This study aimed at describing the psychological intervention to address the psychological difficulties and issues encountered by the hospitalized post-acute COVID-19 patients in a rehabilitation setting.Patients and Methods: Patients’ demographics, medical diagnosis, and neuro-psychological information were collected from March 2nd to May 12th, 2020. The main psychological issues and intervention strategies were collected.Results: A total of 181 patients were hospitalized during this period. Among them, the 47.5% underwent psychological assessment (N=86; age: 74.58± 13.39; 54.7% females). The most common psychological issues were acute stress disorders (18.6%), anxious and demoralization symptoms (26.7%), depression (10.5%%), and troublesome grief (8.1%). Once recovered from COVID-19, many patients were discharged home (38.4%), some received further rehabilitation in non-COVID-19 wards (41.9%), mostly due to pre-existent diseases (72.2%) rather than to COVID-19 complications (27.8%).Conclusion: A great number of the hospitalized post-acute COVID-19 patients showed psychological issues requiring psychological intervention, the most common were anxiety, demoralization, acute stress, depression, and grief. The proposed psychological treatment for hospitalized COVID-19 patients was conducted in a Cognitive Behavioral framework. In particular, during the COVID-19 pandemic, psychological intervention is an important part of rehabilitation in the post-acute phase of the illness to reduce distress symptoms and improve psychological health.Keywords: psychological intervention, COVID-19 patients, clinical psychology, health psychology, rehabilitation

Abstract

Presence of behavioral and psychological symptoms of dementia (BPSD) is very common in subjects with cognitive impairment, representing an important determinant of disease progression, institutionalization, and worse prognosis. Knowledge of the prevalence and correlates of BPSD in community-living old subjects with cognitive impairment is limited so far, but it is essential for establishing specifically tailored care and cure in such a vulnerable population.With this study, we aimed at investigating, in a large sample of old age subjects with cognitive impairment, BPSD prevalence and correlates including the main demographic, clinical, and socio-environmental characteristics.Data were gathered from the ReGAl project (Rete Geriatrica Alzheimer; Geriatric Network on Alzheimer's disease), a large longitudinal Italian multicentric clinical-based study, promoted by the Italian Society of Gerontology and Geriatrics (SIGG).We evaluated data from 4,157 old-age subjects affected by mild cognitive impairment (MCI) (541; 13%) or dementia (3616; 87%). 85.2% of all the population presented with at least one BPSD. Using a factor analysis, we identified four factors of BPSD: psychotic, affective, maniac, and impulse control behaviors. Logistic regression analyses revealed that among the main demographic, clinical, and socio-environmental aspects considered, only comorbidity was associated with all factors, independently of multiple covariates.Identification of BPSD is crucial in everyday clinical practice and necessary to develop specific interventions and to define appropriate outcomes in their management. BPSD occur in a complex psychopathological context, influenced by several demographic and environmental factors that must be taken into account for a correct diagnosis and treatment.

34. Predictive Mapping of the Variable Response of Permafrost Terrain to Climate Change for Optimal Roadway Routing, Design, and Maintenance Forecasting in Northern Canada.

Abstract

Terrain mapping has been successfully applied to the planning and design of linear infrastructure in North America since the 1940s. A natural extension of terrain mapping in northern regions is the interpretation and characterization of permafrost and ground ice potential. This application has become increasingly important as climate change accelerates permafrost degradation, affecting the development and maintenance of infrastructure. It is, therefore, critical to forecast changes in permafrost terrain to optimize roadway routing, design, and maintenance forecasting. The accuracy and resolution of permafrost interpretations are increased markedly when based on newly available, high-resolution digital elevation models derived from structure from motion photogrammetry and airborne LiDAR surveys, in addition to traditional aerial photography. Using these resources, we have developed a predictive mapping approach to identify permafrost terrain that is susceptible to subsidence or thaw-induced mass movements as a result of climate change or anthropogenic disturbance. We have applied this approach to proposed resource access road corridors in two permafrost settings in northern Canada: the Coffee Gold Project in unglaciated west-central Yukon and a portion of the Slave Geological Province Access Corridor in formerly glaciated Northwest Territories. Terrain mapping representing current conditions was first produced to establish the distribution of distinct terrain units and indicators of excess ground ice. We then derived predictive mapping representing the variable response of distinct permafrost terrain units to projected climate conditions or disturbance. This mapping highlights the most hazardous areas, which are commonly explained by the surficial geological setting, as a basis for optimizing geotechnical field investigations. 35. Permafrost-Related Erosional Effects of Water Level Regulation and Climate Change along the Shorelines of Aishihik and Canyon Lakes, Southwest Yukon.

Abstract

Lake shoreline erosion is caused by wind-generated waves (mechanical erosion) and, in areas of permafrost, can be compounded by thawing ground ice (thermal erosion). In southwest Yukon, erosion patterns along the shorelines of Aishihik and Canyon Lakes reflect the combined effects of climate change and lake level regulation for hydroelectricity, which began in 1975. An investigation characterizing shoreline erosion and aiming to determine the relative contributions of lake level regulation and climate change was undertaken to support an environmental and socio-economic effects assessment associated with hydroelectric facility relicensing. The investigation involved three phases: (i) compilation and review of pertinent topographic, geological, and hydrological information; (ii) helicopter- and ground-based field reconnaissance; and (iii) mapping of shoreline types, relative severity of erosion and recession from former beaches at select sites. Three types of shorelines were identified based on their distinct responses to lake level regulation and climate change. Type 1 shorelines exhibit a moderate slope of coarse-grained sediment and may contain ice-poor permafrost; they are relatively insensitive to climate change and quickly self-stabilize when exposed to higher lake levels. Type 2 shorelines typically exhibit an abruptly steep embankment of ice-rich, fine-grained sediments; they are vulnerable to recession through long-term retrogressive thaw and slumping in response to climate change and lake level regulation. Type 3 shorelines are typically low-relief and composed of fine-grained, ice-rich sediments; they are most sensitive to higher lake levels due to inundation-induced thaw. Erosion along the Type 2 and Type 3 shorelines of Aishihik and Canyon Lakes appears to be driven primarily by climatic effects, despite an anomalous peak in lake level at the onset of regulation that locally initiated and exacerbated erosion. 36. Immediate mandibular rehabilitation with endosseous implants: simultaneous extraction, implant placement, and loading.

Abstract

PURPOSE: This report of a clinical patient series indicates the relative safety and illustrates the procedures involved in the extraction of remaining teeth followed by immediate implant placement and loading with a simple acrylic resin fixed denture. MATERIALS AND METHODS: Ten consecutive patients who selected tooth extraction and implant-supported fixed denture rehabilitation of the mandible were treated using a 1-visit approach for extraction, implant placement, and restoration. Healthy individuals (10 women) were treated under local anesthesia. Fifty-four implants were placed in 10 patients. Five or 6 Astra Tech implants (11 or 13 mm long) were placed into the edentulous parasymphyseal region of the mandible. Four to 6 implants (48 of 54) were immediately loaded by the fabrication of a simple acrylic resin fixed denture. The criterion for loading was clinical judgment of primary stability, ie, the absence of axial or lateral mobility with physical resistance to rotation. Patients were recalled at 1, 3, and 12 weeks. At 12 weeks, impressions were made for the fabrication of a screw-retained fixed denture. The fixed dentures were completed using conventional fabrication and prosthetic techniques. RESULTS: After a period of 6 to 18 months, all 54 implants had survived and were considered 100% successful by independent testing of mobility and radiographic evidence of osseointegration. There were no surgical complications. Fracture and debonding of the acrylic resin provisional denture occurred for 1 patient during the first 12 weeks of treatment. DISCUSSION: Advantages to extraction with simultaneous replacement include the maintenance of vertical dimension, elimination of reline procedures and interim denture therapy, and potential improvement of soft tissue healing.
