1. Impact of Corticosteroid Dosing and Glucose Levels on Readmission Rates in Chronic Obstructive Pulmonary Disease Exacerbation Patients: A Study from Tertiary Care Center

Abstract

Background: Chronic obstructive pulmonary disease (COPD) exacerbations often require hospitalization, and corticosteroids are commonly used for treatment. The impact of corticosteroid dosing and glucose levels on patient outcomes, particularly readmission rates, remains unclear. Aims and Objectives: This study aims to examine the impact of corticosteroid dosing and blood glucose levels on readmission rates in hospitalized COPD patients. The specific objectives include assessing the association between corticosteroid dosing adherence, glucose levels, and readmission rates within 30 and 90 days post-discharge. Materials and Methods: A retrospective analysis was conducted on 200 hospitalized COPD patients with exacerbations. Data collected included patient demographics, corticosteroid dosing, blood glucose levels, and discharge plans. The study evaluated readmission rates within 30 days and between 31 and 90 days post-discharge, comparing these rates against corticosteroid dosing adherence and glucose levels during hospitalization. Results: No significant association was found between adherence to recommended corticosteroid dosing and readmission rates within 30 days or between 31 and 90 days post-discharge. Average glucose levels during hospitalization did not affect readmission rates. Patients discharged to home with home health services or post-hospital facilities had higher 30-day readmission rates compared to those discharged home without additional care. Subgroup analysis revealed no significant trends in readmission rates among patients with prior diabetes diagnoses. Conclusion: The study indicates that adherence to recommended corticosteroid dosing and average glucose levels during hospitalization do not significantly affect readmission rates for COPD exacerbations. However, discharge plans involving home health services or post-hospital facilities are associated with higher 30-day readmission rates. While higher corticosteroid doses are not advocated, clinicians should be cautious of hyperglycemia risks, particularly in diabetic patients. The study’s limitations include its retrospective design and variability in steroid administration, suggesting a need for further investigation into in-hospital management strategies for COPD exacerbations and their impact on post-discharge outcomes.

Abstract

Vitamin D deficiency has been implicated in various non-skeletal diseases, including coronary artery disease. However, the relationship between serum vitamin D levels and CAD severity remains unclear, particularly in Indian populations with a high prevalence of vitamin D deficiency. This prospective observational study included 252 patients who underwent coronary angiography at a tertiary care center. Serum vitamin D levels were categorized as normal, insufficient or deficient. CAD severity was assessed by the number of vessels involved, and SYNTAX scores as low, intermediate or high. Statistical analyses were performed to determine correlations between vitamin D levels and CAD parameters. The mean age of the study population was 57.79 ± 12 years with 57% being male. Vitamin D deficiency was observed in 27.8% of the participants, while 34.5% had insufficient levels, and 37.7% had normal levels. CAD was present in 71.4% of participants, with 21% having SVD, 25.4% DVD and 25% TVD. No significant difference in serum vitamin D levels was found between participants with normal coronaries and those with CAD (27.95 vs. 28.21 ng/mL, p = 0.8). Vitamin D levels showed no significant correlation with the number of diseased vessels (p=0.3). However, patients with high SYNTAX scores (=33) had significantly higher mean vitamin D levels compared to those with low and intermediate scores (p=0.001). No significant association between vitamin D levels and the presence or extent of CAD based on the number of vessels involved was found. However, an unexpected positive correlation between vitamin D levels and high SYNTAX scores warrants further investigation. Abstract

The aim of this study was to estimate the best covariance function structure, by means of Legendre polynomials using random regression models (RRM) to describe the genetic variability of the weight growth trajectory of quails and to estimate the genetic parameters. Body weight records of animals at 7, 14, 21, 28, 35, and 42 days of age were used. The orders of fit of the polynomials were gradually increased in orders from 3 to 6 for comparison between the models the Akaike information criteria were used. The models included fixed effects of the contemporary group, the fixed regression coefficient of weight on the Legendre polynomial, random regression coefficients of the direct additive genetic, and the permanent environment effects of the animal, in addition to the residual random effect. The RRM with orders five for the additive genetic effect and six for the permanent environment effect, under the assumption of homogeneity, was the most suitable to describe the genetic variability of the birds’ ponderal growth. There was greater expression of additive genetic variability after 21 days, resulting in distinct and increasing heritability estimates between two periods in ponderal development. Estimates of additive genetic correlations for weights between the ages evaluated indicated that genetic associations are more correlated between body weights at closer ages when compared to ages more distant along the growth trajectory. Thus, it is concluded that selection in birds is more efficient from 28 days of age due to the higher heritability values. [ABSTRACT FROM AUTHOR]

Abstract

Objectives: The study aimed to evaluate quality of nephrolithometric nomograms to predict stone-free rates (SFRs) and complication rates (CRs) in case of minimally invasive percutaneous nephrolithotomy (PNL). In the last decade, nomograms have been introduced to estimate the SFRs and CRs of PNL. However, no data are available regarding their reliability in case of utilization of miniaturized devices. Herein we present a prospective multicentric study to evaluate reliability of Guy's stone score (GSS), the stone size, tract length, obstruction, number of involved calyces, and essence of stone (S.T.O.N.E.) nephrolithometry score and Clinical Research Office of the Endourological Society (CROES) score in patients treated with minimally invasive PNL. Methods: We evaluated SFRs and CRs of 222 adult patients treated with miniaturized PNL. Patients were considered stone-free if no residual fragments of any size at post-operative unenhanced computed tomography scan. Patients demographics, SFRs, and CRs were reported and analysed. Performances of nomograms were evaluated with the area under the curve (AUC). Results: We included 222 patients, the AUCs of GSS, CROES score and S.T.O.N.E. nephrolithometry score were 0.69 (95% confidence interval [CI] 0.61–0.78), 0.64 (95% CI 0.56–0.73), and 0.62 (95% CI 0.52–0.71), respectively. Regarding SFRs, at multivariate binomial logistic regression, only the GSS had significance with an odds ratio (OR) of 0.53 (95% CI 0.31–0.95, p=0.04). We did not find significant correlation with complications, with only a trend for GSS. Conclusion: This is the first study evaluating nomograms in miniaturized PNL. They still show good reliability; however, our data showed lower performances compared to standard PNL. We emphasize the need of further studies to confirm this trend. A dedicated nomogram for minimally invasive PNL may be necessary.

Abstract

Purpose Therapeutic strategies for prostate cancer (PCa) have been evolving dramatically worldwide. The current article reports on the evolution of surgical management strategies for PCa in Italy. Methods The data from two independent Italian multicenter projects, the MIRROR-SIU/LUNA (started in 2007, holding data of 890 patients) and the Pros-IT-CNR project (started in 2014, with data of 692 patients), were compared. Differences in patients' characteristics were evaluated. Multivariable logistic regression models were used to identify characteristics associated with robot-assisted (RA) procedure, nerve sparing (NS) approach, and lymph node dissection (LND). Results The two cohorts did not differ in terms of age and prostate-specific antigen (PSA) levels at biopsy. Patients enrolled in the Pros-IT-CNR project more frequently were submitted to RA (58.8% vs 27.6%,p < 0.001) and NS prostatectomy (58.4% vs. 52.9%,p = 0.04), but received LND less frequently (47.7% vs. 76.7%,p < 0.001), as compared to the MIRROR-SIU/LUNA patients. At multivariate logistic models, Lower Gleason Scores (GS) and PSA levels were significantly associated with RA prostatectomy in both cohorts. As for the MIRROR-SIU/LUNA data, clinical T-stage was a predictor for NS (OR = 0.07 for T3, T4) and LND (OR = 2.41 for T2) procedures. As for Pros-IT CNR data, GS >= (4 + 3) and positive cancer cores >= 50% were decisive factors both for NS (OR 0.29 and 0.30) and LND (OR 7.53 and 2.31) strategies. Conclusions PCa management has changed over the last decade in Italian centers: RA and NS procedures without LND have become the methods of choice to treat newly medium-high risk diagnosed PCa.

Abstract

Background There are several treatments available to newly diagnosed prostate cancer (PCA) patients. Although surgery and radiotherapy (RT) with or without androgen deprivation therapy (ADT) are widely adopted treatment options for localized PCA together with active surveillance (AS), there is no consensus nor randomised trials on treatment selection, prospective quality of life (QOL), along with toxicity outcomes and according to treatment modality in the Italian population. The current study aimed to describe clinical-therapeutic features and QOL at PCA diagnosis, according to different treatment patterns in a large prospective, Italian population, enrolled in the Pros-IT CNR study. Methods The Pros-IT CNR is an on-going national, multicenter, observational, prospective study on patients affected by PCA who have been referred by 97 Italian Urology, Radiation Oncology and Medical Oncology facilities participating in the project. The possible relationships between the treatment patterns reported in the 6 month follow-up case report form and patients' features at diagnosis were evaluated using exploratory multiple correspondence analysis (MCA) and other data analysis method. Results At diagnosis, surgery and AS patients were significantly younger, had fewer comorbidities, lower PSA levels and Gleason Score (GS) values; they were also diagnosed at an earlier stage of disease with respect to the RT or ADT patients who showed significantly worse QoL scores at the time of diagnosis. Conclusions An analysis of the data collected at baseline and 6 months later uncovered substantial differences in ages, comorbidities, clinical and QOL features in the various treatment groups. These findings do not fully reflect the current PCA treatment guidelines and suggest the need for a multidisciplinary consensus guideline to ameliorate both the counselling and treatments of PCA patients.

Abstract

Radiotherapy (RT) represents an important therapeutic option for the treatment of localized prostate cancer. The aim of the current study is to examine trajectories in patients’ reported quality of life (QoL) aspects related to bowel function and bother, considering data from the PROState cancer monitoring in ITaly from the National Research Council (Pros-IT CNR) study, analyzed with growth mixture models. Data for patients who underwent RT, either associated or not associated with androgen deprivation therapy, were considered. QoL outcomes were assessed over a 2-year period from the diagnosis, using the Italian version of the University of California Los Angeles-Prostate Cancer Index (Italian-UCLA-PCI). Three trajectories were identified for the bowel function, having three or more comorbidities and the use of 3D-CRT technique for RT were associated with the worst trajectory (OR = 3.80, 95% CI 2.04–7.08, OR = 2.17, 95% CI 1.22–3.87, respectively). Two trajectories were identified for the bowel bother scores, diabetes and the non-Image guided RT method were associated with being in the worst bowel bother trajectory group (OR = 1.69, 95% CI 1.06–2.67, OR = 2.57, 95% CI 1.70–3.86, respectively). The findings from this study suggest that the absence of comorbidities and the use of intensity modulated RT techniques with image guidance are related with a better tolerance to RT in terms of bowel side effects.
