Background: Folfirinox (FFX) and gemcitabine/nab-paclitaxel (GN) are both standard first-line treatments in patients with metastatic pancreatic cancer (mPC). However, data comparing these two chemotherapeutic regimens and their sequential use remain scarce. Methods: Data from two independent cohorts enrolling patients treated with FFX ( n = 107) or GN ( n = 109) were retrospectively pooled. Primary endpoint was overall survival (OS). Progression-free survival (PFS) was the secondary endpoint. A propensity score based on age, gender, performance status (PS), and presence of liver metastases was used to make groups comparable. Results: In the whole study population, OS was significantly higher in FFX (14 months; 95% CI: 10–21) than in GN groups (9 months; 95% CI: 8–12) before ( p = 0.008) and after ( p = 0.021) adjusting for age, number of metastatic sites, liver metastases, peritoneal carcinomatosis and CA19.9 level at baseline. PFS tends to be higher in FFX (6 months) than GN groups (5 months; p = 0.053). After matching ( n = 49/group), patients were comparable for all baseline characteristics including PS. In the matched population, there was a trend toward greater OS in patients treated with FFX (HR = 0.67; p = 0.097). However, survival in each group was not solely a result of the first-line regimen. The proportion of patients who were fit for GN after FFX failure (FFX–GN sequence) was higher (46.9%) than the reverse sequence (20.4%; p = 0.01), which suggests a higher feasibility for the FFX–GN sequence. Corresponding median OS were 19 months versus 9.5 months, respectively ( p = 0.094). Conclusion: This study shows greater OS with FFX than with GN in patients with mPC. GN after FFX failure appears more feasible than the reverse sequence.

Upper gastrointestinal (GI) tract involvement in adult Crohn's disease (CD) is rare and severe complications unusual. Stenosis has been reported, but gastroduodenal fistulae are seldom detected during surgery and most of the fistulae are cologastric or ileogastric. In complicated gastroduodenal CD, medical treatments are often effective and surgery is only considered in exceptional cases. We here report the unusual case of a 23-year-old patient with upper GI CD presenting a hyperalgic giant ulcer of the bulb fistulized in the pancreatic gland. The failure of steroids and two lines of combined treatment led us to a salvage surgical option. Abdominal exploration showed a plate stomach with an inflammatory bulboduodenal block. Cephalic duodenopancreatectomy and cholecystectomy were performed; histological analysis reported large fissuring pylorus ulceration with micro abscesses reaching the pancreas and the presence of non-caseating granulomas. Six months after the surgery, the patient had stopped antalgic treatment and did not have residual abdominal pain. He had gained 11 kg in weight and had no diarrhea with pancreatic enzymes. To our knowledge, we report the first case of an upper GI and fistulizing CD patient heavily treated with steroids and combined immunosuppressant agents requiring salvage cephalic duodenopancreatectomy.

Neste ensaio avaliou-se o efeito de diferentes misturas de substrato comercial e vermicomposto na produção de biomassa, composição química e rendimento dos óleos essenciais de Thymus zygis. Utilizaram-se 5 substratos resultantes da mistura de um vermicomposto (V) com um substrato orgânico comercial (O), em cinco proporções diferentes (v/v): V100 (100%V); VO75-25 (75%V+25%O); VO50 -50 (50%V+50%O); VO 25-75 (25%V + 75%O) e O100 (100%O). A inclusão do vermicomposto no substrato não melhorou a produção de biomassa. Na modalidade O100 produziu-se mais biomassa, tendo sido na modalidade VO25-75 onde se obteve o valor mais baixo. Na modalidade V100 todas as plantas secaram tendo-se ainda apresentado nesta modalidade o valor mais elevado de pH e de densidade real e inversamente o menor valor de espaço poroso total. Os monoterpenos oxigenados predominaram nos óleos extraídos das plantas nas modalidades O100 e VO25-75, contrariamente ao que se verificou para os monoterpenos não oxigenados tendo estes predominado nas modalidades VO75-25 e VO50-50. A concentração de timol foi elevada nas misturas testadas, em particular na modalidade VO25-75.In this trial we evaluated the effect of different mixtures of organic growing media substratum and vermicompost and their physical properties on biomass, chemical composition and yield of essential oils of Thymus zygis. Five treatments were established based on two types of organic materials (O, commercial growing media, and V, vermicompost). The following proportions were studied (v/v): V100 (100%V); VO75-25 (75%V+25%O); VO50-50 (50%V+50%O); VO 25-75 (25%V+75%O) and O100 (100%O). The inclusion of vermicompost did not improve biomass production. The treatment O100 produced more biomass and the lowest was obtained in VO25-75. On V100 treatment all plants were dead and also have showed the highest pH and particle density and conversely presented the lowest value of total pore space. Oxygenated monoterpenes predominated in the oils extracted from plants in O100 and VO25-75, contrary to what was found for non-oxygenated monoterpenes having these prevailed in VO75-25 and VO50-50. The concentration of thymol was high in the mixtures tested, particularly in VO25-75.

OBJETIVOS: comparar a evolução materna e perinatal após a utilização da analgesia peridural contínua versus analgesia combinada raqui-peridural em parturientes primigestas. MÉTODOS: foi realizado ensaio clínico aleatorizado com 128 gestantes primigestas em trabalho de parto, divididas em dois grupos: analgesia peridural (APC) com 65 mulheres e grupo analgesia combinada raqui-peridural (ACRP) com 63, admitidas no pré-parto de duas maternidades na cidade de Jundiaí - SP. Foram estudadas as variáveis: tempo de latência de instalação da analgesia, intensidade da dor e tempo total decorrido até a dilatação completa, índice de Apgar no primeiro e quinto minutos, tempo de resolução do parto, grau de bloqueio motor, efeitos adversos como náuseas, vômitos, prurido, hipotensão arterial, e grau de satisfação materna. Foram critérios de inclusão: primigestas, estado físico ASA 1 e 2, feto único, apresentação cefálica, de termo, dilatação cervical de 3 a 6 cm e solicitação de analgesia pelo obstetra. Foram excluídas mulheres com morbidades, ruptura de membranas, anormalidades fetais e uso de opioides até quatro horas antes. Para a análise estatística utilizou-se o teste de Mann-Whitney para as variáveis contínuas não paramétricas e os testes exato de Fisher e χ2 de Pearson, para variáveis categóricas. RESULTADOS: não houve diferença entre os grupos para velocidade de dilatação cervical, tempo para resolução do parto, parâmetros hemodinâmicos maternos, vitalidade do recém-nascido, complementações analgésicas durante o trabalho de parto e modo de parto. Houve maior rapidez de instalação da analgesia no grupo da ACRP e menor bloqueio motor no grupo de APC. Não foram observadas diferenças em relação aos efeitos adversos como náuseas, vômitos, prurido e hipotensão, sendo hipotensão mais frequente no grupo APC (16,9 versus 6,3%) e náusea no grupo ACRP (6,3 versus 3,1%). CONCLUSÕES: as duas técnicas mostraram-se seguras e eficientes, porém a ACRP ofereceu analgesia mais rápida, com alívio precoce da dor. O bloqueio motor menos intenso no grupo APC proporcionou movimentação mais ativa no leito e colaboração mais efetiva das gestantes durante o período expulsivo. A grande maioria das mulheres referiu satisfação com a analgesia recebida. As doses de anestésicos locais e opioides utilizadas em ambas as técnicas analgésicas e as doses complementares, iguais nos dois grupos, não produziram efeitos adversos maternos significativos ou alteraram a vitalidade dos recém-nascidos.PURPOSE: to compare maternal and perinatal results, after the use of continuous peridural versus combined rachidian and peridural analgesia in primiparous parturients. METHODS: randomized clinical trial with 128 primiparous pregnant women in labor, divided into two groups: peridural analgesia (CPA) with 65 women, and combined continuous rachidian and peridural analgesia (CRPA) with 63, all of them admitted in the pre-labor period at two maternities in Jundiai, in the state of São Paulo. The variables studied were: latency of analgesia onset, pain intensity, total time until complete dilation, Apgar index at the first and fifth minutes, labor duration, degree of motor blockade, adverse effects such as vomiting, pruritus, arterial hypotension and degree of maternal fulfillment. Inclusion criteria were: primiparous, physical condition ASA 1 and 2, an only foetus, cephalic presentation, term delivery, 3 to 6 cm cervical dilation, and analgesia required by the obstetritian. Women with morbidity, membrane rupture, fetal abnormality and use of opioids up to four hours before were excluded. The Mann-Whitney test was used for the statistical analysis of non-parametric continuous variables, and Fisher's exact and Pearson's χ2 tests, for the categoric variables. RESULTS: there was no difference between the groups concerning the length of cervical dilation, labor duration, maternal hemodynamic parameters, newborn vitality, analgesic complementation during labor and type of delivery. Analgesia onset was quicker in the CRPA group, and the CPA group presented less motor blockade. Differences concerning adverse effects such as nausea, vomiting, pruritus and hypotension have not been observed, but hypotension was more frequent in the CPA group (6.3 versus 3.1%). CONCLUSIONS: both techniques have proved to be safe and efficient, but CRP has provided an earlier onset, with quicker pain relief. The lesser motor blockade in the CPA group has allowed the parturients more active movements in bed and more effective collaboration during the expulsion period. Most women were pleased with the analgesia they got. The doses of local anesthetics and opioids used in both analgesic techniques and the complementary doses, equal in both groups, neither had any significant adverse effect nor affected newborn vitality.

OBJETIVO: conhecer as expectativas de primigestas com relação à via de parto, bem como os motivos de sua escolha, procurando melhorar a qualidade do relacionamento médico-paciente. MÉTODOS: foi realizado estudo do tipo qualitativo por meio da análise do sujeito coletivo, incluindo primigestas atendidas de setembro a novembro de 2003 nos pronto-socorros dos serviços da Faculdade de Medicina de Jundiaí. Foi aplicado questionário especialmente elaborado para responder aos objetivos propostos pela pesquisa o questionário é baseado nas dúvidas apresentadas por pacientes que freqüentaram o serviço meses antes da elaboração do projeto. O consentimento livre e esclarecido, assinado pela gestante e por um dos pesquisadores responsáveis. Foi obtido para fins de padronização da amostra a seleção das pacientes seguiu critérios de inclusão: idade maior que 16 anos, primigestas, que estivessem recebendo assistência pré-natal e consentimento pós-informado lido e esclarecido. Foram considerados critérios de exclusão pacientes mentalmente incapacitadas e gestantes de alto risco com doenças que pudessem interferir na escolha da paciente. RESULTADOS: a população estudada teve como perfil mais prevalente mulheres com mais de 21 anos, brancas, casadas, com segundo grau completo e que estavam no terceiro trimestre da gestação. A via de parto de preferência da maioria das mulheres (90%) foi o parto vaginal normal, sendo que as principais justificativas foram: a praticidade para sua realização (94%) e o medo do sofrimento e dor no pós-parto causados pela cesárea. Encontramos relação entre a preferência pelo parto vaginal com mulheres de maior idade, casadas, não havendo diferença significativa entre as raças. CONCLUSÃO: estes resultados nos mostram enorme contraste entre a preferência das mulheres e os altos índices de cesárea no Brasil. Concluímos que deve haver falha de informação, falta de diálogo entre os profissionais da saúde e a paciente sobre as possíveis dificuldades, dúvidas e anseios que permeiam a escolha por determinada via de parto. Do ponto de vista ético concluímos que os obstetras devem questionar cada indicação para a realização de uma cesárea e respeitar a autonomia da escolha materna sem ignorar os verdadeiros critérios clínicos que levam à decisão médica pela via de parto.PURPOSE: to find out the preference in regard to the way of delivery among primigravidae, as well as the reasons for their choice, in order to improve the quality of the doctor-patient relationship. METHODS: a qualitative-type study was conducted through analysis of the collective subject, including primigravidae attended from September to November 2003 at the emergency rooms of the hospital of the "Faculdade de Medicina de Jundiaí". A questionnaire, specially developed to accomplish the proposed objectives was applied. An informed and free consent, signed by the pregnant woman and one of the researchers in charge was obtained. This questionnaire was based on doubts of patients attended at this hospital some months before the trial. For the purpose of sample standardization, the patients' selection followed some inclusion criteria: age above 16, primigravidae that were receiving prenatal assistance and a post-informed and free consent. Mental disorders and clinical and/or obstetric pathologies that could interfere in the patient's choice were considered exclusion criteria. RESULTS: the studied population had as prevailing profile women in the third quarter of gestation, above 21 years of age, white, married and with completed school. Most of the women (90%) preferred vaginal delivery for the following main reasons: ease to be done (94%) and the fear of suffering and pain during the postpartum period caused by cesarean section. There was a relationship between older and married women and the preference for vaginal delivery, with no significant difference between races. CONCLUSION: these results show an enormous contrast between women's preference and the high cesarean section rates in Brazil. We conclude that there may be a lack of information and dialogue between the health professionals and patients about the possible difficulties, doubts and anxieties that involve the women's choice for a specific way of delivery. From an ethical point of view, we conclude that obstetricians should question every cesarean section indication and take into account the women's right to choose, without ignoring clinical criteria, when making the medical decision about the way of delivery.

OBJETIVO: comparar a efetividade e segurança do misoprostol, em duas diferentes dosagens (12,5 e 25 mig), administradas por via vaginal, para preparo cervical e indução do parto em gestações de termo com indicação de antecipação do parto. MÉTODO: estudo piloto de ensaio clínico controlado aleatorizado unicego, incluindo 40 gestantes tratadas com uma das duas diferentes doses de misoprostol. A variável independente foi a dose de misoprostol e as principais variáveis dependentes foram tipo de parto, tempo entre o início da indução e o parto, complicações perinatais e efeitos maternos adversos. As principais variáveis de controle foram idade materna, idade gestacional, escolaridade, paridade, cor e estado do colo uterino no início da indução. Na análise dos dados foram utilizados os testes t de Student, chi2, exato de Fisher, Wilcoxon, Kolmogorov-Smirnof e análise de sobrevivência. RESULTADOS: os grupos utilizando 12,5 e 25 mig mostraram-se similiares e não apresentaram diferenças significativas no tempo de início de atividade uterina (20,9 ± 20,4 e 16,6 ± 9,8 h respectivamente), tempo entre o início da atividade uterina até o parto (7,8 ± 3,4 e 6,9 ± 5,0 h), parto vaginal (65 e 80%) e efeitos indesejados maternos e perinatais (índice de Apgar e síndrome de hiperestimulação uterina similares). CONCLUSÃO: a maior proporção de partos vaginais e menor tempo para o parto com a dose de 25 mig, embora não significativas, não permitem recomendar a utilização de doses de 12,5 mig como mais vantajosa para o preparo cervical e indução do parto.

Objetivos: comparar, em uma população de gestantes brasileiras com rotura prematura de membranas a termo (RPM-T), as condutas expectante ou ativa com ocitocina. Métodos: ensaio clínico prospectivo, randomizado e multicêntrico, avaliando as variáveis relativas ao tempo para o início do trabalho de parto e parto e tempo de internação hospitalar materna e neonatal. Foram selecionadas 200 gestantes com RPM-T, atendidas em quatro instituições públicas do estado de São Paulo de novembro de 1995 a fevereiro de 1997. As pacientes foram divididas aleatoriamente em dois grupos de conduta: ativa, com indução do trabalho de parto com ocitocina iniciando até 6 h de RPM, e expectante, aguardando-se o início espontâneo do trabalho de parto por um período máximo de 24 h. Os dados foram analisados com o auxílio dos programas Epi-Info e SPSS-PC+, utilizando-se os testes estatísticos do chi², t de Student e Log-rank. Resultados: indicam que as diferenças entre a utilização da conduta ativa com ocitocina e a conduta expectante dizem respeito ao maior tempo necessário no grupo de conduta expectante para o início do trabalho de parto e parto, além da maior proporção de mulheres e recém-nascidos com internação superior a três dias. Conclusões: o tempo entre a admissão e o parto, o período de latência e o tempo entre a rotura das membranas e o parto foram maiores quando se adotou conduta expectante.Objective: to compare the expectant versus active management with oxytocin in a Brazilian population of pregnant women with premature rupture of membranes (PROM) at term. Methods: a prospective, randomized and multicenter clinical trial was performed, evaluating variables concerning the time from PROM until the onset of labor and delivery, and maternal and neonatal hospitalization periods. Two hundred pregnant women with PROM at term were selected from four public hospitals in São Paulo state, from November 1995 to February 1997. They were randomly divided into two groups: active management, with oxytocin induction of labor until 6 h of PROM; and expectant management, waiting for the spontaneous onset of labor up to 24 h. The data were analyzed with the Epi-Info and SPSS-PC+ packages, using the statistical c², Student's t and log-rank tests. Results: the results indicate that the differences between the two managements concern to the longer time needed for the expectant management group until onset of labor and delivery, besides the higher number of women and neonates who remained in hospital for more than three days. Conclusions: the time between admission and onset of labor and delivery, and also the latent period were longer in the expectant management group.

The involvement of local people in the management of scarce resources, such as water available for agriculture, is a desirable approach to address some of the difficulties in the decision-making processes. This paper reports the experience of a research team of sociologists and geographers involved in the MULINO project (Multi-sectoral, Integrated and Operational decision support system for sustainable use of water resources at the catchment scale). In the framework of this international and interdisciplinary project, a social network analysis was developed in a catchment of south Portugal. This analysis was structured in five fundamental steps: Stakeholder Identification; Data Collection; Data Treatment; Network Visualisation; and Local Network Analysis. The information was gathered mainly through a questionnaire, bibliographic references, statistical inputs and field observations. In order to obtain a typology of stakeholders and their interactions, a factorial analysis of multiple correspondences and a cluster analysis were applied. The social network analysis, which is the measuring and visualisation of relationships and flows between people, groups, organisations or other information/knowledge processing entities, supported the description of two types of patterns that link different sets of actors: social groups (sets of actors closely linked together) and social positions (sets of actors who are linked into the overall social system in similar ways). The analysis of the Caia Catchment network shows that this type of analysis is important not only as a means to characterising certain parameters about the network’s characteristics, but also as a significant tool to help improve the communication within the social network.
