com estratégias de lógica financeirizada; se apresentam como “alternativa” ao SUS, uma suposta “lacuna” entre planos de saúde privados e serviços públicos, e como “novo” trabalho médico. As empresas ofertam consultas com especialistas a preços “populares” e sem fila de espera. A assistência prestada é restrita e os profissionais não têm garantia de direitos trabalhistas. Para os entrevistados, o acesso à saúde representa acessibilidade geográfica e temporal de serviço a preço reduzido. O direito universal à saúde e princípios do SUS são confrontados com a defesa da autonomia dos clientes e dos profissionais visando suas necessidades: saúde e trabalho.

Abstract Objective To assess the direct, indirect, and total effects of violence during pregnancy on perinatal outcomes, and to evaluate the effect of violence as a moderator of the mediated relationship of depression with perinatal outcomes. Methods Data was collected from the prenatal study and follow-ups of the BRISA cohort, São Luís, Maranhão, Brazil. The perinatal outcomes investigated were: birth weight (BW), intrauterine growth restriction (IUGR) and gestational age (GA). Violence against women was evaluated using the World Health Organization Violence against Women instrument (Violence during pregnancy – regardless of the type of violence; Physical violence during pregnancy; Psychological violence during pregnancy). Depressive symptoms during pregnancy were evaluated as a mediating variable. Moderated mediation analysis was performed to estimate the effects of violence and depression on perinatal outcomes. Results Three types of violence analyzed by depression had an indirect effect in BW and GA. None of the types of violence showed an association with IUGR. All types of violence analyzed showed a moderated mediation effect with BW and GA. Only among women who experienced violence were birth weight and gestational age lower the higher the values of depressive symptoms. Conclusion Violence and depression are only associated with lower BW and GA when they occur simultaneously.

ABSTRACT Objective: To understand the challenges in the care of children born to mothers with COVID-19 during the pandemic. Method: Qualitative research, conducted from November 2020 to May 2021, in two public maternity hospitals, with women who had children during the first wave of the pandemic, diagnosed with COVID-19 during pregnancy and/or childbirth. There were nineteen semi-structured interviews with thematic analysis. Symbolic Interactionism was used as a theoretical framework. Results: Changes in the care for newborn children were identified. In the domestic environment, hygiene measures with the newborn children were intensified, social isolation restricted the support network and mothers felt alone and overwhelmed. In terms of health care, there was a setback in neonatal care and interruption of professional care, such as the suspension of appointments. Conclusion: The pandemic has restructured traditional models of family care, intensified difficulties in accessing healthcare, and exposed children to inherent risks due to a lack of proper follow-up.

Abstract Background A nutrient-poor and hypocaloric diet may be associated with lower handgrip strength (HGS), whereas a high-quality or balanced diet may be associated with higher HGS. However, no study has used the NOVA system for classifying food by their degree of processing. Objective To analyze the association between food consumption according to the degree of food processing and HGS in Brazilian teenagers. Methods This cross-sectional study included teenagers aged 18 and 19 years old from the 1997/98 São Luís’ birth cohort, Maranhão, Brazil. HGS (kilogram-force) was measured via a Jamar Plus + dynamometer. Food consumption was assessed using a semiquantitative food frequency questionnaire. The energy intake of culinary preparations (unprocessed or minimally processed food and processed culinary ingredients), processed, and ultra-processed foods was evaluated in percentages and categorized in tertiles. The associations between each food group intake and HGS was estimated via crude and adjusted linear regression models. A directed acyclic graph was used to identify confounding factors. Results We evaluated 2,433 teenagers, 52.1% of which were girls. For boys, adjusted analysis showed an association between the highest HGS and the 3rd tertile of culinary preparation consumption (β: 1.95; 95%CI: 0.80; 3.10) and between the lowest HGS and the 3rd tertile of ultra-processed food consumption (β: -2.25; 95%CI: -3.40; -1.10). Among girls, the consumption of culinary preparations in the 3rd tertile was associated with higher HGS (β: 0.76; 95%CI: 0.05; 1.46). Conclusions Higher consumption of culinary preparations and lower consumption of ultra-processed foods can contribute to reduce the chance of lower HGS in adult life. Interventions to promote the development and preservation of muscle strength should include dietary recommendations.

Abstract Background The use of telemedicine, or the provision of healthcare and communication services through distance-based technologies, has increased substantially since the 2019 novel coronavirus (COVID-19) pandemic. However, it is still unclear what are the innovative features of the widespread use of such modality, its forms of employment and the context in which it is used across pluralist health systems, particularly in low- and middle-income settings. We have sought to provide empirical evidence on the above issues by analysing the responses of medical doctors in a representative cross-sectional survey in two states in Brazil: São Paulo and Maranhão. Methods We analysed the responses of 1,183 physicians to a survey on the impact of COVID-19 on their livelihood and working practice. Two independent samples per state were calculated based on a total of 152,511 active medical registries in São Paulo and Maranhão. Proportional stratified sampling was performed and the distributions for gender, age, state and location of address (capital or countryside) were preserved. The survey contained questions on the frequency of physicians’ employment of telemedicine services; the specific activities where these were employed, and; the forms in which the pandemic had influenced the adoption or consolidation of this technology. We performed descriptive and univariate analysis based on the chi-square test or Fisher's exact test for the qualitative data, and the Mann–Whitney test in the quantitative cases. Data were shown as absolute frequency and proportion with a 95% confidence interval. Results In our sample of physicians, telemedicine was employed as a form of clinical collaboration by most doctors (76.0%, 95 CI 73.6–78.5), but only less than a third of them (30.6%, 95 CI 28.0–33.3) used it as a modality to provide healthcare services. During the pandemic, telemedicine was used predominantly in COVID-19-related areas, particularly for hospital-based in-patient services, and in private clinics and ambulatory settings. ObjectiveTo analyze the experiences of maternal health workers in three Brazilian cities, located in the Northeast (São Luís), Southeast (Niterói), and South (Pelotas) regions during the first year of the COVID-19 pandemic.MethodsQualitative research carried out between December 2020 and February 2021. Interviews were conducted, in person or remotely, with 30 health workers, doctors and nurses, working in maternity hospitals of different degrees of complexity.ResultsSociodemographic characteristics, employment relationships and professional qualification of the interviewees were described. Two thematic axes were identified: 1) changes in hospital organization and dynamics in the pandemic; 2) Illness and suffering of health workers. The majority of respondents were women. Most physicians had work relationships in the public and private sectors. In Niterói, health workers had better professional qualifications and more precarious work relationships (as temporary hires), compared to São Luís and Pelotas. In the context of the uncertainties resulting from the pandemic, this situation generated even more insecurity for those workers. The statements at the beginning of the pandemic covered topics such as changes in the organizational dynamics of services, healthcare, telemedicine, and interaction between health workers and users. In the health workers' perception, the initial period of the health emergency, which resulted in intense changes in the provision of services, was marked by an increase in preterm births, perinatal mortality, and fetal losses. Work overload, fear of contamination, concern for family members and uncertainties regarding the new disease caused intense suffering in health workers who had little institutional support in the cities studied. The suffering experienced by health workers went beyond the work dimension, reaching their private life.ConclusionChanges caused by the pandemic required immediate adjustments in professional practices, generating insecurities in healthcare regardless of the location studied. The method of hiring health workers remained the same as the previously practiced one in each city. Due to the risk of disease transmission, measures contrary to humanization practices, and more restrictive in São Luís, were reported as harmful to obstetric care. The Covid-19 pandemic was a huge challenge for the Brazilian health system, aggravating the working conditions experienced by health workers. In addition to the work environment, it was possible to briefly glimpse its effects on private life.

Abortion, particularly when illegal, highlights inequities in different populations. Although abortion-related mortality is lower compared to other obstetric causes, abortion complications tend to be more lethal. Delays in seeking and obtaining care are determinants of negative outcomes. This study, nested within the GravSus-NE, analyzed healthcare delays and their association with abortion-related complications in three cities of northeastern Brazil (Salvador, Recife and São Luís). Nineteen public maternity hospitals were involved. All eligible women ≥18 years old hospitalized between August and December 2010 were evaluated. Descriptive, stratified and multivariate analyses were performed. Youden's index was used to determine delay. One model was created with all the women and another with those admitted in good clinical conditions, thus determining complications that occurred during hospitalization and their associated factors. Of 2,371 women, most (62.3%) were ≤30 years old (median 27 years) and 89.6% reported being black or brown-skinned. Most (90.5%) were admitted in good condition, 4.0% in fair condition and 5.5% in poor/very poor condition. Median time between admission and uterine evacuation was 7.9 hours. After a cut-off time of 10 hours, the development of complications increased considerably. Black women and those admitted during nightshifts were more likely to experience a wait time ≥10 hours. Delays were associated with severe complications (OR 1.97; 95%CI: 1.55-2.51), including in the women admitted in good condition (OR 2,56; 95%CI: 1.85-3.55), and even following adjustment for gestational age and reported abortion type (spontaneous/induced). These findings corroborate the literature, highlighting the social vulnerability of women hospitalized within Brazil's public healthcare system in a situation of abortion. The study strongpoints include having objectively measured the time between admission and uterine evacuation and having established a cut-off time defining delay based on conceptual and epidemiological criteria. Further studies should evaluate other settings and new measurement tools for effectively preventing life-threatening complications.

The COVID-19 pandemic has impacted public and private health systems around the world, impairing good practices in women's health care. However, little is known about the experiences, knowledge, and feelings of Brazilian women in this period. The objective was to analyze the experiences of women, seen at maternity hospitals accredited by the Brazilian Unified Health System (SUS, acronym in Portuguese), regarding health care during pregnancy, childbirth, and postpartum periods, their interpersonal relationships, and perceptions and feelings about the pandemic. This was a qualitative, exploratory research, carried out in three Brazilian municipalities with women hospitalized in 2020, during pregnancy, childbirth, or postpartum period, with COVID-19 or not. For data collection, semi-structured individual interviews (in person, by telephone, or by digital platform) were conducted, recorded and transcribed. The content analysis of thematic modalities was displayed as per the following axes: i) Knowledge about the disease; ii) Search for health care in prenatal, childbirth, and postpartum periods; iii) Experience of suffering from COVID-19; iv) Income and work; and v) Family dynamics and social support network. A total of 46 women were interviewed in São Luís-MA, Pelotas-RS, and Niterói-RJ. Use of media was important to convey information and fight fake news. The pandemic negatively impacted access to health care in the prenatal, childbirth, and postpartum periods, contributing to worsening of the population's social and economic vulnerabilities. Women experienced diverse manifestations of the disease, and psychic disorders were very frequent. Social isolation during the pandemic disrupted the support network of these women, who found social support strategies in communication technologies. Women-centered care-including qualified listening and mental health support-can reduce the severity of COVID-19 cases in pregnant, parturient, and postpartum women. Sustainable employment and income maintenance policies are essential to mitigate social vulnerabilities and reduce risks for these women.

O objetivo deste estudo foi compreender a perspectiva de médicos acerca do processo de trabalho em Clínicas Populares de Saúde. Para isso, realizamos uma pesquisa qualitativa na qual foram entrevistados oito jovens médicos especialistas que atuam nessas empresas em São Luís, Maranhão. Identificamos condições precarizadas de trabalho, mas com pouca resistência por parte dos médicos, considerando que essas empresas são encaradas como espaços de projeção no mercado de trabalho local. Devido às limitações do modelo assistencial do tipo queixa-conduta, os entrevistados acionam uma rede informal e os usuários das Clínicas Populares de Saúde são encaminhados para o Sistema Único de Saúde (SUS). Assim, o processo de trabalho do médico é desafiado por esse modelo assistencial fragmentado, o que faz borrar os limites nas interfaces público-privado na medida em que uma dupla porta de entrada para o SUS é naturalizada.

Abstract Objectives: to analyze the prevalence and factors associated with unplanned pregnancy in a Brazilian capital in the Northeast. Methods: a cross-sectional study nested to a hospital birth cohort with a probable sample of 5,110 puerperal women. Associated factors were analyzed using a hierarchical theoretical model in three levels: distal (women’s socioeconomic and demographic characteristics), intermediate (reproductive characteristics, maternal habits and BMI), and proximal level (partner's characteristics). Multivariate Poisson regression analysis was performed. Results: the prevalence of unplanned pregnancy was 68.1% (CI95%=66.8-69.4). Multivariate analysis showed association with black skin color/race (PR=1.03; CI95%=1.01- 1.07), mother's age group up to 19 years old (PR=1.09; CI95%=1.06-1.12) and 20 to 24 years old (PR=1.04; CI95%=1.01-1.07), not living with partner (PR=1.09; CI95%=1.07- 1.11), highest number of people in the household: 5 people (PR= 1.10; CI95%=1.08-1.13) and 3 to 4 (PR=1.08; CI95%=1.05-1.10), number of ≥4 children (PR=1.09; CI95%=1.06- 1.13) and 2 or 3 children (PR=1.03; CI95%=1.02-1.05), alcohol consumption (PR=1.03; CI95%=1.01-1.05), malnourished pre-pregnancy BMI (PR=1.03; CI95%=1.01-1.06) and partner’s low schooling (5 to 8 years) (PR=1.03; CI95%=1.01-1.07). Prior abortion was inversely associated with planned pregnancy (PR=0.95; CI95%=0.93-0.97). Conclusions: the prevalence of unplanned pregnancy was high and was associated with socioeconomic and demographic characteristics that reflect on the combination of the complex inequalities that impact women and their partners

Little is known about the evolution of head circumference (HC) in children with congenital Zika syndrome (CZS). This study aims to evaluate HC growth in children with CZS in the first three years of life and identify associated factors. HC data obtained at birth and in neuropediatric consultations from 74 children with CZS were collected from the Child’s Health Handbook, parents’ reports, and medical records. Predictors of HC z-score were investigated using different mixed-effects models; Akaike’s information criterion was used for model selection. The HC z-score decreased from -2.7 ± 1.6 at birth to -5.5 ± 2.2 at 3 months of age, remaining relatively stable thereafter. In the selected adjusted model, the presence of severe brain parenchymal atrophy and maternal symptoms of infection in the first trimester of pregnancy were associated with a more pronounced reduction in the HC z-score in the first three years of life. The decrease of HC z-score in CZS children over the first three months demonstrated a reduced potential for growth and development of the central nervous system of these children. The prognosis of head growth in the first 3 years of life is worse when maternal infection occurs in the first gestational trimester and in children who have severe brain parenchymal atrophy.

Este estudo objetivou analisar os fatores sociodemográficos e reprodutivos associados ao acolhimento ao parto em estabelecimentos de saúde vinculados à Rede Cegonha no Brasil, na perspectiva das usuárias. Foram selecionados 606 serviços com amostra de 10.540 puérperas. O desfecho foi a variável latente acolhimento da puérpera, composta por cinco indicadores: apresentação dos profissionais com nome e função, chamar a gestante pelo nome, compreensão das informações dadas, se sentir bem tratada e respeitada e ter as necessidades respondidas pela equipe. As variáveis explicativas foram: idade, escolaridade, cor da pele, situação conjugal, tipo de parto, paridade e peregrinação. Foi utilizada modelagem de equações estruturais. Observaram-se maiores percentuais de mulheres com idade de 20 a 34 anos (68,31%), que se autodeclararam como pardas (56,3%), com escolaridade entre 9 e 11 anos de estudo (56,1%) e que tinham companheiro (78,8%). Houve predominância de puérperas que tiveram parto vaginal (56,6%), com um a dois filhos (46%) e que relataram não peregrinar (91,9%). Apresentaram efeito direto positivo sobre o acolhimento mulheres com maior idade (CP = 0,094; p < 0,001) e maior escolaridade (CP = 0,096; p < 0,001). O parto cesáreo apresentou efeito direto negativo (CP = -0,059; p < 0,002) e cor da pele preta e parda apresentou efeitos direto e indireto negativos (CP = -0,081; p < 0,001 e CP = -0,014; p < 0,001). Puérpera com maior idade, maior escolaridade e que tiveram parto vaginal tiveram percepção mais positiva do acolhimento nos serviços de saúde.

Resumo: Este trabalho analisou a influência da ocorrência de violência psicológica no desenvolvimento de transtorno de estresse pós-traumático (TEPT) em adolescentes. Tratou-se de um estudo transversal aninhado a uma coorte, cujo segundo segmento foi realizado em 2016. Aplicou-se um questionário em 2.486 adolescentes, por meio do qual se abordou aspectos individuais, familiares, sociais e da vivência de violência psicológica. Utilizou-se o escore de propensão para se criar o inverso da probabilidade de seleção (IPS). Dessa forma, atribuiu-se uma probabilidade para cada adolescente, sendo 1/IPS aqueles que compõem o grupo dos expostos e 1/(1-IPS) aqueles do grupo dos não expostos. Essa condição tornou os dois grupos mais homogêneos e comparáveis entre si. A associação entre a ocorrência de violência psicológica e de TEPT foi estimada pela odds ratio (OR), enquanto o intervalo de 95% de confiança (IC95%) foi estimado por meio da regressão logística binária bruta e ajustada, ponderada pelo IPS. Verificou-se que 30,3% relataram ter sofrido violência severa. A prevalência do TEPT foi de 4,8% entre os expostos contra 1,5% entre os não expostos à violência psicológica. Observou-se uma associação da violência severa com o TEPT nas duas análises realizadas. Porém, a magnitude no modelo estruturado pelo escore de propensão (OR = 1,97; IC95%: 1,08-3,56) indicou um ajuste da medida de associação da análise bruta (OR = 3,40; IC95%: 2,03-5,69). Nesse sentido, este estudo contribui para a escassa literatura sobre a exposição à violência psicológica e a sua associação com o desenvolvimento de TEPT, confirmando o impacto negativo dessa forma de abuso na saúde mental do indivíduo.

Abstract: This paper describes the history, objectives and methods used by the nine Brazilian cohorts of the RPS Brazilian Birth Cohorts Consortium (Ribeirão Preto, Pelotas and São Luís) Common thematic axes are identified and the objectives, baseline periods, follow-up stages and representativity of the population studied are presented. The Consortium includes three birth cohorts from Ribeirão Preto, São Paulo State (1978/1979, 1994 and 2010), four from Pelotas, Rio Grande do Sul State (1982, 1993, 2004 and 2015), and two from São Luís, Maranhão State (1997 and 2010). The cohorts cover three regions of Brazil, from three distinct states, with marked socioeconomic, cultural and infrastructure differences. The cohorts were started at birth, except for the most recent one in each municipality, where mothers were recruited during pregnancy. The instruments for data collection have been refined in order to approach different exposures during the early phases of life and their long-term influence on the health-disease process. The investigators of the nine cohorts carried out perinatal studies and later studied human capital, mental health, nutrition and precursor signs of noncommunicable diseases. A total of 17,636 liveborns were recruited in Ribeirão Preto, 19,669 in Pelotas, and 7,659 in São Luís. In the studies starting during pregnancy, 1,400 pregnant women were interviewed in Ribeirão Preto, 3,199 in Pelotas, and 1,447 in São Luís. Different strategies were employed to reduce losses to follow-up. This research network allows the analysis of the incidence of diseases and the establishment of possible causal relations that might explain the health outcomes of these populations in order to contribute to the development of governmental actions and health policies more consistent with reality.

ABSTRACT OBJECTIVES: To estimate the prevalence and factors associated with hesitancy in getting the vaccine against SARS-CoV-2 in Maranhão, Brazil. METHODS: This is a cross-sectional population-based study conducted from October 19 to 30, 2020. The estimates were calculated based on clustering, stratification, and non-response. A three-stage sampling was adopted, considering stratum, census tracts, and domicile. After systematic analysis, thirty sectors were selected in each stratum, totaling 150 sectors. Each sector contained a fixed number of 34 households, thus totaling 5,100 households. One individual within each household (resident for at least six months and aged one year or more) was selected by a simple random sampling. We questioned participants about their vaccination intention. Univariate association between independent variables and the outcome were verified using descriptive analysis (weighted frequencies) and Pearson's chi-square test (p < 0.05). Robust multivariate analysis was performed using a three-level hierarchical model. RESULTS: We found 17.5% (95%CI 16.1–19.1%) of the 4,630 individuals interviewed to report hesitancy to be vaccinated against covid-19. After final model adjustment, vaccination hesitancy was statistically higher among residents of the cities of Imperatriz (24.0%; RP = 1.48; IC95% 1.09–2.02) and municipalities of the Grande Ilha de São Luís (20.7%; RP = 1.34; 95%CI 1.02–1.76), female individuals (19.8%; RP = 1.44; 95%CI 1.20–1.75), older adults (22.8%; RP = 1.79; IC95% 1.30–2.46), evangelicals (24.1%; RP = 1.49; 95%CI 1.24–1.79), and those without reported symptoms (18.6%; RP = 1.24; 95%CI 1.02–1.51). We found no statistical differences for other socioeconomic and demographic characteristics, as well as variables related to the labor market, behaviors, and health conditions of the interviewees. CONCLUSION: The prevalence of vaccine hesitancy in Maranhão and its association with individual, contextual, and clinical factors enable us to identify the groups and contexts of greatest resistance, requiring special attention from public strategies to ensure wide vaccination.

Resumo: O objetivo foi estimar a prevalência de indicadores de saúde de adolescentes em São Luís, Maranhão, Brasil, em 2016. Foram estudadas condições sociodemográficas, hábitos de vida, composição corporal, qualidade do sono, atividade física, habilidade cognitiva e risco de suicídio de 2.515 adolescentes com 18 e 19 anos. Os adolescentes pertencem à coorte de nascimento 1997/1998 ou foram incluídos, retrospectivamente, utilizando-se o banco do SINASC (Sistema de Informações sobre Nascidos Vivos). Os adolescentes eram, principalmente, da classe econômica C (50,1%), 69,5% estudavam, 40,3% trabalhavam e 25,2% não estudavam nem trabalhavam; 60,3% já foram vítimas de assalto e 48,7% tinham pais separados. Apresentaram padrão de consumo nocivo, excessivo ou provável dependência de álcool 19,4%, 19,1% usaram ou estavam usando drogas ilícitas, 53,7% tinham qualidade de sono ruim, 40,8% referiram dor de cabeça frequente, 34,3% declararam tempo de tela por dia maior que cinco horas e 4,1% apresentaram alto risco para suicídio. A prevalência de hipertensão arterial foi de 12% e de obesidade, 6%. As meninas eram mais inativas (80,7%) e apresentaram maior percentual de gordura corporal alto (15,8%) e muito alto (21,5%), já os meninos tiveram maior prevalência de hipertensão arterial (21,2%) e menor prevalência de inatividade física (40,9%). As elevadas prevalências de fatores de risco à saúde aumentam a vulnerabilidade dos adolescentes, expondo estes indivíduos precocemente a fatores que levam ao acometimento cada vez mais cedo de doenças e agravos à saúde.

Introduction Although economic crises are common in low/middle-income countries (LMICs), the evidence of their impact on health systems is still scant. We conducted a comparative case study of Maranhão and São Paulo, two unevenly developed states in Brazil, to explore the health financing and system performance changes brought in by its 2014–2015 economic recession.Methods Drawing from economic and health system research literature, we designed a conceptual framework exploring the links between macroeconomic factors, labour markets, demand and supply of health services and system performance. We used data from the National Health Accounts and National Household Sample Survey to examine changes in Brazil’s health spending over the 2010–2018 period. Data from the National Agency of Supplementary Health database and the public health budget information system were employed to compare and contrast health financing and system performance of São Paulo and Maranhão.Results Our analysis shows that Brazil’s macroeconomic conditions deteriorated across the board after 2015–2016, with São Paulo’s economy experiencing a wider setback than Maranhão’s. We showed how public health expenditures flattened, while private health insurance expenditures increased due to the recession. Public financing patterns differed across the two states, as health funding in Maranhão continued to grow after the crisis years, as it was propped up by transfers to local governments. While public sector staff and beds per capita in Maranhão were not affected by the crisis, a decrease in public physicians was observed in São Paulo.Conclusion Our case study suggests that in a complex heterogeneous system, economic recessions reverberate unequally across its parts, as the effects are mediated by private spending, structure of the market and adjustments in public financing. Policies aimed at mitigating the effects of recessions in LMICs will need to take such differences into account.

BackgroundEconomic recessions carry an impact on population health and access to care; less is known on how health systems adapt to the conditions brought by a downturn. This particularly matters now that the COVID-19 epidemic is putting health systems under stress. Brazil is one of the world's most affected countries, and its health system was already experiencing the aftermath of the 2015 recession.MethodsBetween 2018 and 2019 we conducted 46 semi-structured interviews with health practitioners, managers and policy-makers to explore the impact of the 2015 recession on public and private providers in prosperous (São Paulo) and impoverished (Maranhão) states in Brazil. Thematic analysis was employed to identify drivers and consequences of system adaptation and coping strategies. Nvivo software was used to aid data collection and analysis. We followed the Standards for Reporting Qualitative Research to provide an account of the findings.ResultsWe found the concept of 'health sector crisis' to be politically charged among healthcare providers in São Paulo and Maranhão. Contrary to expectations, the public sector was reported to have found ways to compensate for diminishing federal funding, having outsourced services and adopted flexible-if insecure-working arrangements. Following a drop in employment and health plans, private health insurance companies have streamlined their offer, at times at the expenses of coverage. Low-cost walk-in clinics were hit hard by the recession, but were also credited for having moved to cater for higher-income customers in Maranhão.ConclusionsThe 'plates' of a health system may shift and adjust in unexpected ways in response to recessions, and some of these changes might outlast the crisis. As low-income countries enter post-COVID economic recessions, it will be important to monitor the adjustments taking place in health systems, to ensure that past gains in access to care and job security are not eroded.

We report the early growth and neurologic findings of 48 infants in Brazil diagnosed with probable congenital Zika virus syndrome and followed to age 1–8 months. Most of these infants had microcephaly (86.7%) and craniofacial disproportion (95.8%). The clinical pattern included poor head growth with increasingly negative z-scores, pyramidal/extrapyramidal symptoms, and epilepsy.

Resumo: O objetivo deste trabalho foi estimar a peregrinação de gestantes no momento do parto e identificar os fatores associados a essa peregrinação em duas cidades brasileiras. Estudo seccional, aninhado à coorte de nascimento BRISA, cuja amostra foi composta por 10.475 gestantes admitidas nas maternidades selecionadas por ocasião do parto em São Luís (Maranhão) e Ribeirão Preto (São Paulo). Entrevistas foram realizadas utilizando-se questionários que continham variáveis sociodemográficas e relacionadas ao parto. Utilizou-se modelagem hierarquizada, e calculou-se o risco relativo utilizando regressão de Poisson. A peregrinação foi mais frequente em São Luís (35,8%) que em Ribeirão Preto (5,8%). Em São Luís, foram fatores associados à maior peregrinação: ser primípara (RR = 1,19; IC95%: 1,08-1,31) e ter escolaridade menor que 12 ou mais anos de estudo. Entretanto, ter 35 anos ou mais (RR = 0,65; IC95%: 0,54-0,84) foi fator associado à menor peregrinação. Em Ribeirão Preto, peregrinaram com maior frequência as gestantes cujos partos foram de alto risco (RR = 2,45; IC95%: 1,81-3,32) e com idade gestacional inferior a 37 semanas (RR = 1,93; IC95%: 1,50-2,50). No entanto, partos com idade gestacional igual ou acima de 42 semanas foi um fator associado à menor peregrinação (RR = 0,57; IC95%: 0,33-0,98). Nas duas cidades, gestantes pobres peregrinaram com maior frequência, e sem garantia de que seriam atendidas, mesmo dentre as que realizaram o pré-natal. O estudo evidenciou ausência da garantia de acesso universal e equânime e reafirmou a desigualdade de acesso à assistência ao parto entre as regiões brasileiras.

Abstract: Around 18 million unsafe abortions occur in low and middle-income countries and are associated with numerous adverse consequences to women’s health. The time taken by women with complications to reach facilities where they can receive appropriate post-abortion care can influence the risk of death and the extent of further complications. All women aged 18+ admitted for abortion complications to public-sector hospitals in three capital cities in the Northeastern Brazil between August-December 2010 were interviewed; medical records were extracted (N = 2,804). Nearly all women (94%) went straight to a health facility, mainly to a hospital (76.6%); the rest had various care-seeking paths, with a quarter visiting 3+ hospitals. Women waited 10 hours on average before deciding to seek care. 29% reported difficulties in starting to seek care, including facing challenges in organizing childcare, a companion or transport (17%) and fear/stigma (11%); a few did not initially recognize they needed care (0.4%). The median time taken to arrive at the ultimate facility was 36 hours. Over a quarter of women reported experiencing difficulties being admitted to a hospital, including long waits (15%), only being attended after pregnant women (8.9%) and waiting for a bed (7.4%). Almost all women (90%) arrived in good condition, but those with longer delays were more likely to have (mild or severe) complications. In Brazil, where access to induced abortion is restricted, women face numerous difficulties receiving post-abortion care, which contribute to delay and influence the severity of post-abortion complications.

O papel dos serviços de saúde é crucial para o alcance da meta 90-90-90 de controle da epidemia do HIV. O estudo avalia a organização dos serviços brasileiros nas ações de promoção, monitoramento e suporte à retenção no seguimento e apoio ao tratamento. Foram comparadas, por meio de variação percentual (VP), as respostas dos serviços a um questionário de avaliação da qualidade organizacional (Qualiaids) em 2007 e em 2010. Analisou-se os 419 serviços que responderam ao questionário em 2007 (83,1% dos respondentes) e 2010 (63,6%). Ações gerenciais relacionadas à retenção e apoio, embora incrementadas no período, permaneceram com baixa frequência, tais como: reuniões sistemáticas para discussão de casos; (32,7% em 2010; VP = 19,8%), registro de faltas em consulta médica (35,3%; VP = 36,8%). Ações assistenciais relacionadas à adesão ao tratamento medicamentoso permanecem majoritariamente exclusivas do médico. O aporte de recursos de provisão federal - medicamentos e exames específicos para HIV - manteve-se alto para a grande maioria dos serviços (~90%). Não se alcançará decréscimo significativo da transmissão do HIV enquanto a permanência no tratamento não for prioridade de todos os serviços de assistência.

Objetivos:estimar a prevalência e investigar os fatores associados ao hábito de fumar de gestantes na cidade de São Luís, Maranhão.Métodos:estudo de corte seccional em amostra de 5212 gestantes assistidas em onze hospitais de São Luís em 2010. Informações sobre hábito de fumar e características socioeconômicas, demográficas, de saúde reprodutiva e hábitos de vida foram obtidos utilizando questionário padronizado. Estimativa da prevalência e cálculo de odds ratio (OR), com seus respectivos Intervalos de Confiança de 95% (IC95%), foram obtidos em um modelo de regressão logística. Os dados foram analisados no programa estatístico STATA 12.0.Resultados:a prevalência do hábito de fumar na gravidez foi 4,1% (IC95%:3,55-4,64). Os fatores associados ao hábito de fumar foram: ausência de religião (OR=1,58; IC95%:1,11-2,26); não ter companheiro (OR=1,66; IC95%:1,15-2,38); as categorias 2 a 4 partos (OR=2,25; IC95%:1,54-3,29) e cinco partos ou mais (OR=2,50; IC95%:1,23-5,05); 0 a 3 consultas pré-natais (OR=2,75; IC95%:1,74- 4,34); classe econômica D/E (OR=2,75; IC95%:1,22- 6,19); e consumo de álcool (OR=7,61; IC95%:5,50- 10,55).Conclusões:a prevalência do hábito de fumar foi baixa. Houve redução do tabagismo em São Luís de 5,9%, em 1997/98, para 4,1%, no presente estudo. É necessário que intervenções mais enfáticas sejam dirigidas às gestantes com os fatores de vulnerabilidade encontrados.

The objective of this study was to analyze changes in perinatal health in two birth cohorts started in 1997/1998 and 2010, respectively, in São Luís, Maranhão State, Brazil. A total of 2,493 live born infants were included in 1997/1998 and 5,166 in 2010. Low birth weight (LBW) rate did not change (8.5% in 1997/1998 and 8.6% in 2010). Preterm birth (PTB) rate also remained stable (13.2% in 1997/1998 and 13% in 2010). Teenage deliveries and births to single mothers decreased. Maternal schooling and prenatal care coverage increased. Intrauterine growth restriction (IUGR) decreased from 13.3% to 10.6% (p < 0.001). The perinatal mortality rate decreased from 36.6 to 20.7 per 1,000 (p < 0.001) and the infant mortality rate (IMR) dropped from 28.5 to 12.8 per 1,000 (p < 0.001). The cesarean rate increased from 34.1% to 47.5% (p < 0.001). In conclusion, despite favorable changes in socio-demographic, behavioral, and health service factors and decreasing rates of IUGR and perinatal and infant mortality, LBW and PTB remained stable, while the cesarean rate increased.

Objetivos: avaliar a estrutura de maternidades que prestam atenção a mulheres em situação de abortamento no Sistema Único de Saúde, em Salvador, Recife e São Luís. Métodos: foram selecionadas três maternidades em cada capital. Os dados foram obtidos por entrevista com gerente do serviço ou equipe de direção e observação direta. Utilizou-se instrumento com 120 questões pontuadas abrangendo seis componentes - planta física, recursos materiais, recursos humanos, materiais de consumo, educação em saúde e ferramentas de gestão que incorporam quatro dimensões de avaliação - insumo/ambiente físico; qualidade técnica e gerencial do cuidado; acolhimento/orientação e continuidade da atenção. Os resultados foram categorizados pelo percentual obtido em relação ao máximo esperado: suficiente (B a 80%); intermediário (de 50% a 79%); insuficiente (< 50%). Resultados: os componentes melhor pontuados foram planta física, recursos materiais e material de consumo. Educação em saúde e ferramentas de gestão tiveram pior pontuação. Nenhuma unidade atingiu nível considerado suficiente. Sete foram classificadas no nível intermediário e duas insuficiente. Conclusões: as unidades estudadas não apresentam estrutura adequada para o modelo de atenção ao abortamento preconizado. Há necessidade de intervenções para qualificar a estrutura dos serviços para a atenção ao aborto e promover a huma-nização do cuidado.

The aim of this study was to investigate the association between mental health and physical inactivity in 1,447 pregnant women in the second trimester of pregnancy. Subjects answered the short version of the International Physical Activity Questionnaire. Symptoms of depression and anxiety, and stress levels were assessed using the Center for Epidemiological Studies Depression Scale, the Beck Anxiety Inventory and the Perceived Stress Scale, respectively. The rate of physical inactivity was low (39.8%). The prevalence rates of symptoms of severe depression and severe levels of anxiety were 28.8% and 16.9%, respectively. The average perceived stress score was 24.9. An association was found between physical inactivity and not living with a partner (OR = 1.28), having a manual occupation (OR = 0.71) and, unexpectedly, normal and low levels of anxiety (OR = 1.46 and OR = 1.44, respectively). No association was observed between physical inactivity and symptoms of severe depression and perceived stress. It is plausible to assume that the majority of physical activity practiced by these women was attributable to housework or occupation which may in turn be associated with high levels of anxiety.

Prevalências de desnutrição e excesso de peso em menores de cinco anos e sua associação com fatores socioeconômicos, sanitários e demográficos foram estimadas nos seis maiores municípios do Maranhão, em 2006/2007. Por meio de inquérito domiciliar por amostragem 1.214 crianças menores de cinco anos foram aleatoriamente selecionadas. Foi utilizada amostragem por conglomerados em dois estágios, representativa dos seis municípios maranhenses com mais de cem mil habitantes. Foram aplicados questionários padronizados para as mães ou responsáveis pelas crianças e aferidos peso e estatura. Para classificação da desnutrição foram utilizados os pontos de corte < - 2 escores z pelos indicadores peso para idade, peso para estatura e estatura para idade. Para a classificação do excesso de peso foram considerados > +2 escores z, de acordo com o indicador peso para estatura, seguindo recomendações da Organização Mundial da Saúde. Pelo índice peso para idade a prevalência de desnutrição foi de 4,5%, pelo índice estatura para idade 8,5% estavam com desnutrição pregressa e pelo índice peso para estatura 3,9% encontravam-se com desnutrição atual, enquanto 6,7% apresentavam excesso de peso. Crianças de famílias chefiadas por mulheres apresentaram menores prevalências de desnutrição (Razão de Prevalências = 0,4). Variáveis socioeconômicas não estiveram associadas à desnutrição ou ao excesso de peso. Recebimento de benefício do programa bolsa família não foi associado à desnutrição ou excesso de peso. A prevalência de desnutrição infantil foi baixa, mas o excesso de peso foi mais prevalente do que a desnutrição. Não foi detectada desigualdade social em relação à desnutrição em crianças menores de cinco anos, sugerindo evolução favorável no sentido de maior equidade.

OBJETIVO: Avaliar os serviços do Sistema Único de Saúde brasileiro de assistência ambulatorial a adultos vivendo com aids em 2007 e comparar com a avaliação de 2001. MÉTODOS: Os 636 serviços cadastrados no Ministério da Saúde em 2007 foram convidados a responder a um questionário previamente validado (Questionário Qualiaids) com 107 questões de múltipla escolha sobre a organização da assistência prestada. Analisaram-se as frequências das respostas de 2007 comparando-as com as obtidas em 2001 na forma de variação percentual (VP). RESULTADOS: Responderam o questionário 504 (79,2%) serviços. Cerca de 100,0% dos respondentes relataram ter pelo menos um médico, suprimento sem falhas de antirretrovirais e de exames CD4 e carga viral. Vários aspectos mostraram melhor desempenho em 2007 comparados a 2001: registro de número de faltas à consulta médica (de 18,3 para 27,0%, VP: 47,5%), agendamento de consulta em menos de 15 dias no início da terapia antirretroviral (de 55,3 para 66,2%, VP: 19,7%) e participação organizada do usuário (de 5,9 para 16,7%, VP: 183,1%). Houve manutenção de dificuldades: pequena variação na disponibilidade de exames especializados em até 15 dias, como endoscopia (31,9 para 34,5%, VP: 8,1%), e a piora de indicadores como tempo ideal de acesso a consultas especializadas (55,9 para 34,5% em cardiologia, VP negativa de 38,3%). O tempo médio despendido nas consultas médicas de seguimento manteve-se baixo: 15 minutos ou menos (52,5 para 49,5%, VP negativa de 5,8%). CONCLUSÕES: A avaliação de 2007 mostrou que os serviços contam com os recursos essenciais para a assistência ambulatorial. Houve melhoras em muitos aspectos em relação a 2001, mas persistem desafios. Pouco tempo dedicado à consulta médica pode estar vinculado ao número insuficiente de médicos e/ou à baixa capacidade de escuta e diálogo. A acessibilidade prejudicada a consultas especializadas mostra a dificuldade das infraestruturas locais do Sistema Único de Saúde.

Few studies have used structural equation modeling to analyze the effects of variables on violence against women. The present study analyzed the effects of socioeconomic status and social support on violence against pregnant women who used prenatal services. This was a cross-sectional study based on data from the Brazilian Ribeirão Preto and São Luís birth cohort studies (BRISA). The sample of the municipality of São Luís (Maranhão/Brazil) consisted of 1,446 pregnant women interviewed in 2010 and 2011. In the proposed model, socioeconomic status was the most distal predictor, followed by social support that determined general violence, psychological violence or physical/sexual violence, which were analyzed as latent variables. Violence was measured by the World Health Organization Violence against Women (WHO VAW) instrument. The São Luis model was estimated using structural equation modeling and validated with 1,378 pregnant women from Ribeirão Preto (São Paulo/Brazil). The proposed model showed good fit for general, psychological and physical/sexual violence for the São Luís sample. Socioeconomic status had no effect on general or psychological violence (p>0.05), but pregnant women with lower socioeconomic status reported more episodes of physical/sexual violence (standardized coefficient, SC = -0.136; p = 0.021). We used body mass index (BMI) and waist circumference (WC) as fat indicators to assess whether perinatal and early adulthood factors are associated with adiposity in early adulthood. We hypothesized that risk factors differ between men and women and are also different when WC is used for measuring adiposity as opposed to BMI. We conducted a longitudinal study based on a sample of 2,063 adults from the 1978/1979 Ribeirão Preto birth cohort. Adjustment was performed using four sequential multiple linear regression models stratified by sex. Both perinatal and early adulthood variables influenced adulthood BMI and WC. The associations differed between men and women and depending on the measure of abdominal adiposity (BMI or WC). Living with a partner, for both men and women, and high fat and alcohol intake in men were factors that were consistently associated with higher adulthood BMI and WC levels. The differences observed between sexes may point to different lifestyles of men and women, suggesting that prevention policies should consider gender specific strategies.Utilizou-se o índice de massa corporal (IMC) e a circunferência de cintura (CC) para avaliar se alguns fatores perinatais e da vida adulta se associam com adiposidade na vida adulta jovem. Trabalhou-se com a hipótese de que os fatores de risco diferem entre homens e mulheres e também são diferentes quando a CC é utilizada como medida de adiposidade em vez do IMC. Realizou-se estudo longitudinal baseado em 2.063 pessoas da coorte de nascimentos de 1978/1979 de Ribeirão Preto, São Paulo, Brasil. Foi feito ajuste sequencial em quatro modelos de regressão linear múltipla, estratificados por sexo. Tanto variáveis do início da vida como atuais interferiram no IMC e na CC. As associações foram diferentes para homens e mulheres, e também quando se considerou o IMC ou a CC. Homens e mulheres que vivem com companheira(o) e homens que têm consumo elevado de gordura e álcool apresentam maiores valores de IMC e de CC. As diferenças encontradas podem apontar para estilos de vida diferentes de homens e mulheres, sugerindo que as políticas de prevenção também precisam traçar estratégias diferenciadas segundo gênero.

Resumo O artigo objetiva analisar a prevalência e fatores associados ao uso de drogas ilícitas na gestação. Estudo transversal, aninhado à coorte pré-natal do estudo de coorte BRISA. Utilizou-se medidas de frequência e modelo de regressão logística múltipla hierarquizada. A prevalência estimada foi de 1,45% para o uso de drogas ilícitas, 22,32% para o uso de bebidas alcóolicas e 4,22% para o de cigarro. A população mostrou-se jovem, de 20-34 anos de idade (81%), com escolaridade de 9-11 anos de estudos (75,55%), com mais da metade das mulheres fora da população economicamente ativa (52,18%) e na classe econômica “C” (67,61%). As gestantes apresentaram nível de estresse elevado (24,46%), sintomas de ansiedade de moderada a intensa (40,84%) e sintomas depressivos graves (28,8%). Aproximadamente metade (49,72%) das gestantes relatou ter sofrido algum tipo de violência, tendo a maioria ampla rede (72,77%) e baixo apoio social (65,21%). Uso de drogas lícitas, níveis altos de estresse e monoparentalidade foram os fatores independentemente associados ao uso de drogas ilícitas na gestação.

BACKGROUND: Screening for violence during pregnancy is one of the strategies for the prevention of abuse against women. Since violence is difficult to measure, it is necessary to validate questionnaires that can provide a good measure of the phenomenon. The present study analyzed the psychometric properties of the World Health Organization Violence Against Women (WHO VAW) instrument for the measurement of violence against pregnant women. METHODS: Data from the Brazilian Ribeirão Preto and São Luís birth cohort studies (BRISA) were used. The sample consisted of 1,446 pregnant women from São Luís and 1,378 from Ribeirão Preto, interviewed in 2010 and 2011. Thirteen variables were selected from a self-applied questionnaire. Confirmatory factor analysis was used to investigate whether violence is a uni-or-multidimensional construct consisting of psychological, physical and sexual dimensions. The mean-and-variance-adjusted weighted least squares estimator was used. Models were fitted separately for each city and a third model combining data from the two settings was also tested. Models suggested from modification indices were tested to determine whether changes in the WHO VAW model would produce a better fit. RESULTS: The unidimensional model did not show good fit (Root mean square error of approximation [RMSEA] = 0.060, p < 0.001 for the combined model). The multidimensional WHO VAW model showed good fit (RMSEA = 0.036, p = 0.999 for the combined model) and standardized factor loadings higher than 0.70, except for the sexual dimension for SL (0.65). The models suggested by the modification indices with cross loadings measuring simultaneously physical and psychological violence showed a significantly better fit compared to the original WHO model (p < 0.001 for the difference between the model chi-squares). CONCLUSIONS: Violence is a multidimensional second-order construct consisting of psychological, physical and sexual dimensions. The WHO VAW model and the modified models are suitable for measuring violence against pregnant women.

O artigo mostra o desenvolvimento de uma pesquisa que objetivou avaliar a qualidade da assistência ambulatorial do Programa Brasileiro de DST/ AIDS. A investigação, realizada entre 2001-2003, envolveu três projetos: uma análise do padrão tecnológico da assistência realizada em cinco serviços, uma avaliação qualitativa em 27 serviços e, finalmente, uma avaliação estruturada de 322 serviços de sete estados brasileiros. Mediante a descrição de todas as etapas dos projetos, as autoras discutem questões teóricas e metodológicas envolvidas na avaliação da assistência em programas de saúde. Discutem ainda algumas questões relacionadas à aplicabilidade e ao impacto das avaliações em serviços de saúde.This article discusses a study focused on quality care in health services under the Brazilian AIDS Program. Research in 2001-2003 involved three different projects: an in-depth analysis of the predominant health care pattern in five services, a qualitative evaluation of 27 services, and a structured evaluation of 322 services from seven Brazilian states. Through a description of the projects' stages, the authors discuss issues on theoretical and methodological approaches to evaluation of care in health programs. The discussion also focuses on key problems with the applicability and impact of health services evaluation.

Objetivo:Analisar a associação entre o declínio cognitivo e a qualidade de vida de idosos hipertensos.Métodos:Pesquisa de abordagem quantitativa com delineamento analítico transversal, com 125 idosos hipertensos, de ambos os sexos, atendidos no Programa HIPERDIA, de São Luís-MA. Para a avaliação do declínio cognitivo, aplicou-se o Miniexame do Estado Mental (MEEM) e para avaliar a qualidade de vida, o Medical Outcomes Study 36 - Short-Form Health Survey (SF-36). A normalidade dos dados foi testada por meio do teste de Shapiro-Wilk, utilizando-se ainda o teste de Mann Whitney (qualidade de vida). Para testar a associação entre declínio cognitivo e qualidade de vida, usou-se o coeficiente de Spearman.Resultados:A prevalência de declínio cognitivo foi de 20,80%, com predominância em idosos com baixa escolaridade (45,83%). Idosos hipertensos com declínio cognitivo apresentaram pior qualidade de vida, comparados aos idosos hipertensos sem declínio cognitivo. Houve associação positiva da função cognitiva com a qualidade de vida nos domínios: capacidade funcional (r=0,222; p=0,01), dor (r=0,1871; p=0,04) e aspectos emocionais (r=0,3136; p=0,0005).Conclusão:Os resultados encontrados neste estudo sugerem que o declínio cognitivo afeta diretamente a qualidade de vida do idoso hipertenso, na medida em que limita a capacidade de realização de atividades do cotidiano, principalmente se associado a quadros dolorosos e alterações emocionais.

Este estudo analisou os efeitos do ganho de peso gestacional e do aleitamento materno na retenção de peso pós-parto. Foram acompanhadas 2.607 mulheres da coorte de nascimento BRISA. As variáveis utilizadas foram idade, situação socioeconômica, paridade, índice de massa corporal pré-gestacional, ganho de peso gestacional, duração do aleitamento materno, tempo de acompanhamento após o parto e retenção de peso pós-parto. Foi utilizada modelagem de equações estruturais que permitiu avaliar os efeitos totais, diretos e indiretos das variáveis explicativas na retenção de peso pós-parto. Aumento de um desvio padrão do ganho de peso gestacional correspondeu a um aumento significativo de 0,49 desvio padrão da retenção de peso pós-parto (p < 0,001). Aumento de um desvio padrão da duração do aleitamento materno correspondeu à diminuição média de 0,10 desvio padrão da retenção de peso pós-parto (p < 0,001). Independente do índice de massa corporal pré-gestacional, o ganho de peso gestacional é fator de risco, e a duração do aleitamento materno é fator protetor para a retenção de peso pós-parto.

Abstract: The factors associated with physical violence against pregnant women were analyzed in a cross-sectional study of 1,446 pregnant women from a prenatal cohort who were interviewed in 2010 and 2011 in São Luís, Brazil. In the initial model, socioeconomic status occupied the most distal position, determining sociodemographic factors, social support and the behavioral factors that ultimately determined physical violence, which was investigated as a latent variable. Structural equation modeling was used in the analysis. Pregnant women who were from more disadvantaged backgrounds (p = 0.027), did not reside with intimate partners (p = 0.005), had low social support (p < 0.001) and had a high number of lifetime intimate partners (p = 0.001) reported more episodes of physical violence. Low social support was the primary mediator of the effect of socioeconomic status on physical violence. The effect of marital status was mainly mediated by a high number of lifetime intimate partners.

This study focused on the association between physical activity in the second trimester of pregnancy and adverse perinatal outcomes: low birth weight (LBW), preterm birth (PTB), and intrauterine growth restriction (IUGR). The study used a sample from the BRISA cohort, São Luís, Maranhão State, Brazil, which included women with singleton pregnancy, gestational age from 22 to 25 weeks confirmed by obstetric ultrasound performed at < 20 weeks, and re-interviewed in the first 24 hours postpartum (n = 1,380). Level of physical activity was measured by the International Physical Activity Questionnaire (IPAQ), short version, categorized as high, moderate, and low. A directed acyclic graph (DAG) was used to identify minimum adjustment to control confounding. High physical activity was not associated with LBW (RR = 0.94; 95%CI: 0.54-1.63), PTB (RR = 0.86; 95%CI: 0.48-1.54), or IUGR (RR = 0.80; 95%CI: 0.55-1.15). The results support the hypothesis that physical activity during pregnancy does not result in adverse perinatal outcomes.

This study analyzed the association between the inversion of traditional gender roles and exclusive psychological and physical/sexual intimate partner violence, in a cross-sectional study of Brazilian pregnant women, identified through prenatal services in the municipalities of São Luís, Maranhão State (n = 992) and Ribeirão Preto, São Paulo State (n = 943). The pregnant women ranged from 12 to 45 years. Inversion of traditional gender roles was assessed by calculating differences in age, education and occupation between pregnant women and their co-residing intimate partners and identifying the largest contribution to family income. The conceptual model was tested with structural equation modeling and showed acceptable fit. The prevalence of any type of intimate partner violence was 29.8% in São Luís and 20.1% in Ribeirão Preto. In both municipalities, pregnant women were more likely to suffer exclusive psychological and physical/sexual violence when they had the highest income in the family (p < 0.005). In São Luís, physical/sexual violence was more common among women who were better educated than their partners (standardized coefficient, SC = -0.466; p = 0.007). In Ribeirão Preto, exclusive psychological violence was more frequent among women who had lower status occupations than their partners (SC = 0.236; p = 0.004). Inversion of traditional gender roles is associated with exclusive psychological and physical/sexual violence against pregnant women by their co-residing intimate partners. These findings suggest that women's empowerment at an individual level does not necessarily relieve them of intimate partner abuse in social contexts where traditional gender norms persist.
