The webs can be congenital or acquired, commonly associated with Plummer-Vinson syndrome and rarely with celiac disease, dermatological disorders or graft-versus-host disease. A 54-year-old man was referred to our hospital with a history of high non-progressive dysphagia to solid food, meat impaction and weight loss over last ten months. His medical history and family history were unremarkable nor was the physical examination. Complete blood count and basic biochemical tests were normal. Antigliadin and antiendomysial antibodies for celiac disease were negative. Barium swallow esophagography and upper endoscopy detected cervical esophageal webs. The treatment consisted of endoscopic balloon dilation. The patient’s dysphagia resolved shortly after dilation, and the endoscope passed easily through the esophagus showing normal esophageal, gastric and duodenal mucosa. This report is consistent with a case of esophageal webs rarely documented in males and that is not related with common causes like Plummer-Vinson syndrome. 3. Seasonal observations of the ocean and sea ice environment offshore Oliktok Point, Alaska, using distributed acoustic sensing

Abstract

We present new results from a multiyear effort to record environmental signals in the Beaufort Sea using distributed acoustic sensing (DAS). We collected seafloor DAS data on a 36-km long segment of telecom dark fiber for eight one-week intervals in 2021 and 2022. The eight data collects comprise two samplings of each Arctic “season” (ice bound, ice free, and partial ice coverage during freezing and breakup). We recorded icequakes, floating sea-ice collisions, ice grounding events, ice flexural-gravity waves excited by winds, currents, and hovercrafts, ocean gravity waves, earth microseisms, and submarine soil/permafrost resonance. We have used these signals to characterize both sea ice extent and sea-ice thickness in the Beaufort Sea. Sea ice-extent is determined by clustering ambient noise data in 30-minute windows using a convolutional neural network. The presence or absence of ocean gravity waves is the dominant factor in the ice/no-ice clustering. Sea ice thickness is estimated from the inversion of ice flexural-gravity wave dispersion curves. This novel application has the added benefit of simultaneously inverting for snow thickness on the ice sheet, a confounding factor in other remote sensing methods. Ocean gravity wave parameters will also be presented. Comparison with wave buoys purposefully deployed above the fiber indicates that DAS can capture low-frequency waves with high spatial resolution during both open water and partially ice-covered periods.SNL is managed and operated by NTESS under DOE NNSA contract DE-NA0003525

6. The relevance of oxidative stress in cirrhotic patients with different forms of hepatic encephalopathy

Abstract

While the exact ammonia-induced pathogenic mechanisms in hepatic encephalopathy (HE) are not completely understood, an increasing number of reports have lately been involved in studying the possible relevance of oxidative stress status in HE. Here we evaluated the levels of some oxidative stress markers (two main antioxidant enzymes: superoxide dismutase-SOD and glutathione peroxidase-GPX, as well as a lipid peroxidation marker: malondialdehyde-MDA) in thirty-six cirrhotic patients with overt HE (n=12) or minimal HE (n=24), as compared to a control age-matched group (n=19). We also included a group with cirrhosis without HE (n=42). Our results provide additional evidences of increased oxidative stress in both overt and minimal HE, as expressed by altered serum glutathione peroxidase antioxidant activity and increased levels of lipid peroxidation. Moreover, we demonstrated a significant correlation between the levels of the aforementioned oxidative stress markers and the severity of cirrhosis according to the Child and MELD scales, as well as with the venous blood ammonia level.

Abstract

This article discusses a survey conducted in a tertiary-care center in Romania to assess the perception of European recommendations for environmental sustainability in endoscopy. The survey included medical personnel and asked them to state their level of agreement with statements regarding environmental sustainability in endoscopy. The majority of participants agreed that measures are needed to reduce the negative impact of endoscopy on the environment, but there was disagreement regarding the replacement of endoscopy by alternative investigations and the use of methods other than endoscopy for trainees. Overall, most of the proposed measures for environmental sustainability in endoscopy were agreed upon by the participants. [Extracted from the article]

Abstract

The paper deals with the brain drain phenomenon (rational minds migration), displayed inside the European Union, in close correlation with European tax competition. Speciality literature from the countries of the European Union deals with great responsibility the migration process of the specialists, numerous studies being dedicated to this phenomenon. Through the present paper we aim to study this correlation and the evaluation of the implications for the member states of the European Union, by elaborating a model by means of which there are tested the correlations between different variables and the quantification of the taxation effects upon the brain drain phenomenon. The model is an adaptation of the so-called ”Model of the Brain Drain and human capital formation” elaborated by Mountford (1997) by which there are described the different researched variables: brain gain, brain drain, migration competition and tax competition. 35. Informed consent for gastrointestinal endoscopy: A 2002 ESGE survey

Abstract

Background/Aim: Gastroenterologists are required to obtain informed consent before undertaking any endoscopic examination. Published data indicate that in practice there are many deficiencies in this process. The aim of this survey was to determine the quality of information given to patients before the endoscopic procedures in countries that are members of the European Society of Gastrointestinal Endoscopy (ESGE). Methods: A structured questionnaire was sent to representatives of endoscopic societies that are members of ESGE regarding the quality of informed consent. Results: The response rate was 59% (26/44). The endoscopist is responsible for giving the required information prior to written consent in only 23.1% (6/26) of the countries. Although information about the procedure is given to the patients in 96% of the responding countries, in only 77% is there sufficient time for the patients to ask questions about the nature of the procedure. In 15% (4/26) of the countries neither the diagnostic or therapeutic alternatives to endoscopy are discussed nor the potential complication rates. Finally, the procedure-related mortality is provided in only 23% (6/26) of countries. Conclusion: The procedure of obtaining informed consent for endoscopy varies considerably and may need reevaluation. Copyright (C) 2002 S. Karger AG, Basel.

Abstract

The first attempt to unify Thermodynamics with Finite Speed (TFS) and Thermodynamics with Finite Time (TFT) has been achieved by Petrescu, Harman and Bejan in 1992. Since then, several papers that have been published played an essential role in the whole development of TFS, finally leading to the Validation of the Direct Method and TFS for 12 Stirling engines. The present paper presents the results of the second unification between TFS and TFT by taking into account in addition to the Finite Speed, two other causes of internal irreversibility in thermal machines, namely Friction and Throttling. In this way we achieved quantitatively what was very clearly explained by Thomson Kelvin only qualitatively, namely that "the main causes of irreversibility are: the finite speed, friction and throttling". Abstract

Introduction. Relative adrenal insufficiency (RAI) is common in the setting of critical illness as well as in hemodynamically instable cirrhotic patients with sepsis. Several studies have also shown that RAI is frequent in patients with stable cirrhosis without sepsis. The aim of this study was to prospectively assess the incidence of RAI in patients with stable cirrhosis. Patients and Methods. Forty-seven patients with hemodynamically stable liver cirrhosis without sepsis were prospectively included. RAI, assessed by using low doseshort Synacthen test (LD-SST), was defined as either a basal total cortisol concentration below 3.6 μg/dL or a peak total serum cortisol ≥ 16 μg/dL at 30 min after stimulation. Results. RAI was present in 10 (21.3%) of 47 cirrhotic patients. Peak cortisol level was negatively correlated with the severity of cirrhosis evaluated by Child-Turcotte-Pugh (CTP) (r=-0.46; P=0.001) and Model for End- Stage Liver Disease (MELD) (r=-0.51; P=0.001) scores. The frequency of RAI increased from CTP-A (10%) to CTP-B (30%) to CTP-C (60%). Conclusion. RAI diagnosed by LD-SST is frequent in patients with stable cirrhosis and is related to the severity of liver disease. Abstract

The growth of carbon nanotubes from fullerene fragments produced by catalytic decomposition in the presence of transition metals at about 450 °C was investigated. Multi wall and single wall nanotubes are grown by vapor deposition on highly oriented pyrolytic graphite (HOPG) at room temperature. No further purification or manipulation is necessary for investigations by scanning tunneling microscopy. The templating effect of the substrate is assumed to be an important growth condition. No tubes were found on Au(III) or mica surfaces. Individual tubes and raft like structures of multiwalled tubes and bundles of single walled tubes were found on HOPG. Abstract

Thermal stability of endohedral Li@C[sub 60] molecules was studied with high performance liquid chromatography (HPLC), laser desorption time of flight mass spectrometry and double sublimation thermal desorption spectrometry. HPLC purified material with a 90% nominal content of Li@C[sub 60], was used. The material is transformed by heat into a non-soluble and nonsublimable state. High heating rates (10 K/s) are appropriate to sublime Li@C[sub 60]. Thin films of Li@C[sub 60] were investigated by atomic force microscopy, optical absorption and second harmonic generation (SHG). First evidence for the photopolymerization of Li@C[sub 60] films was obtained from SHG measurements. [ABSTRACT FROM AUTHOR]
