The aim of this study is to describe resources and outcomes of coronary computed tomography angiography plus Stress CT perfusion (CCTA ​+ ​Stress-CTP) and stress cardiovascular magnetic resonance (Stress-CMR) in symptomatic patients with suspected or known CAD. Six hundred and twenty-four consecutive symptomatic patients with intermediate to high-risk pretest likelihood for CAD or previous history of revascularization referred to our hospital for clinically indicated CCTA ​+ ​Stress-CTP or Stress-CMR were enrolled. Stress-CTP scans were performed in 223 patients while 401 patients performed Stress-CMR. Patient follow-up was performed at 1 year after index test performance. Endpoints were all cardiac events, as a combined endpoint of revascularization, non-fatal MI and death, and hard cardiac events, as combined endpoint of non-fatal MI and death. Twenty-nine percent of patients who underwent CCTA ​+ ​Stress-CTP received revascularization, 7% of subjects assessed with Stress-CMR were treated invasively, and a low number of non-fatal MI and death was observed with both strategies (hard events in 0.4% of patients that had CCTA ​+ ​Stress-CTP as index test, and in 3% of patients evaluated with Stress-CMR). According to the predefined endpoints, CCTA ​+ ​Stress-CTP group showed high rate of all cardiac events and low rate of hard cardiac events, respectively. The cumulative costs were 1970 ​± ​2506 Euro and 733 ​± ​1418 Euro for the CCTA ​+ ​Stress-CTP group and Stress-CMR group, respectively. The use of CCTA ​+ ​Stress-CTP strategy was associated with high referral to revascularization but with a favourable trend in terms of hard cardiac events and diagnostic yield in identifying individuals at lower risk of adverse events despite the presence of CAD. Background Cardiac magnetic resonance (CMR) is gaining an important role in the setting of acute coronary syndromes: it gives prognostic information based on oedema and late gadolinium enhancement (LGE) extension in acute myocardial infarction, and has a diagnostic value in myocardial infarction with nonobstructive coronary arteries (MINOCA) thanks to its capability to distinguish, based on different LGE patterns, ischaemic and non-ischaemic myocardial injuries. Case summary We describe a case of acute myocardial infarction involving a recurrent apical branch showing an atypical intramyocardial LGE pattern in the medium inferior septum. Discussion An intramyocardial LGE pattern might be determined by an ischaemic injury involving the interventricular septum. Chronic coronary syndrome (CCS) is one of the leading cardiovascular causes of morbidity, mortality, and use of medical resources. After the introduction by international guidelines of the same level of recommendation to non-invasive imaging techniques in CCS evaluation, a large debate arose about the dilemma of choosing anatomical (with coronary computed tomography angiography (CCTA)) or functional imaging (with stress echocardiography (SE), cardiovascular magnetic resonance (CMR), or nuclear imaging techniques) as a first diagnostic evaluation. The determinant role of the atherosclerotic burden in defining cardiovascular risk and prognosis more than myocardial inducible ischemia has progressively increased the use of a first anatomical evaluation with CCTA in a wide range of pre-test probability in CCS patients. Functional testing holds importance, both because the role of revascularization in symptomatic patients with proven ischemia is well defined and because functional imaging, particularly with stress cardiac magnetic resonance (s-CMR), gives further prognostic information regarding LV function, detection of myocardial viability, and tissue characterization. Emerging techniques such as stress computed tomography perfusion (s-CTP) and fractional flow reserve derived from CT (FFRCT), combining anatomical and functional evaluation, appear capable of addressing the need for a single non-invasive examination, especially in patients with high risk or previous revascularization. Furthermore, CCTA in peri-procedural planning is promising to acquire greater importance in the non-invasive planning and guiding of complex coronary revascularization procedures, both by defining the correct strategy of interventional procedure and by improving patient selection. This review explores the different roles of non-invasive imaging techniques in managing CCS patients, also providing insights into preoperative planning for percutaneous or surgical myocardial revascularization. Polarons and spin-orbit (SO) coupling are distinct quantum effects that play a critical role in charge transport and spin-orbitronics. Polarons originate from strong electron-phonon interaction and are ubiquitous in polarizable materials featuring electron localization, in particular 3d transition metal oxides (TMOs). On the other hand, the relativistic coupling between the spin and orbital angular momentum is notable in lattices with heavy atoms and develops in 5d TMOs, where electrons are spatially delocalized. Here we combine ab initio calculations and magnetic measurements to show that these two seemingly mutually exclusive interactions are entangled in the electron-doped SO-coupled Mott insulator Ba2Na1−xCaxOsO6 (0 < x < 1), unveiling the formation of spin-orbital bipolarons. Polaron charge trapping, favoured by the Jahn-Teller lattice activity, converts the Os 5d1 spin-orbital Jeff = 3/2 levels, characteristic of the parent compound Ba2NaOsO6 (BNOO), into a bipolaron 5d2 Jeff = 2 manifold, leading to the coexistence of different J-effective states in a single-phase material. The gradual increase of bipolarons with increasing doping creates robust in-gap states that prevents the transition to a metal phase even at ultrahigh doping, thus preserving the Mott gap across the entire doping range from d1 BNOO to d2 Ba2CaOsO6 (BCOO).The interplay between electron-phonon and spin-orbit interactions has led to the concept of a spin-orbit polaron. Here the authors show that such a regime is realized in a spin-orbit-coupled Mott insulator, leading to a new polaron quasiparticle, and study its effect on the Mott metal-insulator transition. Recent technological advancements have facilitated the extensive collection of movement data from large-scale fishing vessels, yet a significant data gap remains for small-scale fisheries. This gap hinders the development of consistent exploitation patterns and meeting the information needs for marine spatial planning in fisheries management. This challenge is specifically addressed in the Campania region of Italy, where several Marine Protected Areas support biodiversity conservation and fisheries management. The authors have created a spatially-explicit dataset that encompasses both large-scale (vessels exceeding 12 meters in length) and small-scale (below 12 meters) fishing efforts. This dataset (available at https://doi.org/10.6084/m9.figshare.23592006) is derived from vessel tracking data and participatory mapping. It offers insights into potential conflicts between different fishing segments and their interactions with priority species and habitats. The data can assist researchers and coastal management stakeholders in formulating policies that reduce resource competition and promote ecosystem-based fisheries management. Circulating small extracellular vesicles (sEVs) contribute to inflammation, coagulation and vascular injury, and have great potential as diagnostic markers of disease. The ability of sEVs to reflect myocardial damage assessed by Cardiac Magnetic Resonance (CMR) in ST-segment elevation myocardial infarction (STEMI) is unknown. To fill this gap, plasma sEVs were isolated from 42 STEMI patients treated by primary percutaneous coronary intervention (pPCI) and evaluated by CMR between days 3 and 6. Nanoparticle tracking analysis showed that sEVs were greater in patients with anterior STEMI (p = 0.0001), with the culprit lesion located in LAD (p = 0.045), and in those who underwent late revascularization (p = 0.038). A smaller sEV size was observed in patients with a low myocardial salvage index (MSI, p = 0.014). Patients with microvascular obstruction (MVO) had smaller sEVs (p < 0.002) and lower expression of the platelet marker CD41–CD61 (p = 0.039). sEV size and CD41–CD61 expression were independent predictors of MVO/MSI (OR [95% CI]: 0.93 [0.87–0.98] and 0.04 [0–0.61], respectively). In conclusion, we provide evidence that the CD41–CD61 expression in sEVs reflects the CMR-assessed ischemic damage after STEMI. Funding innovation requires knowledge on previous/on-going research and identification of gaps and synergies among actors, networks and projects, but targeted databases remain scattered, incomplete and scarcely searchable. Here we present the BlueBio database: a first comprehensive and robust compilation of internationally and nationally funded research projects active in the years 2003–2019 in Fisheries, Aquaculture, Seafood Processing and Marine Biotechnology. Based on the previous research projects' database realized in the framework of the COFASP ERA-NET, it was implemented within the ERA-NET Cofund BlueBio project through a 4-years data collection including 4 surveys and a wide data retrieval. After being integrated, data were harmonised, shared as open and disseminated through a WebGIS that was key for data entry, update and validation. The database consists of 3,254 "georeferenced" projects, described by 22 parameters that are clustered into textual and spatial, some directly collected while others deduced. Deep-sea fishery in the Mediterranean Sea was historically driven by the commercial profitability of deepwater red shrimp (DWRS), and understanding spatiotemporal dynamics of fishing is key to comprehensively evaluate the status of these profitable resources and prevent stock collapse. A 4-year time series of observed monthly patterns and related frequency of trawling disturbance based on an automatic identification system (AIS) is provided with a resolution of 0.01∘×0.01∘ , accounting for the spatial extent and temporal variability in deepwater (DW) bottom-contact fisheries during the period 2015–2018. The dataset was estimated from 370 fishing vessels that were found to perform trawling in deep water (400–800 m) during the study period, and they represent a significant part of the real fleet exploiting these fishing grounds in the study area. The reconstructed deepwater trawling-effort dataset is available at: 10.17882/89150 (Pulcinella et al., 2022). The knowledge of small‐scale fisheries (SSFs) is important to develop management policies and mitigate the competition for marine resources. However, spatially explicit information is often unavailable at the regional and subregional scale. We designed and tested a novel participatory approach to map the SSF fishing effort using the Mediterranean sea as a case study. We applied the approach in eight countries (Albania, Croatia, Italy, Libya, Malta, Montenegro, Slovenia and Tunisia) characterized by different cultural, social, political and ecological features. The results provided quantitative and spatially explicit information on fishing operations on a fine‐scale resolution, contributing to overcome the pragmatic and budgetary constraints that to date have prevented an accurate assessment of SSFs worldwide. This novel participatory approach is inspired by the principles of governance, adaptive management, cross‐national cooperation and spatial planning, thus supporting the ecosystem approach to fisheries and has the potential to provide a useful complement to traditional assessments. Introduction: Bradyarrhythmias are an established red flag for storage cardiac conditions including Anderson–Fabry disease (AFD). The prevalence of bradyarrhythmias requiring a pacemaker (PM) and their timing in AFD is unresolved. Methods: We evaluated the prevalence and predictors of PM requirement in a large AFD cohort, investigating the occurrence of bradyarrhythmias as initial versus late manifestation. We retrospectively evaluated 82 consecutive AFD patients referred to our multidisciplinary referral center from 1994 to 2020 with a median follow‐up of 6.9 years, identifying those requiring pacing. Univariable analysis was performed to identify cardiac features associated with PM implantation. Results: Five of 82 (6%) AFD patients required PM implantation (5/39, i.e., 13% of those with cardiac involvement), always in the context of advanced cardiomyopathy. In none, bradyarrhythmias were the presenting feature. Indications included sick sinus syndrome in three patients, advanced atrioventricular block in two patients. QRS prolongation during follow‐up strongly correlated with the onset of bradyarrhythmias. Conclusion: Severe bradyarrhythmias are relatively frequent in patients with AFD cardiomyopathy, but do not represent a mode of presentation, occurring late in the disease course and always in the context of advanced cardiac involvement. The COVID-19 pandemic provides a major opportunity to study fishing effort dynamics and to assess the response of the industry to standard and remedial actions. Knowing a fishing fleet's capacity to compensate for effort reduction (i.e., its resilience) allows differentiating governmental regulations by fleet, i.e., imposing stronger restrictions on the more resilient and weaker restrictions on the less resilient. In the present research, the response of the main fishing fleets of the Adriatic Sea to fishing hour reduction from 2015 to 2020 was measured. Fleet activity per gear type was inferred from monthly Automatic Identification System data. Pattern recognition techniques were applied to study the fishing effort trends and barycentres by gear. The beneficial effects of the lockdowns on Adriatic endangered, threatened and protected (ETP) species were also estimated. Finally, fleet effort series were examined through a stock assessment model to demonstrate that every Adriatic fishing fleet generally behaves like a stock subject to significant stress, which was particularly highlighted by the pandemic. Our findings lend support to the notion that the Adriatic fleets can be compared to predators with medium-high resilience and a generally strong impact on ETP species. [ABSTRACT FROM AUTHOR]

Moreover, data were widely metadated, published in a WebGIS (https://adrireef.github.io/sandbox3/ , last access: 3 May 2021), and shared as open data in EMODnet (European Marine Observation and Data Network) Data Ingestion Portal through the SEANOE repository (Ferrà et al., 2020; 10.17882/74880). Purpose of Review: We provide a state of the art of therapeutic options in hypertrophic cardiomyopathy (HCM), focusing on recent advances in our understanding of the pathophysiology of sarcomeric disease. Recent Findings: A wealth of novel information regarding the molecular mechanisms associated with the clinical phenotype and natural history of HCM have been developed over the last two decades. Such advances have only recently led to a number of controlled randomized studies, often limited in size and fortune. Recently, however, the allosteric inhibitors of cardiac myosin adenosine triphosphatase, countering the main pathophysiological abnormality associated with HCM-causing mutations, i.e. hypercontractility, have opened new management perspectives. Mavacamten is the first drug specifically developed for HCM used in a successful phase 3 trial, with the promise to reach symptomatic obstructive patients in the near future. In addition, the fine characterization of cardiomyocyte electrophysiological remodelling has recently highlighted relevant therapeutic targets. Summary: Current therapies for HCM focus on late disease manifestations without addressing the intrinsic pathological mechanisms. However, novel evidence-based approaches have opened the way for agents targeting HCM molecular substrates.
