Allergic rhinitis (AR) remains a significant pediatric health problem because of the burden of uncontrolled symptoms on daily activities and on general well being. AIM: to assess the impact of AR on health-related quality of life (HRQL) of children and adolescents using a generic instrument, the Child Health Questionnaire (CHQ - PF50). METHODS: Between January and November 2004, parents or caregivers of 23 children and adolescents with AR without comorbidities and with positive prick tests for at least one air allergen were invited to participate of a cross-sectional study and asked to answer the self-administered CHQ-PF50. The scores were compared to those of healthy children and adolescents. The objective of this research was to evaluate lamb diets with a high proportion of concentrate and various levels of physically effective neutral detergent fibre (NDF) using tropical ingredients. Four diets were formulated: D1 with 16.5% physically effective NDF (peNDF), D2 with 21.4% peNDF, D3 with 23.3% peNDF, and D4 with 24.6% peNDF. Twenty male Santa Inês lambs at four months old with an average liveweight (LW) of 18.6 ± 3.4 kg were fed for 49 days. There were no differences (P >0.05) among the diets in dry matter (DM) intake. Crude protein (CP) intake, relative to either LW or metabolic live weight (LW0.75) or LW, was greater (P =0.05) for D2 and D3 than for D1 or D4. Lower (P =0.05) NDF intake, relative to LW0.75 or LW, was lower for D1 than for the other diets. Selection indexes (SIs) were similar (P >0.05) among diets. Animals that consumed D1 had less (P =0.05) rumination time per bolus. Meaningful variations were not observed in performance and efficiency of finishing lambs that were related to differences in peNDF among the diets. Santa Inês lambs were able to adapt to diets with low levels of peNDF by increasing rumination time as a consequence of increased fibre consumption and greater particle size selection. Objective: The goal of this study was to translate, adapt and validate the items of the Gap-Kalamazoo Communication Skills Assessment Form for use in the Brazilian cultural setting. Methods: The Gap-Kalamazoo Communication Skills Assessment Form was translated into Portuguese by two independent bilingual Brazilian translators and was reconciled by a third bilingual healthcare professional. The translated text was then assessed for content using a modified Delphi technique and adjusted as needed to assure content validity. A total of nine phrases in the completed tool were adjusted. The final tool was then used to assess videotaped simulations as a means of validation. Response process was assessed using exploratory factor analysis and internal structure was assessed via Cronbach’s Alpha (internal consistency) and Intraclass Correlation (test-retest reliability and inter-rater reliability). Results: One hundred and four (104) videotaped communication skills simulations were assessed by 38 subjects (6 staff physicians, 4 faculty physicians, 8 resident physicians, 4 professional actors with experience in simulation, and 16 other allied healthcare professionals). Measures of Internal consistency (Cronbach’s alpha = 0.818) and test-retest reliability (intra-class correlation coefficient = 0.942) were high. Exploratory factor analysis confirmed the uni-dimensionality of the instrument. Conclusions: Our results support the validity and reliability of the Brazilian Gap-Kalamazoo Communication Skills Assessment Form when used among Brazilian medical residents. The Brazilian version of Gap-Kalamazoo Communication Skills Assessment Form was found to be adequate both in the linguistic and technical aspects. The use of this instrument in Brazilian medical education can enhance the assessment of physician-patient-team relationships on an ongoing basis.

Cashew nut meal (CNM) is widely used in tropical countries due to the high protein and energy levels; therefore, it has potential to be an alternative feed supplementation for livestock. Our objective was to evaluate the use of CNM as feed supplement for lambs. Twenty-four lambs were divided into a randomized block design with four treatments, starting with a diet control of Tifton 85 (Cynodon spp.) hay and CNM as a supplement at three different levels representing 6, 12, and 18% of the total mixed ration (TMR) provided. There were evaluated intake (g/day and g/kgBW 0.75 ); the digestibility of DM and nutrients; nitrogen balance; and ingestive behavior. The CP and ether extract (EE) intake (g/day) as well as DM, and organic matter (OM) intake (g/kgBW 0.75 ) were influenced by supplementation with CNM in a positive linear increase (P < 0.05). The digestibility of DM, OM and NDF increased according to the levels of CNM up to 12% and markedly decreased at the higher level (P < 0.05). The EE and CP digestibility raised according to the CNM levels (P < 0.05) and consequently increased the nitrogen retention resulting in a positive nitrogen balance. The protein and energetic characteristics of CNM show that it can be used as an alternative supplementation to low-quality forages for lambs.
