Abstract

A cooperative effort between the NASA/Johnson Space Center (JSC) and the Baylor College of Medicine (BCM) has been underway since 1988 to develop a long-term implantable Ventricular Assist Device (VAD). The VAD is intended to boost the cardiac output of patients with deteriorated cardiac function. For many of these patients, the best alternative is heart transplantation. Heart transplantation is a complex and expensive procedure and usually requires a long waiting period for a donor heart. The condition of the patient often deteriorates during this waiting period which complicates the pre and post-operative care. Because of these factors, the need for a long-term implantable VAD for use as a bridge-to-transplant device or as a permanent assist device has become the focus of much research. The need for a VAD has been estimated at 50,000 to 60,000 patients per year in the United States alone. A device which satisfies all the system performance and reliability requirements has yet to be achieved. However, the development of the NASA/Baylor VAD has progressed to a state in which commercial viability can being to be considered. The device is small, simple, efficient and reliable which meets all requirements for a totally implantable VAD.

Abstract

Background and purpose: Tapering immunosuppressants is desirable in patients with well‐controlled myasthenia gravis (MG). However, the association between tapering of calcineurin inhibitor dosage and reduction‐associated exacerbation is not known. The aim of this study was to clarify the frequency of reduction‐associated exacerbation when tacrolimus is tapered in stable patients with anti‐acetylcholine receptor antibody‐positive MG, and to determine the factors that predict exacerbations. Methods: We retrospectively analyzed 115 patients in whom tacrolimus dosage was tapered. The reduction‐associated exacerbation was defined as the appearance or worsening of one or more MG symptoms <3 months after the reduction. Results: Tacrolimus dosage was successfully tapered in 110 patients (96%) without any exacerbation. Five patients (4%) experienced an exacerbation, but symptoms were reversed in all patients when the tacrolimus dose was increased to the previous maintenance level. No patient developed an MG crisis. The age at onset was significantly earlier (30 vs. 56 years, P = 0.025) and the reduction in dosage was significantly larger (2.0 vs. 1.0 mg/day, P = 0.002) in patients with reduction‐associated exacerbation than in those without exacerbation. The cut‐off values determined in a receiver‐operating characteristic curve analysis were 52 years (sensitivity, 57%; specificity, 100%) for the age at onset and 1.5 mg (sensitivity, 80%; specificity, 100%) for the dose reduction. Conclusion: Tapering of tacrolimus was possible in most patients with well‐controlled anti‐acetylcholine receptor antibody‐positive MG. Early age at onset and a large reduction from maintenance dosage were associated with exacerbation. Reductions ≤1.5 mg/day from the maintenance dosage should be considered for patients with late‐onset disease. [ABSTRACT FROM AUTHOR]

Abstract

The homogeneity ranges of the L12-ordered compounds Ni3Ga and Ni3Ge and the boundaries of the neighboring phases have been examined by electron probe microanalysis using heterophase alloys in the temperature ranges 1073 to 1373 K (Ni-Ga) and 1073 to 1346 K (Ni-Ge). The composition ranges of the L12-ordered phases have been found to be narrower than those reported in the literature, on the Ga-rich side for Ni3Ga and on the Ge-deficient side for Ni3Ge.

Abstract

The objective of this study is to examine the ability of PIXE to distinguish the country of origin of duvets. The stable isotope method that is currently used is not recognized as reliable. Mineral concentrations of down feathers from France, Poland and Taiwan were measured by PIXE. The measurements were statistically analyzed. The results show even a scatter plot of appropriately chosen minerals distinguished between those countries at 95% accuracy. [ABSTRACT FROM AUTHOR]

Abstract

The objective of this study is to examine the variations in hair mineral concentrations measured by particle induced X-ray emission (PIXE). As the concentrations reflect the historical profile of minerals in blood for a few days to several years, hair mineral measurements vary among the hair strands of a sample as well as the locations on hair strands where PIXE beams are irradiated. PIXE analysis presents not only mineral measurements but also standard errors associated with them due to uncertainty in fitting analysis of X-ray spectrum. We developed statistical methods to decompose the intra-individual variance into the variances due to locations and that due to the fitting analysis. This study describes the results of statistical analysis on the variations in six minerals (Cu, Fe, Sr, Ca, Cl and Pb) and compares the results for better understanding of variations in hair mineral measurements obtained from hair analysis using PIXE.
