Background: Nonsurgical treatment of uncomplicated appendicitis is a reasonable option in many cases despite the sparsity of robust, easy access, externally validated, and multimodally informed clinical decision support systems (CDSSs). Developed by OpenAI, the Generative Pre-trained Transformer 3.5 model (GPT-3) may provide enhanced decision support for surgeons in less certain appendicitis cases or those posing a higher risk for (relative) operative contra-indications. Our objective was to determine whether GPT-3.5, when provided high-throughput clinical, laboratory, and radiological text-based information, will come to clinical decisions similar to those of a machine learning model and a board-certified surgeon (reference standard) in decision-making for appendectomy versus conservative treatment. Methods: In this cohort study, we randomly collected patients presenting at the emergency department (ED) of two German hospitals (GFO, Troisdorf, and University Hospital Cologne) with right abdominal pain between October 2022 and October 2023. Statistical analysis was performed using R, version 3.6.2, on RStudio, version 2023.03.0 + 386. Overall agreement between the GPT-3.5 output and the reference standard was assessed by means of inter-observer kappa values as well as accuracy, sensitivity, specificity, and positive and negative predictive values with the “Caret” and “irr” packages. Statistical significance was defined as p < 0.05. Results: There was agreement between the surgeon’s decision and GPT-3.5 in 102 of 113 cases, and all cases where the surgeon decided upon conservative treatment were correctly classified by GPT-3.5. The estimated model training accuracy was 83.3% (95% CI: 74.0, 90.4), while the validation accuracy for the model was 87.0% (95% CI: 66.4, 97.2). This is in comparison to the GPT-3.5 accuracy of 90.3% (95% CI: 83.2, 95.0), which did not perform significantly better in comparison to the machine learning model (p = 0.21). Conclusions: This study, the first study of the “intended use” of GPT-3.5 for surgical treatment to our knowledge, comparing surgical decision-making versus an algorithm found a high degree of agreement between board-certified surgeons and GPT-3.5 for surgical decision-making in patients presenting to the emergency department with lower abdominal pain.

Abstract Anecdotal evidence from preliminary observations has noted multiple instances where osteoporosis is present in elderly patients before the clinical detection of bowel disease, even in the absence of overt gastrointestinal symptoms. However, any potential association between these conditions remains to be further investigated. This computed tomography (CT) study investigates whether patients with gastrointestinal (GI) perforation have lower bone mineral density (BMD) than age and sex matched controls. BMD was measured by drawing 3D regions of interest in the bone marrow of the L1–L3 vertebral bodies on CT scans of each of 37 GI perforations and matched controls. Abstract Intra-arterial nimodipine administration is a widely used rescue therapy for cerebral vasospasm. Although it is known that its effect sets in with delay, there is little evidence in current literature. Our aim was to prove that the maximal vasodilatory effect is underestimated in direct angiographic controls. We reviewed all cases of intra-arterial nimodipine treatment for subarachnoid hemorrhage-related cerebral vasospasm between January 2021 and December 2022. Inclusion criteria were availability of digital subtraction angiography runs before and after nimodipine administration and a delayed run for the most affected vessel at the end of the procedure to decide on further escalation of therapy. We evaluated nimodipine dose, timing of administration and vessel diameters. Delayed runs were performed in 32 cases (19 patients) with a mean delay of 37.6 (± 16.6) min after nimodipine administration and a mean total nimodipine dose of 4.7 (± 1.2) mg. Abstract Since the outbreak of coronavirus disease 2019 (COVID-19), a growing number of cases of acute transverse myelitis associated with COVID-19 have been reported. Here, we present the case of a patient who developed sensory ataxia after COVID-19 with MR lesions suggestive for longitudinal myelitis and in the splenium of the corpus callosum. Abstract Background To investigate if iodine density overlay maps (IDO) and virtual monoenergetic images at 40 keV (VMI40keV) acquired from spectral detector computed tomography (SDCT) can improve detection of incidental skeletal muscle metastases in whole-body CT staging examinations compared to conventional images. Methods In total, 40 consecutive cancer patients who underwent clinically-indicated, contrast-enhanced, oncologic staging SDCT were included at this retrospective study: 16 patients with n = 108 skeletal muscle metastases confirmed by prior or follow-up CT, 18F-FDG-PET, MRI or histopathology, and a control group of 24 patients without metastases. Four independent readers performed blinded, randomized visual detection of skeletal muscle metastases in conventional images, IDO and VMI40keV, indicating diagnostic certainty for each lesion on a 5-point Likert scale. Quantitatively, ROI-based measurements of attenuation (HU) in conventional images and VMI40keV and iodine concentration in IDO were conducted. CNR was calculated and receiver operating characteristics (ROC) analysis of quantitative parameters was performed. Results Regarding subjective assessment, IDO (63.2 (58.5–67.8) %) and VMI40keV (54.4 (49.6–59.2) %) showed an increased sensitivity for skeletal muscle metastases compared to conventional images (39.8 (35.2–44.6) %). Specificity was comparable in VMI40keV (69.8 (63.2–75.8) %) and conventional images (69.2 (60.6–76.9) %), while in IDO, it was moderately increased to 74.2 (65.3–78.4) %. Quantitative image analysis revealed that CNR of skeletal muscle metastases to circumjacent muscle was more than doubled in VMI40keV (25.8 ± 11.1) compared to conventional images (10.0 ± 5.3, p ≤ 0.001). Iodine concentration obtained from IDO and HU acquired from VMI40kev (AUC = 0.98 each) were superior to HU attenuation in conventional images (AUC = 0.94) regarding differentiation between healthy and metastatic muscular tissue (p ≤ 0.05). Conclusions IDO and VMI40keV provided by SDCT improve diagnostic accuracy in the assessment of incidental skeletal muscle metastases compared to conventional CT.

Objective: To identify factors associated with early angiographic reperfusion improvement (EARI) following intra-arterial fibrinolytics (IAF) after failed or incomplete mechanical thrombectomy (MT). Methods: A subset of patients treated with MT and IAF rescue after incomplete reperfusion included in the INFINITY (INtra-arterial FIbriNolytics In ThrombectomY) multicenter observational registry was analyzed. Multivariable logistic regression was used to identify factors associated with EARI. Heterogeneity of the clinical effect of EARI on functional independence (defined as modified Rankin Score ≤2) was tested with interaction terms. Results: A total of 228 patients (median age: 72 years, 44.1% female) received IAF as rescue for failed or incomplete MT and had a post-fibrinolytic angiographic control run available (50.9% EARI). A cardioembolic stroke origin (adjusted odds ratio (aOR) 3.72, 95% confidence interval (CI) 1.39–10.0) and shorter groin puncture to IAF intervals (aOR 0.82, 95% CI 0.71–0.95 per 15-min delay) were associated with EARI, while pre-interventional thrombolysis showed no association (aOR 1.15, 95% CI 0.59–2.26). The clinical benefit of EARI after IAF seemed more pronounced in patients without or only minor early ischemic changes (Alberta Stroke Program Early Computed Tomography Score (ASPECTS) ≥9, aOR 4.00, 95% CI 1.37–11.61) and was absent in patients with moderate to severe ischemic changes (ASPECTS ≤8, aOR 0.94, 95% CI 0.27–3.27, p for interaction: 0.095). Conclusion: Early rescue and a cardioembolic stroke origin were associated with more frequent EARI after IAF. The clinical effect of EARI seemed reduced in patients with already established infarcts. If confirmed, these findings can help to inform patient selection and inclusion criteria for randomized-controlled trials evaluating IAF as rescue after MT.

PurposeCardiovascular comorbidity anticipates severe progression of COVID-19 and becomes evident by coronary artery calcification (CAC) on low-dose chest computed tomography (LDCT). The purpose of this study was to predict a patient's obligation of intensive care treatment by evaluating the coronary calcium burden on the initial diagnostic LDCT.MethodsEighty-nine consecutive patients with parallel LDCT and positive RT-PCR for SARS-CoV-2 were included from three centers. The primary endpoint was admission to ICU, tracheal intubation, or death in the 22-day follow-up period. CAC burden was represented by the Agatston score. Multivariate logistic regression was modeled for prediction of the primary endpoint by the independent variables "Agatston score > 0", as well as the CT lung involvement score, patient sex, age, clinical predictors of severe COVID-19 progression (history of hypertension, diabetes, prior cardiovascular event, active smoking, or hyperlipidemia), and laboratory parameters (creatinine, C-reactive protein, leucocyte, as well as thrombocyte counts, relative lymphocyte count, d-dimer, and lactate dehydrogenase levels).ResultsAfter excluding multicollinearity, "Agatston score >0" was an independent regressor within multivariate analysis for prediction of the primary endpoint (pConclusionCAC scoring on LDCT might help to predict future obligation of intensive care treatment at the day of patient admission to the hospital.

Background Patients aged ≥90 were excluded or under‐represented in past thrombectomy trials; thus, uncertainty remains whether treatment benefits can be expected regardless of age. This study investigates outcome and safety of thrombectomy in nonagenarians to improve decision making in a real‐world setting. Methods and Results All currently available data of patients aged ≥90 enrolled in the GSR‐ET (German Stroke Registry–Endovascular Treatment) were combined with a smaller cohort from 3 tertiary stroke centers. Baseline characteristics, procedural (Thrombolysis in Cerebral Infarction scale) and functional outcomes (modified Rankin Scale; mRS), as well as complications (symptomatic intracranial hemorrhage, serious adverse events; SAEs) were analyzed. Good functional outcome was defined as mRS ≤3 at 90‐days. 203 patients with anterior circulation stroke and prestroke mRS ≤3 were included. The rate of successful recanalization (Thrombolysis in Cerebral Infarction scale ≥2b) was 75.9% (154/203). Good functional outcome (mRS ≤3) was observed in 21.6% (41 of 193) at 90‐days. BackgroundCardiovascular comorbidity anticipates poor prognosis of SARS-CoV-2 disease (COVID-19) and correlates with the systemic atherosclerotic transformation of the arterial vessels. The amount of aortic wall calcification (AWC) can be estimated on low-dose chest CT. We suggest quantification of AWC on the low-dose chest CT, which is initially performed for the diagnosis of COVID-19, to screen for patients at risk of severe COVID-19.MethodsSeventy consecutive patients (46 in center 1, 24 in center 2) with parallel low-dose chest CT and positive RT-PCR for SARS-CoV-2 were included in our multi-center, multi-vendor study. The outcome was rated moderate (no hospitalization, hospitalization) and severe (ICU, tracheal intubation, death), the latter implying a requirement for intensive care treatment. The amount of AWC was quantified with the CT vendor's software.ResultsOf 70 included patients, 38 developed a moderate, and 32 a severe COVID-19. The average volume of AWC was significantly higher throughout the subgroup with severe COVID-19, when compared to moderate cases (771.7 mm3 (Q1 = 49.8 mm3, Q3 = 3065.5 mm3) vs. 0 mm3 (Q1 = 0 mm3, Q3 = 57.3 mm3)). Within multivariate regression analysis, including AWC, patient age and sex, as well as a cardiovascular comorbidity score, the volume of AWC was the only significant regressor for severe COVID-19 (p = 0.004). For AWC > 3000 mm3, the logistic regression predicts risk for a severe progression of 0.78. If there are no visually detectable AWC risk for severe progression is 0.13, only.ConclusionAWC seems to be an independent biomarker for the prediction of severe progression and intensive care treatment of COVID-19 already at the time of patient admission to the hospital; verification in a larger multi-center, multi-vendor study is desired.

Background: Clinical outcome in patients undergoing mechanical thrombectomy (MT) due to intracranial large vessel occlusion (LVO) in the anterior circulation is influenced by several factors. The impact of a concomitant extracranial carotid artery stenosis (CCAS) contralateral to the intracranial lesion remains unclear.Methods: Retrospective analysis of 392 consecutive patients treated with MT due to intracranial LVO in the anterior circulation in two comprehensive stroke centers between 2014 and 2017. Clinical (including demographics and NIHSS), imaging (including angiographic evaluation of CCAS via NASCET criteria), and procedural data were evaluated. Primary endpoint was an unfavorable clinical outcome defined as modified Rankin Scale 3–6 at 90 days.Results: In 27/392 patients (7%) pre-interventional imaging exhibited a CCAS (>50%) contralateral to the intracranial lesion compared to 365 patients without relevant stenosis. Median baseline NIHSS, procedural timings, and reperfusion success did not differ between groups. Median volume of the final infarct core was larger in CCAS patients (176 cm3, IQR 32-213 vs. 11 cm3, 1-65; p < 0.001). At 90 days, unfavorable outcome was documented in 25/27 CCAS patients (93%) vs. 211/326 (65%; p = 0.003) with a mortality of 63 vs. 19% (p = 0.001), respectively. Presence of CCAS was associated with an unfavorable outcome at 90 days independent of age and baseline NIHSS in multivariate logistic regression (OR 2.2, CI 1.1-4.7; p < 0.05).Conclusion: For patients undergoing MT due to intracranial vessel occlusion in the anterior circulation, the presence of a contralateral CCAS >50% is a predictor of unfavorable clinical outcome at 90 days.

Purpose: The aim of this study was to evaluate the technical feasibility and rate of mid-term occlusion in aneurysms treated solely with the Pipeline Embolization Device (PED) in a German tertiary care university hospital. Materials and Methods: Forty-nine non-consecutive intracranial aneurysms underwent endovascular treatment using the PED exclusively between March 2011 and May 2017 at our institution. Primary endpoint was a favorable aneurysm occlusion defined as OKM C1-3 and D (O'Kelly Marotta Scale). Secondary endpoints were retreatment rate and delayed complications. Median follow-up was 200 days. Results: The mean aneurysm size was 7.1 ± 5.3 mm. Forty-four aneurysms were located in the anterior circulation (90%). Ten aneurysms were ruptured (20%). Branching vessels from the sac were observed in 11 aneurysms (22%). Favorable obliteration immediately after PED placement was seen in 13/49 aneurysms (27%), of those nine aneurysms were completely occluded (18%). Angiographic and clinical follow-up was available for 45 cases (92%); 36/45 aneurysms (80%) were occluded completely and 40/45 aneurysms (89%) showed a favorable occlusion result. A branching vessel arising from the aneurysm sac was associated with incomplete occlusion (P < .05). All electively treated patients had good outcome (mRS 0). Three aneurysms (6%) required additional treatment due to aneurysm recurrence. Conclusion: In our series, treatment of intracranial aneurysms with the PED was associated with favorable occlusion rates and low complication rates at mid-term follow-up.
