Patients often prefer one knee over the other following staged bilateral total knee arthroplasty (BTKA). Our study compared patient-reported outcomes scores of each knee following BTKA and identified factors that may contribute to the identified discrepancies. All patients who underwent staged BTKA between July 2014 and August 2022 were identified. The patient-reported outcomes were collected preoperatively and at 2 weeks, 6 weeks, 1 year, and 2 years postoperatively. Each knee's results were compared using paired t -tests and McNemar tests. Preoperative Kellgren-Lawrence Grade (KLG), postoperative range of motion (ROM), reoperation rates, and manipulations under anesthesia (MUAs) were collected. Results were stratified based on time between TKAs (< 3 months, 3 to 12 months, 1 to 2 years, and > 2 years). There were 911 patients who underwent staged BTKA, with a mean 4.1-year follow-up. The ROM, patient satisfaction, MUAs, and reoperations were not significantly different between knees. Comparing the KLG of the first and second knees, 71% had the same KLG for both knees, 21% had a lower KLG, and 7% of the second knees had a higher KLG. The first knee had greater pain reduction (−10.6 at 2 weeks, −27.4 at 6 weeks) compared to the second (9.3 at 2 weeks, −8.1 at 6 weeks) (P <.0001) and better improvement in Knee Injury and Osteoarthritis Outcome Score for Joint Replacement (KOOS JR) score (8.5 at 2 weeks, 16.9 at 6 weeks) compared to the second (−5.8 at 2 weeks, 5.0 at 6 weeks) (P <.0001). The 1-year outcomes between first and second knees, or recovery curves, were not different when stratifying by time between TKAs. The second knee in a staged BTKA has less delta improvement in KOOS JR and pain scores at early follow-up, likely due to higher starting KOOS JR and Patient-Reported Outcomes Measurement Information System scores, despite similar final patient satisfaction and clinical outcome measures. Purpose: In patients with moderate-to-severe bilateral medial knee osteoarthritis, it is difficult to determine the necessity of the subsequent contralateral total knee arthroplasty (TKA) after performing the first procedure. This study aimed to compare patient characteristics and clinical outcomes between patients who underwent staged bilateral TKA (group B) and those who underwent unilateral TKA (group U) to identify predictors of contralateral TKA requirement. Methods: Among 153 patients with bilateral medial knee osteoarthritis of Kellgren-Lawrence grade 3 or 4, 43 and 110 patients underwent unilateral and bilateral TKA, respectively. Clinical evaluations using the patient-reported outcome measure (the Japanese Knee Osteoarthritis Measure [JKOM]), the Knee Society Score (KSS), and the timed “Up and Go” test (TUG) were performed preoperatively, at 12 months, and once every year up to 5 years after the unilateral and the contralateral TKA in groups U and B, respectively. Receiver Operating Characteristic (ROC) analysis was used to determine the cut-off value for preoperative JKOM score that indicates the contralateral TKA. Results: There were no significant differences in patient characteristics between groups B and U. The preoperative JKOM scores were significantly worse in group B. Postoperative improvements in JKOM, KSS, and TUG were statistically significant in both groups; however, there were no significant differences in post-procedure scores between the two groups. ROC analysis showed that the optimal cut-off value of the preoperative JKOM score was 41.5 points. Conclusion: A preoperative JKOM score of ≥41.5 is a possible predictor of the contralateral TKA in patients with moderate-to-severe bilateral medial knee osteoarthritis.

Aims: Up to one in five patients undergoing primary total hip (THA) and knee arthroplasty (TKA) require contralateral surgery. This is frequently performed as a staged procedure. This study aimed to determine if outcomes, as determined by the Oxford Hip Score (OHS) and Knee Score (OKS) differed following second-side surgery. Methods: Over a five-year period all patients who underwent staged bilateral primary THA or TKA utilizing the same type of implants were studied. Eligible patients had both preoperative and one year Oxford scores and had their second procedure completed within a mean (2 SDs) of the primary surgery. Patient demographics, radiographs, and OHS and OKS were analyzed. Results: A total of 236 patients met the inclusion criteria, of which 122 were THAs and 114 TKAs. The mean age was 66.5 years (SD 9.4), with a 2:1 female:male ratio. THAs showed similar significant improvements in outcomes following first- and second-side surgery, regardless of sex. In contrast for TKAs, although male patients demonstrated the same pattern as the THAs, female TKAs displayed significantly less improvement in both OKS and its pain component following second-side surgery. Conclusion: Female patients undergoing second-side TKA showed less improvement in Oxford and pain scores compared to the first-side. This difference in outcome following second-side surgery did not apply to male patients undergoing TKA, or to either sex undergoing THA.

Background: Patients with hip and knee arthritis often undergo bilateral total hip arthroplasty (THA) and total knee arthroplasty (TKA) in a staged or simultaneous fashion. However, when staged, the incidence and factors associated with having both procedures performed by the same surgeon or different surgeon are not well studied.Methods: All patients undergoing nonsimultaneous bilateral THA or TKA for osteoarthritis were abstracted from the 2010 to 2020 PearlDiver Mariner administrative database. The National Provider Identifier number was used to determine whether the same surgeon performed both surgeries. Demographics, comorbidities, and 90-day complications after the first joint replacement were assessed as possible independent predictors of utilizing a different surgeon for the contralateral joint.Results: Of 87,593 staged bilateral THAs, the same surgeon performed 40,707 (46.5%) arthroplasties. Of 147,938 staged bilateral TKAs, the same surgeon performed 77,072 (52.1%) arthroplasties. Notably, older cohorts of patients had independent, stepwise, and significantly greater odds of changing surgeons for the contralateral THA and TKA. Those patients who were insured by Medicare and Medicaid had significantly lower odds of changing surgeons. For both THA and TKA, surgical and implant-related adverse events (surgical site infection/periprosthetic joint infection, periprosthetic fracture, dislocation, manipulation) carried the greatest odds of undergoing the contralateral replacement with a different surgeon.Conclusion: Patients covered by Medicaid and sicker patients were significancy less likely to switch surgeons for their contralateral THA or TKA. Additionally, patients experiencing a surgery-related adverse event within 90 days of their first THA or TKA had significantly, increased odds of switching surgeons for their subsequent TJA. Background: There are many risk factors for arthrofibrosis and manipulation under anesthesia (MUA) following total knee arthroplasty (TKA). However, no study has elucidated whether a history of MUA increases the risk of contralateral MUA in patients undergoing staged bilateral TKA.Methods: A retrospective review of an institutional database of TKAs was performed. All patients aged ≥18 years who underwent primary staged bilateral TKAs were screened for inclusion. Staged bilateral TKAs were viewed as 2 distinct events based on the temporal order in which they occurred: TKA#1 (occurred first) and TKA#2 (occurred second). Following TKA#1, patients were split into 2 groups: those who underwent MUA (Group MANIP) and those who did not (Group NO MANIP). The subsequent risk of undergoing MUA following TKA#2 was then assessed and compared between the 2 groups. Chi-squared tests were used for comparison.Results: A total of 5,330 patients who underwent primary uncomplicated staged bilateral TKAs (10,660 knees) during the study period were identified. Overall, 2.1% of patients underwent MUA following TKA#1 and 1.9% of patients underwent MUA following TKA#2. In the MANIP group, 21.4% of patients underwent MUA following TKA#2, while only 1.5% underwent MUA in the NO MANIP group. This 14.3-fold increase in the risk of MUA in the MANIP group following TKA#2 was statistically significant (21.4% vs 1.5%, absolute risk reduction = 19.9%, relative risk reduction = 93.0%, P < .0001).Conclusion: Patients who undergo MUA during the first TKA of a staged bilateral TKA are 14.3 times more likely to undergo a subsequent MUA than those who did not undergo MUA following their first TKA. Background: The objective of the study was to analyze if the length of interval time between stages influenced functional and quality of life outcomes in patients with staged bilateral primary total knee arthroplasty (TKA).Methods: This is a retrospective comparative study between 93 patients with an interval between stages of 6-8 months (6-month group), 112 of 12-14 months (1-year group), and 108 of 24-26 months (2-year group). Outcome variables were Knee Society scores, Western Ontario and McMaster Universities, Short Form, and patient satisfaction.Results: Overall, the mean follow-up for the first TKA was 8.2 (range, 7-10) years, and for the second TKA, 6.7 (range, 5-10) years. At last follow-up, functional and patient-related outcomes were similar for both knees, regardless of the interval. However, mental score and patient satisfaction were significantly better for the second than for the first TKA in the 2-year group. Age did not correlate significantly with the functional scores but was significantly correlated with the mental score.Conclusion: The performing staged bilateral TKA with a wide interval between surgeries provided equivalent functional outcomes and quality of life for both knees. Postoperative outcomes were not affected by the length of the time interval between procedures or age. Our results can help the surgeon to inform the patients reliably about what they can expect in the delay of a second knee replaced. Thus, patients could make an informed decision. Abstract: Simultaneous vs staged bilateral total hip arthroplasty (THA) continues to evoke controversy, with perioperative complications representing the most significant issue. Previous authors have reported significant cost savings by performing bilateral THA simultaneously, but have failed to address issues of surgeon and hospital reimbursement. This study compares 277 consecutive patients undergoing either simultaneous (334 THAs) or staged bilateral cementless THA (220 THAs) in lateral decubitus position using a single cementless stem design, with emphasis on perioperative complications and reimbursement to surgeon and hospital. There were significantly more inpatient complications and adverse events in patients undergoing simultaneous bilateral THA in the lateral decubitus position, significantly higher transfusion requirement, and more patients failing to reach physical therapy goals during admission requiring more transfers to rehabilitation facilities. Need for subsequent hip surgery was also significantly higher in simultaneous bilateral patients. In addition to these negative results, the hospital system realized a 28% reduction and the surgeon suffered a 15% reduction in potential reimbursement. [Copyright &y& Elsevier]

Background: Long-term follow-up after total knee arthroplasty (TKA) is crucial to providing survivorship updates on available TKA implants to guide patient expectations when undergoing TKA. The purpose of this study was to compare implant survivorship between patients undergoing primary single, simultaneous bilateral, and staged bilateral TKA with 15-year minimum follow-up., Methods: This retrospective study included 826 patients (n = 1089) who underwent primary single (n = 454), simultaneous bilateral (n = 266), or staged bilateral (n = 369) TKA using a single TKA system by a single surgeon from 1991-2005 with 15-year minimum follow-up. Demographics, clinical variables, and surgical outcomes were collected and compared using Student's t-test, chi-squared tests, or Kaplan-Meier analyses. Reoperation was defined as all surgeries performed on the knee after the index procedure; revision TKA was defined as complete implant exchange. p < 0.05 significant., Results: Patients in the staged TKA cohort were younger (65.4 years = staged, 67.8 = simultaneous, 67.1 = single, p < 0.019). Women were less likely to receive simultaneous TKAs (22 %vs.29%, p < 0.001). Postoperative range of motion was similar (116.8° = simultaneous, 114.9° = staged, 114.8° = single, p = 0.11). Overall 15-year implant survival based on revision TKA was similar (97.7% = simultaneous, 97.2% = staged, 96.7% = single, p = 0.45). The estimated 15-year reoperation rate was 7.0% (95% CI, 5.5-8.7%). Reoperations were secondary to infection (35.5%), implant wear (26.3%), arthrofibrosis (18.4%), traumatic injuries (10.5%), pain (6.6%), pathologic lesion (1.3%), and avascular necrosis (1.3%)., Conclusion: This study demonstrated a high implant survival rate of 95-97% at 20-years after primary single, staged bilateral and simultaneous bilateral TKA. Background: Accuracy of lead placement within the brain can affect the outcome of deep brain stimulation (DBS) surgery. Whether performing unilateral lead implantation, simultaneous bilateral lead implantation, or staged bilateral lead implantation affects accuracy has not yet been assessed. We compare lead placement errors to evaluate whether one approach affords greater lead accuracy., Methods: We retrospectively reviewed 205 leads placed in 125 DBS surgeries. The accuracy of lead placement, defined by differences in x, y, and z coordinates and error vector magnitudes, was compared between three surgery groups: unilateral leads, bilateral leads placed simultaneously, and bilateral leads placed in staged surgeries. We also compared accuracies between first and second leads within each bilateral cohort and between second leads of the bilateral cohorts. Finally, we examined the effect of target and age on accuracy., Results: The accuracy of lead placement was comparable among unilateral, simultaneous bilateral, and staged bilateral leads. Timing of placement of the second lead in bilateral cases was not found to affect accuracy. The mean number of microelectrode trajectories was greater for first leads in simultaneous bilateral DBS (p = 0.032). No significant correlation between either age or target and accuracy was found., Conclusion: Although there may be other important reasons for performing DBS in a staged fashion, our study finds that neither laterality nor timing of second lead placement, patient age, or target site have significant impact on DBS lead accuracy, a finding that indicates with appropriate approach selection based on patient factors, accuracy does not have to be significantly compromised., (© 2017 International Neuromodulation Society.)
