Collapsing glomerulopathy(CG), characterized by collapse of the glomerular capillary loops onto the mesangial stalks is rarely associated to systemic lupus erythematosus (SLE). Recently a genetic predisposition to HIV associated nephropathy (HIVAN) has been shown in Afro-Americans: MYH9 polymorhism in 2008 and then APOL1 variants (G1 and G2 alleles) in 2010 were shown to be strongly associated with HIVAN.We describe here for the first time the association of CG in a young Afro-American female with SLE having a homozygous mutation of APOL1. The clinical history, laboratory findings and immunofluorescence all confirmed a diagnosis of SLE. However, studies for factors associated with collapsing glomerulopathy in other situations were consistently negative. As this Afro-American patient developed a CG, we performed genotyping of APOL1. Sixty HIV-infected patients presenting renal symptoms who underwent percutaneous renal biopsies were analysed. According to the CDC classification, 44 patients were staged in group IV, five in group III, and 11 in group II. Patients were divided in two groups according to their ethnic origin (29 black patients and 31 white patients). Risk factors such as homosexuality, multiple transfusions or intravenous drug abuse (IVDA) were identified in all white patients except two, but in only nine (31%) of the black patients. Three main patterns of renal disease were observed: focal and segmental glomerulosclerosis (FSGS) was found predominantly in black patients (23 black patients versus 3 Caucasians, < 0.001) and was associated with the nephrotic syndrome; immune-complex-type glomerulonephritis (ICGN) was frequent in black and white patients (21% and 52% respectively) including four cases of IgA nephritis all seen in white patients; and 10 cases of lupus-like nephritis (4 black and 6 white patients). The frequent hypergammaglobulinaemia in those patients suggests a pathogenic role of polyclonal B cell activation in ICGN. Interstitial nephritis was present in 48 and 52% of the black and white patients respectively and did not seem related to drug toxicity or superimposed infectious disease. In addition to interstitial nephritis, the coexistence of multivisceral lymphocytic infiltration involving accessory salivary glands, liver and/or lung, found in six patients possibly suggests a virus-induced immune disorder. L'évolution des vascularites associées aux ANCA (VAA) est émaillée de rechutes. Aucun signe clinicobiologique n'est spécifique d'une rechute rénale rendant son diagnostic difficile. Pourtant, aucune recommandation n'existe concernant les indications de rebiopsie rénale. Les objectifs de l'étude étaient de déterminer l'impact des rebiopsies rénales et déterminer les facteurs clinicobiologiques associés à la présence de lésions actives à l'histologie. À ce jour, nous avons inclus de manière rétrospective 37 patients qui avaient bénéficié chacun d'au moins 2 biopsies rénales (BR) entre 2002 et 2018. La première biopsie (B1) était toujours réalisée au diagnostic et la deuxième (B2) était réalisée pour cause (maladie réfractaire ou suspicion de rechute rénale). Nous avons étudié les modifications histologiques observées entre B1 et B2, en particulier la présence de lésions d'activité et les lésions chroniques. Les lésions actives étaient définies par la présence de croissants cellulaires et/ou de nécrose fibrinoïde. Les lésions chroniques étaient quantifiées selon le pourcentage de glomérules scléreux et le pourcentage de fibrose interstitielle/atrophie tubulaire (FI/AT). À la rebiopsie, les lésions chroniques étaient plus sévères qu'au diagnostic. De manière surprenante, les lésions actives n'étaient présentes que dans 35 % des cas alors que l'on suspectait une activité résiduelle ou une rechute rénale chez tous les patients. Nous avons confronté les anomalies cliniques avec les lésions retrouvés et nous avons retenu 5 facteurs de risque associés à une activité histologique : ascension des ANCA, présence de signes extra-rénaux, CRP > 30 mg/L, fibrose interstitielle peu étendue et pourcentage de glomérules normaux élevé au diagnostic. Au total, la rechute rénale n'est pas un diagnostic aisé. La rebiopsie a permis de corriger 65 % des suspicions cliniques et éviter un traitement immunosuppresseur dans 45,9 % des cas. La prévalence des atteintes rénales et de la gammapathie monoclonale (GM) augmente avec l'âge. En présence de ces deux pathologies, la biopsie rénale (BR) est souvent réalisée afin d'exclure une néphropathie en rapport avec la GM (NRGM) pouvant justifier d'un traitement spécifique. Étude rétrospective unicentrique incluant les patients ayant bénéficié d'une BR et présentant une GM circulante pour déterminer la prévalence des NRGM et identifier des facteurs prédictifs du résultat histologique. Interrogation de l'entrepôt de données informatisées de l'hôpital pour recenser et collecter les données clinicobiologiques et histologiques de tous les patients ayant eu une BR entre 2007 et 2018 et présentant une GM authentifiée par une immunofixation plasmatique ou urinaire positive. Sur les 2727 BR réalisées, 328 patients (12 %) avaient une GM. Une insuffisance rénale (DFGe < 60 mL/min) était présente chez 77,4 % des cas, une protéinurie chez 87,2 %. L'âge médian était de 67 ans (60 % d'hommes), 21,3 % étaient diabétiques, 53,1 % hypertendus, 25,3 % avaient une hémopathie maligne avérée. La BR a révélé chez 91 (27,7 %) patients une NRGM : tubulopathie myélomateuse (37 %), amylose AL (35 %), LCDD (18 %), cryoglobuminémie monoclonales (8 %). L'insuffisance rénale était plus sévère (créatininémie médiane à 176 vs 149 μmol/L, p = 0,003), la protéinurie plus élevée (3,9 vs 2,0 g/g créatininurie, p < 0,001), comparés aux patients sans NRGM. Parmi les patients sans hémopathie maligne avérée (n = 245), 16 % présentait une monoclonal gammopathy of renal significance (MGRS). Les marqueurs biologiques prédictifs étaient une protéinurie de Bence–Jones (PBJ) positive (OR : 4,7 ; IC95 % [2,2–10,3] ; p < 0,001), un rapport Kappa/Lambda sérique anormal (OR : 4,2 ; IC95 % [1,7–10,7] ; p < 0,004) et un pic à l'électrophorèse > 1,5 g/L (OR : 5,9 ; IC95 % [2,6–13,5] ; p < 0,001) avec des valeurs prédictives négatives de 89/86/88 %, respectivement. Parmi les patients biopsiés ayant une GM, 27,7 % ont une néphropathie spécifique (NRGM). Intra-vascular lymphoma is usually reported as a rare and fatal disorder. We describe here the first case of an intra-vascular lymphoma revealed by a nephrotic syndrome for which a durable remission has been obtained by 8 cycles of bi-mensual CHOP and Rituximab therapy. Introduction L’association séquentielle d’une MAT suivie d’une cryglobulinémie avec atteinte rénale au cours des gammapathies monoclonales n’a jamais été rapportée. Nous présentons ici les données cliniques et histologiques de trois patientes présentant une évolution séquentielle et stéréotypée associant une microangiopathie thrombotique puis une GNC. Observation Les trois cas répondent aux critères suivants : – l’existence initiale d’une gammapathie monoclonale (IgMκ : n = 2, IgGλ : n = 1) ; – la survenue dans un premier temps d’au moins un épisode d’insuffisance rénale aiguë (IRA) liée à une MAT ; – la survenue dans un second temps d’une GNC ; – l’absence d’anomalie quantitative ou de mutation de facteurs régulateurs de la voie alterne du complément, reconnus comme cause de syndrome hémolytique et urémique atypique. Dans tous les cas, le tableau inaugural a été déclenché par un processus infectieux, et s’est manifesté par une MAT combinant l’atteinte hématologique et une atteinte rénale prouvée histologiquement. L’épisode inaugural a été traité efficacement par plasmaphérèses (deux cas) ou eculizumab (dernier cas). Une cryoglobuline de type II a été détectée chez deux patientes, d’emblée ou lors du deuxième épisode d’IRA ; dans le dernier cas, il s’agissait d’une cryoglobuline « flottante » non typable, découverte au 4e épisode d’IRA. Chez cette patiente, les deuxièmes et troisièmes épisodes ont montré la coexistence de lésions de MAT rénale et de lésions évocatrices de GNC (glomérulonéphrite endocapillaire, thrombis hyalins et dépôts d’IgMκ). Les diagnostics finaux retenus ont été une maladie de Waldenström, un myélome IgGλ et un lymphome de la zone marginale. Une excellente récupération de la fonction rénale finale a été observée après l’initiation d’une chimiothérapie. Conclusion On relève le caractère stéréotypé des observations autour des points suivants :. – l’existence d’une gammapathie monoclonale ; – la documentation d’une MAT hématologique et rénale ; – l’évolution secondairement vers une GNC avec, dans un cas, un chevauchement des lésions histologiques associant MAT et GNC ; – un effet « trigger infectieux » à l’origine des épisodes d’IRA ; – la susceptibilité de l’épisode de MAT au traitement par échanges plasmatiques ou à l’eculizumab, suggérant le rôle d’un facteur circulant interférant avec le complément dans la survenue de la MAT. À supposer que la cryoglobuline puisse jouer ce rôle, le mécanisme par lequel il induit la MAT reste incertain. Introduction La glomérulonéphrite d’origine infectieuse est une pathologie dont l’épidémiologie a évolué ces dernières décennies en raison d’une augmentation de l’incidence des infections à staphylocoque [1]. La forme histologique à dépôts d’IgA a rarement été rapportée en Europe [2]. L’objectif était d’analyser les données cliniques, histologiques et évolutives de patients ayant un diagnostic de glomérulonéphrite d’origine infectieuse à dépôts d’IgA. Patients/matériels et méthodes Les données cliniques, biologiques, évolutives et les biopsies rénales d’une cohorte rétrospective de patients provenant de 11 hôpitaux français ont été analysées. L’expression en immunohistochimie du C4d dans des GN d’origine infectieuse avec IgA, sans IgA et des néphropathies à IgA a été comparée. Résultats Parmi les 27 patients (23 hommes, 4 femmes, âge moyen = 62 ± 15 ans) inclus, 44,4 % étaient diabétiques, 77,8 % avaient une infection à Staphylococcus Aureus (sensible à la méticilline dans 66,7 % des cas), le plus souvent d’origine ostéoarticulaire (44,4 %), cutanée (40,7 %) ou associée à une bactériémie (40,7 %). Un syndrome néphrotique (66,7 %) et une glomérulonéphrite rapidement progressive (55,6 %) étaient fréquemment observés. Une hématurie était présente dans 95,2 % des cas, une protéinurie était observée chez tous les patients. La moitié des patients avaient une insuffisance rénale sévère, 16 % avaient une hypocomplémentémie. Le délai médian entre l’infection et la biopsie était de 42 jours. On observait fréquemment une glomérulonéphrite (GN) proliférative mésangiale (88,9 %) ou endocapillaire (88,9 %) riche en polynucléaires neutrophiles (81,4 %) et des dépôts extra-membraneux de type « humps » (48,1 %). Une nécrose tubulaire aiguë et une fibrose interstitielle avec atrophie tubulaire étaient observées chez 85,1 % des patients. Les dépôts d’IgA étaient dominants ou codominants dans 66,6 % des cas. Un marquage glomérulaire diffus et global par le C4d n’était observé que dans des GN d’origine infectieuse à dépôts d’IgA (17,4 % des cas). Le suivi était disponible pour 26 patients (médiane : 13,2 mois) : 23,1 % des patients sont décédés, 46,2 % avaient une insuffisance rénale chronique, 15,4 % avaient une insuffisance rénale terminale malgré un traitement par antibiotiques (100 %) et corticoïdes (37 %). Le mauvais pronostic rénal était corrélé à la sévérité de la fibrose interstitielle avec atrophie tubulaire. Discussion La glomérulonéphrite d’origine infectieuse à dépôts d’IgA, souvent secondaire à une infection staphylococcique, est de diagnostic difficile avec une présentation clinique atypique et un spectre lésionnel histologique étendu. Conclusion Il s’agit d’une néphropathie de mauvais pronostic posant le problème du diagnostic différentiel avec la néphropathie à IgA aux pronostic et implication thérapeutique différents [3]. Les vascularites associées aux ANCA (VAA) sont une cause fréquente de glomérulonéphrite rapidement progressive (GNRP). Au décours d'une poussée, le pronostic est dominé par le risque d'évolution vers une insuffisance rénale terminale (IRT) ou le décès. L'étude prospective multicentrique PEXIVAS, incluant des patients en poussée sévère de VAA avec hémorragie alvéolaire et/ou insuffisance rénale (avec un DFGe < 50 mL/min/1,73 m2), a montré que les échanges plasmatiques (EP) ne réduisaient pas la survenue d'IRT et/ou de décès. Cette étude n'a cependant pas évalué l'impact pronostique de la biopsie rénale sur le devenir de la fonction rénale. Notre objectif était d'évaluer si les lésions histologiques rénales pouvaient prédire le devenir de la fonction rénale chez les patients avec VAA traités par EP. Nous avons inclus dans cette étude rétrospective multicentrique des patients avec granulomatose avec polyangéite (GPA), polyangéite microscopique (PAM) ou vascularite limitée au rein, traités par EP pour une GNRP et ayant eu une biopsie rénale initiale. Deux classifications histologiques ont été utilisées : celle de Berden distinguant 4 classes (focale, à croissants, mixte, scléreuse), et celle de Brix distinguant 3 groupes à risque (faible, intermédiaire, élevé) et utilisant le pourcentage de glomérules normaux, le pourcentage de fibrose interstitielle et atrophie tubulaire et le DFGe initial. Le critère de jugement principal était l'indépendance de la dialyse et la survie à 12 mois. Les critères secondaires étaient un DFGe > 30 mL/min/1,73 m2 à 12 mois et un gain de DFGe > 15 mL/min/1,73 m2 à 12 mois par rapport au début de la poussée. Nous avons inclus 163 patients : 99 (60 %) hommes, 92 (56 %) avec ANCA-MPO, 65 (40 %) avec ANCA-PR3 ; créatininémie initiale 555 ± 274 μmol/L, 62 (38 %) avec hémorragie alvéolaire ; 139 (85 %) ont reçu du cyclophosphamide et 30 (18 %) du rituximab ; 74 (45 %) ont nécessité initialement le recours à l'épuration extra-rénale. Le nombre moyen d'EP était de 7,0 ± 2,3. Les classes de Berden étaient focales dans 19 %, à croissants dans 39 %, mixtes dans 20 % et scléreuses dans 21 %. Selon la classification de Brix, 11 % des patients avait un risque bas, 48 % un risque intermédiaire et 41 % un risque élevé. Les classifications de Berden et de Brix étaient toutes les deux fortement associées à la survenue du critère de jugement principal : indépendance de la dialyse et survie à 12 mois (respectivement c224,9, p < 0,0001, et c211,7, p = 0,003). De plus, la créatininémie initiale n'était pas différente dans les deux groupes (526 ± 252 μmol/L dans le groupe avec bon pronostic vs 597 ± 300 μmol/L dans le groupe avec mauvais pronostic, p = 0,11). Concernant les critères secondaires : un DFGe > 30 mL/min/1,73 m2 à 12 mois était associé à la classification de Berden (c214,0, p = 0,003) et de Brix (c217,3, p = 0,0002) ; et un gain de DFGe > 15 mL/min/1,73 m2 était également associé avec la classification de Berden (c218,1, p = 0,0004) et de Brix (c217,7, p = 0,0001). Dans cette cohorte de patients traitées par EP pour poussée rénale de VAA, nos résultats suggèrent que les résultats de la biopsie rénale initiale sont fortement associés au devenir de la fonction rénale à 12 mois. Background: Antiphospholipid syndrome (APS) nephropathy (APSN) is a rare pattern with specific features resulting from microvascular lesions. The prognosis of APSN, outside of lupus nephritis, is unknown. The aim of this study was to describe the renal, vascular and overall outcomes of patients with APSN., Methods: Retrospective multicenter study of patients with antiphospholipid antibodies (aPL) associated with histological APSN lesions and no other nephropathy, identified through a national call for medical records. End-stage renal disease (ESRD)-free survival, thrombosis recurrence-free survival and overall survival were assessed., Results: Thirty patients were included (19 women) with a median age of 40 years (34-52 years). Fifteen patients had APS, 26/28 had lupus anticoagulant, and 15/26 had triple positivity for aPL. Median eGFR was 50 (31-60) mL/min/1.73 m 2 . Glomerular thrombotic microangiopathy was found in 12/24 cases, fibrous intimal hyperplasia in 12/22 cases and focal cortical atrophy in 17/29 cases. Nineteen patients had moderate to severe interstitial fibrosis (>25%). Six patients developed ESRD at a median follow-up of 6.2 (1.8-9.1) years. The ESRD-free survival rates at 5 and 10 years were 80.0% (95% CI 57.6%-91.4%) and 72.7% (95% CI, 46.9%-87.4%) respectively. None of the histological factors considered was significantly associated with a decrease in eGFR at 12 months. Thrombosis recurrence-free survival was 77.8% (95% CI 48.2%-91.6%) at 10 years. Overall survival was 94% at 10 years (95% CI 65.0%-99.2%)., Conclusions: The renal prognosis of isolated APSN is poor. Background: Data from the PEXIVAS trial challenged the role of plasma exchange (PLEX) in ANCA-associated vasculitides (AAV). We aimed to describe kidney biopsy from patients with AAV treated with PLEX, evaluate whether histopathologic findings could predict kidney function, and identify which patients would most benefit from PLEX., Methods: We performed a multicenter, retrospective study on 188 patients with AAV and AKI treated with PLEX and 237 not treated with PLEX. The primary outcome was mortality or KRT at 12 months (M12)., Results: No significant benefit of PLEX for the primary outcome was found. To identify patients benefitting from PLEX, we developed a model predicting the average treatment effect of PLEX for an individual depending on covariables. Using the prediction model, 223 patients had a better predicted outcome with PLEX than without PLEX, and 177 of them had >5% increased predicted probability with PLEX compared with without PLEX of being alive and free from KRT at M12, which defined the PLEX-recommended group. Risk difference for death or KRT at M12 was significantly lower with PLEX in the PLEX-recommended group (-15.9%; 95% CI, -29.4 to -2.5) compared with the PLEX not recommended group (-4.8%; 95% CI, 14.9 to 5.3). Microscopic polyangiitis, MPO-ANCA, higher serum creatinine, crescentic and sclerotic classes, and higher Brix score were more frequent in the PLEX-recommended group. An easy to use score identified patients who would benefit from PLEX. The average treatment effect of PLEX for those with recommended treatment corresponded to an absolute risk reduction for death or KRT at M12 of 24.6%., Conclusions: PLEX was not associated with a better primary outcome in the whole study population, but we identified a subset of patients who could benefit from PLEX. The mechanisms underlying the development of glomerular lesions during aging are largely unknown. It has been suggested that senescence might play a role, but the pathophysiological link between senescence and lesion development remains unexplained. Here, we uncovered an unexpected role for glomerular endothelial cells during aging. In fact, we discovered a detrimental cross-talk between senescent endothelial cells and podocytes, through PAI-1. In vivo, selective inactivation of PAI-1 in endothelial cells protected glomeruli from lesion development and podocyte loss in aged mice. In vitro, blocking PAI-1 in supernatants from senescent endothelial cells prevented podocyte apoptosis. Consistently, depletion of senescent cells prevented podocyte loss in old p16 INK-ATTAC transgenic mice. Importantly, these experimental findings are relevant to humans. We showed that glomerular PAI-1 expression was predictive of poor outcomes in transplanted kidneys from elderly donors. In addition, we observed that in elderly patients, urinary PAI-1 was associated with age-related chronic kidney disease. Rationale & Objective: Fibrinogen A α-chain amyloidosis (AFib amyloidosis) is a form of amyloidosis resulting from mutations in the fibrinogen A α-chain gene (FGA), causing progressive kidney disease leading to kidney failure. Treatment may include kidney transplantation (KT) or liver-kidney transplantation (LKT), but it is not clear what factors should guide this decision. The aim of this study was to characterize the natural history and long-term outcomes of this disease, with and without organ transplantation, among patients with AFib amyloidosis and various FGA variants., Study Design: Case series., Setting & Participants: 32 patients with AFib amyloidosis diagnosed by genetic testing in France between 1983 and 2014, with a median follow-up of 93 (range, 4-192) months, were included., Results: Median age at diagnosis was 51.5 (range, 12-77) years. Clinical presentation consisted of proteinuria (93%), hypertension (83%), and kidney failure (68%). Manifestations of kidney disease appeared on average at age 57 (range, 36-77) years in patients with the E526V variant, at age 45 (range, 12-59) years in those with the R554L variant (P<0.001), and at age 24.5 (range, 12-31) years in those with frameshift variants (P<0.001). KT was performed in 15 patients and LKT was performed in 4. In KT patients with the E526V variant, recurrence of AFib amyloidosis in the kidney graft was less common than with a non-E526V (R554L or frameshift) variant (22% vs 83%; P=0.03) and led to graft loss less frequently (33% vs 100%). Background: Infection-related glomerulonephritis with IgA deposits (IRGN-IgA) is a rare disease but it is increasingly reported in the literature. Data regarding epidemiology and outcome are lacking, especially in Europe. We aimed to assess the clinical, pathologic and outcome data of IRGN-IgA., Methods: Clinical and outcome data from patients from 11 French centers over the 2007-2017 period were collected retrospectively. We reviewed pathologic patterns and immunofluorescence of renal biopsies and evaluated C4d expression in IRGN-IgA. We analyzed the correlation between histological presentation and outcome., Results: Twenty-seven patients (23 men, mean age: 62 ± 15 years) were included. Twenty-one (78%) had Staphylococcus aureus infection and twelve (44%) were diabetic. At the time of biopsy, 95.2% had haematuria, 48.1% had a serum creatinine level of > 4 mg/dL, and 16% had hypocomplementemia. The most common pathologic presentation included mesangial (88.9%) and endocapillary proliferative glomerulonephritis (88.9%) with interstitial fibrosis and tubular atrophy (IF/TA) (85.1%). Diffuse and global glomerular C4d expression was found in 17.8%, mostly in biopsies with acute or subacute patterns, and was associated with a short delay between infection and renal biopsy compared to segmental and focal staining. After median follow-up of 13.2 months, 23.1% died, 46.2% had persistent renal dysfunction and 15.4% reached end-stage renal disease. Renal outcome was correlated to IF/TA severity., Conclusions: Infection-related glomerulonephritis with IgA deposits is usually associated with Staphylococcus infections and mainly affects adult men. This entity has a poor prognosis which is correlated to interstitial fibrosis and tubular atrophy severity.

Background: Ifosfamide, a widely prescribed antineoplasic agent, is frequently associated with kidney dysfunction. Its nephrotoxicity is well documented in children, but data are lacking in adult patients., Methods: The aim of this retrospective study was to describe the clinical, biological and histological characteristics of ifosfamide nephrotoxicity., Results: We report 34 patients (median age: 41 years) admitted in six French nephrology departments for kidney failure and/or tubular dysfunction. Fifteen patients (44.1%) received cisplatin as part of their chemotherapy. In 6 patients (17.7%), ifosfamide nephrotoxicity was revealed by a proximal tubular dysfunction (PTD), in 5 patients (14.4%) by an acute kidney injury (AKI), in 6 patients (17.7%) by a chronic kidney disease (CKD) and in 17 patients (49.7%) by an association of PTD and AKI. Fourteen renal biopsies (41.2%) were performed and revealed acute tubular necrosis (85.7%), vacuolation (78.6%) and nuclear atypias (71.4%) of renal epithelial cells, interstitial inflammation (71.4%) and fibrosis (57.1%). Electron microscopy showed mitochondrial enlargement and dysmorphic changes suggestive of mitochondrial toxicity. Ten patients (29.4%) progressed to Stage 5 CKD, six (17.6%) required haemodialysis and six patients died during a median follow-up period of 31 months. The mechanisms driving the development of extracapillary lesions in focal segmental glomerulosclerosis (FSGS) and crescentic glomerulonephritis (CGN) remain poorly understood. A key question is how parietal epithelial cells (PECs) invade glomerular capillaries, thereby promoting injury and kidney failure. Here we show that expression of the tetraspanin CD9 increases markedly in PECs in mouse models of CGN and FSGS, and in kidneys from individuals diagnosed with these diseases. Cd9 gene targeting in PECs prevents glomerular damage in CGN and FSGS mouse models. Mechanistically, CD9 deficiency prevents the oriented migration of PECs into the glomerular tuft and their acquisition of CD44 and β1 integrin expression. These findings highlight a critical role for de novo expression of CD9 as a common pathogenic switch driving the PEC phenotype in CGN and FSGS, while offering a potential therapeutic avenue to treat these conditions.
