Background: Chlorhexidine Gluconate (CHG), Hibiclens (4% CHG with 4% Isopropyl Alcohol Detergent), and Chloraprep (i.e. labeled CHG-based solutions), utilized as preoperative surgical preparatory solutions may all cause severe oculotoxicity and ototoxicity. Alternatively, 10% Povidone-Iodine (PI) solutions without detergent demonstrate minimal toxic effects on the eyes and ears. Methods: Based on studies from 1984 to 2021, we compared the safety/efficacy of CHG-based versus PI-based solutions utilized for presurgical skin preparation near the cornea/eyes and ears (i.e., predominantly for cranial or cervical spine surgery). Results: Some studies documented that even minimal exposure (i.e., "splash risk") during face/neck skin preparation with CHG-based solutions could result in irreversible corneal injury and ototoxicity. Within minutes to hours, CHG-based non-detergent solutions posed the risks of; corneal epithelial edema, anterior stromal edema, conjunctival chemosis, bullous keratopathy, and de-epithelialization. Notably, even occlusive dressings like Tegaderm could not protect against CHG penetration. Alternatively, PI-based solutions posed no to minimal ocular and/or ototoxicity, while often demonstrating comparable protection against surgical site infections (SSI). Conclusion: Chlorhexidine Gluconate (CHG), Hibiclens, and Chloraprep (i.e. CHG-based solutions) are often used as skin preparations near the face/eyes/spine (i.e., particularly anterior/posterior cervical procedures). However, if these solutions come in contact with the eyes, corneal irritation, abrasions, and even blindness may result. Background: Povidone-iodine (PVI) is an effective disinfection solution for surgical wounds. However, there is some reports of its adverse effects on wound healing and bone reunion. Here, we evaluate the efficacy and safety of PVI irrigation in the prevention of surgical site infection in spinal surgery. Methodology: From February 2011 to June 2019, 936 candidates of spinal fusion surgery were randomized to receive PVI 3% (n = 468) or normal saline (NS) irrigation (n = 468) using convenient sampling. Before bone grafting, the surgical wounds were irrigated with NS 0.9% or PVI 3% followed by NS for a maximum of two min. The rate of postoperative surgical site infection, wound dehiscence, vertebral fusion, pain intensity, and function score were evaluated. The pain was scored using the visual analogue scale (VAS). Results: The wound infection rate was significantly less in the PVI group compared with the NS group [5 (1.1%) vs. 21(4.48%), p = 0.032]. There was no significant difference in the rate of vertebral union or wound dehiscence between the two groups. The pain VAS and function scores were both improved after surgery while this improvement was comparable between the study groups. Conclusion: Lavage of surgical wounds with povidone-iodine 3% prevents surgical site infection in spinal surgeries without influencing wound healing, bone union or the clinical outcome. Objective: The efficacy and safety of povidone-iodine in wound dressing and irrigation of some operative cavities were established by many in vitro and in vivo experimental reports and clinical series. However, its use in defective tissue in neural structures has not been confirmed yet. The aim of the present study was to histopathologically investigate its effect on neural tissues when applied on the upper side of defective dura. Methods: Wistar rats were randomly divided into two experimental groups: control and povidone-iodine groups. In the control group, durotomy was performed following laminectomy, and the spinal cord was covered with a dry sponge. In the study group, the same procedure was performed, but open duras were covered with a sponge that had been wetted with 0.1 % povidone-iodine solution. Three weeks after surgery, all experimental animals were sacrificed, and histopathological evaluations were conducted. Results: Myelin changes were absent or minimal in all cases of the control group but were present as markedly increased myelin degeneration in nearly all cases in the study group. Axonal degeneration and hypoxic neuronal damage were absent in the control group, whereas they were marked in half of the study group. No statistically significant differences were established in Schwann cell proliferation, venous congestion, and lymphocytic proliferation between the two groups. Conclusions: Based on the present study, 0.1 % povidone-iodine solution cannot be recommended for wound dressing for neural structures such as myelomeningocele cases because of possible damage to underlying neural tissues. In peritoneal dialysis (PD) the dialysate is introduced into the peritoneal cavity, and the peritoneal membrane is used as the dialysis membrane. In PD, patients exchange the dialysate by themselves through the connection tube attached to the indwelling catheter that is inserted into the peritoneal cavity. Microbes may enter the peritoneal cavity during dialysate exchange, and, therefore, peritonitis is a potential complication of PD. To prevent microbial contamination, the connection tube tip is generally sealed with a protection cap containing povidone-iodine (PVP-I) during the dwelling time. This cap is designed to make direct contact with the tube tip so that microbes attached during dialysate exchange are killed by the next dialysate exchange. However, if excess PVP-I flows into the peritoneal cavity and is absorbed into the body, the complications, including thyroid dysfunction, peritoneal inflammation, and fibrous thickening, can develop. Therefore, in this study, a new manual connection system (Zero System, JMS Co., Ltd., Hiroshima, Japan) for continuous ambulatory peritoneal dialysis was investigated to confirm that the PVP-I solution within the protection cap of the new system would not flow into the fluid passing through the tube. An experiment was also performed to confirm that the microbes on the connector tip become completely nonviable after attachment of the cap for 3 hours. The cap is fitted with a sponge containing a 10% PVP-I solution, the same as for the conventional cap system. However, the system is designed to achieve disinfection without contact, unlike with the conventional system, in which disinfection is achieved by direct contact of the PVP-I-containing sponge with the open end of the attached connector. The test results demonstrated that adequate disinfection with this system can be achieved by the next exchange, while avoiding entry of PVP-I into the peritoneal cavity from the cap. The results suggest that the use of this connection system can avoid adverse reactions arising from the absorption of PVP-I and prevent the onset of peritonitis caused by microbial invasion of the peritoneal cavity. The article discusses a study on cleansing the periurethral area before surgery for gynecology patients. The researchers examined the effects of two cleansing solutions, water and povidone-iodine solution and compared the bacteria development that resulted. The study found that there wasn't a difference in bacteria levels or urinary tract infections when cleansing around the urethral opening with water or povidone iodine solution.

Background: Povidone-iodine (PVP-I) is widely used in antiseptic agents. Immediate allergic reaction to PVP-I preparations is very rare and often overlooked, as it is difficult to diagnose. Polyvinylpyrrolidone (PVP) is thought to play a role in the underlying mechanism. We examined the usefulness of the histamine release test (HRT) for definite diagnosis of PVP allergy. Method: A 9-year-old boy with eosinophilia (1,500/μl) and elevated total IgE (1,376 IU/ml) was suspected clinically of having a PVP allergy, as he had anaphylaxis twice when he was administered a PVP-I solution for impetigo contagiosum. Skin prick tests (SPTs) were performed with a PVP-I solution, PVP (K30), gentamicin sulfate and 2 other medicines containing PVP. HRT was assessed using peripheral blood basophils. Results: SPTs to PVP-I solution, PVP-K30 and other medicines were all negative. Histamine release was observed on stimulation by PVP in the presence of autologous serum, although it was not observed in the absence of autologous serum. Conclusions: This observation was in line with the clinical findings that anaphylaxis had not developed despite the long use of PVP-I solution, but developed only when he received PVP-I solution treatment where basophils could contact PVP-I in the presence of serum, which was probably due to a broken skin and vessel condition. Furthermore, our results suggest the usefulness of HRT in the diagnosis of PVP allergy, and the possibility that negative SPT does not entirely rule out PVP allergy. Copyright © 2008 S. Intra-operative incidental contamination of surgical wounds is not rare. Povidone-iodine solution can be used to disinfect surgical wounds. Although povidone-iodine is a good broad-spectrum disinfecting agent, it has occasionally been reported to have a negative effect on wound healing and bone union. Therefore, its safety in a spinal surgery is unclear. A prospective, single-blinded, randomized study was accordingly conducted to evaluate the safety of povidone-iodine solution in spinal surgeries. Ascertained herein was the effect of wound irrigation with diluted povidone-iodine solution on wound healing, infection rate, fusion status and clinical outcome of spinal surgeries. Materials and methods: From January 2002 to August 2003, 244 consecutive cases undergoing primary instrumented lumbosacral posterolateral fusion due to degenerative spinal disorder with segmental instability had been collected and randomly divided into two groups: the study group (120 cases, 212 fusion levels) and the control group (124 cases, 223 fusion levels). Excluded were those patients with a prior spinal surgery, spinal trauma, malignant tumor, infectious spondylitis, rheumatoid arthritis, ankylosing spondylitis, metabolic bone disease, skeletal immaturity or with an immunosuppressive treatment. In the former group, wounds were irrigated with 0.35% povidone-iodine solution followed by normal saline solution just before the bone-grafting and instrumentation procedure. However, only with normal saline solution in the latter. All the operations were done by the same surgeon with a standard technique. All the patients were treated in the same postoperative fashion as well. Later on, wound healing, infection rate, spinal bone fusion and clinical outcome were evaluated in both groups. Results: A significant improvement of back and leg pain scores, modified Japanese Orthopedic Association function scores (JOA) and ambulatory capacity have been observed in both groups. One hundred and seven patients in the study group and one hundred and nine in the control group achieved solid union. There was no infection in the study group but six deep infections in the control group. Wound dehiscence was noted in one group 1 and two group 2 patients. A subsequent statistical analysis revealed higher infection rate in the control group ( P<0.05), but no significant difference in fusion rate, wound healing, improvement of pain score, function score and ambulatory capacity between the two groups. Conclusion: Diluted povidone-iodine solution can be used safely in spinal surgeries, and it will not influence wound healing, bone union and clinical outcome. [ABSTRACT FROM AUTHOR]

Introduction: Topical application of tranexamic acid to the knee joint before closure in total knee arthroplasty reduces postoperative bleeding without increase in complication. However, it is unknown the effectiveness of topic TXA performed with other topical medications, like povidone-iodine solution., Materials and Methods: One hundred and twenty-five patients were randomized to receive 100mL of povidone-iodine solution (control: group A) or 1.5 (group B) and 3.0 g (group C) of topical TXA in povidone-iodine solution applied into the knee before closure in total knee arthroplasty., Results: The patients in the TXA groups had higher mean postoperative hemoglobin levels (P=0.01 and P=0.03 in groups B and C, respectively) and a reduced postoperative blood loss in the TXA groups (P=0.07 and P=0.09 in groups B and C, respectively). Objective: Few studies have evaluated the risk of bacteremia and infectious complications after endoscopic ultrasonography-guided fine needle aspiration (EUS-FNA). Therefore, we aimed to study the frequency of bacteremia and search for a method to potentially reduce bacterial infection after EUS-FNA. We also investigated the effect of taking proton pump inhibitors (PPIs) before examination on the occurrence of bacteremia., Methods: A total of 28 healthy adult dogs were randomly assigned into three groups: control group, povidone-iodine group and omeprazole group. The dogs in the povidone-iodine group were administered with 0.5% povidone-iodine solution (10 mL) to wash gastrointestinal mucosa, while the dogs in the omeprazole group were fed with 20 mg omeprazole orally twice a day for 3 days before the EUS-FNA procedure. Blood samples were collected for cultures before EUS examination, between EUS and FNA, and 5 min, 15 min and 30 min after FNA., Results: There were 3 true-positive cases of bacteremia in the control group while there was 1 true-positive case of bacteremia in each of the two experiment groups. The differences in the occurrences of bacteremia between the control group and both experiment groups were not statistically significant., Conclusion: There are no statistically significant differences in the frequencies of bacteremia between the two experiment groups and the control group. Therefore, washing of the gastrointestinal mucosa with 0.5% povidone-iodine solution may not reduce the risk of bacterial infection and taking the PPIs does not increase the risk of bacteremia after EUS-FNA.
