Background: The quantification of electromyographic activity using surface electrodes is invaluable for understanding gait disorders in patients with central nervous system lesions. We propose to evaluate a commercially available low-cost system compared to a reference system in participants with stroke-related movement disorders in functional situations. Methods: Three hemiparetic participants performed three functional tasks: two treadmill walks at different speeds and a sit-to-stand test. The vastus lateralis and gastrocnemius medialis muscles were equipped with two EMG sensors. The comparison between the two EMG systems was based on 883 identified cycles. Spearman's correlation coefficients (SCs), linear correlation coefficients (LCCs), and cross-correlation coefficients (CCCs) were calculated. Results: The main results indicate good to very good similarity of the EMG signals collected from the two tested sEMG systems. In the comfortable-walking condition, an SC of 0.894 ± 0.091 and an LCC of 0.909 ± 0.094 were noted. In the fast-walking condition, an SC of 0.918 ± 0.064 and an LCC of 0.935 ± 0.056 were observed. For the 1 min sit-to-stand test, an SC of 0.880 ± 0.058 and an LCC of 0.881 ± 0.065 were noted. Conclusions: This study demonstrates good to very good similarity between the two sEMG systems, enabling the analysis of muscle activity during functional tasks. [ABSTRACT FROM AUTHOR]

Background: Three-dimensional motion analysis using optoelectronic cameras and force platforms is typically used to quantify gait disorders. However, these systems have various limitations, particularly when assessing patients in an ecological environment. To address these limitations, several wearable devices have been developed. However, few studies have reported metrological information regarding their repeatability and sensitivity to change. Methods: A healthy adult performed 6 min walking tests with FeetMe® system insoles under different walking conditions overground and on a treadmill. The standard error of measurement (SEM), the minimum detectable differences (MDDs), and the effect size (ES) were calculated for spatio-temporal parameters, and the ground reaction force was calculated from the 16,000 steps recorded. Results: SEM values were below 3.9% for the ground reaction force and below 6.8% for spatio-temporal parameters. ES values were predominantly high, with 72.9% of cases between overground and treadmill conditions with induced asymmetry, and 64.5% of cases between treadmill conditions with and without induced asymmetry exhibiting an ES greater than 1.2. The minimum detectable differences ranged from 4.5% to 10.7% for ground reaction forces and 2.1% to 18.9% for spatio-temporal parameters. Conclusion: Our study demonstrated that the FeetMe® system is a reliable solution. The practice of the wheelchair rugby is becoming more and more worldwide. However, few biomechanical studies have focused on this sport. The aim of this study was to compare kinematic parameters of wheelchair rugby players, classified as defensive players (LP-D) versus offensive players (HP-O). Twenty-nine wheelchair rugby players (17 LP-D and 12 HP-O) performed a 20-m sprint test. The peak velocities, temporal parameters (propulsion phase time, deceleration phase time, cycle time and cycle frequencies) and asymmetries (the difference in peak velocities between the right and the left wheels) were measured at the acceleration and constant peak velocity phases of the sprint by an inertial measurement unit which was placed on each rear wheel. At the acceleration and constant peak velocity phases, peak velocities and cycle frequencies were higher in HP-O players than LP-D players. The deceleration phase times and the cycle times were higher in LP-D players than HP-O players. However, no significant difference in asymmetry was found between LP-D players and HP-O players. The HP-O players showed superior performance than the LP-D players, but they could be more exposed at risk of injury at their upper limbs than LP-D players. [ABSTRACT FROM AUTHOR]

In 200‐ and 400‐m races, 58% of the total distance to cover is in the curve. In the curve, the sprinting performance is decreased in comparison to the straight. However, the reasons for this decreased performance is not well understood. Thus, the aim of this study was to identify the kinetic parameters underpinning the sprinting performance in the curve in comparison to the straight. Nineteen experienced‐to‐elite curve specialists performed five sprints in the straight and in the curve (radius 41.58 m): 10, 15, 20, 30, and 40 m. The left and the right vertical, anterior–posterior, medial‐lateral, and resultant ground reaction forces (respectively FV$$ {F}_{\mathrm{V}} $$, FA−P$$ {F}_{\mathrm{A}-\mathrm{P}} $$, FM−L$$ {F}_{\mathrm{M}-\mathrm{L}} $$, and FTOT$$ {F}_{\mathrm{TOT}} $$), the associated impulses (respectively IMPV$$ {IMP}_{\mathrm{V}} $$, IMPA−P$$ {IMP}_{\mathrm{A}-\mathrm{P}} $$, IMPM−L$$ {IMP}_{\mathrm{M}-\mathrm{L}} $$, and IMPTOT$$ {IMP}_{\mathrm{TOT}} $$) and the stance times of each side were averaged over each distance. In the curve, the time to cover the 40‐m sprint was longer than in the straight (5.52 ± 0.25 vs. 5.47 ± 0.23 s, respectively). Additionally, the left and the right FA−P$$ {F}_{\mathrm{A}-\mathrm{P}} $$ and IMPA−P$$ {IMP}_{\mathrm{A}-\mathrm{P}} $$ were lower than in the straight while the left and the right FM−L$$ {F}_{\mathrm{M}-\mathrm{L}} $$ increased, meaning that the FM−L$$ {F}_{\mathrm{M}-\mathrm{L}} $$ was more medial. The left FV$$ {F}_{\mathrm{V}} $$ was also lower than in the straight while the left stance times increased to keep the left IMPV$$ {IMP}_{\mathrm{V}} $$ similar to the straight to maintain the subsequent swing time. Overall, the sprinting performance was reduced in the curve due to a reduction in the left and the right FA−P$$ {F}_{\mathrm{A}-\mathrm{P}} $$ and IMPA−P$$ {IMP}_{\mathrm{A}-\mathrm{P}} $$, that were likely attributed to the concomitant increased FM−L$$ {F}_{\mathrm{M}-\mathrm{L}} $$ to adopt a curvilinear motion. Background: For manual wheelchair users, overuse of the upper limbs can cause upper limb musculoskeletal disorders, which can lead to a loss of autonomy. The main objective of this study was to quantify the risk level of musculoskeletal disorders of different slope propulsions in manual wheelchair users using fuzzy logic. Methods: In total, 17 spinal cord injury participants were recruited. Each participant completed six passages on a motorized treadmill, the inclination of which varied between (0° to 4.8°). A motion capture system associated with instrumented wheels of a wheelchair was used. Using a biomechanical model of the upper limb and the fuzzy logic method, an Articular Discomfort Index (ADI) was developed. Results: We observed an increase in articular discomfort during propulsion on a slope with increasing discomfort at the shoulder, elbow and wrist, due to an increase in kinetics. There was a kinetically significant change in the kinetic global ADI (22 to 25%) and no change in the kinematic. The ADI increased from 14 to 36% during slope propulsion for each joint. Conclusion: The quantification of the level of discomfort helps us to highlight the situations with the most high-risk exposures and to identify the parameters responsible for this discomfort. Background: The aim of this study was to compare energy expenditure (EE) predicted by accelerometery (EEAcc) with indirect calorimetry (EEMETA) in individuals with hemiparesis. Methods: Twenty-four participants (12 with stroke and 12 healthy controls) performed a six-minute walk test (6MWT) during which EEMETA was measured using a portable indirect calorimetry system and EEACC was calculated using Bouten's equation (1993) with data from a three-axis accelerometer positioned between L3 and L4. Results: The median EEMETA was 9.85 [8.18;11.89] W·kg−1 in the stroke group and 5.0 [4.56;5.46] W·kg−1 in the control group. The median EEACC was 8.57 [7.86;11.24] W·kg−1 in the control group and 8.2 [7.05;9.56] W·kg−1 in the stroke group. The EEACC and EEMETA were not significantly correlated in either the control (p = 0.8) or the stroke groups (p = 0.06). The Bland–Altman method showed a mean difference of 1.77 ± 3.65 W·kg−1 between the EEACC and EEMETA in the stroke group and −2.08 ± 1.59 W·kg−1 in the controls. Conclusions: The accuracy of the predicted EE, based on the accelerometer and the equations proposed by Bouten et al., was low in individuals with hemiparesis and impaired gait. Purpose: The continued development of microprocessor-based knee prostheses has improved the independence of people with a femoral amputation in many environments. This study aimed to describe the effect of slopes on kinematic joint variables and segmental asymmetry. Methods: Ten individuals with transfemoral amputation fitted with microprocessor-controlled knees performed 5 sessions of treadmill walking at their preferred speed in an immersive virtual environment in 5 incline conditions (Level, 3° and 6° Uphill, and 3° and 6° Downhill). The Human Body Model was used to quantify kinematic joint variables from motion capture system data. The perimeter-to-area method was used to determine the symmetry ratio of the trajectory of the leg segments in the sagittal plane. Results: There was a significant effect of the Uphill conditions on step length and width on the intact side and on all kinematic joint variables on both sides, although the changes differed according to the phase of the gait cycle. The segmental symmetry index was significantly modified in all slope conditions compared to Level. Conclusion: Kinematic joint variables are affected by slopes; the effect was greater for the Uphill than Downhill conditions compared to the Level condition. The perimeter-to-area symmetry ratio differed from the Level condition for all slope conditions. These results indicate that, although microprocessor knees improve the autonomy of prosthesis users, work is required to improve their capacity of adaptation to varied terrain to reduce kinematic asymmetry. Use of a power-assistance wheelchair could reduce the risk of musculoskeletal disorders (MSDs), however, a comprehensive biomechanical evaluation of these systems has not been carried out. This study aimed to evaluate and compare biomechanical UL propulsion variables, and physiological exercise-related variables during the use of a wheelchair with rear drive power assist device (RD-PAD) and a standard manual wheelchair (MW). Twenty-two adults with spinal cord injury were recruited. RD-PAD (SmartDrive system) was fitted to their own MW. An instrumented wheel was used to measure handrim forces, and gas exchange and heart rate were monitored. Participants performed repeated out and back runs for 6 min on a straight outdoor course. Distance covered was significantly greater with the RD-PAD (538 ± 104 m versus 470 ± 124 m). Peak mechanical effort during the propulsion phase was significantly lower with the RD-PAD (p < 0.001). Heart rate, metabolic equivalent of task (MET), tidal volume, minute volume, oxygen consumption, and peak oxygen consumption were all significantly lower with the RD-PAD (p < 0.001). The results showed that use of RD-PAD increased the distance covered by MW users and reduced the energy costs of propulsion. The biomechanical results indirectly suggest that RD-PAD may reduce the risk of MSD. Using the SmartDrive system as propulsion assistance increases the travel autonomy. The SmartDrive system reduces the biomechanical constraints propelling the wheelchair on a slope and low slope. The objective of this exploratory research is to study the impact of holding a tennis racket while propelling a wheelchair on kinetic and temporal parameters in a field-based environment. 13 experienced wheelchair tennis players with disabilities (36.1 ± 8.2 years, 76.8 ± 15.3 kg, 174.8 ± 17.1 cm) classified between 30/8 and first series performed two 20 m sprints in a straight line, on a tennis court: one while holding a tennis racket and the second without a tennis racket. They used their own sports wheelchair. Potential participants were excluded if they had injuries or pain that impaired propulsion. Maximal total force, maximal propulsive moment, rate of rise, maximal power output, push and cycle times and maximal velocity were measured. Sprinting while holding a tennis racket increased the cycle time by 0,051 s and push time by 0,011s. Sprinting while holding a tennis racket decreased the maximal propulsive moment, maximal power output, rate of rise and maximal velocity during propulsion by 6.713 N/m, 151.108 W, 672.500 N/s and 0.429 m/s, respectively. Our results suggest that the biomechanical changes observed associated with racket propulsion are generally in a direction that would be beneficial for the risk of injury. But sprinting holding a racket seems to decrease players propulsion performance. Working on forward accelerations with a tennis racket would be a line of work for coaches. [ABSTRACT FROM AUTHOR]

Plethysmography is an indispensable component of clinical lung function testing. However, lung volume measurement in the supine position using an optoelectronic system requires the placement of reflective markers on the anterior and lateral torso surface. The conventional method computes breath-by-breath changes in the volume between the markers and the bed, which serves as the reference plane. In contrast, the surface method consists of measuring the volume delineated by the surface area of the marker network at the onset and end of inspiration. We compared these 2 methods to spirometry during spontaneous breathing in 11 healthy volunteers and in 14 patients receiving routine visits for neuromuscular disease. Bland-Altman plots showed that agreement with spirometry was better for the surface method that the conventional method. Background and Objective: Individuals who survive a stroke often display considerable gait impairments that occur in part due to inadequate muscle force production. This study aimed to investigate lower limb muscle forces in poststroke patients during walking. Methods: Kinematics, kinetics, and electromyographic (EMG) measurements were performed on nine poststroke and healthy individuals walking at natural speed in a cross-sectional study. Recorded parameters were used in an EMG-driven model to estimate the forces exerted by the muscles around the knee and ankle joints during the stance (braking and propulsion) and swing phases. Results: For hemiparetic patients, in comparison to healthy controls, the paretic side exhibited (i) lower forces generated by plantar-flexors and quadriceps respectively during the braking and propulsion phases, but (ii) higher knee-flexors forces during the propulsion phase. Regarding the non-paretic side, it displayed (i) higher forces generated by knee-flexors and quadriceps (only for the propulsion phase) forces during the stance phase, and (iii) higher plantar-flexors forces during the swing phase, in comparison to controls. Conclusion: Reduced forces exerted by the plantar-flexors and the knee-extensors along with increased force generated by the knee-flexors on the paretic side give possible explanation for hemiparetic gait abnormalities. Because progressive respiratory muscle weakness leads to decreased chest-wall motion with eventual ribcage stiffening, the purpose was to compare vital capacity (VC) and contributions of chest-wall compartments before and after volume recruitment–derecruitment manoeuvres (VRDM) in Duchenne muscular dystrophy (DMD). We studied nine patients with DMD and VC lower than 30% of predicted. VRDM was performed using 15 insufflations–exsufflations of +30 to −30 cmH2O. VC and three-dimensional chest-wall motion were measured, as well as oxygen saturation, transcutaneous partial pressure of carbon dioxide and the rapid shallow breathing index (respiratory rate/tidal volume) before (baseline) and immediately and 1 hour after VRDM. VC increased significantly immediately after VRDM (108% ± 7% of baseline, p = 0.018) but returned to baseline within 1 hour, and the rapid shallow breathing index increased significantly. The non-dominant side systematically increased immediately after VRDM (p = 0.0077), and in the six patients with abnormal breathing asymmetry (difference >10% of VC) at baseline, this asymmetry was corrected immediately and/or 1 hour after VRDM. VRDM improved VC and reduced chest-wall motion asymmetry, but this beneficial effect waned rapidly with respiratory muscle fatigue, suggesting that VRDM may need to be repeated during the day to produce lasting benefits. Understanding locomotor behavior is important to guide rehabilitation after stroke. This study compared lower-limb kinematics during the walking and turning sub-tasks of the Timed Up and Go (TUG) test in stroke patients and healthy subjects. We also determined the parameters which explain TUG sub-task performance time in healthy subjects. Biomechanical parameters were recorded during the TUG in standardized conditions in 25 healthy individuals and 29 patients with chronic stroke using a 3D motion-analysis system. Parameters were compared between groups and a stepwise regression was used to indicate parameters which explained performance time in the healthy subjects. The percentage difference in step length between the last and first steps was calculated, during walking sub-tasks for each group. Speed, cadence, step length, percentage paretic single support phase, percentage non-paretic swing phase, peak hip extension, knee flexion and ankle dorsiflexion were significantly reduced in the Stroke group compared to the Healthy group (p<0.05). In the Healthy group, step length and cadence explained 91% of variance for Go and 86% for Return (walking sub-tasks), and none of the parameters explained the Turn. Previous study in patients with stroke showed that the same parameters explained the variance during the walking sub-tasks and balance-related parameters explained the Turn. The present results showed that step length was differently modulated in each group. Background: The Timed Up and Go (TUG) test is widely used to assess locomotion in patients with stroke and is considered to predict the risk of falls. The analysis of locomotor trajectories during the TUG appears pertinent in stroke patients. The aims of this study were i) to analyze locomotor trajectories in patients with stroke during the walking and turning sub-tasks of the TUG, and to compare them with healthy subjects, ii) to determine whether trajectory parameters provide additional information to that provided by the conventional measure (performance time), iii) to compare the trajectory parameters of fallers and non-fallers with stroke and of patients with right and left hemisphere stroke, and iv) to evaluate correlations between trajectory parameters and Berg Balance Scale scores. Methods: 29 patients with stroke (mean age 54.2±12.2 years, 18 men, 8 fallers) and 25 healthy subjects (mean age 51.6±8.7 years, 11 men) underwent three-dimensional analysis of the TUG. The trajectory of the center of mass was analyzed by calculation of the global trajectory length, Hausdorff distance and Dynamic Time Warping. The parameters were compared with a reference trajectory during the total task and each sub-task (Go, Turn, Return) of the TUG. Results: Values of trajectory parameters were significantly higher for the stroke group during the total TUG and the Go and Turn sub-tasks (p<0.05). Moreover, logistic regression indicated that these parameters better discriminated stroke patients and healthy subjects than the conventional timed performance during the Go sub-task. In addition, fallers were distinguished by higher Dynamic Time Warping during the Go (p<0.05). There were no differences between patients with right and left hemisphere stroke. Discussion and Conclusion: The trajectories of the stroke patients were longer and more deviated during the turn and the preceding phase. Trajectory parameters provided additional information to timed performance of this locomotor task. Focusing rehabilitation programs on lead-up to turn and turning could be relevant for stroke patients since the Turn was related to the balance and the phase preceding the turn seemed to distinguish fallers. Background: The Timed Up and Go (TUG) test is often used to estimate risk of falls. Foot clearance and displacement of the center of mass (COM), which are related to risk of tripping and dynamic stability have never been evaluated during the TUG. Accurate assessment of these parameters using instrumented measurements would provide a comprehensive assessment of risk of falls in hemiparetic patients. The aims of this study were to analyze correlations between TUG performance time and displacement of the COM and foot clearance in patients with stroke-related hemiparesis and healthy subjects during the walking and turning sub-tasks of the TUG and to compare these parameters between fallers and non-fallers. Methods: 29 hemiparetic patients and 25 healthy subjects underwent three-dimensional gait analysis during the TUG test. COM and foot clearance were analyzed during the walking and turning sub-tasks of the TUG. Results: Lateral displacement of the COM was greater and faster during the walking sub-tasks and vertical displacement of the COM was greater during the turn in the patients compared to the healthy subjects (respectively p<0.01 and p<0.05). Paretic foot clearance was greater during walking and displacement of the COM was slower during the turn in the patients (p<0.01). COM displacement and velocity during the turn were correlated with TUG performance in the patients, however, vertical COM displacement was not. These correlations were significant in the healthy subjects. There were no differences between COM parameters or foot clearance in fallers and non-fallers. Discussion and Conclusion: Hemiparetic patients are less stable than healthy subjects, but compensate with a cautious gait to avoid tripping. Background: The timed up and go test (TUG) is a functional test which is increasingly used to evaluate patients with stroke. The outcome measured is usually global TUG performance-time. Assessment of spatiotemporal and kinematic parameters during the Oriented gait and Turn sub-tasks of the TUG would provide a better understanding of the mechanisms underlying patients’ performance and therefore may help to guide rehabilitation. The aim of this study was thus to determine the spatiotemporal and kinematic parameters which were most related to the walking and turning sub-tasks of TUG performance in stroke patients. Methods: 29 stroke patients carried out the TUG test which was recorded using an optoelectronic system in two conditions: spontaneous and standardized condition (standardized foot position and instructed to turn towards the paretic side). They also underwent a clinical assessment. Stepwise regression was used to determine the parameters most related to Oriented gait and Turn sub-tasks. Relationships between explanatory parameters of Oriented gait and Turn performance and clinical scales were evaluated using Spearman correlations. Results: Step length and cadence explained 82% to 95% of the variance for the walking sub-tasks in both conditions. Percentage single support phase and contralateral swing phase (depending on the condition) respectively explained 27% and 56% of the variance during the turning sub-task in the spontaneous and standardized conditions. Discussion and Conclusion: Step length, cadence, percentage of paretic single support phase and non-paretic swing phase, as well as dynamic stability were the main parameters related to TUG performance and they should be targeted in rehabilitation.
