1. Imputación del instante de seroconversión al VIH en cohortes de hemofílicos

Abstract

Objetivos: Describir la técnica utilizada para imputar la fecha de seroconversión al virus de la inmunodeficiencia humana (VIH) en cohortes de hemofílicos y validar su utilización en sujetos con fecha de seroconversión datada dentro de un intervalo de las cohortes del proyecto GEMES (Grupo Español Multicéntrico para el Estudio de Seroconvertores al VIH). Método: Se dispone de 632 hemofílicos procedentes de tres unidades de hemofilia identificados como VIH+ entre 1983 y 1985 (fechas de las primeras pruebas VIH+) y 1.092 individuos procedentes de 5 cohortes del proyecto GEMES con una ventana de seroconversión (tiempo entre pruebas VIH- y VIH+) menor de 3 años en los que se asume el punto medio (PM) como fecha de seroconversión. Se imputa la fecha de seroconversión para los dos grupos a partir de la estimación de la distribución de probabilidad de seroconversión que se obtiene mediante el algoritmo EM. Se utilizan dos métodos de imputación: uno a partir del valor esperado de la distribución estimada y otro de la media geométrica de 5 muestras aleatorias de la distribución estimada. Las imputaciones se han validado en las cohortes de no hemofílicos al comparar con el PM y el tiempo libre de sida y la supervivencia con las diferentes estimaciones de seroconversión. Resultados: La mediana de la seroconversión se sitúa en mayo de 1993 para los no hemofílicos y en 1982 para los hemofílicos. En las cohortes de no hemofílicos no se observan grandes diferencias entre las fechas de la seroconversión estimada como el punto medio y las medianas estimadas a partir de los dos métodos de imputación. Lo mismo ocurre en el caso de las cohortes de hemofílicos. No se observan diferencias en las estimaciones del tiempo libre de sida y supervivencia en ambos grupos de cohortes. Conclusiones: La imputación de la media geométrica de varias muestras aleatorias y la imputación de valor esperado proporcionan una buena estimación de la fecha de seroconversión frente al VIH que puede ser utilizada para estimar el tiempo libre de sida y la supervivencia en una cohorte de hemofílicos para los que se desconoce la fecha de seroconversión

2. Comparación de dos métodos en el análisis del efecto a corto plazo de la contaminación atmosférica en la salud

Abstract

Objetivo: Se comparan dos metodologías para el análisis de la relación entre contaminación atmosférica y salud. Una de las metodologías (ME) está basada en el modelo lineal generalizado, mientras que la otra (MN) incorpora el modelo aditivo generalizado (GAM). Aparte del modelo estadístico utilizado, las dos metodologías presentan discrepancias adicionales en cuanto al tipo y número de variables empleadas en el control de la confusión. Método: Se realizó un análisis para cada retardo de humos negros (de 0 a 5) y diversos indicadores de salud con datos relativos a la ciudad de Valencia y siguiendo ambas metodologías. Los resultados obtenidos se compararon mediante el examen de la secuencia y retardo de mayor efecto, el cálculo de la razón de las amplitudes de los intervalos de confianza y el cálculo de la diferencia porcentual en las estimaciones. Resultados: El cambio en los resultados según la metodología fue pequeño. La forma de la secuencia y, en general, el retardo de mayor efecto permanecieron invariables. Los coeficientes se mantuvieron estables (la diferencia porcentual de MN metodológicas con respecto a ME no superó el 10%) y los intervalos de confianza resultaron más precisos con MN, aunque la disminución de la amplitud de los intervalos de confianza no fue drástica (la razón de amplitudes se situó en torno al 90%). Conclusiones: La escasa variación de los resultados ofrece garantías en cuanto a la aplicabilidad de ambas metodologías, MN y ME, si bien la reducción sistemática del error estándar con MN apoya el uso de este último método como el procedimiento más eficiente en el control de la confusión.

Abstract

Objetivo: Estimar la asociación a corto plazo entre los valores de contaminación atmosférica por partículas en suspensión y diferentes indicadores de morbilidad y mortalidad en la ciudad de Valencia. Métodos: Estudio ecológico de series temporales en el que se relacionan las variaciones diarias en los valores de contaminación atmosférica por humos negros e indicadores de mortalidad, ingresos hospitalarios y visitas a urgencias en la ciudad de Valencia, durante el período 1994-1996. La magnitud de la asociación se estimó mediante regresión autorregresiva de Poisson utilizando modelos aditivos generalizados. Se examinó la forma de la relación, el efecto retardado de la contaminación y la posible modificación de efecto por otros contaminantes o períodos del año. Resultados: Existe una asociación significativa entre los valores de partículas en suspensión y varios indicadores analizados en Valencia. La forma de dicha relación es lineal. Un incremento de 10 μg/m³ en los valores diarios de humos negros se asoció con un incremento del 1,8% (intervalo de confianza [IC] del 95%, 0,9-2,7%) en el número de defunciones del día siguiente. El mismo incremento en la contaminación se asoció con un aumento del 1,5% (0,1-2,8%) en las defunciones por causa circulatoria, en un 1,3% (0,0-2,6%) en los ingresos por causas circulatorias y en un 5,4 (0,6-10,4%) en el número de visitas a urgencias por asma. Esta asociación no está confundida por los valores de otros contaminantes. No se encontró asociación de los humos negros con la mortalidad por enfermedades respiratorias, los ingresos hospitalarios por enfermedades cerebrovasculares ni las visitas a urgencias por enfermedad pulmonar obstructiva crónica (EPOC). Conclusiones: Este estudio proporciona evidencia de que los valores de partículas en suspensión en Valencia se asocian con las urgencias por asma, los ingresos por enfermedades circulatorias y las defunciones diarias. La asociación encontrada es consistente con los resultados de otros estudios realizados y es coherente con conocimientos recientes sobre los mecanismos fisiopatológicos.

Abstract

Spectral observations of Neptune made in 2019 with the Multi Unit Spectroscopic Explorer (MUSE) instrument at the Very Large Telescope (VLT) in Chile have been analyzed to determine the spatial variation of aerosol scattering properties and methane abundance in Neptune's atmosphere. The darkening of the South Polar Wave at ∼60°S, and dark spots such as the Voyager 2 Great Dark Spot is concluded to be due to a spectrally dependent darkening (λ < 650 nm) of particles in a deep aerosol layer at ∼5 bar and presumed to be composed of a mixture of photochemically generated haze and H2S ice. We also note a regular latitudinal variation of reflectivity at wavelengths of very low methane absorption longer than ∼650 nm, with bright zones latitudinally separated by ∼25°. This feature, which has similar spectral characteristics to a discrete deep bright spot DBS‐2019 found in our data, is found to be consistent with a brightening of the particles in the same ∼5‐bar aerosol layer at λ > 650 nm. We find the properties of an overlying methane/haze aerosol layer at ∼2 bar are, to first‐order, invariant with latitude, while variations in the opacity of an upper tropospheric haze layer reproduce the observed reflectivity at methane‐absorbing wavelengths, with higher abundances found at the equator and also in a narrow "zone" at 80°S. Finally, we find the mean abundance of methane below its condensation level to be 6%–7% at the equator reducing to ∼3% south of ∼25°S, although the absolute abundances are model dependent. Plain Language Summary: Observations of Neptune in visible light, made with the Multi Unit Spectroscopic Explorer (MUSE) instrument at ESO's Very Large Telescope, reveal the different layers of clouds and gases within this Ice Giant atmosphere, and how they change with height and latitude. The properties of the 1–2‐bar layer of methane ice and haze are found to be roughly constant with latitude. However, a diffuse upper layer is thickest at the equator and near the south pole, indicating air rising at mid‐latitudes and descending near the equator and poles. Conversely, the distribution of methane between the deep 5‐bar clouds (hydrogen sulphide ice and haze) and the middle layers decreases from 6% to 7% at the equator to ∼3% near the south pole, suggesting instead that air is rising at the equator and descending elsewhere. Finally, a blue‐green darkening of the particles in the deep layer can explain Neptune's dark spots and the dark "South Polar Wave" at 60°S, whereas a brightening of the same particles at red and infrared wavelengths matches occasional discrete deep bright spots and a set of previously unnoticed bright "zones", separated by 25° latitude. All this is evidence that the atmospheric circulation changes as a function of height and latitude in complex and surprising ways. Abstract

Context. A stellar occultation by Neptune's main satellite, Triton, was observed on 5 October 2017 from Europe, North Africa, and the USA. We derived 90 light curves from this event, 42 of which yielded a central flash detection. Aims. We aimed at constraining Triton's atmospheric structure and the seasonal variations of its atmospheric pressure since the Voyager 2 epoch (1989). We also derived the shape of the lower atmosphere from central flash analysis. Methods. We used Abel inversions and direct ray-tracing code to provide the density, pressure, and temperature profiles in the altitude range ∼8 km to ∼190 km, corresponding to pressure levels from 9 μbar down to a few nanobars. Results. (i) A pressure of 1.18 ± 0.03 μbar is found at a reference radius of 1400 km (47 km altitude). (ii) A new analysis of the Voyager 2 radio science occultation shows that this is consistent with an extrapolation of pressure down to the surface pressure obtained in 1989. (iii) A survey of occultations obtained between 1989 and 2017 suggests that an enhancement in surface pressure as reported during the 1990s might be real, but debatable, due to very few high S/N light curves and data accessible for reanalysis. The volatile transport model analysed supports a moderate increase in surface pressure, with a maximum value around 2005-2015 no higher than 23 μbar. The pressures observed in 1995-1997 and 2017 appear mutually inconsistent with the volatile transport model presented here. (iv) The central flash structure does not show evidence of an atmospheric distortion. We find an upper limit of 0.0011 for the apparent oblateness of the atmosphere near the 8 km altitude. ©

Mars&apos;s mystery plume In March and April 2012, amateur astronomers observing Mars recorded what seemed like an eruption of some kind, a protrusion from the planet&apos;s disk that took about 10 hours to develop and lasted for more than a week. Agustin Sánchez-Lavega and colleagues have now analysed these observations in detail and describe the highest-reaching plume-like feature ever detected on Mars. It reached an altitude of 250 kilometres -- previous high-altitude features reached no higher than 100 kilometres -- and extended more than 500 kilometres in both the north-south and east-west directions. Suggested explanations for this mysterious plume include clouds of CO.sub.2 or H.sub.2O ice or a phenomenon related to auroral emissions, but our current understanding of Martian atmospheric dynamics and plume formation cannot account for the creation of such enormous plumes. The Martian limb (that is, the observed &apos;edge&apos; of the planet) represents a unique window into the complex atmospheric phenomena occurring there. Clouds of ice crystals (CO.sub.2 ice or H.sub.2O ice) have been observed numerous times by spacecraft and ground-based telescopes, showing that clouds are typically layered and always confined below an altitude of 100 kilometres; suspended dust has also been detected at altitudes up to 60 kilometres during major dust storms.sup.1,2,3,4,5,6. Highly concentrated and localized patches of auroral emission controlled by magnetic field anomalies in the crust have been observed at an altitude of 130 kilometres.sup.7. Here we report the occurrence in March and April 2012 of two bright, extremely high-altitude plumes at the Martian terminator (the day-night boundary) at 200 to 250 kilometres or more above the surface, and thus well into the ionosphere and the exosphere.sup.8,9. They were spotted at a longitude of about 195° west, a latitude of about -45° (at Terra Cimmeria), extended about 500 to 1,000 kilometres in both the north-south and east-west directions, and lasted for about 10 days. The features exhibited day-to-day variability, and were seen at the morning terminator but not at the evening limb, which indicates rapid evolution in less than 10 hours and a cyclic behaviour. We used photometric measurements to explore two possible scenarios and investigate their nature. For particles reflecting solar radiation, clouds of CO.sub.2-ice or H.sub.2O-ice particles with an effective radius of 0.1 micrometres are favoured over dust. Alternatively, the plume could arise from auroral emission, of a brightness more than 1,000 times that of the Earth&apos;s aurora, over a region with a strong magnetic anomaly where aurorae have previously been detected.sup.7. Importantly, both explanations defy our current understanding of Mars&apos; upper atmosphere.

Abstract

Context. Until recently, camera networks designed for monitoring fireballs worldwide were not fully automated, implying that in case of a meteorite fall, the recovery campaign was rarely immediate. This was an important limiting factor as the most fragile - hence precious - meteorites must be recovered rapidly to avoid their alteration. Aims. The Fireball Recovery and InterPlanetary Observation Network (FRIPON) scientific project was designed to overcome this limitation. This network comprises a fully automated camera and radio network deployed over a significant fraction of western Europe and a small fraction of Canada. As of today, it consists of 150 cameras and 25 European radio receivers and covers an area of about 1.5 × 10km. Methods. The FRIPON network, fully operational since 2018, has been monitoring meteoroid entries since 2016, thereby allowing the characterization of their dynamical and physical properties. In addition, the level of automation of the network makes it possible to trigger a meteorite recovery campaign only a few hours after it reaches the surface of the Earth. Recovery campaigns are only organized for meteorites with final masses estimated of at least 500 g, which is about one event per year in France. No recovery campaign is organized in the case of smaller final masses on the order of 50 to 100 g, which happens about three times a year; instead, the information is delivered to the local media so that it can reach the inhabitants living in the vicinity of the fall. Results. Nearly 4000 meteoroids have been detected so far and characterized by FRIPON. The distribution of their orbits appears to be bimodal, with a cometary population and a main belt population. Sporadic meteors amount to about 55% of all meteors. A first estimate of the absolute meteoroid flux (mag < -5; meteoroid size ≥∼1 cm) amounts to 1250/yr/10km. This value is compatible with previous estimates. Finally, the first meteorite was recovered in Italy (Cavezzo, January 2020) thanks to the PRISMA network, a component of the FRIPON science project., FRIPON was initiated by funding from ANR (grant N.13- BS05-0009-03), carried by the Paris Observatory, Muséum National d’Histoire Naturelle, Paris-Saclay University and Institut Pythéas (LAM-CEREGE). Abstract

BackgroundA persistently low CD4/CD8 ratio has been reported to inversely correlate with the risk of non-AIDS defining cancer in people living with human immunodeficiency virus (HIV; PLWH) efficiently treated by combination antiretroviral therapy (cART). We evaluated the impact of the CD4/CD8 ratio on the risk of Kaposi sarcoma (KS) or non-Hodgkin lymphoma (NHL), still among the most frequent cancers in treated PLWH.MethodsPLWH from the Collaboration of Observational HIV Epidemiological Research Europe (COHERE) were included if they achieved virological control (viral load ≤ 500 copies/mL) within 9 months following cART and without previous KS/LNH diagnosis. Cox models were used to identify factors associated with KS or NHL risk, in all participants and those with CD4 ≥ 500/mm3 at virological control. We analyzed the CD4/CD8 ratio, CD4 count and CD8 count as time-dependent variables, using spline transformations.ResultsWe included 56 708 PLWH, enrolled between 2000 and 2014. At virological control, the median (interquartile range [IQR]) CD4 count, CD8 count, and CD4/CD8 ratio were 414 (296–552)/mm3, 936 (670–1304)/mm3, and 0.43 (0.28–0.65), respectively. Overall, 221 KS and 187 NHL were diagnosed 9 (2–37) and 18 (7–42) months after virological control. Low CD4/CD8 ratios were associated with KS risk (hazard ratio [HR] = 2.02 [95% confidence interval {CI } = 1.23–3.31]) when comparing CD4/CD8 = 0.3 to CD4/CD8 = 1) but not with NHL risk. High CD8 counts were associated with higher NHL risk (HR = 3.14 [95% CI = 1.58–6.22]) when comparing CD8 = 3000/mm3 to CD8 = 1000/mm3). Similar results with increased associations were found in PLWH with CD4 ≥ 500/mm3 at virological control (HR = 3.27 [95% CI = 1.60–6.56] for KS; HR = 5.28 [95% CI = 2.17–12.83] for NHL).ConclusionsLow CD4/CD8 ratios and high CD8 counts despite effective cART were associated with increased KS/NHL risks respectively, especially when CD4 ≥ 500/mm3.
