Abstract Nano round polycrystalline adsorbent (NRPA) of chicken bones origin was utilize as effective adsorbent in Congo red dye removal via aqueous media. The NRPA adsorbent was prepared via thermal decomposition and its structure was investigated with the aids of Transmission Electron Microscopy, Fourier Infrared Spectroscopy (FT-IR), Scanning Electron Microscopy, Energy Dispersive X-ray Analysis (EDX), and X-ray Diffractometer (XRD). A monophasic apatite phase was confirmed from XRD investigation, while functional groups analysis showed that NRPA possessed CO3 2− , PO4 3− and OH− absorption bands. The maximum adsorption capacities derived from Langmuir isotherm is 98.216 mg g−1. From the combined values of n from Freundlich and separation factor (RL) of Langmuir models, the adsorption of CR by NRPA is favourable. Thermodynamic values of 5.280 kJ mol−1 and 16.403 kJ mol−1 K−1 were found for ΔH° and ΔS° respectively. The entire values of ΔG° which ranges from − 35.248 to − 459.68 kJ mol−1 were all negative at different temperatures. Thus, nano polycrystalline adsorbent of chicken bone origin can serve as excellent adsorbent in Congo red dye removal from waste water.

Gastric cancer (GC) is a prevalent form of malignancy characterized by significant heterogeneity. The development of a specific prediction model is of utmost importance to improve therapy alternatives. The presence of H. pylori can elicit pyroptosis, a notable carcinogenic process. Furthermore, the administration of chemotherapeutic drugs is often employed as a therapeutic approach to addressing this condition. In the present investigation, it was observed that there were variations in the production of 17 pyroptosis-regulating proteins between stomach tissue with tumor development and GC cells. The predictive relevance of each gene associated with pyroptosis was assessed using the cohort from the cancer genome atlas (TCGA). Key Clinical Message Poncet's disease is an acute onset reactive polyarthritis associated with tuberculosis infection. Although uncommon, the diagnosis should be considered among patients presenting with symmetrical polyarthritis in tuberculosis‐endemic regions. Abstract This is a case report of Poncet's disease presenting as bilateral knee and wrist pain associated with swelling. Joint x‐rays and immunological assays were normal. A chest x‐ray and Gene‐Xpert diagnosed tuberculosis. A complete resolution of symptoms was attained following the completion of antituberculous therapy regimen.

Abstract Background: Hemoglobin A1C (HbA1c) levels are known to be linked to a higher risk of stroke. However, no research data is available on the impact of HbA1C on the severity of acute ischemic stroke in Sudan. Methods: This study is a descriptive, cross-sectional hospital-based study of 40 cases of acute ischemic stroke. Ischemic stroke was confirmed using computed tomography (CT) scan at admission; all subjects' blood HbA1C levels were also measured. Participants were divided into two subgroups based on HbA1C at admission, good glycemic control (GGC) ( < 7 HbA1C) and poor glycemic control (PGC) ( > 7 HbA1C), and neurological impairment was assessed using the National Institutes of Health Stroke Scale (NIHSS). Results: The age distribution of the participants was 45-85 years, with an average age of 63.5 ± 9.2 years with the highest frequency (67.5%) in the age group of 55-75 years. PGC had a statistically significant high HbA1C value of 8.9 ± 1.3 (P=0.000), when compared to GGC subgroups 5.1 ± 0. The association between stroke severity and HbA1C levels on admission in this study was statistically significant (P value=0.005), on admission (78.6%) PGC had moderate to severe stroke ( > 18.8 NIHSS) versus (33.3%) that of the GGC ( > 10.4NIHSS).The frequency of elevated HbA1C levels in patients with acute ischemic stroke was 70% in this study. Conclusion: PGC was shown to be linked to the occurrence of stroke and to its severity.

The presented research applied the modified Halloysite nanoclay to boost the adsorption efficacy of heavy metals from the water. To improve As (III) adsorption effectiveness from water, the study assessed the characteristics of the prepared materials and improved the experimental conditions. The study was optimized the experimental condition with a dosage of 1 g/L, contact time of 90 min, the solution pH of 8, and the initial concentration of 5 ppm of As (III). The optimization was performed in distilled water and later the experiments were conducted in the real polluted water. The modified Halloysite nanoclay’s physical characteristics were investigated using techniques like X-ray diffraction, scanning and transmission electron microscopy, Fourier transform infrared spectroscopy, and surface area analysis. The experimental result shows the adsorption efficiency of 82.4 % of As (III) at the optimized condition during the usage of modified Halloysite nanoclay. To create a suitable mathematical model for a better description of the interactions between pollutants and solid adsorbents, it is helpful to analyze the process kinetically. The removal process of As (III) was studied kinetically and the observation shows the pseudo-second order kinetics.

The Mehri Language is an endangered language spoken in eastern Yemen, a sub-group of the Semitic language family, and a Southern Arabic language. The syntax of Mehri wh-questions has not been explored within minimalism; hence, there is a morpho-syntactic need to provide a modern analysis of wh-questions in order to show how the interrogative structures can be derived. This study aims to examine the syntax of the wh-question movement in Mehri's unaccusative/ergative and unergative structures and answer the following questions within Chomsky's (2000 and 2008) Phase-based Theory: (i) Does the Mehri language allow fronting of wh-phrases to [Spec-CP]? And (ii) how can wh-movement in Mehri unaccusative and (un)ergative structures be accounted for? This work presents a novel analysis of wh-question movement in unaccusative/ergative and unergative structures in Mehri; it demonstrates that the source head C triggers the movement of wh-adjunct and wh-subject phrases. In wh-adjunct extraction, two strategies are employed: overt wh-fronting and wh-in-situ; when the head Foc inherits an edge feature from C, wh-adjunct overtly undergoes movement from its original position within v*P to the left peripheries of [Spec-FocP] and subsequently to [Spec-CP]. When the lexical wh-adjunct remains within v*P, its question features covertly move to [Spec-CP], because the head Foc does not inherit an edge feature from C. In wh-subject extraction, the wh-subject overly undergoes movement to [Spec-CP] because C obligatorily inherits the edge feature to the head Top, which triggers movement of the illogical subject in unaccusative/ergative structures and the logical external specifier in unergative structures to [Spec-CP]. Moreover, Mehri obeys the Phase Impenetrability Condition of Chomsky, where wh-subject and wh-adjunct phrases must pass through certain phases until [Spec-CP].

Although the incidence of chronic mesenteric ischemia (CMI) is uncommon, it could become life threatening. With improvements in diagnostic imaging and endovascular therapy over the past few decades, the treatment of CMI has evolved. Even though endovascular revascularization for CMI is prominent, it is unclear if the early advantages outweigh the long-term patency rates. This study conducts a systematic review and meta-analysis to present comprehensive insights into the validity of endovascular revascularization versus open revascularization as effective therapeutic modalities. Studies up to 2022 were searched in MEDLINE, Cochrane Database of Systematic Reviews, Scopus, and Embase. Additionally, references from earlier studies and review papers were manually searched for additional relevant reports. At least two authors examined all the results, making sure they fit the inclusion and exclusion criteria. In the event of disagreement, the authors were able to reach an understanding. Open revascularization is superior to endovascular approaches in maintaining vessel patency and reducing symptoms over the long term. Perioperative problems are more common in patients undergoing open procedures. Each patient's anatomy and physiology call for a different revascularization approach to be applied in treating this condition.

Abstract Epilepsia partialis continua is a rare epileptic syndrome presenting with simple, partial, repetitive motor seizures. It can be a result of structural central nervous system lesion or due to metabolic causes. We report a case of a 65‐year‐old male patient diagnosed with EPC secondary to diabetic ketoacidosis.

Abstract Background Various tumor characteristics might lead to inaccurate local MRI-defined stage of rectal cancer and the purpose of this study was to explore the clinicopathological factors that impact on the precision pre-treatment MRI-defined stage of rectal cancer. Methods A retrospectively analysis was conducted in non-metastatic rectal cancer patients who received radical tumor resection without neoadjuvant treatment during 2007–2015 in the Sixth Affiliated Hospital of Sun Yat-sen University. Clinical T stage and N stage defined by pelvic enhanced MRI and pathological stage were compared and patients were subdivided into accurate-staging, over-staging and under-staging subgroups. Logistic regressions were used to explore risk factors for over-staging or under-staging. Results Five hundred fifty-one cases of patients were collected. Among them, 109 cases (19.4%) of patients were over-T-staged and 50 cases (8.9%) were under-T-staged, while 78 cases (13.9%) were over-N-staged and 75 cases (13.3%) were under-N-staged. The distribution pattern of nitrate in Al-Arbaeen lagoon is a matter of concern due to its elevated levels, which pose significant risks to the environment and human health. This particular research aims to address the issue by exploring the remediation potential of a photocatalyst composed of multiwalled carbon nanotube-modified titanium dioxide (CNT/TiO2). The study focuses on assessing the efficiency of this photocatalytic process in degrading nitrate and reducing its concentration in water samples collected from Al-Arbaeen lagoon. By modifying TiO2 with carbon, the photocatalyst's activity is enhanced, enabling efficient degradation of nitrate under sunlight and expanding its absorption range to visible light. A batch reactor equipped with the carbon-modified TiO2 catalyst is employed, and changes in nitrate concentration over time are analyzed. The carbon-modified TiO2 photocatalyst effectively decreases the concentration of nitrate in the water samples. As a result, the photocatalytic process facilitates the conversion of nitrate into harmless byproducts, ultimately enhancing the overall water quality. The researchers conducted various characterizations of the prepared photocatalyst using SEM, EDX, XRD, FTIR, and UV–Vis spectrophotometer. Various factors, including the amount of catalyst used, the initial concentration of nitrate, and the duration of the reaction, were observed to affect the efficiency of degradation. The study also investigates the kinetics and mechanism of the photocatalytic process, shedding light on the degradation pathway of nitrate and the role of carbon modification in improving the photocatalyst's performance. Introduction As of 2015, as part of the implementation of the Welsh Government primary care plan and primary care clusters, the Welsh Government has encouraged non-medical healthcare professionals working in primary care to train as independent prescribers (IPs).Objectives This research aimed to identify the number of NMIPs in primary care in Wales and describe their prescribing trend of items between 2011 and 2018, in order to compare their prescribing pattern before and after the implementation of primary care clusters for Wales.Design Retrospective secondary data analysis and interrupted time series analysis in order to compare prescribing by non-medical independent prescribers (NMIPs) preimplementation and postimplementation of primary care clusters across Wales.Results Over the study period, 600 NMIPs (nurses n=474 and pharmacists n=104) had prescribed at least one item. The number of nurse IPs increased by 108% and pharmacists by 325% (pharmacists had the largest increase between July 2015 and March 2018). IntroductionDiarrhoea is a frequent concern in the intensive care unit (ICU) and is associated with prolonged mechanical ventilation, increased length of ICU stay, skin breakdown and renal dysfunction. However, its prevalence, aetiology and prognosis in the critically ill have been poorly studied. The primary objectives of this study are to determine the incidence, risk factors and consequences of diarrhoea in critically ill adults. The secondary objectives are to estimate the incidence of Clostridium difficile-associated diarrhoea (CDAD) in ICU patients and to validate the Bristol Stool Chart and Bliss Stool Classification System characterising bowel movements in the ICU. Our primary outcome is the incidence of diarrhoea . Our secondary outcomes include: CDAD, ICU and hospital mortality and ICU and hospital length of stay.Methods and analysisThis international prospective cohort study will enrol patients over 10 weeks in 12 ICUs in Canada, the USA, Poland and Saudi Arabia. We will include all patients 18 years of age and older who are admitted to the ICU for at least 24 hours and follow them daily until ICU discharge. Our primary outcome is the incidence of diarrhoea based on the WHO definition, during the ICU stay. Our secondary outcomes include: CDAD, ICU and hospital mortality and ICU and hospital length of stay. We will use logistic regression to identify factors associated with diarrhoea (as defined using WHO criteria) and the kappa statistic to measure agreement on diarrhoea rates between the WHO definition and the Bristol Stool Chart and Bliss Stool Classification System.Ethics and disseminationThe protocol has been approved by the research ethics board of all participating centres. The diarrhoea interventions, consequences and epidemiology in the intensive care unit (DICE-ICU) study will generate evidence about diarrhoea and its frequency, predisposing factors and consequences, to inform critical care practice and future research.Lay summaryDiarrhoea is a frequent clinical problem for hospitalised patients including those who are critically ill in the ICU. The artificial neural network (ANN) technique, one of the most sophisticated and advanced technologies of the present day, is leveraged across various fields due to its precision and superior performance. One such field that significantly relies on ANN is the detection of errors and classification of signals, especially in non-linear dynamic systems such as robotic systems. Mechanical and electrical faults at the robot's joints often manifest as disturbances, causing vibration at the robot arm's tip. This paper explores the offline detection and diagnosis of these tip acceleration values using eight levels of discrete wavelet transform (DWT). The output from the DWT serves as the input for the signal classification process. A comparative analysis between two neural network types, multilayer perceptron and the Jordan recurrent neural network, is undertaken to achieve signal classification. Signals from various non-healthy scenarios, along with the healthy case, are utilized to train the neural networks. The errors under consideration occur either individually at the robot's joints or at different joints simultaneously. The results indicate that, when applied to the same set of offline recorded data, the multi-layer perceptron neural network demonstrates 100% accuracy, while the Jordan recurrent neural network achieves 97% accuracy in data classification. Thus, for error classification in the robot arm, the multi-layer perceptron neural network outperforms the Jordan neural network. [ABSTRACT FROM AUTHOR]

Sleeve gastrectomy has become one of the most common medical therapies for patients who complain of obesity and comorbidities associated with it. However, numerous reports have shown an increase in body weight after sleeve gastrectomy; therefore, this area requires further investigation. Evidence has shown that there are changes in appetite hormones post sleeve gastrectomy including leptin, ghrelin, cholecystokinin, glucagon-like-peptide 1, peptide tyrosine-tyrosine, and insulin. The main objectives of the current study are 1) to assess the changes in appetite before and after meal consumption at baseline, 3 months, 6 months, and 12 months after sleeve gastrectomy, 2) to assess the interrelationship between dietary intake, body composition, energy expenditure, and physical activity prior and post sleeve gastrectomy. Patients (n=150) will be recruited from King Abdullah bin Abdulaziz University Hospital, Saudi Arabia. A general physical activity assessment, basal measurements including body height, weight, waist circumference, resting metabolic rate measurement, body composition assessment using bioelectrical impedance, dual-energy X-ray absorptiometry, and 24 h food records will be conducted during all the sessions. Blood samples and a visual analog scale will be obtained to measure appetite at four time points prior and an hour following meal consumption. The potential behavior of Cr (VI) in the water makes the significance of the current research. The addition of multi-walled carbon nanotubes (MWCNT) into the TiO2 alters its properties and will enhance the photoactivity for the reduction of Cr (VI). The photocatalyst was synthesized using sol–gel method and characterized using SEM, EDS, XRD, UV–Vis, and FTIR. The UV–Vis spectra of CNT/TiO2 are clearly reflecting the absorption of light in the visible region and supporting the better efficiency compared to bare TiO2. The XRD spectra of CNT-TiO2 show the monophase of the anatase structure. The presence of C modification was further confirmed by EDS. The surface characterization and chemical composition were confirmed with SEM and FTIR. The maximum efficiency of Cr (VI) to Cr (III) was achieved by the optimized conditions of pH 3, initial dosage of 1 g/L. The removal efficiency of Cr (VI) was achieved within 90 min of natural sunlight illumination. The kinetic studies of Cr (VI) follow the pseudo-first-order kinetics. Except for TiO2, both CNTs and CNT/TiO2 showed higher removal efficiency under natural sunlight illumination when comparing removal efficiency between UV and natural sunlight. Formic acid was used in all of the experiments, and it worked well as a hole scavenger. Due to high heat generation within closely packed smart electronic devices, some efficient thermal management systems are required for their reliable performance, avoid overheating, long lifecycle and safety. In this study, a novel thermal management system based on forced air cooling having three airflow configurations is developed to explore the thermal characteristics of each configuration. A customized cavity is designed to have provision for three airflow configurations (axial, cross, and reverse flow) and temperature profiles are investigated within the cavity for each configuration. The experiments are performed at three heat generation rates, i.e., 10 W, 20 W, and 30 W to analyze the cooling effectiveness at a variable heat generation rate. It was observed that the maximum temperature within the setup increases with the increase in heat generation rate. In axial flow air configuration, cavity temperature has been reduced remarkably by 69 and 82.4% at 10 W and 30 W, respectively. Second to axial flow, cross flow configuration performs better than reverse flow and an overall 65.7~78.6% temperature drop is obtained compared with enclosed cavity from 10 W to 30 W, respectively. Furthermore, a similar cooling rate trend in the cavity is obtained for an increased heat generation rate in the cavity. [ABSTRACT FROM AUTHOR]

Rigid environmental regulations and the energy crisis are pursuing researchers to investigate eco-friendly and more energy-generating alternatives to conventional resources. Biomass has the required potential and is drawing keen attention, especially in the field of energy. This work includes the thermal decomposition of almond shells under pyrolysis (N2) conditions in a thermogravimetric analyzer (TGA) and estimated pyrolysis product gas composition and thermo-kinetic parameters. Pyrolysis experiments were conducted at heating rates of 10°C/min, 20°C/min, and 30°C/min. Using 11 reaction models, the Coats–Redfern method was applied to determine kinetic and thermodynamic parameters. The pyrolysis product gas composition was estimated using correlations based on elemental composition and pyrolysis temperature. Thermal decomposition mainly occurred in the temperature range 150–550°C with a peak temperature of 292°C, 310°C, and 321°C for the three heating rates, i.e. 10°C/min, 20°C/min, and 30°C/min, respectively, and 99.40% total weight loss. High regression coefficients (R2) in the range of 0.90–0.99 were obtained for the Coats–Redfern models. High mean relative reactivity (0.04%min−1°C−1) and low activation energies showed a high tendency to degrade thermally. The overall activation energy Eα range for almond shell was 2–84 kJ/mol at all heating rates. The pyrolysis gas composition showed good agreement with the experimental results of similar biomass. Background: Stay-at-home orders in response to the COVID-19 pandemic have forced abrupt changes to daily routines and lifestyles. This study assessed changes in mental health and social activity during the confinement period, in response to the global pandemic. Methods: This study was a face-to-face interview survey targeting adults in Duhok city, Kurdistan Region of Iraq. The questionnaire was distributed from October 2021 to February 2022, and collected information on mental well-being, mood, and social activity participation. All questions were presented as "before" and "during" the COVID-19 pandemic. Results: In total, 407 adult participants have included: 207 females and 200 males. Overall, during the pandemic, mental well-being score was reduced by 13.4% (particularly for the employed), and social participation by 11.1% (particularly for the older age group). Around the same time, the bad mood and feelings score increased by 51.3%. These changes are expected and have been reporworldwidewide. Conclusion: The COVID-19 pandemic produced significant negative lifestyle effects well beyond the virus itself, and confining people has led to a decline in mental health and social activities. People need support to counteract the effects of confinement on mental well-being, e.g., by providing basic life needs and enhancing at-home communication and physical activity. Objectives: This research aimed to compare the incidence and severity of acute appendicitis (AA) before and during the COVID-19 pandemic. Materials and Methods: Two southern Saudi Arabian high-volume hospitals hosted this retrospective investigation. The COVID pandemic period (March-June 2020) and the pre-COVID period were identified (March-June 2019). The frequency, clinical manifestation, and severity of AA were compared for patients who underwent appendectomies during these periods. Results: In the study hospitals, total admissions decreased by 52.3% (95% confidence interval [CI]: 51.1%-53.3%) from 6231 patients during the pre-COVID period to 3261 patients during the COVID period. In the 2019 study period, 107 cases of AA were surgically treated, compared to 61 cases in 2020. The observed reduction rate was 57.1% (95% CI: 47.5%-66.8%). During the COVID period, the duration from the onset of symptoms to hospital admission and hospital admission to surgery was significantly shorter for patients. There were no significant differences between the COVID and pre-COVID periods in terms of the clinical picture or severity of AA. Conclusions: During the COVID era, there was a significant decrease in the number of AA patients hospitalized, which may indicate the efficacy of home treatment for mild cases. The successful measures taken by the Saudi Arabian government during the curfew, along with the optimal utilization of resources and logistics, resulted in the early presentation and management of AA, yielding results comparable to the pre-COVID era. Participants were divided into two subgroups based on HbA1C at admission, good glycemic control (GGC) (<7 HbA1C) and poor glycemic control (PGC) (>7 HbA1C), and neurological impairment was assessed using the National Institutes of Health Stroke Scale (NIHSS). Results: The age distribution of the participants was 45-85 years, with an average age of 63.5±9.2 years with the highest frequency (67.5%) in the age group of 55-75 years. PGC had a statistically significant high HbA1C value of 8.9±1.3 (P=0.000), when compared to GGC subgroups 5.1±0. The association between stroke severity and HbA1C levels on admission in this study was statistically significant (P value=0.005), on admission (78.6%) PGC had moderate to severe stroke (> 18.8 NIHSS) versus (33.3%) that of the GGC (>10.4NIHSS). The frequency of elevated HbA1C levels in patients with acute ischemic stroke was 70% in this study. Conclusion: PGC was shown to be linked to the occurrence of stroke and to its severity.
