Objectives:To investigate choriocapillaris flow voids (FV) with a new optical coherence tomography angiography (OCTA) image processing strategy that can eliminate artifacts caused by vitreous opacities, sub-retinal pigment epithelium fluid and deposits, and subretinal fluid (SRF) by thresholding the en-face OCT image of the outer retina.Materials and Methods:We retrospectively reviewed medical records of patients with drusen and patients with active central serous chorioretinopathy (CSC). Objectives:To evaluate the outcomes of 23-gauge pars plana vitrectomy (PPV) under air compared with standard PPV for idiopathic macular hole (MH).Materials and Methods:In this prospective, comparative, interventional case series, 42 eyes of 42 patients with idiopathic MH were enrolled. Twenty-one eyes had vitrectomy with an air-infused technique and 21 eyes underwent vitrectomy with a traditional balanced salt solution-infused technique as a control group. Objectives:To compare the outcomes and complications of dislocated intraocular lens (IOL) extraction and secondary iris-claw IOL (ICIOL) implantation in vitrectomized and non-vitrectomized eyes.Materials and Methods:This retrospective study included 19 vitrectomized eyes and 11 non-vitrectomized eyes that underwent dislocated IOL extraction and secondary anterior chamber ICIOL implantation between June 2014 and September 2017 and had at least one year of follow-up.Results:There were no significant differences between the groups in terms of demographic data, operative time, baseline anatomic and functional measurements, or postoperative changes in these measurements (all p>0.05). Postoperative best corrected visual acuity was significantly higher than preoperative values in both groups (both p0.05). In both groups, endothelial cell density was significantly lower at postoperative 1 year compared to preoperative measurements. There was no significant difference between groups regarding endothelial cell loss (p=0.49). One vitrectomized eye had corneal decompensation. Other complications included hyphema, transient increase of intraocular pressure, secondary glaucoma, pupillary irregularity, and dislocation of ICIOL. Mean operative time was 26.4±5.9 minutes.Conclusion:Dislocated IOL extraction and secondary anterior chamber ICIOL implantation is a safe treatment option in both vitrectomized and non-vitrectomized eyes.

Objectives:To assess outcomes of a risk-based algorithm-guided treatment protocol for neovascular age-related macular degeneration.Materials and Methods:Two hundred and ten eyes of 184 patients managed with anti-vascular endothelial growth factor (anti-VEGF) agents according to a protocol consisting of one of three initial regimens depending on risk with at least 2 years of follow-up were retrospectively evaluated. The “short-term monthly injections” protocol was used for low-risk patients with low-risk lesions and good fellow-eye vision. Patients with low-risk lesions but without good fellow-eye vision, or those with good fellow-eye vision and high-risk lesions were managed according to the “short-term treat-and-extend (TREX)” protocol. The “extended TREX” protocol was for patients with high-risk lesions and low fellow-eye visual acuity.Results:The initial treatment plan consisted of short-term monthly injections in 62 eyes (30%), the short-term TREX regimen in 120 eyes (57%), and the extended TREX regimen in 28 eyes (13%). Overall, 63% of cases met the criteria for cessation of treatment. Approximately 58% of these cases had recurrence, at a mean of 13 months. The mean change in VA from baseline was +9.0 letters at 12 months and +8.0 letters at 24 months. VA improved during a mean follow-up of 46.8±22 months, with a mean of 3.4±1.6 anti-VEGF injections per year.Conclusion:The risk-based algorithm-guided treatment protocol yielded visual outcomes similar to those of the common alternative treatment and monitoring regimens, with a dramatically reduced number of injections, as required by the individual lesion and vision in the fellow eye.

Purpose: To describe the effect of rituximab on full-field electroretinography (ERG) in a patient with nonparaneoplastic autoimmune retinopathy (npAIR). Observations: A 58-year-old male patient with visual complaints, positive anti-retinal antibodies and negative work-up for cancer was diagnosed with npAIR. Visual acuity and ancillary tests were normal except abnormal ERG in both eyes. The patient was given one course of rituximab 375 mg/m2/week for 4 weeks and cyclophosphamide 1 gr/m2/month for 6 months. A second course of rituximab was necessary as autoantibody titers showed no change and as new antibodies were noted after treatment with rituximab and cyclophosphamide. Electroretinography was repeated after the first course of rituximab, after cyclophosphamide, and the second course of rituximab therapy. Conclusions and importance: Rituximab therapy led to marked improvement in full-field ERG readings and regression of symptoms was reported by the patient after rituximab infusions. The effect of rituximab in npAIR was objectively demonstrated with ERG.

Laser devices are widely used for medical, military, industrial and entertainment purposes. This extensive and unregulated use of lasers can cause a variety of maculopathies that can result in permanent vision loss. Uncontrolled and inappropriate use of laser instruments should be prevented with strict rules. We strongly emphasize the importance of changing the general misperception that lasers are safe to use for entertainment purposes. In this study we aim to report the clinical features of three patients with a history of maculopathy caused by exposure to laser light in an entertainment venue.

Occult macular dystrophy is an inherited macular dystrophy characterized by a progressive decline of bilateral visual acuity with normal fundus appearance, fluorescein angiogram and full-field electroretinogram. This case report presents a 20-year-old female patient with bilateral progressive decline of visual acuity for six years. Her visual acuity was 3-4/10 in both eyes. Anterior segment and fundus examination, fluorescein angiogram and full-field electroretinogram were normal. She could read all Ishihara pseudoisochromatic plates. Fundus autofluorescence imaging was normal. There was a mild central hyporeflectance on fundus infrared reflectance imaging in both eyes. Reduced foveal thickness and alterations of the photoreceptor inner and outer segment junction were observed by optical coherence tomography in both eyes. Central scotoma was also found by microperimetry and reduced central response was revealed by multifocal electroretinogram in both eyes. These findings are consistent with the clinical characteristics of occult macular dystrophy

Objectives: To report outcomes of photodynamic therapy (PDT) and combined therapy with PDT and intravitreal bevacizumab (IVB) in patients with polypoidal choroidal vasculopathy (PCV). Materials and Methods: Thirty-four eyes of 31 patients with subfoveal PCV were evaluated. Nine eyes were treated with PDT and 25 eyes treated with combined therapy of PDT and IVB. All eyes had a follow-up period of at least 12 months. In this retrospective study the demographic features, best corrected visual acuity, fundus color photography, optical coherence tomography, fluorescein angiography and indocyanine green angiography of the 34 eyes were evaluated. Results: Visual acuity improved but did not change significantly in the patients treated with PDT and combined PDT+IVB therapy (p=0.149; p=0.087). Although the mean central foveal thickness decreased in both groups, there was no statistically significant difference between groups (p=0.98). The polypoidal lesions regressed in 6 (66.7%) of 9 eyes in the PDT monotherapy group and 16 (64%) of 25 eyes in the PDT+IVB combined therapy group. Conclusion: Both PDT and a combined therapy of PDT and IVB yielded successful outcomes in patients with PCV.

Serous macular detachment has only recently been recognized to occur in a significant number of eyes with macular pathology including diabetic retinopathy, retinal vein occlusion, Behçet disease, Irvine-Gass syndrome and pars planitis. These serous retinal detachments associated with retinal vascular leakage are not suspected clinically or angiographically but can be diagnosed with optical coherence tomography (OCT) beneath the edematous neurosensory retina. The detection of shallow foveal detachment may be helpful in better understanding the pathogenesis of these disorders. In addition, the detection of serous macular detachment may also help to better guide and assess the results of therapy in the future. (Turk J Oph thal mol 2012; 42: 146-9)

Visual acuity is still considered the gold standard in clinical practice of vision testing, but it does not entirely reflect functional vision. Therefore, we need other functional tests to perform more detailed assessment of macular function in retinal diseases. A test of visual function which may help to better understand the characteristics of visual loss in retinal diseases could be microperimetry. With microperimetry, retinal fixation and macular sensitivity may be accurately tested, with strict correspondence between visual parameters and macular morphology. In this paper, we review the literature regarding the results of microperimetry. Results on known diseases and reproducibility of the technique were published, but a lot of research was also performed on the combination of different examination methods. A 35-year-old male patient with blurred vision in both eyes since childhood has been diagnosed with foveaschisis at the age of 14. The patient had 5 brothers, all reported to have similar retinal problems. His best-corrected visual acuity was 4/10 in both eyes. Fundus examination showed retinoschisis in the lower temporal quadrants in both eyes, pigmented laser scars in the same area in the right eye, and a dull foveal reflex was observed in both eyes. The foveal sections examined with high-resolution optical coherence tomography (OCT) demonstrated atrophy in the fovea bilaterally. OCT sections did not show any accumulation of intraretinal or subretinal fluid or any cystic cavities; yet, thinning was observed especially on the superficial retinal layers. Structures under the outer plexiform layer were observed as normal. Even though these OCT findings are not merely diagnostic as in early-stage familial foveal retinoschisis, there is no doubt that they still facilitate the diagnosis. A 32-year-old woman with benign concentric annular macular dystrophy (BCAMD) was examined with multifocal electroretinography (mfERG) and microperimetry. The mfERG demonstrated reduced focal responses in the area corresponding to the scotoma shown by microperimetry. The correspondence between mfERG and microperimetry in a patient with BCAMD shows that inner retinal layers, which include photoreceptors, are affected and these changes lead to visual field defects. Purpose: To evaluate the effect of intravitreal triamcinolone acetonide (IVTA) on retinal sensitivity in cases of macular edema(ME) secondary to branch retinal vein occlusion (BRVO). Materials and Methods: Total of 14 eyes of 14 cases of BRVO were included in this prospective study. In each eye, at baseline and 1, 3, and 6 months after IVTA injection, logMAR visual acuity, central 4° retinal sensitivity by MP-1 microperimetry, and optical coherence tomography foveal thickness were assessed. Results: Cases ages ranged from 60 to 79 years (mean 68 ± 8 years). At 1, 3, and 6 months, the logMAR visual acuity had increased from 0.71 ± 0.21 to 0.42 ± 0.21, 0.46 ± 0.30, and 0.46 ± 0.27; the mean foveal thickness had decreased from 540 ± 88 µm to 254 ± 51 µm, 288 ± 84 µm, and 280 ± 91 µm; and the mean retinal sensitivity had increased from 4.7 ± 2.5 dB to 7.9 ± 2.7 dB, 8.2 ± 3.6 dB, and 8.3 ± 4.6 dB, respectively. Conclusion: In eyes with ME secondary to BRVO, IVTA injections result in a significant increase in not only the visual acuity but also the central 4° retinal sensitivity in 6 months follow-up. Purpose: The purpose of this study was to evaluate the effect of intravitreal triamcinolone acetonide on macular function in cases of macular edema because of central retinal vein occlusion., Methods: Twelve eyes of 12 patients with central retinal vein occlusion were included in this study. In each eye, at baseline and 1, 3, and 6 months after intravitreal triamcinolone acetonide injection, logarithm of the minimum angle of resolution visual acuity, macular sensitivity, fixation stability and fixation location by MP-1 microperimetry, and foveal thickness by optical coherence tomography were assessed., Results: Patients' ages ranged from 50 to 75 years (mean +/- SD, 59 +/- 8 years). All patients were classified as nonischemic. At 1, 3, and 6 months, the mean foveal thickness had decreased from 453 +/- 108 microm to 254 +/- 40.3 microm, 297 +/- 90 microm, and 320 +/- 82 microm and the mean retinal sensitivity had increased from 5.5 +/- 3.3 dB to 9.4 +/- 3.5 dB, 7.8 +/- 3.3 dB, and 7.2 +/- 4.2 dB, respectively. At baseline, fixation was stable in one, relatively unstable in six, and unstable in five eyes. However, 6 months after intravitreal triamcinolone acetonide injection, fixation was stable in 8, relatively unstable in 3, and unstable in one. At baseline, in eyes with macular edema, fixation location was predominantly central in 2, poor central in 4, and predominantly eccentric in 6. And 6 months after treatment, fixation location was predominantly central in 8, poor central in 3, and predominantly eccentric in 1., Conclusion: In eyes with macular edema in central retinal vein occlusion, a short-term improvement in retinal sensitivity and fixation properties can be achieved by intravitreal triamcinolone acetonide injection.

Purpose: To evaluate the effect of intravitreal triamcinolone acetonide (IVTA) treatment on serous macular detachment in patients with central retinal vein occlusion (CRVO)., Methods: Ten eyes of 10 patients with CRVO (6 men and 4 women; age range, 54-78 years) made up the study population. The eligibility criteria for this study included clinically and angiographically detectable cystoid macular edema (CME) in which the presence of serous macular detachment was documented by optical coherence tomography (OCT). After IVTA (4 mg/0.1 mL) injection, visual and anatomical responses were observed., Results: In all eyes, after IVTA injection, CME and serous macular detachment regressed. At 3 months and 6 months, CME and serous macular detachment had recurred in 4 (40%) and 5 eyes (50%), respectively. Patients with recurrence were retreated. No eyes lost vision at 1 month, and all eyes had improvement. At 3 months, no eyes had lost vision from baseline, and 7 eyes (70%) had improvement. At 6 months, again no eyes had lost vision from baseline, and 6 eyes (60%) maintained improved visual acuity., Conclusions: Our study shows that IVTA is effective treatment for serous macular detachment in patients with CRVO.

Purpose: To report three cases of focal foveal detachment evident by optical coherence tomography (OCT) in idiopathic epiretinal membrane (ERM)., Methods: A retrospective case series of three patients who had focal foveal detachment shown by OCT and recent visual deterioration and metamorphopsia due to idiopathic ERM., Results: Three patients with idiopathic ERM had recent visual deterioration and metamorphopsia. Although fundus examination, fluorescein angiography, and central macular thickness findings were stable, focal foveal detachment was identified by OCT in all patients., Conclusion: Our findings suggest a sequence of events to explain the OCT findings for and visual changes in these three patients. It is probable that contraction of the ERM on the surface of the retina may have led to dragging of the outer structure in the retina, resulting in focal detachment of the fovea.

Purpose: To evaluate early changes after photodynamic therapy (PDT) for patients with subfoveal choroidal neovascularization (CNV) due to pathologic myopia by optical coherence tomography (OCT)., Methods: PDT was performed on 10 eyes of 10 patients who presented with subfoveal CNV due to pathologic myopia. OCT was used to evaluate changes 1 day, 3 days, and 7 days after therapy. Changes in intraretinal and subretinal fluid and CNV were examined on the images obtained. The retinal elevation and the height of the neurosensory retinal detachment were calculated. From these two values, the thickness of the neurosensory retina was obtained. The thickness of the neurosensory retina was measured to ascertain the intraretinal fluid change, and the height of the neurosensory retinal detachment was measured to ascertain the subretinal fluid change., Results: The mean pretherapy retinal elevation+/-SD increased from 211+/-28 microm to 230+/-39 microm 1 day after PDT and decreased to 221+/-36 microm 3 days after therapy. At 7 days after therapy, the mean retinal elevation+/-SD was 211+/-22 microm. The retinal elevation was due to a subretinal fluid accumulation, whereas the thickness of the neurosensory retina increased only to a minor extent (range, 0-22 microm) and the foveal architecture remained unchanged. The mean pretherapy height+/-SD of the neurosensory retinal detachment was 6+/-11 microm. It was 18+/-20 microm, 12+/-12 microm, and 3+/-6 microm 1 day, 3 days, and 7 days after therapy, respectively. No change in CNV was observed during follow-up., Conclusion: The results of our study indicate that the acute infiltration observed in patients with pathologic myopia after PDT occurs in the first day and regresses during the first week. Yet, it should be noted that, unlike in patients with age-related macular degeneration, the acute infiltration phase can be observed by OCT only to a limited extent.

Purpose: To evaluate the effect of intravitreal triamcinolone acetonide on serous macular detachment in patients with branch retinal vein occlusion (BRVO)., Methods: Eight eyes of 8 patients with BRVO (6 men and 2 women; age range, 52-76 years) made up the study population. The eligibility criteria for this study included clinically and angiographically detectable cystoid macular edema (CME) in which the presence of serous macular detachment was documented by optical coherence tomography (OCT). After intravitreal injection of 0.1 mL (4 mg) of triamcinolone acetonide, the visual and anatomical responses were observed., Results: In all eyes, after injection of triamcinolone acetonide, CME and serous macular detachment regressed. At 3 and 6 months, CME and serous macular detachment had recurred in 1 eye (12.5%) and 2 eyes (25%), respectively. Patients with recurrence were retreated. No eyes lost vision at 1 month, and all eyes showed improvement. At 3 months, no eyes had lost vision from baseline, and 7 eyes (87.5%) showed improvement. At 6 months, again no eyes had lost vision from baseline, and 7 eyes (87.5%) maintained improved visual acuity., Conclusions: The preliminary results of our study showed prompt resolution of serous macular detachment with corresponding improved visual acuity in patients with CME secondary to BRVO. Further study with longer follow-up and a larger series is warranted to assess the long-term efficacy and safety.
