BACKGROUND: Type 2 diabetes mellitus (DM) is one of the important markers for the development of adverse cardiovascular events after carotid endarterectomy (CEE). However, studies on this issue are based on small sample of patients and do not take into account the type of surgery as an additional factor of potentially negative impact on the course of the postoperative period.AIM: Analysis of hospital and long-term results of eversion and classical CEE with plastic surgery of the reconstruction zone with a biological patch in patients with type 2 diabetes and without it.MATERIALS AND METHODS: In this multicenter retrospective study from January 2010 to December 2020. included 5731 patients. Depending on the presence / absence of type 2 diabetes and the type of implemented CEE, 4 groups were formed: group 1 — 12.2% (n = 702) — patients with type 2 diabetes and eversion CEE; Group 2 — 55.0% (n = 3153) patients without type 2 diabetes and eversion CEE; Group 3 — 8.5% (n = 484) patients with type 2 diabetes and classical CEE; Group 4 — 24.3% (n = 1392) patients without type 2 diabetes and classical CEE. Dynamics of the main indicators on oncological incidence for 3 years is reflected and comparison since 2002 is given, the structure and the gender and age analysis of malignant new growths are presented in detail. The index of accumulation of the contingent, indicators of relative mortality and year lethality are calculated.

92 patients with chronic anal fissure were treated depending on the cause of the disease and the magnitude of the basal pressure anal sphincter. It was found that, compared to the traditional method of surgical treatment, the use of “blind” stitch and developed modified suture, evenly healing the wound from the bottom, in combination with chemical sphincterotomy, contributes to significant acceleration of wound healing and causes fewer complications.

The article presents the data of analysis of the organization of medical care for patients with oncological diseases in outpatient depertment. The type and scope of primary health care? Including at home, by an oncologist as well as by physicians, had been defined. The study enrolled 984 patients (av. age 69,69 ± 0.4 years), which were followed up by oncologist in the primary oncological consulting room. 35,1% of patients were men of 68.6 ± 0,7 years, 64.9% - women of 70,2 ± 0.4 years. Patients visited their doctors about 15 times during a year, including 4,5 visits to therapeutist, 3,7 - to surgeon, 3,2 - to neurologist, etc. The reasons for patients references to the doctors were mostly related diseases The oncological patients visited an oncologist 3,1 times a year, the highest number of visits made by patients with the disease of the prostate and breast. The reason of these visits was monitoring the patient's health status, medication and others. An oncologist paid 3,9 visits to patients with III-IV stage of the disease for palliative care. Of the 48.0% of the patients enjoyed preferential drug provision, being in the list of federal benefit recipients. Goal of research: to evaluate efficiency of videothoracoscopy in diagnosis and treatment of patients with injuries and emergency diseases ща chest organs.Material and methods: Study wasbased on treatment results analysis of 2111 patients with injuries and chest organs emergency diseases, who were treated at Republican Research Centre of Emergency Medicine in 2001-2014. Chest trauma made up 1396 (66,1%) victims. There were 477 (22,6%) patients with spontaneous pneumothorax. At the stages of initial diagnosis, the radiologic evaluations, CT investigations and videothoracoscopies were performed. In chest trauma patients the videothoracoscopy underwent in 844 cases, in spontaneous pneu#mothorax this method was employed in 290 patients. Complicated forms of lung echinococcosis were observed in 238 (11,3%) patients and complicated forms of lung echinococcosis were evident in 72 patients.Results. Videothoracoscopy and video-assisted interventions allowed to eliminate lungs and pleura pathology in 1206 (57,1%) patients, whereas the traditional methods were effective only in 905 cases (42,9%).Conclusions. Investigation methods such as multiplanar radioscopy, radiography, chest CT and videothora-coscopy must be included into algorithm of diagnosis and surgical treatment of chest injuries and emergency diseases of chest organs. At chest trauma the videothoracoscopy allows to avoid broad thoracotomy from 9,4% to 4,7% of cases, to reduce the frequency of repeated interventions from 17,4% to 0,5% and diminish a number of early postsurgery complications from 25,4% to 10,9%. Videothoracoscopy of chest traumas allows to reduce frequency of repeated interventions from 19,8 to 1,7%.
